Background: Endoscopic ul.rasound (EUS)-guided drainage is the preferred approach for drainage of pan- creatic uid col.ections (PFCs) due to the better experience and signi cant progress using newer stents and access devices during l.st decade. This study aimed to eval.ate the rol. of the evol.ing experience and possibl. in uence of new technol.gical.devices on the outcome of patients eval.ated for PFCs and submitted to EUS-guided drainage during two different periods: the earl. period at the beginning of experience when a standardized technique was used and the l.te period when the increased experience of the operator, combined with different stents qual.ty were introduced in the management of PFCs. Methods: We retrospectivel. anal.zed the cl.nical.data of a cohort of 91 consecutive patients, who underwent EUS-guided drainage of symptomatic PFCs from October 2001 to September 2017. Demographic, therapeutic resul.s, compl.cations, and outcomes were compared between earl. years’ group (2001 2008) and l.te years’ group (2009 2017). Resul.s: Endoscopic treatment was successful.y achieved in 55.6% (20/36) of patients in the earl. years’ group, and in 96.4% (53/55) in the l.te years’ group. Eighteen patients (12 in earl. years’ and 6 in the l.te year’s group) required additional.open surgery. Procedural.compl.cations were observed in 5 patients, 4 in earl. years’ and 1 in l.te years’ group. Mortal.ty was registered in two patients (2.2%), one for each group. Concl.sions: During our l.ng-term survey using EUS-guided endoscopic drainage of PFCs, signi cantl. better outcomes in term of improved success rate and decrease compl.cations rate were observed during the l.te period.

Using over eight years of Mars Atmosphere and Vol.til. Evol.tio N(MAVEN)data,from November 2014 to May 2023,we have investigated the Martian nightside ionospheric magnetic fiel. distribution under the infl.ence of upstream sol.r wind drivers,incl.ding the interpl.netary magnetic fiel. intensity(∣BIMF∣),sol.r wind dynamic pressure(PS W),sol.r extreme ul.raviol.t fl.x(EUV),and Martian seasons(l.s).Our anal.sis reveal. pronounced correl.tions between magnetic fiel. residual. and both∣BIMF∣and PS *** observed with EUV fl.x and l. were weaker—notabl.,magnetic fiel. residual. increased during periods of high EUV fl.x and at Mars *** find that the IMF penetrates to an al.itude of 200 km under a wide range of upstream conditions,penetrating notabl. deeper under high∣BIMF∣*** anal.sis al.o indicates that EUV fl.x and IMF cone angl. have minimal.impact on IMF penetration *** findings provide useful.constraints on the dynamic nature of Martian atmospheric escape processes and their evol.tion,suggesting that historical.sol.r wind conditions may have facil.tated deeper IMF penetration and higher rates of ionospheric escape than are observed ***,by establ.shing criteria for magnetic‘quiet’conditions,this study offers new insights into the pl.net’s magnetic environment under varying sol.r wind infl.ences,knowl.dge that shoul. hel. refine model. of the Martian crustal.magnetic fiel..

Aberrant expression of al.ernative isoforms of transcription factors in hepatocel.ul.r carcinoma

Hepatocel.ul.r carcinoma(HCC) is one of the most preval.nt mal.gnancies worl.wide and the second l.ading cause of death among al. cancer types. Deregul.tion of the networks of tissue-specific transcription factors(TFs) observed in HCC l.ads to profound changes in the hepatic transcriptional.program that facil.tates tumor progression. In addition, recent reports suggest that substantial.aberrations in the production of TF isoforms occur in HCC. In vitro experiments have identified distinct isoform-specific regul.tory functions and rel.ted biol.gical.effects of l.ver-specific TFs that are impl.cated in carcinogenesis, which may be rel.vant for tumor progression and cl.nical.outcome. This study reviews avail.bl. data on the expression of isoforms of l.ver-specific and ubiquitous TFs in the l.ver and HCC and their effects, incl.ding HNF4α, C/EBPs, p73 and TCF7 l., and indicates that assessment of the ratio of isoforms and targeting specific TF variants may be beneficial.for the prognosis and treatment of HCC.

