FOLFIRI3-aflibercept in previously treated patients with metastatic colorectal cancer

AIM To evaluate the efficacy and safety of the modified FOLFIRI3-aflibercept as second-line therapy in patients with metastatic colorectal *** This is a retrospective multicenter cohort, evaluating the efficacy and safety of the association of aflibercept with FOLFIRI3(day 1: aflibercept 4 mg/kg, folinic acid 400 mg/m^2, irinotecan 90 mg/m^2, 5-fluorouracil infusion 2400 mg/m^2 per 46 h; day 3: irinotecan 90 mg/m^2) in patients with previously treated metastatic colorectal cancer. The primary endpoint was overall response rate(ORR). Secondary endpoints were disease control rate(DCR), progression-free survival(PFS), overall survival(OS), and *** Among 74 patients treated in four French centers, nine were excluded due to prior use of aflibercept(n = 3), more than one prior treatment line in irinotecanna?ve patients(n = 3), and inadequate liver function(n = 3). In the "irinotecan-na?ve" patients(n = 30), ORR was 43.3% and DCR was 76.7%. Median PFS and OS were 11.3 mo(95%CI: 6.1-29.0) and 17.0 mo(95%CI: 13.0-17.3), respectively. The most common(> 5%) grade 3-4 adverse events were diarrhea(37.9%), neutropenia(14.3%), stomatitis and anemia(10.4%), and hypertension(6.7%). In the "pre-exposed irinotecan" patients(n = 35), 20(57.1%) received ≥ 2 prior lines of treatment. ORR was 34.3% and DCR was 60.0%. Median PFS and OS were 5.7 mo(95%CI: 3.9-10.4) and 14.3 mo(95%CI: 12.8-19.5), *** Minimally modified FOLFIRI has improvement dramatically the FOLFIRI3-aflibercept efficacy, whatever prior use of irinotecan. A prospective randomized trial is warranted to compare FOLFIRI-aflibercept to FOLFIRI3-aflibercept.

We investigated the quantitative relationship between macrophyte community structure and hydromorphology in 648 rivers in the United Kingdom （UK）.Geographical information,geological features,channel morphology,flow pattern and degree of habitat modification were included to characterize the hydromorphology at each *** analysis,partial least squares projection to latent structures （PLS）, revealed a positive correlation between floristic （abundance and species richness） and hydromorphological variables （p＜0.005）.Depth of channel,substrate type and flow habitat type played significant roles on shaping the community structure of *** communities varied between the gradient of two main types of sites: type 1 was characterised by small-sized mountain rivers with variable flow regime whereas type 2 was characterised by large-sized lowland rivers with deeper and wider channel,and higher and more stable flows Dominant species differ between unimpacted and impacted rivers （defined in term of habitat modification and eutrophication）.Seventeen macrophyte species were identified as indicators for different hydromorphological environments in *** these indicator species,submerged and emergent species were dominant over （＞40%） amphibious *** PLS model is sufficiently sensitive （cross validation:Q＞0.4,p＜0.05） to be used in the management of river macrophyte *** can predict the macrophyte communities from hydromorphological variables in rivers,and assess the ecological impacts into five classes based on the ordinal measure of *** impact of physical alteration on macrophyte communities can therefore be quantified from the *** implications of this work on monitoring river health are discussed.

Exogenous application of a cytokinin-like compound forchlorfenuron(CPPU)can promote fruit growth,although often at the expense of dry matter(DM),an important indicator of fruit *** chinensis ***‘Hayward’fruit are very responsive to CPPU treatments,but the mechanism underlying the significant fruit weight increase and associated decrease in DM is *** this study,we hypothesised that CPPU-enhanced growth increases fruit carbohydrate demand,but limited carbohydrate supply resulted in decreased fruit *** fruit development,CPPU effects on physical parameters,metabolites,osmotic pressure and transcriptional changes were assessed under conditions of both standard and a high carbohydrate *** showed that CPPU increased fruit fresh weight but the dramatic DM decrease was not carbohydrate *** glucose and fructose concentrations contributed to an increase in soluble carbohydrate osmotic pressure,which was correlated with increased water accumulation in CPPU-treated fruit and up-regulation of water channel aquaporin gene PIP2.4 at 49 days after *** analysis suggested that the molecular mechanism contributing to increased glucose and fructose concentrations was altered by carbohydrate *** standard carbohydrate supply,the early glucose increase in CPPU fruit was associated with reduced starch synthesis and increased starch *** carbohydrate supply was high,the early glucose increase in CPPU fruit was associated with a general decrease in starch synthesis but up-regulation of vacuolar invertase and fructokinase *** conclude that CPPU affected fruit expansion by increasing the osmotically-driven water uptake and its effect was not carbohydrate supply-limited.

