Comparison of Cardiovascular Parameters and Internal Training Load of Different 1-h Training Sessions in Non-elite CrossFit^(®)Athletes

Purpose The fact that CrossFit^(®)is the best-known and rapidly growing concept for high-intensity interval training(HIIT)and high-intensity functional training(HIFT)results in a continuous increase of athletes performing CrossFit^(®).In the more than 15,000 CrossFit^(®)Affiliates worldwide,the training concept is usually offered in 1-h training sessions containing the CrossFit^(®)-related workout of the day(WOD),as well as a general warm-up,movement demonstrations,and skill ***,we report how physiological parameters measured by heart rate(HR)values vary during four different 1-h CrossFit^(®)training sessions of non-elite athletes(n=27)in a local affiliated training center and what influencing factors may *** The duration of the 1-h training sessions were divided into a warm-up part(WU-part),a skill development part combined with strength exercises(A-part),followed by the WOD part(B-part).Results Analysis of HR values shows high training intensity(≥91%HR_(max))not throughout the duration of each training session,only during *** mean HR values in B-part differ significantly compared to the remaining training parts(P<0.001)for all four training *** of different CrossFit^(®)experience levels revealed no significant differ-ence in acute physiological demands and training load between beginner and experienced CrossFit^(®)*** Our results may suggest that practicing CrossFit^(®)in 1-h training sessions combined anaerobic and aerobic exercise intensities,with the training concept allows beginners and experienced athletes to be trained with the same cardio-vascular responses and training intensities.

Diagnostic value of diffusion-weighted MRI for imaging synovitis in pediatric patients with inflammatory conditions of the knee joint

Background Diffusion-weighted imaging(DWI)of synovitis has been suggested as a possible non-invasive alternative to contrast-enhanced T1w imaging(ce-T1w).We aimed to study DWI for diagnosing synovitis in the knee joint of pediatric patients,to quantify inter-observer agreement on DWI and ce-T1w and to calculate quantitative measures of synovial diffusivity and *** Forty consecutive patients with known or suspected arthritis of the knee(25 girls,median age 12 years)underwent routine 1.5T MRI with ce-T1w and transverse DWI with b values 50 and 800 s/*** apparent diffusion coefficient(ADC)values and signal intensity of inflamed synovium,joint effusion and muscle were measured with regions of interest ***-contrast T1 w images(diagnostic standard)and diffusion-weighted images at b=800 s/mm2 with ADC map were separately rated by three independent and blinded readers with different levels of expertise for the presence and degree of synovitis along with the level of diagnostic *** Thirty-one(78%)patients showed at least some synovial contrast enhancement,17(43%)children were diagnosed with synovitis on *** by the 1st reader on ce-T1w and on DWI for synovitis showed very good agreement(kappa=0.90).Inter-observer agreement on DWI ranged from moderate to substantial with kappa values between 0.68 and 0.79(all P<0.001).Agreement and diagnostic confidence were generally lower in patients with mild and without synovial enhancement,compared to patients with *** yielded higher signal of inflamed synovium *** tissue,but lower signal *** effusion,compared to ce-T1 w(all P<0.001).Conclusions Diffusion-weighted imaging is a promising,though reader-dependent alternative to contrast-enhanced imaging in patients with arthritis of the knee,based on our preliminary *** holds potential for increasing patient safety and comfort.

Climate and Forecast Mode Simulations for Antarctica:Implications for Temperature and Wind

Climate and forecast mode simulations with the regional climate model HIRlam-ECHAM（HIRHAM） are evaluated over a pan-Antarctic domain. The ability of the model to simulate temperature and wind profiles in the troposphere is quantified by comparing its results with radiosonde data acquired from the Davis station for January and July 2007. Compared to the climate mode, the forecast mode was found to deliver improved results for temperature and wind simulations at the near surface and in the lower troposphere. The main remaining model bias found was the under-representation of low-level wind jets. Based on ensemble simulations, it is shown that a distinct internal variability is inherent in the climate mode simulations, and associated areas of reduced predictability over Antarctica are identified.

Effects of Saccharomyces cerevisiae or boulardii yeasts on acute stress induced intestinal dysmotility

