Carpobrotus edulis(L.) ***. extract as a skin preserving agent: From traditional medicine to scientific validation

Objective:Carpobrotus edulis(L.)*** a succulent perennial plant native to South Africa and grows invasively in the Mediterranean *** is commonly used for the treatment of various diseases,including skin wound healing and regeneration,for which experimental validation is *** therefore evaluated the skin healing properties by testing a *** aqueous leaf extract(CAE)on cell cultures and in enzymatic ***:Micro-morphological analysis of leaves was carried out using scanning electron microscopy and light *** features and antioxidant activity of CAE were evaluated by reversed-phase liquid chromatography coupled with diode array detection and electrospray ion trap mass spectrometry(RP-LC-DAD-ESI-MS),and in vitro cell-free *** activities were evaluated using keratinocytes and fibroblasts,as well as elastase,collagenase,and ***:CAE showed high carbohydrates(28.59%±0.68%),total phenols([101.9±6.0]g gallic acid equivalents/kg dry extract[DE]),and flavonoids([545.9±26.0]g rutin equivalents/kg DE).RP-LC-DAD-ESI-MS revealed the predominant presence of hydroxycinnamic acids(51.96%),followed by tannins(14.82%)and flavonols(11.32%).The extract was not cytotoxic,had a strong and dose-dependent antioxidant activity,and inhibited collagenase(>90%at 500 mg/m L)and hyaluronidase(100%at 1000 mg/m L).In cell culture experiments,CAE increased wound closure and collagen production,which was consistent with its high polyphenol ***:Our data support the use of the *** for skin care and the treatment of skin ***,use of *** for skin care purposes could be an eco-friendly solution to reduce its invasiveness in the environment.

Prevalence of functional dyspepsia and its subgroups in patients with eating disorders

AIM:To study the prevalence of functional dyspepsia(FD)(Rome Ⅲ criteria) across eating disorders(ED),obese patients,constitutional thinner and healthy ***:Twenty patients affected by anorexia nervosa,6 affected by bulimia nervosa,10 affected by ED not otherwise specified according to diagnostic and statistical manual of mental disorders,4th edition,nine constitutional thinner subjects and,thirtytwo obese patients were recruited from an outpatients clinic devoted to eating behavior *** healthy volunteers matched for age and gender were enrolled as healthy *** participants underwent a careful clinical *** and anthropometric characteristics were obtained from a structured *** presence of FD and,its subgroups,epigastric pain syndrome and postprandial distress syndrome(PDS) were diagnosed according to Rome Ⅲ *** intensity-frequency score of broader dyspeptic symptoms such as early satiety,epigastric fullness,epigastric pain,epigastric burning,epigastric pressure,belching,nausea and vomiting were studied by a standardized questionnaire(0-6).Analysis of variance and post-hoc Sheffè tests were used for ***:90% of patients affected by anorexia nervosa,83.3% of patients affected by bulimia nervosa,90% of patients affected by ED not otherwise specified,55.6% of constitutionally thin subjects and 18.2% healthy volunteers met the Postprandial Distress Syndrome Criteria(χ 2,P < 0.001).Only one bulimic patient met the epigastric pain syndrome *** fullness intensity-frequency score was significantly higher in anorexia nervosa,bulimia nervosa and ED not otherwise specified groups compared to the score calculated in the constitutional thinner group(4.15 ± 2.08 vs 1.44 ± 2.35,P = 0.003;5.00 ± 2.45vs 1.44 ± 2.35,P = 0.003;4.10 ± 2.23vs 1.44 ± 2.35,P = 0.002,respectively),the obese group(4.15 ± 2.08vs 0.00 ± 0.00,P < 0.001;5.00 ± 2.45vs 0.00 ± 0.00,P < 0.001;4.10 ± 2.23

Bloating and functional gastro-intestinal disorders: Where are we and where are we going?

