Situated at the intersection of technology and medicine,the Internet of Things(IoT)holds the promise of addressing some of healthcare's most pressing challenges,from medical error,to chronic drug shortages,to overburdened hospital systems,to dealing with the COVID-19 ***,despite considerable recent technological advances,the pace of successful implementation of promising IoT healthcare initiatives has been *** inspire more productive collaboration,we present here a simple—but surprisingly underrated—problemoriented approach to developing healthcare *** further assist in this effort,we reviewed the various commercial,regulatory,social/cultural,and technological factors in the development of the *** propose that fog computing—a technological paradigm wherein the burden of computing is shifted from a centralized cloud server closer to the data source—offers the greatest promise for building a robust and scalable healthcare IoT *** this end,we explore the key enabling technologies that underpin the fog architecture,from the sensing layer all the way up to the *** is our hope that ongoing advances in sensing,communications,cryptography,storage,machine learning,and artificial intelligence will be leveraged in meaningful ways to generate unprecedented medical intelligence and thus drive improvements in the health of many people.

In recent years, seasonal blooms of the dinoflagellate Noctiluca miliaris have appeared in the open-waters of the northern Arabian Sea （NAS）. This study provides the first characterization of bacteria from a seasonal bloom of green Noctiluca ofNAS （20°N-17°N and 64°E-70°E）, during the spring-inter-monsoon cruise ofSagar Sampada 253, in March 2007. Bacterial growth as assessed by most-probable number （MPN） and plate counts, revealed ＇variable-physiotypes＇ over a wide range of salinities （0%-25% w/v NaC1）, pH levels （5-8.5）, and organic nutrient strengths, in comparison to non-bloom waters. MPN indices of bacteria in surface waters of bloom stations ＊DWK and ＊PRB, corresponded to （3.08-4.41）×103 cells/mL at 3.5% NaC1 （w/v）, and （2.82-9.49）× 102 cells/mL at 25% （w/v） NaC1 in tryptone-yeast extract broth （TYE）. Plate counts were （1.12-4） × 106 CFU/mL at 0% （w/v） NaC1, （1.28-3.9） × 106 CFU/mL at 3.5% （w/v） NaC1, and （0.4- 7）×104 CFU/mL at 25% NaC1 （w/v） on TYE. One-tenth-strength Zobell＇s gave （0.6-3.74）× 105 CFU/mL at pH 5 to （3.58-7.5）× 105 CFU/mL at pH 8.5. These bacteria were identified to the genera Bacillus, Cellulomonas, Staphylococcus, Planococcus, Dietzia, Diergibacillus, Micrococcus, Sporosarcinae, Leucobacter, and Halomonas. The identity of three strains （GUFBSS253N2, GUFBSS253N30, and GUFBSS253N84） was confirmed through 16S rDNA sequence homology as Bacillus cohnii, Bacillusflexus, and Bacillus cereus. The -2-3-fold higher plate counts of culturable bacteria from the open-waters of the NAS indicate that these bacteria could critically determine the biogeochemical dynamics of the bloom and its milieu. The role of these bacteria in sustaining/terminating the bloom is under evaluation.

Use of Targeted Orographic Smoothing in Very High Resolution Simulations of a Downslope Windstorm and Rotor in a Sub-tropical Highland Location

Nested simulations of a downslope windstorm over Cangshan mountain,Yunnan,China,have been used to demonstrate a method of topographic smoothing that preserves a relatively large amount of terrain detail compared to typical smoothing procedures required for models with terrain-following grids to run *** simulations were carried out using the Met Office Unified Model(MetUM)to investigate downslope *** smoothing method seamlessly blends two terrain datasets to which uniform smoothing has been applied—one with a minimum of smoothing,the other smoothed more heavily to remove gradients that would cause model *** latter dataset dominates the blend where the steepest slopes exist,but this is localised and recedes outside these *** a result,increased detail is starkly apparent in depictions of flow simulated using the blend,compared to one using the default *** includes qualitative flow details that were absent in the latter,such as narrow shooting flows emerging from roughly 1-2 km wide leeside *** separation is more common due to steeper lee *** use of targeted smoothing also results in increased lee side temporal variability at a given point during the windstorm,including over flat ***-/high-pass filtering of the wind perturbation field reveals that relative spatial variability above 30 km in scale(reflecting the background flow)is similar whether or not targeting is *** this scale,when smoothing is targeted,relative flow variability decreases at the larger scales,and increases at lower *** seems linked to fast smaller scale flows disturbing more coherent flows(notably an along-valley current over Erhai Lake).Spatial variability of winds in the model is unsurprisingly weaker at key times than is observed across a local network sampling mesoscale variation,but results are compromised due to relatively few observation locations sampling the *** when targeted smoothing is applied doe

