Evaluation of Functional and Radiological Outcome of Arthroscopic-Assisted Anatomical Coracoclavicular (CC) and Acromioclavicular (AC) Ligament Reconstruction in Chronic AC Joint Dislocation

Introduction: Acromioclavicular (AC) joint dislocation is a common shoulder injury, comprising 9% - 12% of shoulder girdle injuries. Optimal management remains challenging, with treatment decisions guided by the Rockwood classification system. Controversies surround grade III injuries, necessitating further classification. Non-operative treatment has shown favorable outcomes, while surgical interventions vary. Anatomical coracoclavicular reconstruction (ACCR) has demonstrated biomechanical advantages over traditional methods. Arthroscopic techniques offer advantages, minimizing deltoid detachment and allowing concurrent pathology identification. This study evaluates the outcomes of arthroscopic-assisted ACCR in chronic AC joint dislocation. Surgical Technique: Arthroscopic-assisted ACCR involves meticulous portal placement, tendon graft harvesting, diagnostic arthroscopy, and coracoid exposure. The clavicle tunnels were made to mimic the conoid and trapezoid ligament positions, using FibreTape#2 loop and Dog Bone Button for correct placement against the coracoid base, and passing the semitendinosus graft through to reconstruct the conoid ligament, reduction done and graft follow through for anatomical reconstruction. Methods: A retrospective cohort study at Hospital Kuala Lumpur analyzed 35 patients undergoing arthroscopic-assisted ACCR for Rockwood grade III - V AC joint dislocations. Inclusion criteria encompassed trauma ≥ 3 weeks prior, no prior shoulder injuries, and ≥12-month follow-up. Functional and radiological assessments utilized ASES scores and coracoclavicular distances, respectively. Statistical analysis employed descriptive statistics and logistic regression. Results: The mean age was 38.9 years (SD 11.26), and 34 of 35 patients were male. Grade IV injuries were predominant (37.1%). Waiting time for surgery averaged 234.9 days. 目的探究在对急性脑梗死患者进行评估时,采用中性粒细胞与淋巴细胞比值指标的临床价值。方法回顾性分析2021年5月-2023年1月期间安阳市第二人民医院收治的110例急性脑梗死患者的临床资料,按照改良Rankin量表(mRS)评分将所有患者分为两组(预后不良组、预后良好组),分别为18、92例。将两组患者各项基线资料进行对比,并纳入多因素分析,对急性脑梗死患者美国国立卫生研究院卒中量表(NIHSS)、mRS评分与中性粒细胞与淋巴细胞比值、血小板与淋巴细胞比值的关系进行分析。结果单因素结果显示,预后不良组脑梗死病史、大动脉粥样硬化型脑梗死、NIHSS评分≥5分患者占比与预后良好组比升高,发病至治疗时间长于预后良好组,淋巴细胞计数、中性粒细胞计数等指标均于预后良好组升高;多因素回顾分析结果得出,急性脑梗死预后不良的高危因素在于脑梗死病史、大动脉粥样硬化型、NIHSS评分≥5分、发病至治疗时间长、中性粒细胞与淋巴细胞比值、血小板与淋巴细胞比值(OR值=3.327、10.186、1.818、1.527、1.384、1.510)(P<0.05);急性脑梗死患者NIHSS评分与中性粒细胞与淋巴细胞比值、血小板与淋巴细胞比值呈正相关(r=0.598、0.712),mRS评分和中性粒细胞与淋巴细胞比值、血小板与淋巴细胞比值呈负相关关系(r=-0.502、-0.495)(P<0.05)。结论脑梗死病史、大动脉粥样硬化型、NIHSS评分≥5分、发病至治疗时间长、中性粒细胞与淋巴细胞比值、血小板与淋巴细胞比值均是急性脑梗死预后不良的危险因素,临床可对有以上临床特征的患者进行密切关注,临床可早期对患者进行NIHSS、mRS评分评估,以早期发现神经功能的情况,及时采取治疗,改善预后。

