概述:本病例系一位4个孩子的母亲,30岁,来自印度贫困农村,有明显偏执和精神病史6年。患者的症状和社会功能已经越来越恶化,但家人一直没有带其就医,直到她在命令性幻觉的影响下杀死了自己3个月大的女儿。此后使用抗精神病药物治疗,患者的精神病性症状得到控制,心理社会功能也明显改善。该案例是在社区中未能识别和治疗精神疾病造成诸多负面后果的典型案例之一。该患者在弑婴之前已经有长达6年的显而易见的严重症状,但由于家人缺乏关于精神疾病的常识,当地缺少精神卫生保健资源,加上医疗费用相对昂贵,患者未能得到及时治疗,导致了这起原本可以避免的婴儿惨死案件。要改变上述三个在中低等收入国家的农村贫困地区阻碍治疗的因素,我们必须共同应对这一挑战。任务紧迫!这一因精神疾病未得到治疗所致的弑婴病例为我们敲响了警钟。

In vivo measurement of NADH fluorescence lifetime in skeletal muscle via -ber-coupled time-correlated single photon counting

Nicotinamide adenine dinucleotide(NADH)is a cofactor that serves to shuttle electrons during metabolic processes such as glycolysis,the tricarboxylic acid cycle,and oxidative phosphorylation(OXPHOS).NADH is autofluorescent,and itsfluorescence lifetime can be used to infer metabolic dynamics in living ***-coupled time-correlated single photon counting(TCSPC)equipped with an implantable needle probe can be used to measure NADH lifetime in vivo,enabling investigation of changing metabolic demand during muscle contraction or tissue *** study illustrates a proof of concept for point-based,minimally-invasive NADHfluorescence lifetime measurement in *** muscle loss(VML)injuries were created in the left tibialis anterior(TA)muscle of male Sprague Dawley *** lifetime measurements were collected before,during,and after a 30 s tetanic contraction in the injured and uninjured TA muscles,which was subsequently-t to a biexponential decay model to yield a metric of NADH utilization(cytoplasmic vs protein-bound NADH,the A11/A22 ratio).On average,this ratio was higher during and after contraction in uninjured muscle compared to muscle at rest,suggesting higher levels of free NADH in contracting and recovering muscle,indicating increased rates of *** injured muscle,this ratio was higher than uninjured muscle overall but decreased over time,which is consistent with current knowledge of inflammatory response to injury,suggesting tissue regeneration has *** data suggest that-ber-coupled TCSPC has the potential to measure changes in NADH binding in vivo in a minimally invasive manner that requires further investigation.

Hydrological Modelling of Small Gauged and Ungauged Mountainous Watersheds Using SWAT—A Case of Western Ghats in India

Mountainous forested watersheds are important hydrologic systems that are responsible for much of the water supply and run-of-the-river hy-dropower schemes in many parts of the world. In India, the Western Ghats are one of such important hydrologic systems located in southern peninsular region. Several of these watersheds are ungauged. The Soil and Water Assessment Tool (SWAT) has been used to model streamflows for two mountainous forested watersheds, namely, Gurupur (699 km2) (a gauged watershed) and Upper Payaswini (44.6 km2) (an ungauged wa-tershed). Model calibration and validation are performed using monthly and daily streamflow data for the gauged watershed. Sample flow values obtained over a limited period were used for validation of ungauged wa-tershed. Flow duration curves (FDCs) have been derived to assess per-centile flow distributions. Model performance is evaluated using Nash-Sutcliffe coefficient (ENS), percent bias (PBIAS), coefficient of de-termination (R2) and comparison of percentile flow values obtained from observed and simulated FDCs. Sensitivity analysis with Latin Hypercube One-factor-At-a-Time (LH-OAT) indicates five soil-land use related pa-rameters namely, soil available water capacity (SOL_AWC), soil evapora-tion compensation factor (ESCO), soil depth (SOL_Z) and layers, groundwater baseflow (ALPHA_BF), and curve number (CN2 (forest & agriculture)), to be sensitive for simulating both gauged and ungauged wet mountainous forested watersheds. Study shows that lateral flows from dynamic sub-surface zones in such watersheds contribute substan-tially to the total water yield.

Mangrove Restoration and Regeneration Monitoring in Gulf of Kachchh, Gujarat State, India, Using Remote Sensing and Geo-Informatics

Indian coast harbors richly diverse and critical coastal habitats like coral reefs and mangroves. Mangroves form one of the most important ecosystems of coastal and marine areas. It safeguards the ecology of the coastal areas and provides livelihood opportunities to the fishermen and pastoral families living in these areas. In real sense, mangrove is the Kalpvriksh (divine tree which fulfills all the desires) for the coastal communities. The restoration and plantation of mangroves have received a lot of attentions worldwide. To assess the impact of mangrove plantation activities and to monitor the mangrove regeneration and restoration in various villages, a joint study under the Integrated Coastal Zone Management Project (ICZMP) was taken up by Gujarat Ecology Commission (GEC) and Bhaskaracharya Institute for Space Applications and Geo-Informatics (BISAG) in the Gulf of Kachchh, Gujarat State.?The major objective of this study was to monitor the increase in mangrove cover in coastal areas of Gulf of Kachchh using the Indian Remote Sensing Satellite data of 2005, 2011 and 2014. The mangrove regeneration was monitored using multi-temporal Indian Remote Sensing Satellite (IRS) LISS-III and LISS-IV digital data covering Gulf of Kachchh region. The multi-temporal IRS LISS-III data covering Gulf of Kachchh of October-2005, November-2011 and LISS-IV data of April-2014 was analyzed. The mangrove density and mangrove area in different talukas was estimated based on the analysis of IRS LISS-III digital data. The mangroves have been delineated based on the pink colour observed on satellite images and the area was estimated in the Geographic Information System (GIS) environment. The taluka or block-level mangrove areas were estimated and changes in the areas were monitored during the period of six years from 2005 to 2011. An Ecological Assessment of Critically Endangered Chinese Pangolin <i>Manis pentadactyla</i>(Mammalia: Pholidota: Manidae) in the Midhills Region of Nepal

