Systematic review of surgical resection vs radiofrequency ablation for hepatocellular carcinoma

Hepatocellular carcinoma (HCC) represents one of the most common neoplasms worldwide. Surgical resection and local ablative therapies represent the most frequent first lines therapies adopted when liver transplantation can not be offered or is not immediately accessible. Hepatic resection (HR) is currently considered the most curative strategy, but in the last decade local ablative therapies have started to obtain satisfactory results in term of efficacy and, of them, radiofrequency ablation (RFA) is considered the reference standard. An extensive literature review, from the year 2000, was performed, focusing on results coming from studies that directly compared HR and RFA. Qualities of the studies, characteristics of patients included, and patient survival and recurrence rates were analyzed. Except for three randomized controlled trials (RCT), most studies are affected by uncertain methodological approaches since surgical and ablated patients represent different populations as regards clinical and tumor features that are known to affect prognosis. Unfortunately, even the available RCTs report conflicting results. Until further evidences become available, it seems reasonable to offer RFA to very small HCC ( 2 cm and especially if > 3 cm, and/or in tumor locations in which ablation is not expected to be effective or safe, surgical removal is to be preferred.

Short-term changes in soil properties after prescribed fire and mulching with fern in Mediterranean forests

Prescribed fire,although having low intensity and being able to reduce the risk of wildfire may modify soil properties in the short term,with possible increases in runoffand erosion *** mulching with vegetation residues is one of the most common post-fire management *** of fern,which is abundant on the Mediterranean forest floor,may be used to replace straw for mulching fireaffected ***,the effects of prescribed fires are not completely understood,and there is no data regarding the use of fern to protect soil after fire in the *** fill this gap,selected soil chemical parameters were analyzed,on a comparative basis,in three Mediterranean forests(pine,oak and chestnut)in Calabria(Southern Italy).These parameters were measured immediately and one year after fire in unburned,burned and not treated,and burned and mulched *** in soil chemical properties among the different treatments were significant,and the effects of the prescribed fire and mulching were dependent on the time elapsed from their application and forest *** general,mulching was not effective in limiting the changes in the monitored soil properties compared to the pre-fire *** forest species showed different temporal trends in changes of soil properties.

Topical delivery of nerve growth factor for treatment of ocular and brain disorders

Neurotrophins are a family of proteins that support neuronal proliferation, survival, and differentiation in the central and peripheral nervous systems, and are regulators of neuronal plasticity. Nerve growth factor is one of the best-described neurotrophins and has advanced to clinical trials for treatment of ocular and brain diseases due to its trophic and regenerative properties. Prior trials over the past few decades have produced conflicting results, which have principally been ascribed to adverse effects of systemic nerve growth factor administration, together with poor penetrance of the blood-brain barrier that impairs drug delivery. Contrastingly, recent studies have revealed that topical ocular and intranasal nerve growth factor administration are safe and effective, suggesting that topical nerve growth factor delivery is a potential alternative to both systemic and invasive intracerebral delivery. The therapeutic effects of local nerve growth factor delivery have been extensively investigated for different ophthalmic diseases, including neurotrophic keratitis, glaucoma, retinitis pigmentosa, and dry eye disease. Further, promising pharmacologic effects were reported in an optic glioma model, which indicated that topically administered nerve growth factor diffused far beyond where it was topically applied. These findings support the therapeutic potential of delivering topical nerve growth factor preparations intranasally for acquired and degenerative brain disorders. Preliminary clinical findings in both traumatic and non-traumatic acquired brain injuries are encouraging, especially in pediatric patients, and clinical trials are ongoing. The present review will focus on the therapeutic effects of both ocular and intranasal nerve growth factor delivery for diseases of the brain and eye.

