Endoscopic findings in patients with Schatzki rings:Evidence for an association with eosinophilic esophagitis

AIM:To investigate endoscopic findings in patients with Schatzki rings(SRs) with a focus on evidence for eosinophilic esophagitis(EoE).METHODS:We consecutively approached all adult patients scheduled for elective outpatient upper endoscopy for a variety of indications at the German Diagnostic Clinic,Wiesbaden,Germany between July 2007 and July *** patients with endoscopically diagnosed SRs,defined as thin,symmetrical,mucosal structures located at the esophagogastric junction,were prospectively *** endoscopic findings,clinical information and histopathological findings with a focus on esophageal eosinophilia(≥ 20 eosinophils/high power field) were *** criteria for active EoE were defined as:(1) eosinophilic tissue infiltration ≥ 20 eosinophils/hpf;(2) symptoms of esophageal dysfunction;and(3) exclusion of other causes of esophageal *** reflux disease was excluded by proton pump inhibitor treatment prior to *** presence of ≥ 20 eosinophils/hpf in esophageal biopsies in patients that did not fulfil the criteria of EoE was defined as esophageal ***:A SR was diagnosed in 171(3.3%;128 males,43 females,mean age 66 ± 12.9 years) of the 5163 patients that underwent upper *** of the 116 patients(17%) from whom esophageal biopsies were obtained showed histological hypereosinophilia(≥ 20 eosinophils/hpf).Nine of these patients(8 males,1 female,mean age 49 ± 10 years) did not fulfill all diagnostic criteria of EoE,whereas in 11(9%) patients with ≥ 20 eosinophils/hpf,a definite diagnosis of EoE was *** of the 11 patients(27%) with definite EoE had no suspicious endoscopic features of *** contrast,in the 25 patients in whom EoE was suspected by endoscopic features,EoE was only confirmed in 7(28%) *** with EoE were younger(mean age 41.5 ± 6.5 vs 50.5 ± 11.5 years,P = 0.012),were more likely to have a history of allergies(73% vs 29%,P = 0.007

Automatic object annotation in streamed and remotely explored large 3D reconstructions

We introduce a novel framework for 3 D scene reconstruction with simultaneous object annotation,using a pre-trained 2 D convolutional neural network(CNN),incremental data streaming,and remote exploration,with a virtual reality *** enables versatile integration of any 2 D box detection or segmentation *** integrate new approaches to(i)asynchronously perform dense 3 D-reconstruction and object annotation at interactive frame rates,(ii)efficiently optimize CNN results in terms of object prediction and spatial accuracy,and(iii)generate computationally-efficient colliders in large triangulated3 D-reconstructions at run-time for 3 D scene *** method is novel in combining CNNs with long and varying inference time with live 3 D-reconstruction from RGB-D camera *** further propose a lightweight data structure to store the 3 D-reconstruction data and object annotations to enable fast incremental data transmission for real-time exploration with a remote client,which has not been presented *** framework achieves update rates of 22 fps(SSD Mobile Net)and 19 fps(Mask RCNN)for indoor environments up to 800 m^(3).We evaluated the accuracy of 3 D-object *** work provides a versatile foundation for semantic scene understanding of large streamed3 D-reconstructions,while being independent from the CNN’s processing *** code is available for non-commercial use.

Large-scale spatial synchrony and the stability of forest biodiversity revisited

Aims Mechanisms contributing to species coexistence have at least one of two modes of action:(i)stabilization of populations through restoring forces and(ii)equalization of fitness across individuals of different ***,ecologists have begun gleaning the relative roles of these by testing the predictions of neutral theory,which predicts the properties of communities under pure fitness *** null hypothesis was rejected for forests of southern Ontario based on large-scale(;100 km)spatial synchrony evident in the fossil pollen record over the entire Holocene,and the argument that a species’relative abundance would instead vary independently at such distances in the absence of stabilizing *** test of neutral theory was criticized based on the idea that the synchrony might be produced by dispersal ***,I revisit this test of neutral theory by explicitly calculating the synchrony expected in these forests using a novel simulation method enabling examination of the distribution of a species over large spatial and temporal *** A novel neutral simulation algorithm tracking only the focal species was used to calculate the neutral expectation for spatial synchrony properties examined empirically by Clark and MacLachlan[(2003)Stability of forest *** 423:635–8]using fossil pollen data from eight lake *** coefficient of variation(CV)in a species’relative abundance across the eight sites(initiated at about 10%with a small CV)was calculated for 10 runs over a 10000 year time *** CV reflects the level of spatial synchrony in that less synchronous dynamics should lead to more variation across space(a higher equilibrium CV),and in particular,a greater increase in the CV over time from a small initial value.A‘two dimensional t’fat-tailed dispersal kernel was assumed with parameters set to the median derived from seed trap data for deciduous wind-dispersed *** of results to assumed dispersal

