Efficacy and safety of endoscopic prophylactic treatment with undiluted cyanoacrylate for gastric varices

AIM:To evaluate the efficacy and safety of undiluted N-butyl-2 cyanoacrylate plus methacryloxysulfolane(NBCM)as a prophylactic treatment for gastric varices(GV)***:This prospective study was conducted at a single tertiary-care teaching hospital between October 2009 and March *** with portal hypertension(PH)and GV,with no active gastrointestinal bleeding,were enrolled in primary prophylactic treatment with NBCM injection without lipiodol *** diagnosis of GV was based on endoscopy and confirmed with endosonography(EUS); the same procedure was used after treatment to confirm eradication of *** puncturing the GV with a regular injection needle,1 mL of undiluted NBCM was injected intranasally into *** injection was repeated as necessary to achieve eradication or until a maximum total volume of 3 mL of NBCM had been *** were followed clinically and evaluated with endoscopy at 3,6 and 12 *** follow-ups were performed *** main outcome measures were efficacy(GV eradication),safety(adverse events related to cyanoacrylate injection),recurrence,bleeding from GV and mortality related to GV ***:A total of 20 patients(15 male)with PH and GV were enrolled in the study and treated with undiluted NBCM *** 2(10%)patients had no esophageal varices(EV); 18(90%)patients were treated with endoscopic band ligation to eradicate EV before inclusion in the *** patients were followed clinically and endoscopically for a median of 31 mo(range:6-40 mo).Eradication of GV was observed in all patients(13 patients were treated with 1 session and 7 patients with 2 sessions),with a maximum injected volume of 2 mL *** patient had GV recurrence,confirmed by EUS,at 6-mo follow-up,and another had late recurrence with GV bleeding after 35 mo of follow-up; overall,GV recurrence was observed in 2 patients(10%),after 6 and 35 mo of follow-up,and GV bleeding rate was 5%(1 patient).Mild epigastric pain was r

Antral exfoliative cytology for the detection of Helicobacter pylori in the stomach

AIM: To evaluate the efficacy of antral exfoliative cytology method in the diagnosis of Helicobacter pylori (H pylori)infection in the ***: Fifty patients were submitted to upper digestivetract endoscopy due to complaints of dyspepsia. Thematerial for exfoliative cytology was obtained by extensive brushing of the gastric antral mucosa and Papanicolaou stain was used to identify the bacteria. The authors also performed gastric biopsies to collect material for urease tests and histologic studies, with hematoxylin-eosin and fucsin stains in order to identify the microorganism. The gold standard used to detect the presence of H pylori was an analysis of the combined results from the gastric biopsies by urease test and histological method. RESULTS: Antral exfoliative cytology method exhibited 90.3% sensitivity, 66.6% specificity, accuracy of 81.6%, positive predictive value of 82.3% and negative predictive value of 80.0%, in this population with a prevalence of 63.3%.CONCLUSION: Antral exfoliative cytology was demonstrated to be a sensitive, accurate and easy to perform method for investigating H pylori infection in the stomach.

Critical bone defects are considered one of the major clinical challenges in reconstructive bone *** combination of 3D printed conductive scaffolds and exogenous electrical stimulation(ES)is a potential favorable approach for bone tissue *** this study,3D conductive scaffolds made with biocompatible and biodegradable polycaprolactone(PCL)and multi-walled carbon nanotubes(MWCNTs)were produced using the extrusion-based additive manufacturing to treat large calvary bone defects in *** results show that the use of PCL/MWCNTs scaffolds and ES contributes to thicker and increased bone tissue formation within the bone *** and mineralization are also significantly promoted using high concentration of MWCNTs(3 wt%)and ***,scaffolds favor the tartrate-resistant acid phosphatase(TRAP)positive cell formation,while the addition of MWCNTs seems to inhibit the osteoclastogenesis but present limited effects on the osteoclast functionalities(receptor activator of nuclear factor κβ ligand(RANKL)and osteoprotegerin(OPG)expressions).The use of ES promotes the osteoclastogenesis and RANKL expressions,showing a dominant effect in the bone remodeling *** results indicate that the combination of 3D printed conductive PCL/MWCNTs scaffold and ES is a promising strategy to treat critical bone defects and provide a cue to establish an optimal protocol to use conductive scaffolds and ES for bone tissue engineering.