Soil mineral nitrogen dynamics in fallow periods in a rainfed semiarid Mediterranean agricultural system

Rainfed agricultural systems in semiarid Mediterranean environments are subject to erratic but often heavy rainfall *** an agronomic practice,fallow periods can be included even within the existing European Union common policy for crop *** study aimed to quantify the effects of previous mineral fertilization on soil mineral nitrogen(N_(min))content and potential nitrate leaching during no-till fallow periods of crop *** Leaching Estimation and Chemistry Model(LEACHM)was used for soil N_(min)prediction after *** fallow periods,N_(min)measured in the soil profile increased by an average of 125 kg N ha^(-1),whereas the model showed an average increase of 95 kg N ha^(-1).The amount of nitrogen(N)leached was estimated to be 11–38 kg N ha^(-1).The N balance simulated using LEACHM might differ from the actual *** calibrated LEACHM underestimated soil N_(min)after a drought period followed by soil water replenishment,probably due to a pulse in soil C and N mineralization caused by the wetting of dry soil,but overestimated soil N_(min)after occasional rainy spells when soil quickly became saturated,probably due to occasional nitrous oxide emissions not being fully accounted for by the model and specific preferential water flow,which might lead to greater nitrate leaching than that simulated by LEACHM using the convection-dispersion *** results show that soil N_(min)measurements after fallow periods cannot be well predicted by *** findings of this study provide a support for fertilization planning for crop rotation systems including a fallow period to avoid overfertilization in the following cropping season and reduce N environmental impacts.

Stopover behavior of Red-eyed Vireos(Vireo olivaceus) during fall migration on the coast of the Yucatan Peninsula

Background: For migrating birds, stopover requires spending time and energy that otherwise could be allocated to flying. Thus, birds optimally refuel their subsequent migratory flight by reducing stopover duration or foraging activ-ity in food-rich environments. In coastal habitats, birds may forego refueling and take short stopovers irrespective of local food availability. Given the paucity of studies exploring how migrants adjust stopover behavior in response to temporal variation in food availability, especially in the Neotropics, we fixed radio tags to 51 Red-eyed Vireos(Vireo olivaceous) over two years at two sites on the coast of Mexico's Yucatan ***: We applied VHF radio tags during the fall of 2016 and 2017, and tracked birds using automatic and manual receiving units. We estimated stopover duration and activity levels(one site only) for between six and fifteen birds,depending on site and year. We measured fruit availability weekly along the net lanes where we captured birds. We used a generalized linear model to estimate the relationships between stopover duration/activity level and fruit den-sity, bird body mass and year. We interpreted relationships for the model with the lowest AICc ***: We found that approximately half of the birds departed on the same day they were captured. For the birds that stayed longer, we could not discern whether they did so because they were light, or fruit density was high. On the other hand, lighter birds were more active than heavier birds but only in one of the two ***: Given our results, it is unlikely that Red-eyed Vireos refuel along the Yucatan coast. However, they still likely need to recuperate from crossing the Gulf of Mexico, which may necessitate foraging more often if in poor body condition. If the birds then move inland then stopover should be thought of as a large-scale phenomenon, where habitats with different functions may be spread out over a broad landscape.

Phosphorus(P) limitation in the coming decades calls for the utilization of alternative fertilizers in agriculture. Struvite is a promising P source, but its potential role as a fertilizer is dependent on different physical, chemical, and biological properties, which are very heterogeneous in soil, complicating the prediction of the best soil conditions for its application. Here, we evaluated the solubility of struvite in soil, its redistribution into P fractions, and its potential abiotic and biotic drivers in 62 globally distributed soils with contrasting properties through an incubation assay. We found that after 40 d, about 35% of struvite P was redistributed into soil fractions more accessible to plants and microbes. Phosphorus redistribution from struvite was driven by a complex suite of soil physical, chemical, and microbial properties as well as environmental factors that varied across soils. Soil texture played a critical role in determining the redistribution of P in struvite-amended soils in soluble(H2O extraction), labile(NaHCO3 extraction), and moderately labile(NaOH extraction) *** addition, the soil solution cation concentration was one of the most important drivers of available struvite-derived P fractions. The great importance of texture and cations in determining struvite-derived P fractions in soil was contrasted with the relatively minor role of pH. At the microbial level, the number of bacterial operational taxonomic units(OTUs) from the unfertilized soils that correlated with struvite-derived P fractions was higher than that of fungi. The number of OTUs that correlated with the struvite-derived soluble P fraction was dominated by fungi, whereas the number of OTUs that correlated with the struvite-derived labile P fraction was dominated by bacteria. Overall, this study provided a predictive framework for the potential use of struvite as a P fertilizer in contrasting soils.

