Background and Aims:There is an unmet need for new bi-omarkers to improve diagnostics and prognostics in primary biliary cholangitis(PBC)and primary sclerosing cholangitis(PSC).Inter-α-inhibitor heavy chain 4(ITIH4)is an abun-dant,liver-produced protein,and its synthesis may be al-tered in liver *** investigated whether ITIH4 plas-ma concentrations were affected in PBC and PSC ***:We developed an immunoassay specific for ITIH4 and determined ITIH4 plasma concentrations in 66 PBC,126 PSC,92 autoimmune hepatitis(AIH),67 chronic hepatitis C(CHC),33 alcoholic hepatitis(AH)patients and 138 healthy controls(HCs).Hepatic ITIH4 expression was investigated by immunohistochemistry in ***:The mean plas-ma concentration of ITIH4 was almost doubled in PBC[409μg/mL(95%CI:388-431)]and 35%higher in PSC[308μg/mL,(95%CI:296-319)]compared with HCs[226μg/mL(95%CI:221-231);p<0.001].In PBC patients,ITIH4 correlated with IgM(rho=0.49,p<0.001).Responders to ursodeoxycholic acid treatment(UDCA)had lower levels of ITIH4 than incomplete responders[395μg/mL(95%CI:364-425)]vs.460μg/mL(95%CI:421-498);p=0.02].Four weeks of UDCA treatment had no effect(p=0.19).In-creased ITIH4 immunohistochemical staining was seen in a liver biopsy from a PBC ***4 levels in AIH[224μg/mL(95%CI:208-241)]and HCs were similar(p=0.8).ITIH4 levels were lower in AH[199μg/mL(95%CI:175-223)]and CHC[202μg/mL(192-212)]patients than in HCs(p<0.05).Conclusions:The plasma concentration of ITIH4 was highly elevated in patients with PBC and PSC,suggesting that ITIH4 should be further investigated as a biomarker in cholestatic liver disease.

We have previously shown that high expression of the nucleic acid binding factor YB-1 is strongly associated with poor prognosis in a variety of cancer types. The 3-dimensional protein structure of YB-1 has yet to be determined and its role in transcriptional regulation remains elusive. Drug targeting of transcription factors is often thought to be difficult and there are very few published high-throughput screening approaches. YB-1 predominantly binds to single-stranded nucleic acids, adding further difficulty to drug discovery. Therefore, we have developed two novel screening assays to detect compounds that interfere with the transcriptional activation properties of YB-1, both of which may be generalizable to screen for inhibitors of other nucleic acid binding molecules. The first approach is a cell-based luciferase reporter gene assay that measures the level of activation of a fragment of the E2 F1 promoter by YB-1. The second approach is a novel application of the Alpha Screen system, to detect interference of YB-1 interaction with a single-stranded DNA binding site. These complementary assays examine YB-1 binding to two discrete nucleic acid sequences using two different luminescent signal outputs and were employed sequentially to screen 7360 small molecule compounds leading to the identification of three putative YB-1 inhibitors.

Background: To date, there is scant knowledge and inconsistent results about physical activity, exercise and job strain in relation to pelvic girdle pain (PGP). Hence, the aims of the present study were: 1) determine prevalence of PGP and severe PGP during pregnancy and 2) compare background and lifestyle factors in addition to total physical activity level (at work, commuting, housework and recreational exercise) in those who experienced PGP and those who did not in pregnancy. Methods: Healthy pregnant women (n = 467) were allocated to the study from Oslo University Hospital, Norway. The participants filled in a validated self-administered questionnaire, Physical-Activity-Pregnancy-Questionnaire (PAPQ) in gestation-week 32 - 36. The questionnaire contained 53 questions with nine specific questions addressing PGP. The key question on PGP was asked cross-sectionally and also posed retrospectively for pre-pregnancy, 1st and 2nd trimester (During this pregnancy, did you experience any PGP?) (“Yes versus No”). Results: More than half of the women (57.5%) reported to suffer from PGP, with 18.4% reporting severe PGP. Compared the no-PGP group, women with PGP were significantly more likely to be sick-listed, multiparous, they perceived their job to be physically demanding, as well as had physically demanding household chores, including childcare. In addition, they were more prone to have experienced PGP in previous pregnancies and suffered from urinary incontinence (UI) in the current pregnancy. Most women with severe PGP reported to be sedentary in the 3rd trimester;however they performed more strengthening exercises at home than the women without PGP. Severe PGP also showed an association with standing/walking ≥50% at work. Conclusion: Women who exercised regularly in the 3rd trimester were less likely to report PGP, while job strain was associated with higher prevalence of PGP. Background:Living donor kidney transplantations have been performed at Sahlgrenska University Hospital in Gothenburg, Sweden since 1965. In this study we wanted to explore the living kidney donor’s long-term experiences of the donation. Methods: Of 1110 consecutive living donors throughout 1965-2005, 823 were available for our questionnaire study. Results:Totally 692 replied to the questionnaire, 65% were females. The most common relation to the recipients was siblings (284), parents (262) and spouses (96). Time since donation was median 15 years (2 - 43). The initiative to donate came from the donors themselves in 69%. The dominating motives for donation were a wish to help, worries about the recipient not receiving a transplant and the knowledge that one could live a normal life with one kidney. The majority of the donors, felt well informed about potential risks both short-term and long-term. Depression post donation was reported by few donors, 2.3% and persisting pain by 4.3%. Comparisons between sibling donors and spouse donors show a significant difference (p Conclusions:Our study shows that the donor population is in good psychosocial health. It is a positive progress that spouses can be living kidney donors - they seem to be the winners.