粮草畜耦合系统包括作物、牧草和家畜的共同生产,相较于传统单一粮食作物种植系统其自给能力更强、环境成本较低。然而该耦合系统产量优势的形成机制尚不清楚,气候变化条件下系统资源利用规律和生产潜力也不清晰。本研究利用经过验证的APSIM模型,在两种共享社会经济路径（SSP245和SSP585）下,利用27个全球气候模型（GCMs）提供的气候数据,模拟了包含6种不同作物轮作的粮食-牧草-绵羊耦合生产系统（WCWC、WFWC、WFWO、WWB、WWC和WWO）,将纯作物轮作设定为参照,评估了历史时期（1985-2008）、近未来（NF,2033-2056）和远未来（FF,2057-2080）条件下耦合农业系统中牧草、作物和家畜放牧对土壤水分和氮平衡及系统生产力的影响。结果显示,与作物系统相比耦合生产系统将更多的氮（N）还原回土壤,作物生长阶段的氮还原量在历史时期为28.1～28.3 kg ha,在未来气候下为32.2～36.4 kg ha,牧草生长阶段的氮以有机物和尿素的形式返回土壤,SSP245情景下历史时期、NF和FF等不同时期返还量分别为66.3、80.4和83.8 kg ha,SSP585情景下NF和FF返还量分别为85.5和94.4 kg ha。耦合系统中存在时间上的氮素互补作用,不同的轮作和气候情景下作物每年从土壤中吸收25.4～58.2 kg ha的氮。在水分平衡方面,耦合系统显著减少了深层排水,降雨的收集效率比作物系统提高了11.0%-12.3%。耦合系统中更多的水分用于蒸腾作用,蒸腾与蒸散比（T/ET）相较于作物系统增加了38.1%～44.6%。SSP245情景下历史、NF和FF等不同时期常规系统中作物水分利用效率为9.81、10.71和11.03 kg ha mm,耦合系统中增加到10.67、11.60和11.99 kg ha mm,SSP585 NF和FF下进一步增加至为12.33和13.60 kg ha mm。耦合系统粗蛋白水分利用效率比作物系统分别提高了33.1%～61.6%。在未来气候条件下,紫花苜蓿后的第一年小麦作物的产量优势受到土壤水分含量的限制,而其他作物产量优势由于气候变暖而增加。WCWC轮作苜蓿的8年耦合生产系统经济效益最高,而WFWO轮作苜蓿系统的经济效益增加幅度最大。并且,耦合种植系统大幅度增加了土壤有机碳,历史条件下温室气体排放与传统农业系统相当,未来气候下需要通过秸秆还田增加土壤碳汇,以减少耦合农业系统的温室气体排放量。本研究表明,粮草畜耦合农业系统对氮输入的依赖较低,减少了水分渗漏和蒸发损失,提高了水分利用效率和系统产量。在未来的气候条件下,水氮互补利用和产量优势可以进一步增强。建议在旱作农区采用粮食作物-苜蓿轮作饲养家畜的耦合生产方式提高变化环境下农业系统的生产稳定性。

Transition from early intervention program to primary school in children with autism spectrum disorder

AIM To evaluate the characteristics that are associated with successful transition to school outcomes in preschool aged children with *** Twenty-one participants transitioning from an early intervention program were assessed at two time points; at the end of their preschool placement and approximately 5 mo later following their transition to school. Child characteristics were assessed using the Mullen Scales of Early Learning, Vineland Adaptive Behaviour Scales, Social Communication Questionnaire and the Repetitive Behaviour Scale. Transition outcomes were assessed using Teacher Rating Scale of School Adjustment and the Social Skills Improvement System Rating Scales to provide an understanding of each child's school adjustment. The relationship between child characteristics and school outcomes was evaluated. RESULTS Cognitive ability and adaptive behaviour were shown to be associated with successful transition to school outcomes including participation in the classroom and being comfortable with the classroom teacher. These factors were also associated with social skills in the classroom including assertiveness and engagement. CONCLUSION Supporting children on the spectrum in the domains of adaptive behaviour and cognitive ability, including language skills, is important for a successful transition to school. Providing the appropriate support within structured transition programs will assist children on the spectrum with this important transition, allowing them to maximise their learning and behavioural potential.
