many azoospermic men do not possess mature spermatozoa at the time of surgical sperm *** study is a systematic review and meta-analysis evaluating outcomes following round spermatid injection(ROSI),a technique which utilizes immature precursors of spermatozoa for *** electronic search was performed to identify relevant articles published through October *** cohort studies in English involving male patients who had round spermatids identified and used for fertilization with human oocytes were *** rate,pregnancy rate,and resultant delivery rate were assessed following ***-analysis outcomes were analyzed using a random-effects *** were extracted from 22 studies involving 1099 couples and 4218 embryo *** fertilization rate after ROSI was 38.7%(95%confidence interval[CI]:31.5%–46.3%),while the pregnancy rate was 3.7%(95%CI:3.2%–4.4%).The resultant delivery rate was low,with 4.3%of embryo transfers resulting in a delivery(95%CI:2.3%–7.7%).The pregnancy rate per couple was 13.4%(95%CI:6.8%–19.1%)and the resultant delivery rate per couple was 8.1%(95%CI:6.1%–14.4%).ROSI has resulted in clinical pregnancies and live births,but success rates are considerably lower than those achieved with mature *** this technique may be a feasible alternative for men with azoospermia who decline other options,couples should be aware that the odds of a successful delivery are greatly diminished and the prognosis is relatively poor.

Serological biomarkers in inflammatory bowel disease (IBD) are a rapidly expanding list of non-invasive tests for objective assessments of disease activity, early diagnosis, prognosis evaluation and surveillance. This review summarizes both old and new biomarkers in IBD, but focuses on the development and character-ization of new serological biomarkers (identifi ed since 2007). These include fi ve new anti-glycan antibodies, anti-chitobioside IgA (ACCA), anti-laminaribioside IgG (ALCA), anti-manobioside IgG (AmCA), and antibod-ies against chemically synthesized (∑) two major oligomannose epitopes, man α-1,3 man α-1,2 man (∑man3) and man α-1,3 man α-1,2 man α-1,2 man (∑man4). These new biomarkers serve as valuable complementary tools to existing biomarkers not only in differentiating Crohn's disease (CD), ulcerative colitis (UC), normal and other non-IBD gut diseases, but also in predicting disease involvement (ileum vs colon), IBD risk (as subclinical biomarkers), and disease course (risk of complication and surgery). Interestingly, the prevalence of the antiglycan antibodies, including anti-Saccharomyces cerevisiae antibodies (ASCA), ALCA and AmCA, was found to be associated with single nucleotide polymorphisms (SNPs) of IBD susceptible genes such as NOD2/CARD15, NOD1/CARD4, toll-likereceptors (TLR) 2 and 4, and β-defensin-1. Further-more, a gene dosage effect was observed: anti-glycan positivity became more frequent as the number of NOD2/CARD15 SNPS increased. Other new serum/ plasma IBD biomarkers reviewed include ubiquitination factor E4A (UBE4A), CXCL16 (a chemokine), resistin, and apolipoprotein A-IV. This review also discusses the most recent studies in IBD biomarker discovery by the application of new technologies such as proteomics, fourier transform near-infrared spectroscopy, and mul-tiplex enzyme-linked immunosorbent assay (ELISA)'s (with an emphasis on cytokine/chemokine profiling). Starting with philip ***’s 1997 essay on“critical technical practice”,we consider examples of writings from computer science where authors describe“waking up”from a previously narrow technical approach to the world,enabling them to recognize how their previous efforts towards social change had been *** use these examples first to talk about the underlying assumptions of a technology-centric approach to social problems,and second to theorize these awakenings in terms of Paulo Freire’s idea of critical ***,understanding these awakenings among technical practitioners as examples of this more general phenomenon gives guidance for how we might encourage and guide critical awakenings in order to get more technologists working effectively towards positive social change.

Peripheral nerve injuries(PNI)in the lower extremity are an uncommon but highly morbid *** advances in our understanding of nerve physiology and microsurgical techniques have inspired renewed faith in nerve surgery and sparked a creative renaissance in the tools,approaches,and reconstructive schemas available to surgeons in the management of lower extremity *** this article,we review the literature and provide a principles-based approach for the surgical management of lower extremity PNIs with an emphasis on techniques for functional reconstruction after complete nerve *** principles in management include early diagnosis with electrodiagnostics and imaging,early surgical exploration,and opting for nerve and tendon transfers when primary reconstruction of the injured nerve is unfavorable(e.g.,delayed reconstruction,unavailability of proximal or distal nerve stumps,or long regenerative distance).The goal of functional reconstruction should be to restore independent gait,so understanding the roles of major neuromuscular units during the gait cycle informs the selection of donor nerves and tendons for *** on these principles and literature to date,specific algorithms for surgical management are presented for femoral,sciatic,tibial,and common peroneal *** recognize limitations of the current literature,namely the predominance of case series evidence,and call for the accrual of more patient data in surgical management of PNIs.