目的:探讨姜黄素(Cur)对磨损颗粒诱导的体外破骨细胞的作用及机制。方法:将小鼠RAW264.7巨噬细胞分为五组培养,即空白组(完全培养基)、模型组[空白组+钛(Ti)颗粒0.1 mg/ml]、低浓度Cur组(模型组+Cur 1μmol/L)、中浓度Cur组(模型组+Cur 10μmol/L)和高浓度Cur组(模型组+Cur 15μmol/L)。CCK-8法检测细胞增殖活性。抗酒石酸酸性磷酸酶(TRAP)染色法和鬼笔环肽染色法检测成熟破骨细胞数量。酶联免疫吸附(ELISA)法检测细胞上清液中肿瘤坏死因子(TNF)-α、白细胞介素(IL)-1β和基质金属蛋白酶(MMP)-9的表达。Western blot法检测p-p65、磷酸化核因子(NF)-κB抑制蛋白α(p-IκBα)蛋白水平。PCR法检TNF-α、IL-1β、活化T细胞核因子(NFATc)1和TRAP mRNA的表达水平。结果:Cur浓度小于15μmol/L对RAW264.7巨噬细胞增殖活性的影响与空白组比较差异无统计学意义(均P>0.05),因此采用不高于15μmol/L Cur进行后续实验。中、高浓度Cur组TRAP阳性成熟破骨细胞数量低于模型组(均P<0.05)。与模型组比较,不同浓度Cur组TNF-α、IL-1β和MMP-9表达,p-p65与p-IκBα蛋白表达,TRAP、TNF-α、IL-1β和NFATc1 mRNA表达降低(均P<0.05)。结论:Cur对体外磨损颗粒诱导的破骨细胞生成和破骨细胞功能有抑制作用,其机制可能是抑制NF-κB信号通路。

Effects of longitudinal magnetic field on primary dendrite spacing and segregation of directionally solidified single crystal superalloy

The primary dendrite spacing(PDS) and segregation of directionally solidified single crystal(SC) superalloy under the longitudinal magnetic field(LMF) were investigated based on the analysis of the whole cross-sectional microstructure at different solidification distances. The results show that the PDS under the LMF remains basically unchanged at different solidification distances, and it is greater than that under no LMF. With the increase of magnetic field intensity, the PDS increases and the macrosegregation decreases. The increasing PDS and reducing segregation under the LMF can be attributed to the increase of solute boundary layer, which expands the non-equilibrium freezing temperature range and brings the effective partition coefficient closer to 1. The increase of the solute enrichment layer thickness could be caused by the downward secondary circulation generated by the thermoelectric magnetic convection(TEMC) near the interface, which drives the migration of solutes towards the interdendritic region. This work not only clarifies the mechanism of LMF controlling PDS and reducing segregation by TEMC, but also provides theoretical guidance for producing high-quality SC superalloys using magnetic fields.

MicroRNAs(miRNAs)play a significant role in axon regeneration following spinal cord ***,the functions of numerous miRNAs in axon regeneration within the central nervous system(CNS)remain largely ***,we elucidate the positive role of microRNA-2184(miR-2184)in axon regeneration within zebrafish Mauthner cells(M-cells).The upregulation of miR-2184 in a single M-cell can facilitate axon regeneration,while the specific sponge-induced silencing of miR-2184 leads to impeded *** show that syt3,a downstream target of miR-2184,negatively regulates axon regeneration,and the regeneration suppression modulated by syt3 depends on its binding to Ca2+.Furthermore,pharmacological stimulation of the cAMP/PKA pathway suggests that changes in the readily releasable pool may affect axon *** data indicate that miR-2184 promotes axon regeneration of M-cells within the CNS by modulating the downstream target syt3,providing valuable insights into potential therapeutic strategies.

An Information Entropy-Based Methodology to Construct the Avulsion Threshold in the Tail Reach of the Estuarine Alluvial Plain

Channel avulsion is a natural phenomenon that occurs abruptly on alluvial river deltas,which can affect the channel *** causes for avulsion could be generally categorized as topography-and flood-driven ***,previous studies on avulsion thresholds usually focused on topography-driven factors due to the centurial or millennial avulsion timescales of the world’s most deltas,but neglected the impacts of flood-driven *** the current study,a novel demarcation equation including the two driven factors was proposed,with the decadal timescale of avulsion being considered in the Yellow River Estuary(YRE).In order to quantify the contributions of different factors in each category,an entropy-based methodology was used to calculate the contributing weights of these *** factor with the highest weight in each category was then used to construct the demarcation equation,based on avulsion datasets associated with the *** avulsion threshold was deduced according to the demarcation *** avulsion threshold was then applied to conduct the risk assessment of avulsion in the *** results show that:two dominant factors cover respectively geomorphic coefficient representing the topography-driven factor and fluvial erosion intensity representing the flood-driven factor,which were thus employed to define a two dimensional mathematical space in which the demarcation equation can be obtained;the avulsion threshold derived from the equation was also applied in the risk assessment of avulsion;and the avulsion threshold proposed in this study is more accurate,as compared with the existing thresholds.

