AIM： To study the association of three common ABCB11 and ABCC2 polymorphisms （ABCB11： 1331T〉C→V444A; ABCC2： 3563T〉A → V1188E and 4544G 〉A → C1515Y） with intrahepatic cholestasis of pregnancy （ICP） and contraceptive-induced cholestasis （CIC）. METHODS： ABCB11 and ABCC2 genotyping data were available from four CIC patients and from 42 and 33 ICP patients, respectively. Allele-frequencies of the studied polymorphisms were compared with those in healthy pregnant controls and Caucasian individuals. Furthermore, serum bile acid levels were correlated with the presence or absence of the 1331 C allele. RESULTS： The ABCB11 1331T〉C polymorphism was significantly more frequent in cholestatic patients than in pregnant controls： C allele 76.2% （CI, 58.0-94.4） vs 51.3% （CI 35.8-66.7）, respectively （P = 0.0007）; and CC allele 57.1% （CI 36.0-78.3） vs 20% （CI 7.6-32.4）, respectively （P = 0.0065）. All four CIC patients were homozygous carriers of the C allele. In contrast, none of the studied ABCC2 polymorphism was overrepresented in ICP or CIC patients. Higher serum bile acid levels were found in carriers of the 1331CC genotype compared to carriers of the TT genotype. CONCLUSION： Our data support a role for the ABCB11 1331T〉C polymorphism as a susceptibility factor for the development of estrogen-induced cholestasis, whereas no such association was found for ABCC2. Serum bile acid and 7-glutamyl transferase levels might help to distinguish ABCB4- and ABCB11-related forms of ICP and CIC.

Combination of "low-dose" ribavirin and interferon alfa-2a therapy followed by interferon alfa-2a monotherapy in chronic HCV-infected nonresponders and relapsers after interferon alfa-2a monotherapy

AIM To report on the efficacy, safety and tolerability of interferon alfa-2a combined with a "low dose" of ribavirin for relapsers and non responders to alpha interferon *** Thirty-four chronic hepatitis C virus-infected non-responders to interferon alfa2a monotherapy (a course of at least 3 months treatment) and 13 relapsers to interferon alfa 2a monotherapy (a dose of 3 to 6 million units three times per week for at least 20 weeks but not more than 18 months) were treated with the same dose of interferon alfa-2a used before (3 to 6 million units three times per week) and ribavirin (10 mg/ kg daily) for 6 months. In complete responders, interferon alfa-2a was administered for further 6 months at the same dose used before as *** Seven (20.6%) of 34 non-responders stopped the combined therapy due to adverse events, including two patients with histological and clinical Child A cirrhosis. In 17/27 (63%)non-responders, the combined therapy was stopped after three months because of non-response. Ten of the 27 non-responders completed the 1;2-month treatment course. At a mean follow up of 28 months (16- 37 months)after the treatment, 4/10 (15%) previous non-responders still remained complete responders,All 13 previous relapsers completed the 12-month treatment course. While public-private partnerships(PPPs)have surged worldwide since the 1990s,they have been met with growing skepticism during the last years.A recent revision of Germany's constitutional rules on motorway construction and observations on the use of PPPs published by both the German and the European Courts of Auditors illustrate this new *** two examples fit into a general trend towards a revival of the public sector,which can be summarized under the cross-level umbrella term"publicization."It would,however,be remiss to replace the uncritical euphoria that once surrounded privatization with a similarly undifferentiated euphoria regarding ***,it is crucial to identify the most appropriate solution for the fulfilment of each public task from the"toolbox"of publicization on the one hand and privatization on the other hand in order to ensure the most effective completion of public functions.

HCV-RNA positivity in peripheral blood mononuclear cells of patients with chronic HCV-infection: does it really mean viral replication?

AIM: To analyze the association of HCV-RNA with peripheral blood mononuclear cells (PBMC) and to answer the question whether HCV-RNA positivity in PBMC is due to viral replication. METHODS: HCV-RNA was monitored in serum and PBMC preparations from 15 patients with chronic HCV infection before, during and after an IFN-alpha therapy using a nested RT/PCR technique. In a second approach, PBMC from healthy donors were incubated in HCV positive plasma. RESULTS: In the IFN-alpha responding patients,HCV-RNA disappeared first from total RNA preparations of PBMC and then from serum. In contrast, in relapsing patients, HCV-RNA reappeared first in serum and then in PBMC. A quantitative analysis of the HCV-RNA concentration in serum was performed before and after transition from detectable to non detectable HCV-RNA in PBMC-RNA and vice versa. When HCV-RNA was detectable in PBMC preparations, the HCV concentration in serum was significantly higher than the serum HCV-RNA concentration when HCV-RNA in PBMC was not detectable. Furthermore, at no time during the observation period was HCV specific RNA observed in PBMC, if HCV-RNA in serum was under the detection limit. Incubation of PBMC from healthy donors with several dilutions of HCV positive plasma for two hours showed a concentration dependent PCR positivity for HCV-RNA in reisolated PBMC. CONCLUSION: The detectability of HCV-RNA in total RNA from PBMC seems to depend on the HCV concentration in serum. Contamination or passive adsorption by circulating virus could be the reason for detection of HCV-RNA in PBMC preparations of chronically infected patients.