Bloating is one of the most common and bothersome symptoms complained by a large proportion of patients. This symptom has been described with various definitions, such as sensation of a distended abdomen or an abdominal tension or even excessive gas in the abdomen, although bloating should probably be defined as the feeling (e.g. a subjective sensation) of increased pressure within the abdomen. It is usually associated with functional gastrointestinal disorders, like irritable bowel syndrome, but when bloating is not part of another functional bowel or gastrointestinal disorder it is included as an independent entity in Rome III criteria named functional bloating. In terms of diagnosis, major difficulties are due to the lack of measurable parameters to assess and grade this symptom. In addition, it is still unclear to what extent the individual patient complaint of subjective bloating correlates with the objective evidence of abdominal distension. In fact, despite its clinical, social and economic relevance, bloating lacks a clear pathophysiology explanation, and an effective management endorsement, turning this common symptom into a true challenge for both patients and clinicians. Different theories on bloating etiology call into questions an increased luminal contents (gas, stools, liquid or fat) and/or an impaired abdominal empting and/or an altered intra-abdominal volume displacement (abdomino-phrenic theory) and/or an increased perception of intestinal stimuli with a subsequent use of empirical treatments (diet modifications, antibiotics and/or probiotics, prokinetic drugs, antispasmodics, gas reducing agents and tricyclic antidepressants). In this review, our aim was to review the latest knowledge on bloating physiopathology and therapeutic options trying to shed lights on those processes where a clinician could intervene to modify disease course.

An analytical method for Fe(Ⅱ) and Fe(Ⅲ) determination in pharmaceutical grade iron sucrose complex and sodium ferric gluconate complex

A robust voltammetric method has been developed and validated for the determination of Fe（Ⅱ） and Fe（Ⅲ） in pharmaceutical iron polysaccharidic complexes. Undesirable low molecular weight iron complexes, at concentration about 3% in the pharmaceutical formulation, can be easily determined with good accuracy and precision. This methodology can be proposed as a viable, environmentally sustainable substitute for the conventional Normal Pulse Polarographic method in US Pharmacopeia, with better analytical figures of merit, and reduced Hg consumption. A deeper insight in Fe（Ⅱ） and Fe（Ⅲ） composition can be gained by the combined use of a new potentiometric technique after chemical decomposition of the complex.

Statins decrease the risk of acute pancreatitis after endoscopic ultrasound fine-needle aspiration of pancreatic cysts

Background: Basic and clinical studies suggest that statins may prevent and even ameliorate acute pan- creatitis. The present study was to evaluate whether statin decreases the risk of acute pancreatitis in patients undergoing endoscopic ultrasound-guided ne-needle aspiration of pancreatic cysts. Methods: Out of 456 patients with pancreatic cysts referred to our center between 2006 and 2018, 365 were nally included in analyses: 86 were treated with statins and 279 were not at the time of endo- scopic ultrasound ne-needle aspiration. We compared the acute pancreatitis incidence between the two groups, and we also compared other complications such as bleeding and infections. Results: Median age was 64 years [interquartile range (IQR) 62 69] and median cyst size was 24mm (IQR, 21 29). The most frequent histology was intraductal papillary mucinous neoplasm (45.3% and 42.3% in the two groups, respectively;P =0.98). All 13 patients experiencing post-endoscopic ultrasound acute pancreatitis were from the control group (4.7%), of which 3 were classi ed as severe pancreatitis. None of statin users developed post-procedural acute pancreatitis (odds ratio: 0.15;95% con dence interval: 0.03 0.98;P=0.03). No difference was registered with regard to severe pancreatitis and other complications. Conclusions: Statins exert a bene cial role in preventing acute pancreatitis in patients with pancreatic cysts undergoing endoscopic ultrasound-guided ne-needle aspiration. If con rmed in prospective trials, our ndings may pave the way to an extensive use of statins as prophylactic agents in pancreatic inter- ventional endoscopy.

Human bocavirus in children hospitalized for acute respiratory tract infection in Rome

