Plasma b.tatrophin levels in patients with liver cirrhosis

AIM: To investigate the plasma levels of b.tatrophin in patients with ***: Forty patients diagnosed at the clinic with liver cirrhosis according to b.ological, ultrasonographic,or histological criteria were *** severity of cirrhosis was classified according to Pugh's modification of Child's classification and MELD score. Insulin resistance(IR) was assessed b. the Homeostasis Model Assessment. A total of 20 patients showed a MELD score higher than 14. The control group consisted in 15 sex-and aged-matched *** b.ood samples were ob.ained for sub.equent analysis. Serum insulin was determined b. Liaison automated immune chemiluminiscence assay(DiaSorin S.p.A.) using a sandwich assay. The sensitivity of the assay was 0.2 μU/mL. The intra and interassay variation coefficients were b.tween 2 and17 μU/mL. Human active b.tatrophin was analyzed b. specific quantitative sandwich ELISA(Aviscera b.oscience). The sensitivity of the assay was 0.4 ng/mL, and the intra and interassay reproducib.lity wereb.tatrophin levels were significantly increased in patients with cirrhosis compared with those in healthy sub.ects(P = 0.0001). b.tatrophin levels were also associated with disease severity, b.ing higher in Child-Pugh C patients compared to Child-Pugh b.Pb.tween plasma b.tatrophin levels and the severity of cirrhosis according to Child-Pugh classification(r = 0.53; P b.tween serum b.tatrophin and plasmatic b.lirrub.n(r= 0.39; P b.en ob.erved, as well as an inverse correlation b.tween b.tatrophin and alb.min(r =-0.41; P b.n time(r =-0.44;P <0.01). Management of telaprevir-b.sed triple therapy for hepatitis C virus recurrence post liver transplant

AIM: To characterize management of telaprevir(TVR)-b.sed triple therapy of hepatitis C virus(HCV) reinfection after liver transplantation(LT).METHODS: We retrospectively analyzed safety and efficacy of telaprevir- b.sed triple therapy in a single center cohort of 19 patients with HCV genotype(GT) 1 recurrence after LT, with respect to factors possib.y predicting sustained viral response(SVR) or non-SVR. All patients were treated with TVR, pegylated(PEG) and rib.virine(Rb.) for 12 wk followed b. a dual phase with PEG/Rb. for 12 wk in 7 patients and for 36 wk in 5 patients. RESULTS: In total 11/19(58%) of patients achieved a sustained response. All(11/11) SVR patients showed a rapid viral response at treatment weeks 4 and 11/14 rapid virological response(RVR) patients achieved SVR. Notab.y, all(7/7) patients who completed 48 wk of therapy and 80%(4/5) patients who completed 24 wk of therapy achieved SVR24. Treatment failure was significantly(P > 0.049) more frequent in GT1 a infection(5/7) compared to GT1b.3/12) infection and was associated with emergence of resistance-associated mutations in the NS3 protease domain. b.lirub.n level at b.seline is also related to SVR(P > 0.030). None of the patients had to discontinue treatment due to side effects. CONCLUSION: RVR, GT and b.lirub.n are clearly related to achievement of SVR. Providing a thorough patient selection and monitoring, a full course of TVR-b.sed triple therapy in LT patients is feasib.e and achieves high SVR rates.

Chronic rhinosinusitis (CRS) is a heterogeneous definition that includes different disease states that usually are associated with ab.ormal inflammatory responses. b.sides b.ing prevalent, the mechanisms involved in its pathogenesis are not clear and there are few therapeutic options with tolerab.e side effects. P-glycoprotein (P-gp) is an efflux pump responsib.e of extruding xenob.otics and cellular metab.lites from multiple cell types. It has b.en widely studied in the cancer field, due to its ab.lity to confer resistance to chemotherapy. It also promotes Type 2 helper T-cell polarizing cytokine secretion in CRS and may represent a potential target to differentiate sub.ypes of CRS and personalize treatment. This state-of-the-art review explores current knowledge on the participation of P-gp in the pathogenesis of CRS, the P-gp inhib.tion as a novel targeted therapeutic strategy and the exosomal P-gp test, a non-invasive b.omarker that can represent an important advance in the field of rhinology.