Comparison of Cardiovascular Parameters and Internal Training Load of Different 1-h Training Sessions in Non-elite CrossFit^(®)Athletes

Purpose The fact that CrossFit^(®)is the best-known and rapidly growing concept for high-intensity interval training(HIIT)and high-intensity functional training(HIFT)results in a continuous increase of athletes performing CrossFit^(®).In the more than 15,000 CrossFit^(®)Affiliates worldwide,the training concept is usually offered in 1-h training sessions containing the CrossFit^(®)-related workout of the day(WOD),as well as a general warm-up,movement demonstrations,and skill ***,we report how physiological parameters measured by heart rate(HR)values vary during four different 1-h CrossFit^(®)training sessions of non-elite athletes(n=27)in a local affiliated training center and what influencing factors may *** The duration of the 1-h training sessions were divided into a warm-up part(WU-part),a skill development part combined with strength exercises(A-part),followed by the WOD part(B-part).Results Analysis of HR values shows high training intensity(≥91%HR_(max))not throughout the duration of each training session,only during *** mean HR values in B-part differ significantly compared to the remaining training parts(P<0.001)for all four training *** of different CrossFit^(®)experience levels revealed no significant differ-ence in acute physiological demands and training load between beginner and experienced CrossFit^(®)*** Our results may suggest that practicing CrossFit^(®)in 1-h training sessions combined anaerobic and aerobic exercise intensities,with the training concept allows beginners and experienced athletes to be trained with the same cardio-vascular responses and training intensities.

Novel technique for endoscopic en bloc resection(EMR+)-Evaluation in a porcine model

BACKGROUND Endoscopic en bloc resection of larger polyps is relevant because risk of advanced neoplasia or malignancy correlates with tumor size. Recurrence rates after piecemeal endoscopic mucosal resection (EMR) are high and endoscopic submucosal dissection (ESD) is associated with higher complication rates in the western world. AIM To develop a modified endoscopic en bloc resection technique using an external additional working channel and novel agent for submucosal injection. METHODS EMR+ was considered as modified grasp and snare technique. For simultaneous use of a grasping and cutting device a novel additional working channel was used (AWC■, Ovesco Endoscopy, Tübingen, Germany). AWC■ is installed on the outer surface of the endoscope, covered with a plastic sleeve and designed for single use. For submucosal injection a new agent consisting of poloxamers was used (LiftUp■, Ovesco Endoscopy, Tübingen, Germany). The agent is liquid at room temperature and forms a stable and permanent gel cushion after injection. Safety of LiftUp? has been shown in a pre-clinical study in domestic pigs. LiftUp■ is commercially not yet available but approval is expected in early 2019. EMR+ was first developed ex vivo (explanted pig stomach) and subsequently evaluated in vivo (stomach, porcine model, 3 domestic pigs). Main outcome measurements were: Procedure time, macroscopic en bloc resection and adverse events. RESULTS Concept of EMR+ was first developed ex vivo (explanted pig stomach). Ex vivo, 22 resections were performed after technique was established. Median procedure time (measured from begin of injection to extraction of resection specimen) was 7 min (range 5-11, SD 1.68) and median size of resection specimens was 30 mm × 26 mm × 11 mm ex vivo. Subsequently 13 resections were performed in vivo (stomach, porcine model, 3 domestic pigs). Complexation Selectivities of Pillar[5]arenes with Primary Ammonium Salts

The complexation of alkyl-substituted pillar[5]arenes with primary ammonium salts is investigated. 1,4-Bis- （methoxy）pillar[5]arene （MeP5） can form strong complexes with the primary ammonium salts in CDC13. However, 1,4-bis（ethoxy）pillar[5]arene （EtP5） shows weak interaction with these guests, and 1,4-bis（butoxy）pillar[5]arene （BuP5） can not form such a complex at all. These results indicate that the modified alkyl chains of pillar[5]arene play an important role in the complexation selectivity.
