Cirrhosis in children and adolescents: An overview

Several conditions, especially chronic liver diseases, can lead to cirrhosis in children and adolescents. Most cases in clinical practice are caused by similar etiologies. In infants, cirrhosis is most often caused by biliary atresia and genetic-metabolic diseases, while in older children, it tends to result from autoimmune hepatitis,Wilson's disease, alpha-1-antitrypsin deficiency and primary sclerosing cholangitis. The symptoms of cirrhosis in children and adolescents are similar to those of adults. However, in pediatric patients, the first sign of cirrhosis is often poor weight gain. The complications of pediatric cirrhosis are similar to those observed in adult patients, and include gastrointestinal bleeding caused by gastroesophageal varices, ascites and spontaneous bacterial peritonitis. In pediatric patients, special attention should be paid to the nutritional alterations caused by cirrhosis, since children and adolescents have higher nutritional requirements for growth and development. Children and adolescents with chronic cholestasis are at risk for several nutritional deficiencies. Malnutrition can have severe consequences for both pre- and post-liver transplant patients. The treatment of cirrhosis-induced portal hypertension in children and adolescents is mostly based on methods developed for adults. The present article will review the diagnostic and differential diagnostic aspects of end-stage liver disease in children, as well as the major treatment options for this condition.

Treatment Adherence, Quality of Life and Clinical Variables in HIV/AIDS Infection

The purpose of the study was to analyze the relationship between treatment adherence, quality of life and clinical variables in HIV/AIDS Infection. The empirical study was conducted at two Portuguese hospitals (Porto and Lisbon) with a sample of 295 outpatients diagnosed with HIV/AIDS attending the Infectology service and on antiretroviral medication, during a 12-month period (February 2009 to February 2010). Data were collected by voluntary fulfillment of three questionnaires: one for socio-demographic variables, one to Assess Adherence to Antiretroviral Treatment-HIV and the Portuguese version of the WHOQOL-Bref to measure the quality of life (QoL). Clinical records were inspected in order to collect clinical information from the patients. The relationship between these variables was accessed by Student’s t-test and ANOVA using Tukey and LSD as the post Hoc test. Regarding disease stages, the post Hoc analysis has showed that asymptomatic patients have a better level of adherence and quality of life when compared to those in more advanced stages of the disease. Undetectable viral load 500 cells/mm3 were also associated with higher QoL in all overall domains. Patients on NRTI + NNTRI regimens have higher adherence when compared to those on NRTI + PI regimens and higher QoL indexes when compared to twice daily regimens, patients on single dose per day regimens have higher adherence and higher QoL in overall domain. Patients experiencing adverse effects have lower QoL and lower adherence levels when compared to those not experiencing such events. We consider the multiplicity and the interaction of several determinants of influence in the adaptation process during the HIV/AIDS treatment. The results may have implications for the psychological intervention to improve the adherence’s level to the antiretroviral therapy.

ana;">Introduction: Rheumatoid arthritis (RA) is a chronic systemic rheumatic disease which is usually treated with corticosteroids and immunobiologicals. The goal of this article is to carry out an assessment of disease activity indices in a cohort of patients with rheumatoid arthritis. Patients and Methods: This is a prospective cohort study. Individuals from the Initial Rheumatoid Arthritis Brasília Cohort, which is an incident cohort of early RA diagnosed patients, were monitored at the Rheumatology Service of the Hospital Universitário de Brasília (HUB), University of Brasília (UnB), Brazil. A cross-sectional analysis was carried out from 2017 to 2018 to evaluate patients with 15 or more years of follow-up, through a direct interview and review of medical records. The main focus of the study is on the assessment of disease activity, based on the indices: 28-joint Disease Activity Score based on Creactive protein (DAS 28 CPR) and based on erythrocyte sedimentation rate (DAS 28 ESR), Clinical Disease Activity Index (CDAI), and Simple Disease Activity Index (SDAI). The reference remission criteria used were the Composite Disease Activity Indices. Results: 107 patients were evaluated, mostly women, mean age of 55.1 years. Concerning the disease characteristics, 75.5% of the patients were positive for rheumatoid factor and 12 (11.3%) had documented erosive disease. The mean Health Assessment Questionnaire (HAQ) at the time of assessment was 0.6 (median 0.35). The indices analyzed showed: DAS28-ESR 48.6% of patients were in remission and 12.1% had low activity levels, DAS28-CRP 55.1% and 11.2%, SDAI 42% and 26.1%, CDAI 41.1% and 27.1%. These remission and low disease activity levels are higher than those generally found in the literature. Like other democratic societies,Portugal has known an important reduction of gender inequalities,mainly by the action of women’s social movements and some political ***,recent(inter)national studies confirm the social borders and inequalities persistence in many contexts(work,values and symbolic regulations,institutional frames,and daily interactions).In this paper,the authors will focus their analysis on the conjugal dissolution processes and unequal distribution of power between men and *** these matters,there is an extended debate,where they propose an articulation between the concepts of gender and *** authors assume as fruitful an articulation of the Marxist model with the feminist one and,indirectly,a critical and synthetic crossing between(neo)Marxism and Weberianism,being this one also articulated with symbolic *** on official statistics,in the analysis of some interviews about the motivations for/in divorce,and in the empirical evidence from the divorce judicial processes,the authors present some preliminary results of a collective project held on some regions of Portugal named:“Gender inequalities in work and private life:from the norms to social practices”.The central hypothesis of this project is that the forms of gender inequality and domination are tributaries of macro‐economic and institutional mechanisms but they are also playing at the micro level(family,business,public and private institutions),involving both variables,namely,in a crossing of Weberian‐Marxist perspectives,the presence/absence of certain level of empowerment by social *** means that women’s position depends and/or differs in function of factors such as available resources and rewards,the place in the organizational and(re)productive sphere of the family,and the place in the interactions and in roles negotiation.

非洲猪瘟病毒(African Swine Fever Virus,ASFV)是猪的一种强毒型病原体,从2007年起在东欧流行,目前还没有有效的疫苗或治疗方法。ASF病毒是当前的流行毒株,在猪体内带毒和排毒(shedding and excretion)的动力学机理至今尚不清楚。因此,本试验希望通过将易感的家猪与经肌内注射感染ASF病毒格鲁吉亚2007/1毒株的家猪进行接触,以测定ASF病毒在猪栏内和猪栏之间由受感染猪向易感猪进行传播的动力学机理。试验采集了血液、口液、鼻液、肠液(直肠)样本,以通过病毒滴定法和实时定量PCR(q PCR)法检测ASF病毒,利用血清学试验测定ASF病毒特异性抗体。肌内注射和接触传播感染均能造成猪急性发病,不过试验表明因感染途径不同而导致猪的发病机制有所差异。接种感染猪在接种的第3天可在血液中分离到有传染性的ASF病毒,而通过直接接触和间接接触进行感染的猪分别在曝露后第10天和第13天方可从血液中分离到有传染性的ASF病毒。临近出现ASF临床症状时,ASF病毒在血液中的滴度高于在鼻液和肠液中的。口液样本中没有分离到有传染性的ASF病毒,不过ASF病毒的基因组被检测到。只有一头接种感染的猪在接种后第12天检测到ASF病毒特异性抗体。本项试验结果提供了ASF病毒格鲁吉亚2007/1毒株在家猪中带毒和排毒的定量数据,同时表明该毒株引起持续感染的能力有限。