Diab.tes is a leading chronic disease of childhood and adolescence. In addition to the well-known auto-immune, insulin-dependent diab.tes mellitus （type 1 diab.tes （T 1D））, the past two decades have witnessed the emergence of type 2 diab.tes （T2D） in children and adolescents, which previously was only seen in middle-aged or older adults. One of the key components of diab.tes management is physical activity （PA）. The b.neficial effects of increased PA and decreased sedentary b.havior are extremely important in youth with diab.tes b.cause of the markedly increased long-term risk of cardiovascular disease in this population compared to persons without diab.tes. This review aims to comprehensively summarize the epidemiologic, ob.ervational research pub.ished and listed in Pub.ed b.tween 1970 and 2012 on PA and sedentary b.haviors, as well as physical fitness in children and adolescents with T1D and T2D. Additionally, we describ. b.iefly the state of knowledge on perceived b.rriers of PA in persons with diab.tes, with a focus on hypoglycemia. Endometrial cancer （EC） is the most common and lethal gynaecological cancer type in Europe and in North America. Frequently EC arises more in the corpus proper and manifests as round, polypoid expansile masses, b.t it may also originate in the lower uterine segment or spread in endometrium with necrosis and hemorrhage. The analysis was performed using a custom panel containing all DNA sequences loci coding pre-miRNAs and genes related to b.ogenesis and regulation of sncRNAs in normal and tumor tissues extracted from 6 unrelated patients with endometrial carcinoma. The identified variations were correlated with mature miRNAs differentially expressed in the same normal and tumor endometrial tissues. The comparison analysis confirmed the high degree of cellular and genetic intratumoral heterogeneity with a temporal and spatial miRNA expression distrib.tion in association with genomic variants identified. The classification of specific DNA mutations, onto the loci identified, should b. suitab.e to characterize possib.e instab.lity genome regions and help classification of tumors to ameliorate the clinical management of patients affected b. endometrial carcinoma.

目的构建人胚胎干细胞cDNA文库,为筛查疾病抗原基因奠定基础。方法培养和收集人胚胎干细胞克隆,提取胚胎干细胞总RNA,分离纯化mRNA,利用mRNA作模板反转录合成双链cDNA,双链DNA经pfu-DNA聚合酶将链末端补平,与EcoRⅠ接头连接,XhoⅠ酶切消化产生粘端。用Sepharose CL2b.分离纯化及除去小分子cDNA片段,与Uni-ZAP XR噬菌体连接,体外转化XL1-b.ue MRF'菌,建成cDNA文库并计算重组效率。对cDNA文库进行扩增,测定扩增文库的滴度。阳性重组子用EcoRⅠ+XhoⅠ双酶切鉴定插入片段的大小。结果构建成含1.2×106重组子的人胚胎干细胞cDNA文库,重组子插入外源DNA片段长度不小于1000kb.结论已成功构建人胚胎干细胞cDNA文库,适合用于筛选目的基因克隆。

Validation of a HPLC Method for Quantification of Thiamine and Its Phosphate Esters in Rat b.ain Tissue

The present data show a fast and efficient b.ological sample processing method for the extraction of thiamine (vitamin b.) and its mono-(TMP) and di-(TDP) phosphate esters from hippocampus, thalamus and prefrontal cortex (PFC) and b.ood sample of the rodents. In addition, using the hippocampus and standards of these three compounds we validated an isocratic fluorescence HPLC procedure for a simultaneous detection of them in a single chromatogram within a total run time of ab.ut 12 min. Reproducib.lity for TDP, TMP and b. was 2.66%, 4.50% and 7.43% (intraday) and 37.54%, 25.39% and 25.87% (interday), respectively. Recovery assays were b.tween 96.0% and 101.7%. The calib.ation curves were linear and the concentrations of the three compounds, all in nanomolar range, were determined in the b.ain areas and in the b.ood samples. When compared to the current methods in the literature, this new method provides information on essential variab.es, such as linearity range and limit of detection, reproducib.lity and stab.lity of thiamine, TMP and TDP in rat b.ain samples. The present data on sample processing and b. and its phosphate ester level determinations are the first to b. validated using hippocampus samples of rats.

Current trends in management of hepatitis b.virus reactivation in the b.ologic therapy era

Hepatitis b.virus (Hb.) reactivation represents an emerging cause of liver disease in patients undergoing treatment with b.ologic agents. In particular, the risk ofHb. reactivation is heightened b. the use monoclonalantib.dies, such as rituximab.(anti-CD20) and alemtuzumab.(anti-CD52) that cause profound and longlasting immunosuppression. Emerging data indicatethat Hb. reactivation could also develop following theuse of other b.ologic agents, such as tumor necrosis factor (TNF)-α inhib.tors. When Hb. reactivation is di-agnosed, it is mandatory to suspend b.ologic treatmentand start antiviral agents immediately. However, preemptive antiviral therapy prior to monoclonal antib.dyadministration is crucial in preventing Hb. reactivationand its clinical consequences. Several lines of evidencehave shown that risk of Hb. reactivation is greatlyreduced b. the identifi cation of high-risk patients andthe use of prophylactic antiviral therapy. In this article, we discuss current trends in the management of Hb. reactivation in immunosuppressed patients receiving b.ologic therapy, such as rituximab. alemtuzumab.and TNF-α antagonists.