Background The role of human bocavirus (HBoV) as a respiratory pathogen has not been fulfilled *** aimed to describe clinical and serological characteristics of children with HBoV hospitalized for acute respiratory tract infection and to evaluate whether differences occur between HBoV alone and in *** We retrospectively reviewed data from 60 children (median age of 6.2 months,range 0.6-70.9) hospitalized for acute respiratory symptoms,with HBoV detected from a respiratory sample,using a reverse transcriptase-PCR for 14 respiratory viruses (including respiratory syncytial virus (RSV),influenza virus A and B,human coronavirus OC43,229E,NL-63 and HUK1,adenovirus,rhinovirus,parainfluenza virus 1-3,and human metapneumovirus).Results HBoV was detected alone in 29 (48.3%) patients,while in co-infection with other viruses in 31 patients (51.7%),with a peak between December and *** the 60 patients,34 were bronchiolitis,19 wheezing,3 pneumonia,2 upper respiratory tract infection,and 2 whooping *** children (11.6%) required admission to the paediatric intensive care unit (PICU) for respiratory *** differences was observed in age,family history for atopy and/or asthma,clinical presentations,chest X-ray,or laboratory findings in children with HBoV alone *** viral *** was the most frequently co-detected virus (61.3%).When compared with HBoV detection alone,the co-detection of RSV and HBoV was associated with male sex (P=0.013),younger age (P=0.01),and lower blood neutrophil count (P=0.032).Conclusions HBoV can be detected alone and in co-infection respiratory samples of children with an acute respiratory tract infection.A cause-effect relationship between HBoV and respiratory infection is not clear,so further studies are needed to clarify this point.

Efficacy of current guidelines for the treatment of spontaneous bacterial peritonitis in the clinical practice

AIM： To verify the validity of the International Ascites Club guidelines for treatment of spontaneous bacterial peritonitis （SBP） in clinical practice. METHODS： All SBP episodes occurring in a group of consecutive cirrhotics were managed accordingly and included in the study. SBP was diagnosed when the ascitic fluid polymorphonuclear （PIN） cell count was 〉 250 cells/mm^3, and empirically treated with cefotaxime. RESULTS： Thirty-eight SBP episodes occurred in 32 cirrhotics （22 men/20 women; mean age： 58.6 ＋ 22.2 years）. Prevalence of SBP, in our population, was 27%. Ascitic fluid culture was positive in nine （24%） cases only. Eleven episodes were nosocomial and 71% community-acquired. Treatment with cefotaxime was successful in 59% of cases, while 41% of episodes required a modification of the initial antibiotic therapy because of a less-than 25% decrease in ascitic PMN count at 48 h. Change of antibiotic therapy led to the resolution of infection in 87% of episodes. Among the cases with positive culture, the initial antibiotic therapy with cefotaxime failed at a percentage （44%） similar to that of the whole series. In these cases, the isolated organisms were either resistant or with an inherent insufficient susceptibility to cefotaxime. CONCLUSION： In clinical practice, ascitic PMN count is a valid tool for starting a prompt antibiotic treatment andevaluating its efficacy. The initial treatment with cefotaxime failed more frequently than expected. An increase in healthcare-related infections with antibiotic-resistant pathogens may explain this finding. A different first-line antibiotic treatment should be investigated.

Accuracy of the automated cell counters for management of spontaneous bacterial peritonitis

AIM： To evaluate the accuracy of automated blood cell counters for ascitic polymorphonuclear （PMN） determination for：（1） diagnosis, （2） efficacy of the ongoing antibiotic therapy, and （3） resolution of spontaneous bacterial peritonitis （SBP）. METHODS： One hundred and twelve ascitic fluid samples were collected from 52 consecutive cirrhotic patients, 16 of them with SBP. The agreement between the manual and the automated method for PMN count was assessed. The sensitivity/specificity and the positive/negative predictive value of the automated blood cell counter were also calculated by considering the manual method as the ＂gold standard＂ RESULTS： The mean ＋ SD of the difference between manual and automated measurements was 7.8 4- 58 cells/ram3, while the limits of agreement were ＋124 cells/mm3 [95% confidence interval （CI）：＋145 to ＋103] and -108 cells/mm3 （95% CI： -87 to -129）. The automated cell counter had a sensitivity of 100% and a specificity of 97.7% in diagnosing SBP, and a sensitivity of 91% and a specificity of 100% for the efficacy of the ongoing antibiotic therapy. The two methods showed a complete agreement for the resolution of infection. CONCLUSION： Automated cell counters not only have a good diagnostic accuracy, but are also very effectivein monitoring the antibiotic treatment in patients with SBP. Because of their quicker performance, they should replace the manual counting for PMN determination in the ascitic fluid of patients with SBP.