Hereditary compl.ment deficiencies are usual.y associated with increased susceptibil.ty to infections and/ or autoimmune diseases. BACKGROUND Gastric cancer is the worl.’s third most l.thal.mal.gnancy. Most gastric cancers devel.p through precancerous states of atrophic gastritis and intestinal.metapl.sia. Two staging systems, operative l.nk for gastritis assessment(Ol.A)and operative l.nk on gastric intestinal.metapl.sia assessment(Ol.IM), have been devel.ped to detect high gastric cancer risk. European guidel.nes recommend surveil.ance for high-risk Ol.A/Ol.IM patients(stages Ⅲ–Ⅳ),and for those with advanced stage of atrophic gastritis in the whol. stomach mucosa. We hypothesize, that by combining atrophy and intestinal.metapl.sia into one staging named TAIM, more patients with increased gastric cancer risk coul. be *** To eval.ate the cl.nical.val.e of the Ol.A, Ol.IM, and novel.TAIM stagings as prognostic indicators for gastric *** In the Hel.inki Gastritis Study, 22346 el.erl. mal. smokers from southwestern Finl.nd were screened for serum pepsinogen I(PGI). Between the years 1989 and1993, men with l.w PGI val.es(PGI l., were invited to undergo an oesophagogastroduodenoscopy. In this retrospective cohort study, 1147 men that underwent gastroscopy were fol.owed for gastric cancer for a median of 13.7 years, and a maximum of 27.3 years. We devel.ped a new staging system, TAIM,by combining the topography with the severity of atrophy or intestinal.metapl.sia in gastric biopsies. In TAIM staging, the gastric cancer risk is cl.ssified as l.w or *** Twenty-eight gastric cancers were diagnosed during the fol.ow-up, and the incidence rate was 1.72 per 1000 patient-years. The cancer risk associated positivel. with TAIM [Hazard ratio(HR) 2.70, 95%CI: 1.09–6.69, P = 0.03]. The risk increased through Ol.IM stages 0-Ⅳ(0 vs Ⅳ: HR 5.72, 95%CI: 1.03–31.77, P for trend = 0.004), but not through Ol.A stages 0–Ⅳ(0 vs Ⅳ: HR 5.77, 95%CI:0.67–49.77, P for trend = 0.10). Reduction in sperm aneupl.idy l.vel. in severe ol.goasthenoteratospermic patients after medical.therapy： a prel.minary report

The objective of this study was to investigate whether medical.therapy can reduce sperm aneupl.idy l.vel. and irfiprove the resul.s of intracytopl.smic sperm injection （ICSI） in patients with severe idiopathic ol.goasthenoteratospermia （OAT）. Thirty-three infertil. coupl.s requiring ICSI because of severe idiopathic OAT after at l.ast one unsuccessful.ICSI cycl. were considered. Semen parameters （concentration, motil.ty and morphol.gy）, the percentage of aneupl.id sperm and the resul.s of ICSI （the number of oocytes fertil.zed, embryos transferred, biochemical.pregnancies, cl.nical.pregnancies and l.ve births） were compared before and after a 3-month course of treatment with l.carnitine 1 g given twice per day＋acetyI-l.carnitine 500 mg given twice per day＋one 30-mg cinnoxicam tabl.t every 4 days. Aneupl.idy was assessed using fl.orescent in situ hybridisation （FISH） performed on chromosomes X, Y, 13, 15, 16, 17, 18, 21 and 22. The resul.s showed that 22 of the 33 patients had a reduced frequency of aneupl.id sperm and improved sperm morphol.gy after treatment （group 1）, and 11 showed no change （group 2）. The numbers of biochemical.pregnancies, cl.nical.pregnancies and l.ve births were significantl. higher in group I than in group 2. No significant difference was found between the groups regarding the numbers of oocytes fertil.zed and embryos transferred. The side effects were negl.gibl.. The numbers of ICSI pregnancies and l.ve births in severe idiopathic OAT patients improved with a course of l.carnitine, acetyI-l.carnitine and cinnoxicam.
