Ventriculo-atrial shunt with occlusion of the internal jugular vein: operative experience and surgical technique

Background Ventricular shunts are one of the most frequent techniques used for the management of hydrocephalus. The ventriculoperitoneal shunt (VPS) is the most commonly performed procedure, and the ventriculoatrial shunt (VAS) is the second option in most medical centers. The main objective of this study is to introduce and describe a surgical approach for VAS outlining our experience and comparing it with traditional shunting *** In this retrospective cohort comparison study, we included patients with hydrocephalus treated with a surgical procedure between January 2010 and February 2021 at a single academic institution. We categorized the procedures into two groups: patients with VPS and conventional VAS grouped together into the conventional technique (cT) group, and the second group was patients with whom we performed VAS with complete internal jugular vein occlusion (IJVOT). We compared the surgical time, postoperative complications, and occurrence of shunt failure among the groups by performing univariate analysis using the Fisher exact *** Out of the 106 patients included in the analysis, IJVOT was performed in 66 patients, and cT in 40 patients. The median surgical time was 60 min (IQR 60-90) for IJVOT versus 100 min (IQR 60-120) for cT (p cedure, 83.3% of patients with IJVOT and 62.5% of patients with cT did not require shunt removal or shunt revision (p cT while 1.5% and 2.5% for IJVOT at 1 and 6 months after the *** Our findings demonstrate that VAS with IJVOT is a safe method that exhibited shorter surgical times and outcomes comparable to cT. The search for new biomarkers predictive of type 2 diabetes currently constitutes a research avenue in Bioclinical. Total homocysteine remains a preferred target due to its involvement in the occurrence of degenerative complications in type 2 diabetics. The aim of this work was to study hyperhomocysteinemia and other biochemical markers associated with T2D in the congolese population. This was an analytical case-control study carried out between October 2022 and October 2023. The study population consisted of 150 subjects including 100 T2D patients and 50 control subjects. The main clinical data were collected on a pre-established form. Homocysteine determination was carried out by the sandwich ELISA method. The other biochemical markers were measured by colorimetric enzymatic methods. Hyperhomocysteinemia was present in 27.3% (41/150) of the entire study population. Type 2 diabetics had a frequency of hyperhomocysteinemia of 36% (36/100) and control 10% (5/50) (p = 0.001). The mean hyperhomocysteinemia concentration was 31.9 μmol/l with extremes ranging from 18 to 103 μmol/l. Means of biological markers between diabetics and controls showed a statistically significant difference (p = 0.01). The risk factors associated with this HHcy were: sex (OR = 3.5), age (OR = 9.4), sedentary lifestyle (OR = 3.4) and glycosylated hemoglobin (OR = 12) with a p-value ctively. Our results suggest that hyperhomocysteinemia can be considered as a predictive biomarker in the bioclinic of congolese type 2 diabetic patients.

Seismic attribute and well-log analysis for channel characterization in the upper San andres and Grayburg formations of the Midland Basin,Texas

The Permian Basin is one of the most prolific,and currently one of the most active,oil and gas basins in the *** Lower Permian strata in the Permian Basin have produced more than 14 billion barrels of oil(BBO),making it the largest volume of hydrocarbon in the *** in the Midland Basin during late Leonardian through early Guadalupian(ca.272e269 Ma)resulted in progradation of shelf edge and ultimately closure of the basin by Middle Permian *** analyzed a merged seismic survey covering parts of the Permian Basin(i.e.,central Basin Platform and Midland Basin)in Andrews,Ector,and Midland counties,*** seismic survey and well logs show the presence of gently dipping(ca.1°)clinoforms in the Upper San andres and Grayburg Formations on the eastern edge of the central Basin Platform and western Midland *** seismic attributes,curvature,and spectral decomposition identify low sinuosity slope channels oriented north-south,but such channels do not appear beyond the *** shelf edge shifts from north to south during deposition of the Upper San andres and Grayburg *** identify five basinward shifts noted by the migration of the shelf edge toward the basin center and the presence of channel features along the depositional *** petrophysical analysis indicates that channels cut into carbonate rocks and are filled by siliciclastic sediments;this interpretation is supported by the most negative curvature anomalies along the channel axes caused by the differential compaction between the carbonate and siliciclastic *** are a few channels with a northwest-southeast strike,which matches the direction of the concho Lineament observed by satellite *** observations are consistent with previous interpretations of the northern Midland Basin closure during Middle-Late Permian time.

A lattice Boltzmann exploration of two-phase displacement in 2D porous media under various pressure boundary conditions

Effects on physiologic measures of appetite from intragastric balloon and endoscopic sleeve gastroplasty: results of a prospective study

Background: Endoscopic bariatric therapies can help address widening management gaps in obesity. Their ability to facilitate weight loss is largely tied to influences on appetite through perturbations of gastric emptying and accommodation. As these tools gain traction in obesity therapy, their physiologic underpinnings require exploration, which may enhance efficacy, tolerance, and patient-tailored ***: We prospectively assessed consecutive subjects with fluid-filled intragastric balloons (IGBs) (n = 18) placed between October 2016 and June 2017 or underwent endoscopic sleeve gastroplasty (ESG) (n = 23) from March 2018 to June 2018. Patients underwent physiologic appraisal at 3 months with13c-spirulina-based gastric emptying breath test to determine time to half emptying (T50), as well as maximum tolerated volume (MTV) of a standard nutrient drink test. changes in T50 and MTV at 3 months were compared with percent total body weight loss (%TBWL) at 3 and 6 months using best-fit linear ***: The change in T50 at 3 months correlated with %TBWL at 3 months for IGB (P = 0.01) and ESG (P = 0.01) but with greater impact on %TBWL in IGB compared to ESG (R2 = 0.42vs. 0.26). change in T50 at 3 months was predictive of weight loss at 6 months for IGB (P = 0.01) but not ESG (P = 0.11). ESG was associated with greater decrease in MTV compared to IGB (340.25 ± 297.97 mLvs. 183.00 ± 217.13 mL,P = 0.08), indicting an enhanced effect on satiation through decreased gastric accommodation. changes in MTV at 3 months did not correlate with %TBWL for either IGB (P = 0.26) or ESG (P = 0.49) but trended toward significance for predicting %TBWL at 6 months for ESG (P = 0.06) but not IGB (P = 0.19).conclusion: IGB and ESG both induce weight loss but likely through distinct gastric motor function phenotypes, and gastric emptying may predict future weight loss in patients with IGB.
