BACKGROUND: In recent years, MR techniques have been widely used for displaying hepatic vessels and measuring its hemodynamics, especially in the management of the patients with portal hypertension. The aim of this study was to eva- luate the role of magnetic resonance angiography (MRA) in the treatment of patients with portal hypertension. METHODS: A series of 38 patients with portal hypertension and 12 control patients were enrolled in this study. Three dimensional dynamic contrast-enhanced (3D-DCE) MRA and two dimensional phase-contrast (2D-PC) MR were used for the study of portal venous anatomy and its hemo- dynamics, which were compared with those of indirect portal-venography (IPVG) and doppler ultrasonography (DUS). RESULTS: The anatomical imaging of the portal venous system could be displayed clearly in 3D-DCE MRA and the imaging quality of MRA was better than that of IPVG. The hemodynamic information from 2D-PC MR was closely re- lated to that from DUS. CONCLUSIONS: As a non-invasive technique, MRA can display the anatomy of the portal venous system clearly and measure its hemodynamics exactly. It is very useful and can be applied if necessary in the management of patients with portal hypertension.

BACKGROUND: Signal regulatory protein alpha1 (Sirpα1) is a negative regulatory factor, and inhibits receptor tyro- sine kinase-dependent cell proliferating signal. This study was undertaken to observe the effect of signal regulatory proteinα1 ( Sirpα1) on gankyrin, cyclin D1, CDK4 and Fas expression in Sk-hep1 mouse hepatoma carcinoma cell line. METHODS: BOSC 23 packed cells were respectively trans- fected by means of recombinated retrovirus including pLX- SN, pLXSN-Sirpα1 and pLXSN-Sirpα1Δ4Y2 with lipofec- tin, and various plasmid virus media (viral titer 2.1 × 106 CFU/ml) were collected and infected respectively in 80% confluent Sk-hepl cells. Transfected Sk-hep1 cells were se- lectively screened with G418 (1200 μg/ml), and Sk-hep1 cell lines transfected with various plasmids were obtained. The protein expressions of gankyrin, cyclin D1, CDK4 and Fas in various Sk-hep1 lines were determined by Western blotting. Various Sk-hep1 lines were recovered to culture with 10% fetal bovine serum at 12 hours and 24 hours after starving culture with free serum for 72 hours, and cells were collected to determine the percentage of S phase cells of proliferating cycle by flow cytometry. RESULTS: Sirpα1 transfection remarkably downregulated gankyrin and cyclin D1 expression. Sirpα1Δ4Y2 downregu- lation of gankyrin expression was greater than that of Sirpα1(P 0.05). The per- centage of S phase cells significantly decreased in Sk-hep1 cells transfected with Sirpα1 and Sirpα1Δ4Y2 plasmids (vs pLXSN Sk-hep1, P <0.05). The percentage of S phase cells in various Sk-hep1 cells increased when recovering to culture with 10% fetal bovine serum at 12 hours, but the percentage of S phase cells in Sk-hep1 cells transfected with Sirpα1 was the lowest ( vs pLXSN and Sirpα1Δ4Y2 Sk- hepl, P<0.05). BACKGROUND:Proximal migration of pancreatic stent (PMPS)is an infrequent event but its management can be technically challenging and there are no standard retrieval *** study aimed to determine the results of an endoscopic stent retrieval algorithm in terms of feasibility and efficacy of the endoscopic procedure. METHODS:During the period from January 2008 to December 2009,15 patients(8 women and 7 men with a mean age of 51.9 years)with PMPS were included in this *** retrieval was approached initially with balloon extraction followed by rat-tooth forceps and basket.A rescue approach such as using a stent retriever was attempted when other approaches failed. RESULTS:All the PMPSs(six 5Fr,nine 7Fr)were retrieved successfully within one ERCP *** extraction was successful in 9(60%)*** the 6 failed cases of balloon extraction,wire-guided rat-tooth forceps grasp was successful in 4,and stone extraction basket grasp was successful in 1 in whom forceps grasp *** stent was finally rescued with a stent retriever when balloon extraction,forceps and basket grasp all *** patients with successful balloon extraction,44.4% (4/9)developed post-ERCP hyperamylasemia but none of them developed post-procedure pancreatitis. CONCLUSIONS:With this algorithm,5Fr and 7Fr PMPS were successfully retrieved in all of the *** PMPSs can be safely retrieved with the commonly-used approaches in this *** less used approaches can be used as a rescue method.

A global quality control method based on high performance liquid chromatography(HPLC)coupled with diode array detection(DAD),single quadrupole mass spectrometry(MS)and time-of-flight mass spectrometry(TOFMS)was developed for simultaneous determination of seven major components(mangiferin,neomangiferin,timosaponin E1,timosaponin E,timosaponin BⅡ,timosaponin BⅢ,and timosaponin AⅢ)and identification of most components in extracts of Rhizoma Anemarrhenae(RA).HPLC analysis was performed on an Agilent SB-C18 column(4.6 mm×150 mm,5 μm)by gradient elution using acetonitrile and water-acetic acid(100∶0.05,v/v)as the mobile *** major components in RA were successfully *** quantitative method was fully validated in respect of the following performance criteria:linearity,precision,repeatability,stability,accuracy,limits of detection(LOD)and quantification(LOQ).A formula database of known compounds in RA was established,against which,most of the reported components in this herbal extract were identified effectively based on the extract masses acquired by *** qualitative and quantitative method was successfully used to analyze the components in 10 batches of RA samples collected from different regions in *** global quality control method,which consisted of HPLC-DAD-MS assay of seven major components and unambiguous identification of nineteen components,is suitable for routine quantification and comprehensive quality control of RA.

Objective: To investigate the incidence of aberrant bile duct and its management during laparoscopic cholecystectomy (LC). Methods: In 10 000 patients undergoing laparoscopic cholecystectomy from 1992 to July 2001, 3 had the involvement of the right accessory hepatic duct. In patient 1, the aberrant duct drained into the cystic duct was confirmed by open operation. In patient 2, the aberrant duct, which drained to the common bile duct (CBD), was injured and treated with suture and ligature under laparoscopy. In patient 3, the ab- errant duct, which also drained to the CBD, was confirmed and preserved. Results: All patients recovered well except patient 1 who had a transient elevation of ALT. No bile leak- age or other complication occurred. Conclusions: Only variation near the confluence and the entrance of the cystic duct into the bile duct is discovered during laparoscopic cholecystectomy. Right accessory hepatic duct is common and should be preserved during the operation. The accidentally injured small accessory hepatic duct can be treated with ligature without severe disturbance to liver func- tion.

Background: Portal biliopathy producing intrahepa- tic and extrahepatic biliary ductai abnormalities from portal hypertension, particularly with extrahepatic portal vein obstruction (EHPVO) is common. A majority of these patients are asymptomatic, but oc- casionally there is symptomatic biliary obstruction, and cholangitis and choledocholithiasis. Objective: To explore the principles of diagnosis and treatment of portal biliopathy. Data sources: To review the literature of portal bili- opathy. Conclusions: Endoscopic sphincterotomy, stone ex- traction and supportive drainage could effectively re- lieve cholangitis when jaundice is associated with common bile duct stones. Definitive decompressive portal-system vein shunting operation and choledo- cho-jejunostomy are sometimes required when biliary obstruction is recurrent and progressive.