The aim of this paper is to present the linear generator （Stirling solar dish） in the context of a HSSB （hybrid system solar biomass）, project P ＆ D （research and development P ＆ D 0041, in cooperation and partnership with CPFL--LIght and Force Paulista Company-Campinas, Piratininga, S.P. Brazil）. The other components of the system will be the solar ORC （organic Rankine cycle）, the rotary Stifling and the biomass gasifier. The integration of the complete system will be described in the paper, and is projected to be hydraulic one.

Tracking Power System State Evolution with Maximum-correntropy-based Extended Kalman Filter

This paper develops a novel approach to track power system state evolution based on the maximum correntropy criterion,due to its robustness against non-Gaussian *** includes the temporal aspects on the estimation process within a maximum-correntropy-based extended Kalman filter(MCEKF),which is able to deal with both nonlinear supervisory control and data acquisition(SCADA)and phasor measurement unit(PMU)measurement *** representing the behavior of the state variables with a nonparametric model within the kernel density estimation,it is possible to include abrupt state transitions as part of the process noise with non-Gaussian ***,a novel strategy to update the size of Parzen windows in the kernel estimation is proposed to suppress the effects of suspect *** properly adjusting the kernel bandwidth,the proposed MCEKF keeps its accuracy during sudden load changes and contingencies,or in the presence of bad *** with IEEE test systems and the Brazilian interconnected system are carried *** results show that the method deals with non-Gaussian noises in both the process and measurement,and provides accurate estimates of the system state under normal and abnormal conditions.

The CNC （computer numerical control） is a technology which aims to generate, parse and execute sequential actions that describe the behavior of the end effector. This article reports the development of small sized, low cost, prototype CNC system, based on a modular system with the capability of communication through USB. The report includes the basics of the electronic devices developed to run a CNC machine and proposes, as well, software developed in LabVIEW to control and establish the communication between the machine and the computer. The main objective of this work is the development of a machine which allows future researches on the process performance, on educational purposes, the demonstration of the parts involved on the CNC machine.

Surgery is still the primary curative treatment for gastric cancer,which includes resection of the tumor with adequate margins and extended *** order to improve the operative results and the quality of life of patients,several endeavors have been made toward precision medicine through image-guided surgery,allowing access to real-time intraoperative anatomy and accurate tumor *** goal of the surgeon is to achieve a more precise,individualized,and less invasive surgery without compromising oncological efficiency and *** this perspective,we have demonstrated the role of indocyanine green(ICG)and near-infrared(NIR)fluorescence imaging method in gastric cancer *** technique may be used to improve localization of the tumor,detection of sentinel lymph nodes(SLN),real-time lymphatic mapping,and blood flow assessment(anastomosis perfusion).

Proactive reconciliation as a tool for integrating mining and milling operations

Historically, reconciliation has been viewed as a quality test of model estimates as well as a powerful tool for detecting and correcting problems in all stages of mine operations from resource estimation to metal production. If used correctly, reconciliation helps to better predict the life of mine(LOM), improves the adherence of production plans to the respective budget, and allows for effective control of the mining and milling processes. However, the accuracy of reconciliation results requires that all input data must be generated in accordance with the principles of sampling correctness. Furthermore, complete reconciliation systems can become extremely complex and must be carefully analyzed in order to provide realistic and helpful conclusions. Based on these concepts, this paper presents a successful proactive reconciliation system applied to a copper and gold mine in Brazil for monitoring and integrating mining and milling operations.
