Peptic ulcer disease(PUD) affects 4 million people worldwide annually. The incidence of PUD has been estimated at around 1.5% to 3%. Perforated peptic ulcer(PPU) is a serious complication of PUD and patients with PPU often present with acute abdomen that carries high risk for morbidity and mortality. The lifetime prevalence of perforation in patients with PUD is about 5%. PPU carries a mortality ranging from 1.3% to 20%. Thirtyday mortality rate reaching 20% and 90-d mortality rate of up to 30% have been reported. In this review we have summarized the current evidence on PPU to update readers. This literature review includes the most updated information such as common causes, clinical features, diagnostic methods, non-operative and operative management, post-operative complications and different scoring systems of PPU. With the advancement of medical technology, PUD can now be treated with medications instead of elective surgery. The classic triad of sudden onset of abdominal pain, tachycardia and abdominal rigidity is the hallmark of PPU. Erect chest radiograph may miss 15% of cases with air under the diaphragm in patients with bowel perforation. Early diagnosis, prompt resuscitation and urgent surgical intervention are essential to improve outcomes. Exploratory laparotomy and omental patch repair remains the gold standard. Laparoscopic surgery should be considered when expertise is available. Gastrectomy is recommended in patients with large or malignant ulcer.

Analysis of Motor Synergies Utilization for Optimal Movement Generation for a Human-like Robotic Arm

Controlling human-like robots with musculoskeletal structure has been a challenging problem in engineering. In biological studies, motor synergy hypothesis has been proposed as a solution in order to control high degree-of-freedom and complex human body. In this paper, we focus on exploring the applicability of motor synergies in generating goal-directed movements by optimal control in a human-like robotic arm. We focus on three problems： 1） Can motor synergies facilitate the solving of optimal control problem？ 2） What properties should motor synergies have in order to achieve tasks？ 3） How should motor synergies be utilized better？ For the first problem we show that goal-directed movements can be achieved by utilizing motor synergies which have properties of achieving the goals. For the second problem, we testify motor synergies which have different properties and discover that energy efficiency is an important aspect in motor synergies which can also be utilized to achieve goal-directed movements. This discovery also implies that we can obtain motor synergies by other ways rather than from goal-directed optimal control signals. For the third problem, we show that the control complexity can be further reduced by utilizing a subset of motor synergies which are effective to achieve goals.

Recent experiments revealed that the dielectric dispersion spectrum of fission yeast cells in a suspension was mainly composed of two sub-dispersions. The low-frequency sub-dispersion depended on the cell length, while the high-frequency one was independent of it. The cell shape effect was simulated by an ellipsoidal cell model but the comparison between theory and experiment was far from being satisfactory. Prompted by the discrepancy, we proposed the use of spectral representation to analyze more realistic cell models. We adopted a shell-spheroidal model to analyze the effects of the cell membrane. It is found that the dielectric property of the cell membrane has only a minor effect on the dispersion magnitude ratio and the characteristic frequency ratio. We further included the effect of rotation of dipole induced by an external electric field, and solved the dipole-rotation spheroidal model in the spectral *** agreement between theory and experiment has been obtained.

This paper proposes a static-output-feedback based robust fuzzy wheelbase preview control algorithm for uncertain active suspensions with time delay and finite frequency ***,a Takagi-Sugeno(T-S)fuzzy augmented model is established to formulate the half-car active suspension system with consideration of time delay,sprung mass variation and wheelbase preview ***,in view of the resonation between human’s organs and vertical vibrations in the frequency range of 4–8 Hz,a finite frequency control criterion in terms of H∞norm is developed to improve ride ***,other mechanical constraints are also considered and satisfied via generalized H2 ***,in order to maintain the feasibility of the controller despite of some state variables are not online-measured,a two stage approach is adopted to derive a static output feedback ***,numerical simulation results illustrate the excellent performance of the proposed controller.

Experimental investigation was conducted to characterize the responses of pseudo-ductile cementitious composites (PDCCs) when subjected to uniaxial and biaxial *** PDCCs is a class of fiber reinforced cementitious composites with ultra-high ductility by using a low volume fraction (2%) of polyvinyl alcohol (PVA) *** different strength grades of PDCC were examined with cubic specimen size of 100 mm in the *** specimens were loaded with a servo-hydraulic jack at different stress *** principle stresses and strains of the specimens were recorded,and the failure modes with various stress states were *** test results indicated that the ultimate strength of PDCCs increased due to the lateral confinement in the other principal stress direction,and the maximum ultimate strength occurred at the biaxial stress ratio of 0.25,which was very different from common concrete *** the PDCC specimens,the biaxial strength may be lower than the uniaxial strength when subjected to biaxial compression with the stress ratio of 1.0,and the failure mode showed a shear-type failure because of the bridging effect of ***,a failure criterion was proposed for PDCCs under biaxial compression.

Objective： This paper aimed to review the current literature on the surface modification ofintraocular lenses （IOLs）. Data Sources： All articles about surface modification of IOLs published up to 2015 were identified through a literature search on both PubMed and ScienceDirect. Study Selection： The articles on the surface modification of 1OLs were included, but those on design modification and surface coating were excluded. Results： Technology of surface modification included plasma, ion beam, layer-by-layer self-assembly, ultraviolet radiation, and ozone. The main molecules introduced into IOLs surface were poly （ethylene glycol）, polyhedral oligomeric silsesquioxane, 2-methacryloyloxyethyl phosphorylcholine, TiO2, heparin, F-heparin, titanium, titanium nitride, vinyl pyrrolidone, and inhibitors of cytokines. The surface modification either resulted in a more hydrophobic lens, a more hydrophilic lens, or a lens with a hydrophilic anterior and hydrophobic posterior surface. Advances in research regarding surface modification of ｜OLs had led to a better biocompatibility in both in vitro and animal experiments. Conclusion： The surface modification is an efficient, convenient, economic and promising method to improve the biocompatibility of IOLs.

Objectives:*** examine the characteristics of patients with erectile dysfunction in a family physician led erectile dysfunction clinic;*** review association of chronic disease spectrum and erectile dysfunction;*** review initial treatment pattern and ***:Retrospective case series ***:All consecutive patients seen in a regional hospital family physician led erectile dysfunction clinic from April 2014 to March *** outcome measures:*** severity of erectile dysfunction,based on International Index of Erectile Function(IIEF-5).*** associated chronic comorbidities of *** patterns and patient ***:One hundred and eighty three patients presented with erectile dysfunction(ED)with mean age 58.7(range 23 to 82)years old were seen during the study *** hundred and twenty seven patients(69.4%)had comorbidity of chronic diseases,including 50.8%had hypertension,38.8%had diabetes mellitus and 33.9%had *** mean body mass index was 25.2 kg/m2,the mean blood pressure was 137.3/79.5 mm Hg(1 mm Hg=0.133 kPa).According to IIEF-5 score,50.3%,30.6%and 18.6%had severe,moderate and mild erectile dysfunction *** average duration of ED before seeking medical help was 3.9 *** 5(PDE5)inhibitors were prescribed to 119 patients(65%),and 57.1%of them achieved good *** nine patients(15.8%)were referred to other specialty for further management,including 27.6%had contraindication for PDE5 ***:High proportion of erectile dysfunction patients had comorbitiy of chronic dis-eases.57.1%of those patients receiving PDE5 inhibitors showed good response.
