Objective:This study aimed to compare the historical incidence rate of severe oral mucositis(OM)in head and neck cancer patients undergoing definitive concurrent chemoradiation therapy(CRT)versus a prospective cohort of patients with locally advanced head and neck squamous cell carcinoma(HNSCC)treated with prophylactic photobiomodulation therapy(PBMT).Methods:This US-based,institutional,single-arm,phaseⅡprospective clinical trial was initiated in 50 patients(age≥18 years,Karnofsky Performance Scale Index>60,with locally advanced HNSCC(excluding oral cavity)receiving definitive or adjuvant radiation therapy(RT)with concurrent platinum-based chemotherapy(CT).PBMT was delivered three times per week throughout RT utilizing both an intraoral as well extraoral delivery *** outcome measure was incidence of severe OM utilizing both the National Cancer Institute Common Toxicity Criteria,version 4.0(NCI-CTCAe)Grade≥3 and the World Health Organization Mucositis Grading Scale(WHO)Grade≥3 versus historical controls;secondary outcome measures included time to onset of severe OM following therapy ***:At baseline,all patients included in final analysis(N=47)had OM Grade *** RT and CT dose was(66.3±5.1)Gy and(486.1±106.8)mg/m ^(2),*** OM was observed in 11 of 47 patients(23%,confidence interval:12,38).OM toxicity grade trended upward during treatment,reaching a maximum at 7 weeks(WHO:1.8 ***-CTCAe:1.7).Subsequently,OM grade returned to baseline 3 months following completion of *** mean time to onset of severe OM was(35±12)*** mean time to resolution of severe OM was(37±37)***:Compared to historical outcomes,PBMT aides in decreasing severe OM in patients with locally advanced *** represents a minimally invasive,prophylactic intervention to decrease OM as a major treatment-related side effect.

Klatskin tumor treated by inter-disciplinary therapies including stereotactic radiotherapy: A case report

In view of the poor prognosis of patients with cholangiocarcinoma （CCC）, there is a need for new therapeutic strategies. Inter-disciplinary therapy seems to be most promising. Radiotherapy is an effective alternative to surgery for hilar CCC （Klatskin tumors） if an adequate radiation dose can be delivered to the liver hilus. Here, we describe a patient for whom we used a stereotactic radiotherapy technique in the context of an inter-disdplinary treatment concept. We report a 45-year-old patient with a locally advanced Klatskin tumor. explorative laparotomy showed that the tumor was not resectable. A metallic stent was implanted and the patient was treated by stereotactic radiotherapy using a body frame. A total dose of 48 Gy （3×4 Gy/wk） was administered. Therapy was well tolerated. After 32 too, local tumor recurrence and a chest wall metastasis developed and were controlled by radio-chemotherapy. After more than 56 mo with a good quality of life, the patient died of advanced neoplastic disease. Stereotactic radiotherapy led to a long-term survival of this patient with a locally advanced Klatskin tumor. In the context of inter-disciplinary treatment concepts, this radiotherapy technique is a promising choice of treatment for patients with hilar CCC.

To encounter a fellow-being does not mean only to see her face, to notice the color of her eyes, but to meet her eyes and to be addressed by her. Who one is irreducible to any objective property or value, and likewise cannot be comprehended through propositional statements in the manner of ＂talking about ...＂ something. Rather, such comprehension demands an account of giving way to the appearance of the other as other. This account, prominently linked to e. Levinas＇＂ethics as first philosophy,＂ has also been developed as phenomenological personalism. While Max Scheler developed his ＂ethical Personalism＂ within his material Value ethics, his Philosophy of Fellow-Feeling and in his late Philosophical Anthropology of human spirit, Paul Ricoeur develops his personalism through different approaches： from his early attempts on need and desire as the affective basis of our values to the perspective on the particular way we lead our lives in narratively constituting our personal identity, and finally to his concept of recognition. Reconstructing personalism as a philosophy of discovering the other person in her otherness and as a concept of social practice are the aims of this paper.

Three striking and impactful extreme cold weather events successively occurred across east Asia and North America during the mid-winter of 2020/*** events open a new window to detect possible underlying physical *** analysis here indicates that the occurrences of the three events resulted from integrated effects of a concurrence of anomalous thermal conditions in three oceans and interactive Arctic-lower latitude atmospheric circulation processes,which were linked and influenced by one major sudden stratospheric warming(SSW).The North Atlantic warm blob initiated an increased poleward transient eddy heat flux,reducing the Barents-Kara seas sea ice over a warmed ocean and disrupting the stratospheric polar vortex(SPV)to induce the major *** Rossby wave trains excited by the North Atlantic warm blob and the tropical Pacific La Nina interacted with the Arctic tropospheric circulation anomalies or the tropospheric polar vortex to provide dynamic settings,steering cold polar air *** long memory of the retreated sea ice with the underlying warm ocean and the amplified tropospheric blocking highs from the midlatitudes to the Arctic intermittently fueled the increased transient eddy heat flux to sustain the SSW over a long time *** displaced or split SPV centers associated with the SSW played crucial roles in substantially intensifying the tropospheric circulation anomalies and moving the jet stream to the far south to cause cold air outbreaks to a rarely observed extreme *** results have significant implications for increasing prediction skill and improving policy decision making to enhance resilience in“One Health,One Future”.

