Standardizing mI-TLIF, a Proposal for a Reproducible Technique

Background: minimally invasive transforaminal lumbar interbody fusion (mI TLIF) is a widely known and performed technique, however its versatility among different physicians continues to hinder its replication and results. Therefore, this study aimed to provide a step-by-step surgical guide to perform a safe mI-TLIF, based on the results obtained in patients operated on by a single surgeon over a period of 12 years. Patients and methods: A retrospective, single center, longitudinal, and observational cohort study was conducted with 931 patients who underwent mI TLIF by a single surgeon between 2010 and 2022 using the technique described on this paper, each with a minimum follow-up of 12 months. Criteria included Schizas classification, listhesis according to meyerding classification, number of levels treated, cage size, and complications (screw repositioning or cerebrospinal fluid leak). Patient clinical outcomes were assessed using the Oswestry Disability Index (ODI), Visual analog Scale (VAS) for pre- and postoperative radicular pain. Thin slice CT scans were used to assess the progression of the fusion using the Bridwell classification. In the statistical analysis, percentages, median, and interquartile range (IQR) were calculated. Results: Nine hundred and thirty one patients underwent mI TLIF using the technique described, eight hundred and eighty (94.5%) had a single level treated and fifty one (5.5%) had a 2 level procedure (982 levels), an 8mm cage was placed on five hundred and seventeenlevels (52.7%), six hundred and sixty three levels(67.6%) achieved grade I fusion, two hundred and sixty six levels (27.1%) achieved grade II fusion, 52 levels (5.3) achieved grade III fusion and one level (0.1) achieved a grade IV fusion or non-union. Revision surgery was performed on 3 patients (0.3%) for screw repositioning, cerebrospinal fluid leak was present on 2 patients during surgery and treated before closure. Obesity has a complex multifactorial etiology and is characterized by excessive accumulation of adipose *** adipose tissue has deleterious effects on health because it secretes large amounts of inflammatory *** calorie restriction associated with strength training may be useful in managing chronic systemic *** study aimed to evaluate the acute effect of a single strength-training session on plasma adipokine levels in sedentary,overweight,and obese young *** study included twelve men(Age:[34.95±9.77]years;Height:[174.16±3.66]centimeter[cm];Weight:[97.83±12.87]kilogram(kg);body mass index[BmI]:[32.30±4.51]kg/m2),who performed a single strength training *** strength training protocol consisted of 4 sets of 12 repetitions in the following six exercises,45°leg press,bench press,leg extension,machine row,leg curl,and shoulder *** samples were collected before,immediately after,and 1-h subsequent after strength *** plasma levels of resistin and leptin were measured.A significant decrease in resistin levels were found 1 h after the strength training session if compared to levels before the training session(pre-[before][2390±1199]picograms per milliliter[pg/mL]vs post-1 h[1-h subsequent][1523±798],6 pg/mL,p=0.0028).The plasma leptin levels did not differ at any time *** conclusion,a very well controlled single session of strength training significantly decreased the plasma levels of resistin without altering the concentration of leptin in overweight and obese *** effect,at least in part,supports the benefits of exercise by reducing the low grade inflammation and insulin resistance in obesity.

Considering the recommended indications for Helicobacter pylori(***)eradication therapy and the broad spectrum of available diagnostic methods,a reliable diagnosis is mandatory both before and after eradication *** highly accurate tests should be used in clinical practice,and the sensitivity and specificity of an adequate test should exceed 90%.The choice of tests should take into account clinical circumstances,the likelihood ratio of positive and negative tests,the cost-effectiveness of the testing strategy and the availability of the *** review concerns some of the most recent developments in diagnostic methods of *** infection,namely the contribution of novel endoscopic evaluation methodologies for the diagnosis of *** infection,such as magnifying endoscopy techniques and *** addition,the diagnostic contribution of histology and the urea breath test was explored recently in specific clinical settings and patient *** studies recommend enhancing the number of biopsy fragments for the rapid urease *** culture from the gastric biopsy is the gold standard technique,and is recommended for antibiotic susceptibility *** is used for initial screening and the stool antigen test is particularly used when the urea breath test is not available,while molecular methods have gained attention mostly for detecting antibiotic resistance.