The expansion and limitation of behavioral finance research methods

Behavioral finance inherits the standard research methods in the finance, and it makes outstanding contribution to leading the experimental research into the field of finance. It uses the cognitive science as the method guide to expand its scientific research tools. On one hand, the intelligent development of the cognitive science provides an important and effective means of a tool for the behavioral finance research; on the other hand, precisely because behavioral finance theories of progress makes a certain extent, dependent on the modem cognitive science research results and thus the development of cognitive science, its results application and limitation will be confined to a direct impact on the development of behavioral finance. That is why we need an in-depth study of the new method of behavioral finance research and grasps their limitations.

Diagnostic Utility of Diffusion-weighted Magnetic Resonance Imaging in Differentiating Small Solid Renal Tumors （≤4 cm） at 3.0T Magnetic Resonance Imaging

Background： The aim of this study was to assess the performance of apparent diffusion coefficient （ADC） measurement obtained with diffusion-weighted magnetic resonance imaging （DW-MRI） to distinguish renal cell carcinomas （RCCs） from small benign solid renal tumors （〈4 cm）. Methods： In this cross-sectional study, 49 consecutive patients with histopathologically confirmed small solid renal tumors, and seven healthy volunteers were imaged using nonenhanced MRI and DW-MRI. The ADC map was calculated using the b values of 0, 50, 400, and 600 s/ mm2 and values compared via the Kruskal-Wallis and Mann-Whitney tests. The utility of ADC for differentiating RCCs and benign lesions was assessed using a receiver operating characteristic curve. Multiple nonenhanced MRI features were analyzed by Logistic regression. Results： The tumors consisted of 33 cases of clear-cell RCCs （ccRCCs） and 16 cases of benign tumors, including 14 cases of minimal fat angiomyolipomas and 2 cases ofoncocytomas. The ADCs showed significant differences among benign tumors （[0.90 ±0.52] x 10 x mm2/s）, ccRCCs （[1.53 ± 0.31 ] x 10 ^3 mm2/s） and the normal renal parenchyma （[2.22 ± 0.12]x 10^-3 mmVs）（P 〈 0.001）. Moreover, there was statistically significant difference between high and low-grade ccRCCs （P = 0.004）. Using a cut-offADC of 1.36± 10 3 mm2/s, DW-MRI resulted in an area under the curve （AUC）, sensitivity, and specificity equal to 0.839, 75.8%, and 87.5%, respectively. Nonenhanced MRI alone and the combination of imaging methods led to an AUC, sensitivity and specificity equal to 0.919, 93.9%, and 81.2%, 0.998, 97%, and 100%, respectively. The Logistic regression showed that the location of the center of the tumor （inside the contour of the kidney） and appearance of stiffblood vessel were significantly helpful for diagnosing ccRCCs. Structural regulation of coal-derived hard carbon anode for sodium-ion batteries via pre-oxidation

Hard carbon(HC)is broadly recognized as an exceptionally prospective candidate for the anodes of sodium-ion batteries(SIBs),but their practical implementation faces substantial limitations linked to precursor factors,such as reduced carbon yield and increased ***,a cost-effective approach is proposed to prepare a coal-derived HC anode with simple pre-oxidation followed by a post-carbonization process which effectively expands the d_(002)layer spacing,generates closed pores and increases defect *** these modifications,the resulting HC anode attains a delicate equilibrium between plateau capacity and sloping capacity,showcasing a remarkable reversible capacity of 306.3 mAh·g^(-1)at 0.03 A·g^(-1).Furthermore,the produ ced HC exhibits fast reaction kinetics and exceptional rate performance,achieving a capacity of 289 mAh·g^(-1)at 0.1 A·g^(-1),equivalent to~94.5%of that at 0.03 A·g^(-1).When implemented in a full cell configuration,the impressive electrochemical performance is evident,with a notable energy density of 410.6 Wh·kg^(-1)(based on cathode mass).In short,we provide a straightforward yet efficient method for regulating coal-derived HC,which is crucial for the widespread use of SIBs anodes.

提出了高吸水树脂(SAP)吸水率比指标,研究了吸水率比对SAP在水泥浆体离心液中的吸水动力学及内养护混凝土流变性能、抗压强度、收缩性能的影响。结果表明:SAP在水泥离心液中吸水率呈迅速增加、逐渐降低和最后稳定的规律;当SAP吸水率比增大时,吸水率峰值增大、释水时间延长。伴随SAP吸水率比逐渐增大,内养护混凝土拌合物屈服应力逐渐增大、塑性黏度逐渐降低。SAP可降低混凝土早期强度,对后期强度影响较小,且可明显降低混凝土收缩。在吸水率比为1.94~6.16范围内,SAP对混凝土抗压强度和收缩性能的作用规律一致。