AIM To investigate the capacity of Saccharomyces cerevisiae(S. cerevisiae) and Saccharomyces boulardii(S. boulardii) yeasts to reverse or to treat acute stress-related intestinal *** Adult Swiss Webster mice were stressed for 1 h in a wire-mesh restraint to induce symptoms of intestinal dysmotility and were subsequently killed by cervical dislocation. Jejunal and colon tissue were excised and placed within a tissue perfusion bath in which S. cerevisiae, S. boulardii, or their supernatants were administered into the lumen. Video recordings of contractility and gut diameter changes were converted to spatiotemporal maps and the velocity, frequency, and amplitude of propagating contractile clusters(PCC) were measured. Motility pre- and post-treatment was compared between stressed animals and unstressed controls. RESULTS S. boulardii and S. cerevisiae helped to mediate the effects of stress on the small and large intestine. Restraint stress reduced jejunal transit velocity(mm/s)from 2.635 ± 0.316 to 1.644 ± 0.238, P < 0.001 and jejunal transit frequency(Hz) from 0.032 ± 0.008 to 0.016 ± 0.005, P < 0.001. Restraint stress increased colonic transit velocity(mm/s) from 0.864 ± 0.183 to 1.432 ± 0.329, P < 0.001 and frequency to a lesser degree. Luminal application of S. boulardii helped to restore jejunal and colonic velocity towards the unstressed controls; 1.833 ± 0.688 to 2.627 ± 0.664, P < 0.001 and 1.516 ± 0.263 to 1.036 ± 0.21, P < 0.001, respectively. S. cerevisiae also had therapeutic effects on the stressed gut, but was most apparent in the jejunum. S. cerevisiae increased PCC velocity in the stressed jejunum from 1.763 ± 0.397 to 2.017 ± 0.48, P = 0.0031 and PCC frequency from 0.016 ± 0.009 to 0.027 ± 0.007, P < 0.001. S. cerevisiae decreased colon PCC velocity from 1.647 ± 0.187 to 1.038 ± 0.222, P < 0.001. The cycloaddition of epoxides and carbon dioxide represents a straightforward and atom-efficient method for synthesis of cyclic carbonates and utilization of CO2. So far, homogeneous metal complexes have been mainly applied for such transformations. Here, we describe the synthesis of novel heterogeneous Zn-based catalysts, which were conveniently prepared by pyrolysis of an active- carbon-supported phenanthroline-ligated Zn(OAc)2 complex. Detail structural characterizations proved the existence of single zinc sites in the active material. Compared to a Zn-based nanoparticle (Zn-NP) catalyst, the resulting single metal atom catalyst (SAC) displayed improved activity and stability for the cycloaddition of epoxides. By applying the optimal catalyst, a variety of carbonates were successfully obtained in high yields with good functional group tolerance.

An ordered nanostructure formed by epitaxial crystallization of a semicrystalline block copolymer on a substrate has been used as a patterned template for the selective deposition of thermally evaporated gold nanoparticles, resulting in the formation of structure-guiding host nanocomposites in which the ordered distribution of the guest particles is guided by the ordering of the host nanostructured block copolymer matrix. This opens new perspectives in the field of polymeric composites related to the maximum enhancement of effective physical properties and to the numerous possible applications that arise due to the presence of long-range order in the spatial distribution of functional nanoparticles.

Long-term prognosis in patients with severe late radiation enteropathy:A prospective cohort study

AIM: To assess persistent symptoms and mortality in a cohort of patients with severe (grade 3-4) radiation enteropathy,59 patients were followed up after 15-18 years. METHODS: Fifty-nine patients were prospectively enrolled by twelve surgical departments. Primary malignant disease,radiation therapy and surgical management were recorded at inclusion. The cause of death or persistence of symptoms was examined in public death records or by interview of survivors. RESULTS: Thirty-nine patients had received radiation therapy for gynaecological cancers,twelve for urological cancers,four for gastrointestinal cancers and four for other malignancies. Forty-five patients (76%) required surgical intervention. Complications occurred in 11 (25%) operated patients. Forty-seven patients had died at the time of follow-up,seven (12%) died as a direct result of radiation enteropathy,while radiation enteropathy contributed to death in an additional seven patients. Four of the twelve surviving patients suffered from chronic debilitating symptoms of radiation enteropathy,while three had moderate symptoms. CONCLUSION: Patients with severe delayed radiation enteropathy have a high risk of persistence of symptoms after surgery. At least one in ten patients dies from radiation-induced bowel injury.

Klatskin tumor treated by inter-disciplinary therapies including stereotactic radiotherapy: A case report

In view of the poor prognosis of patients with cholangiocarcinoma （CCC）, there is a need for new therapeutic strategies. Inter-disciplinary therapy seems to be most promising. Radiotherapy is an effective alternative to surgery for hilar CCC （Klatskin tumors） if an adequate radiation dose can be delivered to the liver hilus. Here, we describe a patient for whom we used a stereotactic radiotherapy technique in the context of an inter-disdplinary treatment concept. We report a 45-year-old patient with a locally advanced Klatskin tumor. Explorative laparotomy showed that the tumor was not resectable. A metallic stent was implanted and the patient was treated by stereotactic radiotherapy using a body frame. A total dose of 48 Gy （3×4 Gy/wk） was administered. Therapy was well tolerated. After 32 too, local tumor recurrence and a chest wall metastasis developed and were controlled by radio-chemotherapy. After more than 56 mo with a good quality of life, the patient died of advanced neoplastic disease. Stereotactic radiotherapy led to a long-term survival of this patient with a locally advanced Klatskin tumor. In the context of inter-disciplinary treatment concepts, this radiotherapy technique is a promising choice of treatment for patients with hilar CCC.