Composites offer the option of coupling the individual benefits of their constituents to achieve unique material properties,which can be of extra value in many tissue engineering *** combining hydrogelswith fibre-based scaffolds can create tissue constructs with enhanced biological and structural ***,developing efficient and scalable approaches to manufacture such composites is ***,we use a droplet-based bioprinting system called reactive jet impingement(ReJI)to integrate a cell-laden hydrogel with a microfibrous *** system uses microvalves connected to different bioink reservoirs and directed to continuously jet bioink droplets at one another in mid-air,where the droplets react and form a hydrogel that lands on a microfibrous ***–hydrogel–fibre composites are produced by embedding human dermal fibroblasts at two different concentrations(5×10^(6) and 30×10^(6) cells/mL)in a collagen–alginate–fibrin hydrogel matrix and bioprinted onto a fibre-based *** results show that both types of cell–hydrogel–microfibre composite maintain high cell viability and promote cell–cell and cell–biomaterial *** lower fibroblast density triggers cell proliferation,whereas the higher fibroblast density facilitates faster cellular organisation and infiltration into the ***,the fibrous component of the composite is characterised by high swelling properties and the quick release of calcium *** data indicate that the created composite constructs offer an efficient way to create highly functional tissue precursors for laminar tissue engineering,particularly for wound healing and skin tissue engineering applications.

Association between EGF +61A/G polymorphism and gastric cancer in Caucasians

AIm: To investigate the association between epidermal growth factor (EGF) +61A/G polymorphism and susceptibility to gastric cancer, through a cross-sectional study. mETHODS: Polymerase chain reaction resctriction fragment lenght polymorphism analyses were used to genotype EGF +61 in 207 patients with gastric lesions (162 patients with gastric adenocarcinomas, 45 with atrophy or intestinal metaplasia) and 984 controls. All subjects were Caucasian. RESULTS: Genotype distribution was 23.5% for GG and 76.5% for GA/AA in the control group, 18.4% for GG and 68.6% for GA/AA in the entire group with gastric lesions and 17.9% for GG and 82.1% for GA/AA in the group with gastric adenocarcinoma. No statistically significant associations were found between EGF +61 variants and risk for developing gastric cancer [odds ratios (OR) = 1.41, 95% confidence intervals (CI): 0.90-2.21, P = 0.116]. However, the stratification of individuals by gender revealed that males carrying A alleles (EGF +61A/G or AA) had an increased risk for developing gastric cancer as compared to GG homozygous males (OR = 1.55, 95% CI: 1.05-2.28, P = 0.021). CONCLUSION: In summary, we found that males who were A carriers for EGF +61 had an increased risk for developing gastric cancer. This result may be explained by the suggestion that women secrete less gastric acid than men.

Effects of a long-term physical exercise program with and without diet on obese boys after six-month detraining

Background:most studies on physical fitness and detraining have been conducted on normal-weight *** results indicate that any gains regress to the untrained control values during the detraining ***,therefore,seems necessary to determine how detraining affects the different fitness parameters in obese *** aim of the present study was to evaluate the effects of detraining(6 months)on kinanthropometry and the components of physical fitness after an intervention(31 months)consisting of a program of exercise and/or diet for obese ***:The participants were 18 boys,aged between 8 and 11 years,divided into E and E+D groups according to the program they *** E group followed a physical exercise program(three 90-minute sessions/week),and the E+D group the same physical exercise program plus a low calorie *** fitness was assessed by the European physical fitness test battery including flamingo balance,plate tapping,sit-andreach,standing broad jump,hand-grip strength,sit-ups,bent-arm hang,10×5-metre shuttle run,and 20-metre endurance shuttle *** Kruskal-Wallis test was applied to reveal overall intergroup differences(E and E+D groups),and measurements showing significant differences were further analysed for differences between individual groups by the mann-Whitney U ***:In both groups,changes were observed in various physical fitness parameters,especially limb speed(E group,P=0.001;E+D group,P=0.002),agility(E group,Pm intervention based on the combination of exercise program and exercise program plus diet in obese boys does not affect the changes attained during the intervention.

Ni/Al Intermetallics Plasma Transferred Arc Processing

In-situ alloy development during surface processing allows for a limitless materials selection to protect components exposed to severe service conditions. In fact surface alloying offers the possibility to strengthen surface components with alloys that would not be possible to process otherwise. This work used Plasma transferred arc (PTA) hardfacing for surface alloying. Different amounts of aluminium powder, 5-25%, were added to a Ni based superalloy, from Hastealloy C family, in the atomized form. The mixture was homogeneized in a ball mill and PTA deposited on carbon steel substrate. The influence of different processing parameters on the final surface alloy was evaluated as current intensity and depositing velocity were varied. Coatings were characterized by optical and scanning electronic microscopy, X-ray diffraction and Vickers microhardness profiles, under a 500g load. Results showed that PTA hardfacing is an adequate surface alloying. For the conditions tested increasing hardness was obtained by solid solution for the lower amounts of Al added and due to the new intermetallic phases for the richer Al mixture.