Linking the effect of localised pitting corrosion with mechanical integrity of a rare earth magnesium alloy for implant use

This study presents a computational framework that investigates the effect of localised surface-based corrosion on the mechanical performance of a magnesium-based alloy.A finite element-based phenomenological corrosion model was used to generate a wide range of corrosion profiles,with subsequent uniaxial tensile test simulations to predict the mechanical response to *** python-based detection framework PitScan provides detailed quantification of the spatial phenomenological features of corrosion,including a full geometric tracking of corroding *** this approach,this study is the first to quantitatively demonstrate that a surface-based non-uniform corrosion model can capture both the geometrical and mechanical features of a magne-sium alloy undergoing corrosion by comparing to experimental *** this verified corrosion modelling approach,a wide range of corrosion scenarios was evaluated and enabled quantitative relationships to be established between the mechanical integrity and key phenomenological corrosion *** particular,we demonstrated that the minimal cross-sectional area parameter was the strongest predictor of the remaining mechanical strength(R2=0.98),with this relationship being independent of the severity or spatial features of localised surface ***,our analysis demonstrated that parameters described in ASTM G46-94 showed weaker correlations to the mechanical integrity of corroding specimens,compared to parameters determined by *** study establishes new mechanistic insight into the performance of the magnesium-based materials undergoing corrosion.

Recent results emphasize the supportive effects of adipose-derived multipotent stem/progenitor cells(ADSPCs)in peripheral nerve *** under hypoxia is considered to enhance the release of the regenerative potential of *** study aimed to examine whether peripheral nerve regeneration in a rat model of autologous sciatic nerve graft benefits from an additional custom-made fibrin conduit seeded with hypoxic pre-conditioned(2%oxygen for 72 hours)autologous ADSPCs(n=9).This treatment mode was compared with three others:fibrin conduit seeded with ADSPCs cultivated under normoxic conditions(n=9);non-cell-carrying conduit(n=9);and nerve autograft only(n=9).A 16-week follow-up included functional testing(sciatic functional index and static sciatic index)as well as postmortem muscle mass analyses and morphometric nerve evaluations(histology,g-ratio,axon density,and diameter).At 8 weeks,the hypoxic pre-conditioned group achieved significantly higher sciatic functional index/static sciatic index scores than the other three groups,indicating faster functional ***,histologic evaluation showed significantly increased axon outgrowth/branching,axon density,remyelination,and a reduced relative connective tissue *** pre-conditioned ADSPCs seeded in fibrin conduits are a promising adjunct to current nerve *** studies are needed to understand the underlying cellular mechanism and to investigate a potential application in clinical practice.

α-diversity describes species diversity at local *** Simpson’s and Shannon-Wiener indices are widely used to characterizeα-diversity based on species abundances within a fixed study site(e.g.,a quadrat or plot).Although such indices provide overall diversity estimates that can be analyzed,their values are not spatially continuous nor applicable in theory to any point within the study region,and thus they cannot be treated as spatial covariates for analyses of other ***,we extended the Simpson’s and Shannon-Wiener indices to create point estimates ofα-diversity for any location based on spatially explicit species occurrences within different bandwidths(i.e.,radii,with the location of interest as the center).For an arbitrary point in the study region,species occurrences within the circle plotting the bandwidth were weighted according to their distance from the center using a tri-cube kernel function,with occurrences closer to the center having greater weight than more distant *** novel kernel-basedα-diversity indices were tested using a tree dataset from a 400 m×400 m study region comprising a 200 m×200 m core region surrounded by a 100-m width buffer *** newly extendedα-diversity indices did not disagree qualitatively with the traditional indices,and the former were slightly lower than the latter byedium and large band *** present work demonstrates the feasibility of using kernel-basedα-diversity indices to estimate diversity at any location in the study region and allows them to be used as quantifiable spatial covariates or predictors for other dependent variables of interest in future ecological *** continuousα-diversity indices are useful to compare and monitor species trends in space and time,which is valuable for conservation practitioners.

eGFR and HeR2 expression in advanced biliary tract cancer

AIM:To analyze the pathogenetic role and potential clinical usefulness of the epidermal growth factor receptor(eGFR)and the human epidermal growth factor receptor 2(HeR2)in patients with advanced biliary tract cancer(BTC). MeTHODS:eGFR and HeR2 expression was studied in biopsy samples from 124 patients(51%women; median age 64.8 years),with advanced BTC diagnosed between 1997 and *** micrometers sections of paraffin embedded tissue were examined by standard, FDA approved *** with scores of 2+or 3+for HeR2 expression on immunochemistry were additionally tested for HeR2 gene amplification by fluorescence in situ hybridisation(FISH).ReSULTS:34/124 patients(27.4%)had gallbladder cancer,47(37.9%)had intrahepatic BTC and 43(34.7%)had extrahepatic or perihilar *** expression was examined in a subset of 56 samples. eGFR expression was absent in 22/56 tumors(39.3%). Of the remaining samples expression was scored as 1+in 12(21.5%),2+in 13(23.2%)and 3+in 9(16%), ***2 expression was as follows:score 0 73/124(58.8%),score 1+27/124(21.8%),score 2+ 21/124(17%)and score 3+4/124(3.2%).HeR2 gene amplification was present in 6/124,resulting in an overall amplification rate of 5%. CONCLUSION:Our data suggest that routine testing and therapeutic targeting of HeR2 does not seem to be useful in patients with BTC,while targeting eGFR may be promising.