Illness beliefs about hypertension among non-patients and healthy relatives of patients

Objectives: Personal beliefs about illnesses have received increasing interest because these cognitions help to explain and predict preventive and therapeutic coping efforts, adjustment to a disease and health outcomes. We sought to explore and compare non-specialised illness representations of hypertension among adults never suffering from hypertension who had and had not lived with hypertensive patients. Design: Hypertension representations were explored in a community-based, convenient sample of normotensive Spanish adults of both genders from different educational backgrounds and with different family experience with this illness. Method: An adapted Illness Perception Questionnaire-R was used to assess such perceptions among healthy people in nine dimensions: Identity, Time-line, Consequences, Personal Control, Treatment Control, Illness Coherence, Evolution, Emotional Representations and Causes. Results: The participants’ beliefs mixed accurate and folk knowledge, while gender, age and education level had little impact, family experience (having or not having a relative with hypertension) strongly determined the content of hypertension representations. Participants with family experience held significantly stronger beliefs of controllability of the disease, both by patients and treatments, considered the disease as less stable and reported a lower emotional impact when thinking on suffering from hypertension. Family experience was the only significant predictor of illness cognitions. Conclusions: This study allowed us to know the perceptions of hypertension among non-patients and healthy relatives of patients. Our findings are useful in designing interventions aimed at hypertension prevention, particularly considering family experience with the disease.

Background:Autoimmune pancreatitis(AIP)is a well-recognized fibroinflammatory disease of the *** the significant number of studies published on AIP type 1 and 2,no studies have been focused on AIP type not otherwise specified(NOS)and therefore very little is known about clinical features and long-term outcomes of these *** aim of this study was to investigate clinical and radiological features of AIP type ***:Patients classified as AIP type NOS at clinical onset included in our database prospectively maintained since 1995 were ***,clinical data were collected and ***:Forty-six patients were included in the *** clinical onset was mainly characterized by weight loss,jaundice and acute *** patients(17.4%)were reclassified as AIP type 2 during follow-up because of the development of ulcerative *** patients(15.2%)experienced relapse after steroid treatment but only one(2.2%)needed immunosuppressive drugs because of recurrent ***:AIP type NOS shares clinical features similar to AIP type 2 and a relevant proportion of patients was reclassified as AIP type 2 during follow-up because of the development of ulcerative *** risk of relapse is low but not irrelevant.

Pre-illness changes in dietary habits and diet as a risk factor for inflammatory bowel disease: A case-control study

AIM: To evaluate whether symptoms of inflammatory bowel disease (IBD), before diagnosis modify dietary habits, and to investigate the pre-illness diet in patients with recent IBD in comparison with an age-matched healthy control group. METHODS: Overall, 83 new cases of IBD (41 ulcerative colitis, 42 Crohn's disease) and 160 healthy controls were studied. Portions per week of 34 foods and beverages before onset of symptoms were recorded using a validated questionnaire. Duration of symptoms before IBD diagnosis, presence of specific symptoms and their impact on subjective changes in usual dietary habits were also recorded. The association between diet and IBD was investigated by multiple logistic regression and dietary patterns were assessed by factor analysis. RESULTS: Changes in dietary habits, due to the presence of symptoms, were reported by 38.6% of patients and were not significantly related to specific symptoms, rather to long duration of symptoms, only in Crohn's disease patients. In IBD patients who did not change dietary habits, moderate and high consumption of margarine (OR = 11.8 and OR = 21.37) was associated with ulcerative colitis, whilst high consumption of red meat (OR = 7.8) and high intake of cheese were associated with Crohn's disease. CONCLUSION: More than one third of IBD patients change dietary habits before diagnosis. margarine, red meat and cheese increase the risk of ulcerative colitis and Crohn's disease.