We describe an operator splitting technique based on physics rather than on dimension for the numerical solution of a nonlinear system of partial differential equations which models three-phase flow through heterogeneous porous *** model for three-phase flow considered in this work takes into account capillary forces,general relations for the relative permeability functions and variable porosity and permeability *** our numerical procedure a high resolution,nonoscillatory,second order,conservative central difference scheme is used for the approximation of the nonlinear system of hyperbolic conservation laws modeling the convective transport of the fluid *** scheme is combined with locally conservative mixed finite elements for the numerical solution of the parabolic and elliptic problems associated with the diffusive transport of fluid phases and the pressure-velocity *** numerical procedure has been used to investigate the existence and stability of nonclassical shock waves(called transitional or undercompressive shock waves)in two-dimensional heterogeneous flows,thereby extending previous results for one-dimensional flow *** experiments indicate that the operator splitting technique discussed here leads to computational efficiency and accurate numerical results.

Segmentation of Diabetic Retinopathy Lesions: The Common Fallacy and Evaluation of Real Segmenters

In the context of automated analysis of eye fundus images, it is an important common fallacy that prior works achieve very high scores in segmentation of lesions, and that fallacy is fueled by some reviews reporting very high scores, and perhaps some confusion with terms. A simple analysis of the detail of the few prior works that really do segmentation reveals scores between 7% and 70% in sensitivity for 1 FPI. That is clearly sub-par with medical doctors trained to detect signs of Diabetic Retinopathy, since they can distinguish well the contours of lesions in Eye Fundus Images (EFI). Still, a full segmentation of lesions could be an important step for both visualization and further automated analysis using rigorous quantification or areas and numbers of lesions to better diagnose. I discuss what prior work really does, using evidence-based analysis, and confront with segmentation networks, comparing on the terms used by prior work to show that the best performing segmentation network outperforms those prior works. I also compare architectures to understand how the network architecture influences the results. I conclude that, with the correct architecture and tuning, the semantic segmentation network improves up to 20 percentage points over prior work in the real task of segmentation of lesions. I also conclude that the network architecture and optimizations are important factors and that there are still important limitations in current work.

Revascularization for Iliac-femoral Artery Pseudoaneurysm with Greater Saphenous Vein

Objective To investigate the role of revascularization procedures with autologous greater saphenous vein in surgical management of iliac-femoral artery pseudoaneurysm in parenteral drug abusers. Methods Twenty-one patients with iliac-femoral artery pseudoaneurysm caused by parenteral drug abuse from 2004 to 2007 were enrolled. Among them,15 patients were male and 6 were female; their average age was 31.3 years. The size of pseudoaneurysms ranged from 3.0 cm to 7.5 cm. Common femoral artery and distal external iliac artery were often involved. We performed arterial reconstruction on these patients with autologous greater saphenous vein as a graft after excising iliac-femoral artery pseudoaneurysm through a single curved inguinal incision. All patients were followed up,and the complications were recorded. Results The surgical procedures were finished without intraoperative mortality or perioperative complications. All patients were free of claudication symptoms after the surgery except one case with preoperative popliteal artery stenosis. One case of infection and wound tissue fistula was found later. One case had inguinal incisional hematoma and another complained of numbness in thigh skin. Conclusions The use of autologous greater saphenous venous grafts for arterial reconstruction after pseudoaneurysm excision in drug abusers is safe and effective. This technique offers more advantages than arterial ligation alone without revascularization. An optimal greater saphenous venous graft is a prerequisite for revascularization.