The Chinese Pangolin (Manis pentadactyla) is a “Critically Endangered” insectivorous mammal. There is a lack of information on the ecology, distribution and behavior of this species in Nepal, even though community forests in the Midhills Region of Nepal are considered to be prime habitats for the species. We conducted an ecological assessment of the habitat use of this species in nine community forests covering an area of 170.69 ha in Balthali village in Kavrepalanchok district, Nepal. We divided each forest into 50 × 50 m grids and searched for Chinese Pangolin burrows along 50 m transects in each grid. We categorized burrows into two types, new and old burrows, and recorded 10 environmental variables for each burrow. We found a total of 542 burrows, including 323 old burrows and 219 new burrows. The average width and height of a burrow opening was 21.66 ± 3.94 cm (n = 542) and 22.42 ± 5.02 cm (n = 542). The total density of burrows in the study area was 3.1 ha−1, with new burrows density of 1.2 ha−1 and old burrows density of 1.8 ha−1. We found most burrows in sites with an elevation range of 1400 - 1500 m above sea level, gentle slopes, brown colored soil, high density of tree crown cover and undergrowth vegetation, southern exposure, within 150 m from a water source and less than 200 m from a human settlement. Our study has generated baseline data on the habitat use of Chinese Pangolin, which we hope will help in designing site-specific action plans for Chinese Pangolin conservation in Kavrepalanchok district, Nepal.

PURPOSE: To present a case of a child with an extensive facial hemangioma in whom the suspicion of intracranial involvement was raised by visual evoked potential (VEP) evaluation. DESIGN: Observational case report. METHODS: VEP analysis was performed using flash and pattern stimuli to determine whether treatment intervention other than occlusion therapy was needed. RESULTS: VEP analysis indicate d a marked transoccipital asymmetry in the flash and pattern VEP suggestive of r ight hemispheric dysfunction. Neuroimaging showed intracranial extension of the hemangioma. CONCLUSION: The VEP evaluation is a noninvasive technique performed in the awake child. This case highlights the usefulness of VEP analysis in the m anagement of extensive periocular capillary hemangiomas and its use as a modalit y to identify potential intracranial involvement. Large plaquelike facial capill ary hemangiomas may have possible underlying dural involvement suggestive of PHA CES syndrome.

Objective: To compare visual acuity, optic disc appearance, and transient pattern reversal visual evoked potentials as markers of possible visual dysfunction in children with syndromic craniosynostosis. Methods: Serial visual acuity, optic disc appearance, and pattern reversal visual evoked potential data were recorded in 8 patients with syndromic craniosynostosis before and after cranial vault expansion. The pattern reversal visual evoked potentials were analyzed using linear regression modeling, applied to the N80 to P100 amplitude. Results: Serial optic disc appearances were available for all 8 patients and visual acuities for 7 patients. The visual acuity deteriorated in only 1 patient, improved in 4, and fluctuated in 2, before surgery. Of the 8 patients, 3 showed no papilledema in either eye at any time, 3 showed progressive bilateral swelling before surgery, and 2 exhibited only unilateral disc swelling. In all 8 patients, there was a trend for the N80 to P100 amplitude to decrease before surgery and to increase, in all but 2 patients, after surgery. Conclusions: This study suggests that neither optic disc appearance nor visual acuity assessment alone is a reliable marker of potential visual dysfunction in children with syndromic craniosynostosis. It also suggests that the pattern reversal visual evoked potential can provide early evidence of visual dysfunction before vault expansion surgery in these children; this dysfunction may recover postoperatively.

Purpose: To report a case of unilateral keratoconus in a 7- yearold female, secondary to chronic persistent eye- rubbing in the absence of any systemic condition. Design: Observational case report. Methods: In the case of this child, no organic cause was found to explain the onset of keratoconus other than persistent eye- rubbing in the affected eye. Results: Nocturnal eye padding was instigated and the condition stabilized. She was reviewed over a 2- year period of follow- up. Conclusions: Eye- rubbing has been implicated in the pathogenesis of keratoconus. Eyerubbing is also often a feature of a number of conditions linked to keratoconus such as Down syndrome, atopic keratoconjunc- tivitis, mental retardation, and Lebers’ congenital amaurosis. There are a number of reports linking eye- rubbing and keratoconus in children, typically in relation to these conditions. However, in this case the keratoconus was secondary to chronic and persistent eye- rubbing in a healthy child.