To encounter a fellow-being does not mean only to see her face, to notice the color of her eyes, but to meet her eyes and to be addressed by her. Who one is irreducible to any objective property or value, and likewise cannot be comprehended through propositional statements in the manner of ＂talking about ...＂ something. Rather, such comprehension demands an account of giving way to the appearance of the other as other. This account, prominently linked to E. Levinas＇＂ethics as first philosophy,＂ has also been developed as phenomenological personalism. While Max Scheler developed his ＂Ethical Personalism＂ within his material Value Ethics, his Philosophy of Fellow-Feeling and in his late Philosophical Anthropology of human spirit, Paul Ricoeur develops his personalism through different approaches： from his early attempts on need and desire as the affective basis of our values to the perspective on the particular way we lead our lives in narratively constituting our personal identity, and finally to his concept of recognition. Reconstructing personalism as a philosophy of discovering the other person in her otherness and as a concept of social practice are the aims of this paper.

Meta-analysis quantifying the potential of dietary additives and rumen modifiers for methane mitigation in ruminant production systems

Increasingly countries are seeking to reduce emission of greenhouse gases from the agricultural industries,and livestock production in particular,as part of their climate change *** many reviews update progress in mitigation research,a quantitative assessment of the efficacy and performance-consequences of nutritional strategies to mitigate enteric methane(CH_(4))emissions from ruminants has been lacking.A meta-analysis was conducted based on 108 refereed papers from recent animal studies(2000-2020)to report effects on CH4 production,CH_(4) yield and CH_(4) emission intensity from 8 dietary *** interventions(oils,microalgae,nitrate,ionophores,protozoal control,phytochemicals,essential oils and 3-nitrooxypropanol).Of these,macroalgae and 3-nitrooxypropanol showed greatest efficacy in reducing CH_(4) yield(g CH_(4)/kg of dry matter intake)at the doses *** confidence intervals derived for the mitigation efficacies could be applied to estimate the potential to reduce national livestock emissions through the implementation of these dietary interventions.

Using weighted blankets in an inpatient mental health hospital to decrease anxiety

Objective:Patients who are involuntarily committed to a psychiatric facility often experience anxiety or increased anxiety in response to being placed in the institutional *** weighted blanket introduced a proactive treatment *** purpose of this study was to evaluate patients’anxiety symptoms before and after weighted blanket,compared to a group that did not use a weighted blanket to control ***:This study was conducted in an inpatient mental health facility from June 10,2019,through November 7,2019,with psychiatric patients who were not actively *** were offered the choice of weighted or unweighted blankets for a 20-minute *** treatment group was comprised of individuals who had opted to use a 14-pound weighted blanket,20-pound weighted blanket or 5-pound weighted lap *** in the comparison group were active in a wider range of *** application of the blankets,pulse rate was measured using a pulse oximeter,and anxiety was measured using the Spielberger State-Trait Anxiety Inventory shortened form(STAI:Y-6).Both measures were taken again after the intervention.A two-way mixed analysis of variance(ANOVA)was run to examine the interaction effects between time(pre/post)and group(comparison/weighted blanket).Simple main effects were then further examined for the comparison/weighted blanket groups using a repeated measures *** the weighted blanket group,additional two-way mixed ANOVA was run to determine if gender or blanket weight made a statistically significant ***:There was a statistically significant difference(P<0.05)among those who used weighted blankets(n=61)and those who did not(n=61)based on the pre/post data for both the STAI:Y-6 inventory and the patients’pulse *** results of two-way ANOVA indicated a significant interaction effect between intervention time and group(P<0.001).Repeated measures ANOVA indicated a change between pre/post for the we
