Diagnostic accuracy and cost-effectiveness of the CAR-OLT score in predicting cardiac risk for liver transplantation

BACKGROUND The CAR-OLT score predicts major adverse cardiovascular events 1 year after liver transplant(LT).AIM To test the hypothesis that the CAR-OLT score may help avoid cardiac stress tests in LT *** This retrospective single-center cohort study included all adult patients undergoing elective evaluation for first cadaveric donor orthotopic LT for liver cirrhosis with or without hepatocellular carcinoma at Fondazione Policlinico Universitario Agostino Gemelli Istituto di Ricerca e Cura a Carattere Scientifico in Rome,*** contraindications for LT listing were defined after a center-specific cardiac workup,which included cardiac stress tests for most *** diagnostic accuracy of the CAR-OLT score was evaluated using the area under the receiver operating characteristic(AUROC)*** A total of 342 LT candidates were evaluated between 2015 and 2019,with a moderate cardiovascular risk profile(37%diabetes,34%hypertension,22%obesity).Of these,80(23%)candidates underwent coronary ***-one(6%)candidates were given cardiac contraindications to LT listing,48%of which were due to coronary artery *** CAR-OLT score predicted cardiac contraindications to LT listing with an AUROC of *** optimal cut-off for sensitivity was a CAR-OLT score≤23,which showed a 99%negative predictive value for cardiac contraindications to LT listing.A total of 84(25%)LT candidates with a CAR-OLT score≤23 underwent 87 non-invasive cardiac tests and 13 coronary angiographies pre-listing,with estimated costs of approximately 48000€.The estimated savings per patient was€574.70 for the Italian National Health *** A CAR-OLT score≤23 can identify LT candidates who can be safely listed without the need for cardiac stress tests,providing time and cost *** findings require external validation.

Could metabolic syndrome lead to hepatocarcinoma via non-alcoholic fatty liver disease?

It was estimated that from 2002 to 2008 the risk of developing cancer increased a quarter-fold in men and two-fold in women due to excessive BMI. Obesity, metabolic syndrome and type 2 diabetes mellitus are strictly related and are key pathogenetic factors of non-alcoholic fatty liver disease (NAFLD), the most frequent liver disease worldwide. The most important consequence of the “metabolic epidemics” is the probable rise in the incidence of hepatocarcinoma (HCC), and NAFLD is the major causative factor. Adipose tissue is not merely a storage organ where lipids are preserved as an energy source. It is an active organ with important endocrine, paracrine, and autocrine actions in addition to immune functions. Adipocytes produce a wide range of hormones, cytokines, and growth factors that can act locally in the adipose tissue microenvironment and systemically. In this article, the main roles of insulin growth factor (IGF)-1 and IGF-2 are discussed. The role of IGF-2 is not only confined to HCC, but it may also act in early hepato-carcinogenesis, as pre-neoplastic lesions express IGF-2 mRNA. IGF-1 and IGF-2 interact with specific receptors (IGF-1R and IGF-2R). IGF-1R is over-expressed in in vitro and in animal models of HCC and it was demonstrated that IGF ligands exerted their effects on HCC cells through IGF-1R and that it was involved in the degeneration of pre-neoplastic lesions via an increase in their mitotic activity. Both IGF-2R and TGF β, a growth inhibitor, levels are reduced in human HCC compared with adjacent normal liver tissues. Another key mechanism involves peroxisome proliferator-activated receptor (PPAR)γ. In in vitro studies, PPARγ inhibited various carcinomas including HCC, most probably by regulating apoptosis via the p21, p53 and p27 pathways. Finally, as a clinical consequence, to improve survival, efforts to achieve a “healthier diet” should be promoted by physicians and politicians.

Growing and aging of hematopoietic stem cells

In the hematopoietic system, a small number of stem cells produce a progeny ofseveral distinct lineages. During ontogeny, they arise in the aorta-gonadmesonephrosregion of the embryo and the placenta, afterwards colonise the liverand finally the bone marrow. After this fetal phase of rapid expansion, thenumber of hematopoietic stem cells continues to grow, in order to sustain theincreasing blood volume of the developing newborn, and eventually reaches asteady-state. The kinetics of this growth are mirrored by the rates of telomereshortening in leukocytes. During adulthood, hematopoietic stem cells undergo avery small number of cell divisions. Nonetheless, they are subjected to aging,eventually reducing their potential to produce differentiated progeny. The causalrelationships between telomere shortening, DNA damage, epigenetic changes,and aging have still to be elucidated.

