Liver transplantation and multivisceral transplantation in the management of patients with advanced neuroendocrine tumours

Orthotopic liver transplantation(OLT) represents a generally accepted albeit somewhat controversially discussed therapeutic strategy in highly selected patients with non-resectable hepatic metastases from neuroendocrine tumours(NET). Whilst there are some exclusion criteria, these are not universally followed, and the optimal set of inclusion parameters for deeming patients eligible has not yet been elucidated. This is due to heterogeneity in the study populations, as well differing approaches employed and also divergences in selection criteria between centres. Recent data have suggested that OLT may represent the most efficacious approach in terms of overall and disease-free survival to the management of NET metastatic to the liver when conducted in accordance with the modified Milan criteria. Therefore, a consensus set of selection criteria requires definition to facilitate stringent and fair allocation of deceased-donor organs, as well as consideration for living-donor organs. In the context of classically non-resectable metastatic tumour bulk, multivisceral transplantation with or without the liver may also be indicated, yet experience is very limited. In this review, we discuss the diagnostic work-up of patients in whom the aforementioned transplantation approaches are being considered, critically analyse the published experience and also anticipate future developments in this field, including a discussion of immediate and longer-term research priorities.

Owing to the far-reaching environmental consequences of agriculture and food systems,such as their contribution to climate change,there is an urgent need to reduce their *** and national governments set sustainability targets and implement corresponding ***,critics of the globalized system claim that a territorial administrative scale is better suited to address sustainability ***,at the subnational level,local authorities rarely apply a systemic environmental assessment to enhance their action *** paper employs a territorial life cycle assessment methodology to improve local environmental agri-food *** objective is to identify significant direct and indirect environmental hotspots,their origins,and formulate effective mitigation *** methodology is applied to the administrative department of Finistere,a strategic agricultural region in North-Western *** environmental criteria including climate change,fossil resource scarcity,toxicity,and land use are *** findings reveal that the primary environmental hotspots of the studied local food system arise from indirect sources,such as livestock feed or diesel *** reduction and organic farming conversion emerge as the most environmentally efficient strategies,resulting in a 25%decrease in the climate change ***,the overall modeled impact reduction is insufficient following national objectives and remains limited for the land use *** results highlight the innovative application of life cycle assessment led at a local level,offering insights for the further advancement of systematic and prospective local agri-food ***,they provide guidance for local authorities to enhance the sustainability of planning strategies.

In the present review we discuss the recent developments and future directions in the multimodal treatment of locally advanced rectal cancer, with respect to staging and re-staging modalities, to the current role of neoadjuvant chemo-radiation and to the conservative and more limited surgical approaches based on tumour response after neoadjuvant combined therapy. When initial tumor staging is considered a high accuracy has been reported for T pre-treatment staging, while preoperative lymph node mapping is still suboptimal. With respect to tumour re-staging, all the current available modalities still present a limited accuracy, in particular in defining a complete response. The role of short vs long-course radiotherapy regimens as well as the optimal time of surgery are still unclear and under investigation by means of ongoing randomized trials. Observational management or local excision following tumour complete response are promising alternatives to total mesorectal excision, but need further evaluation, and their use outside of a clinical trial is not *** preoperative selection of patients who will benefit from neoadjuvant radiotherapy or not, as well as the proper identification of a clinical complete tumour response after combined treatment modalities,will influence the future directions in the treatment of locally advanced rectal cancer.

Mo Yan＇s historical novel Sandalwood Death revisits the Boxer Uprising, exploring a local structure of feeling from the point of view of oral transmissions that, one hundred years after the events, appears gradually to e receding into oblivion. It is a project of recuperation or, rather, aesthetic reconstruction of local knowledge. The staging of a variety of local performances, such as Maoqiang opera, seasonal festivals, military and religious parades, as well as of scenes of excessive violence in executions and battle scenes, appears to be a strategy for the cultural reclamation of these local experiences. The story challenges the ingrained dualism between foreign, modern imperialist and nationalist forms of rationality, and pre-modem, local patterns of beha- viour and thought. Employing polyphony and multivalent historical represent- tations, the novel aspires to portray the social dynamics in a given geo- historical circumstances by measuring the spatiotemporal as well as the cognitive distance between the witnessed event, the testifying witness and the future receivers of the transmitted stories. Thus, the inquiry does not focus on the historical events as facts, but rather on their cultural afterlife in （founding） narratives. In times of a growing gap between the modernist vision of human liberation and the actual conditions of growing inequality, delegitimization and dispossession, this tale of unrest in the wake of globalization has as much to say about the world＇s peoples around the year 2000, when the novel was published, as about the microcosm of Shandong Gaomi County around the year 1900, when the historical events took place. Taking into account that the novel was written as a local Maoqiang opera in the making and that theatres are major providers of cultural space for the enactment of the human self as the subject of history, Sandalwood Death can perhaps best be described as a theatre of reclamation.

The control of thermal expansion is essential in applications where thermal stability is required from fiber optics coatings,high performance fuel cell cathodes to tooth *** thermal expansion(NTE)materials,although rare,are fundamental for this *** work focuses on studying tetracyanidoborate salt CuB(CN)_(4),an interesting cubicstructure material that displays large isotropic NTE.A joint study of synchrotron x-ray diffraction,temperature-dependent Raman spectroscopy,and lattice dynamics calculations was conducted,showing that not only low-frequency optical modes(transverse thermal vibrations of N and C atoms)but also the acoustic modes(the vibrations of Cu atoms as a collective torsion of the neighboring atoms),contribute to *** a result,new insights were gained into the NTE mechanism of CuB(CN)_(4) and related framework materials.

Endoluminal vacuum-assisted therapy(EVT)has been introduced recently to treat colorectal anastomotic leaks in clinically stable non-peritonitic *** application has been mainly reserved to low colorectal and colo-anal *** main advantage of this new procedure is to ensure continuous drainage of the abscess cavity,to promote and to accelerate the formation of granulation tissue resulting in a reduction of the abscess *** reported results are promising allowing a higher preservation of the anastomosis when compared to conventional treatments that include trans-anastomotic tube placement,percutaneous drainage,endoscopic clipping of the anastomotic defect or stent ***,despite this procedure is gaining acceptance among the surgical community,indications,inclusion criteria and definitions of success are not yet standardized and extremely heterogeneous,making it difficult to reach definitive conclusions and to ascertain which are the real benefits of this new ***,long-term and functional results are poorly *** present review is focused on critically analyzing the theoretical benefits and risks of the procedure,short-and long-term functional results and future direction in the application of EVT.

This paper describes scientific research conducted to highlight the potential of an integrated GPR-UAV system in engineering-geological *** analysis focused on the stability of a natural scree slope in the Germanasca Valley,in the western Italian *** a consequence of its steep shape and the related geological hazard,the study used different remote sensed methodologies such as UAV photogrammetry and geophysics survey by a GPR-drone integrated ***,conventional in-situ surveys led to the collection of geological and geomorphological *** use of the UAV-mounted GPR allowed us to investigate the bedrock depth under the detrital slope deposit,using a non-invasive technique able to conduct surveys on inaccessible areas prone to hazardous conditions for *** collected evidence and the results of the analysis highlighted the stability of the slope with Factors of Safety,verified in static conditions(i.e.,natural static condition and static condition with snow cover),slightly above the stability limit value of *** the contrary,the dynamic loading conditions(i.e.,seismic action applied)showed a Factor of Safety below the stability limit *** UAV-mounted GPR represented an essential contribution to the surveys allowing the definition of the interface debris deposit-bedrock,which are useful to design the slope model and to evaluate the scree slope stability in different conditions.