The coexistence of fast and slow diffusion processes in the life cycle of Aedes aegypti mosquitoes

A new model that describes the life cycle of mosquitoes of the species Aedes aegypti,main carriers of vector-borne diseases,is *** novelty is to include in the model the coexistence of two independent diffusion processes,one fast which obeys the constitutive Fick’s law,the other slow which satisfies the Cattaneo evolution *** analysis of the corresponding ODE model shows the overall stability of the Mosquitoes-Free Equilibrium(MFE),together with the local stability of the other equilibrium point admitted by the *** wave type solutions have been investigated,providing an estimate of the minimal speed for which there are monotone waves that connect the homogeneous equilibria allowed by the system.A special section is dedicated to the analysis of the hyperbolic model obtained neglecting the fast diffusive *** particular case is suitable to describe the biological process as it overcomes the paradox of infinite speed propagation,typical of parabolic *** numerical simulations compare the existing models in the literature with those presented in this discussion,showing that although the generalized model is parabolic,the associated wave velocity admits upper bound represented by the speed of the waves linked to the classic parabolic model present in the published literature,so the presence of a slow flux together with a fast one slows down the speed with which a population spreads.

Notes for developing a molecular test for the full characterization of circulating tumor cells

The proved association between the circulating tumor cell （CTC） levels and the patients＇ survival parameters has been growing interest to investigate the molecular profile of these neoplastic cells among which hide out precursors capable of initiating a new distant metastatic lesion. The full characterization of the tumor cells in peripheral blood of cancer patients is expected to be of help for understanding and （prospectively） for counteracting the metastatic process. The major hitch that is hampering the successful gaining of this result is the lack of a consensus onto standard operating procedures （SOPs） for performing what we generally define as the ＂liquid biopsy＂. Here we review the more recent acquisitions in the analysis of CTCs and tumor related nucleic acids, looking to the main open questions that are hampering their definitive employ in the routine clinical practice.

We emphasize the importance of studying the primate brain in cognitive neuroscience and suggest a new mind-set in primate experimentation within the boundaries of animal welfare ***,we list the advantages of investigating both genes and neural mechanisms and processes in the emergence of behavioral and cognitive functions,and propose the establishment of an open field of primate *** latter may be conducted by implementing and harmonizing experimental practices with ethical guidelines that regulate(1)management of natural parks with free-moving populations of target nonhuman primates,(2)establishment of indoor-outdoor labs for both system genetics and neuroscience investigations,and(3)hotel space and technologies which remotely collect and dislocate information regarding primates geographically located elsewhere.

Protective potential of glucagon like peptide 2(GLP-2) against the neurodegeneration

Neurodegeneration consists in loss of neuron specific types, pattern and distribution, leading to progressive dysfunctions of the central nervous system. Neurodegenerative diseases include diverse pathological conditions, among which Alzheimer’s and Parkinson’s diseases are the most prevalent ones. Alzheimer’s disease is known as a growing dementia, characterized by progressive language, memory, and cognitive loss, while Parkinson’s disease is primarily characterized as a motor disorder. Senile plaques, caused by amyloid β peptide, hyperphosphorylated taubased neurofibrillary tangles and synapse loss, are the principal pathological hallmarks of Alzheimer’s disease.

Community batteries(CBs)are emerging to support and even enable energy communities and generally help consumers,especially space-constrained ones,to access potential techno-economic benefits from storage and support local grid ***,the economic viability of CB projects is often *** this regard,typical feasibility studies assess CB value for behind-the-meter(BTM)operation or whole-sale market participation,i.e.,front-of-meter(FOM).This work proposes a novel techno-economic operational framework that allows systematic assessment of the different options and introduces a two-meter architecture that co-optimizes both BTM and FOM benefits.A real CB project application in Australia is used to demonstrate the significant two-meter co-optimization opportunities that could enhance the business case of CB and energy communities by multi-service provision and value stacking.
