Hospitalizations and in-hospital mortality for inflammatory bowel disease in Brazil

BACKGROUND Inflammatory bowel disease(IBD)is associated with complications,frequent hospitalizations,surgery and *** introduction of biologic drugs into the therapeutic arsenal in the last two decades,combined with an expansion of immunosuppressant therapy,has changed IBD management and may have altered the profile of hospitalizations and in-hospital mortality(IHM)due to *** to describe hospitalizations from 2008 to 2018 and to analyze IHM from 1998 to 2017 for IBD in *** this observational,retrospective,ecological study used secondary data on hospitalizations for IBD in Brazil for 2008-2018 to describe hospitalizations and for 1998-2017 to analyze *** data were obtained from the Hospital Information system of the Brazilian Unified Health system and population data from demographic *** following variables were analyzed:Number of deaths and hospitalizations,length of hospital stay,financial costs of hospitalization,sex,age,ethnicity and type of hospital *** there was a reduction in the number of IBD hospitalizations,from 6975 admissions in 1998 to 4113 in 2017(trend:y=-0.1682x+342.8;R^(2)=0.8197;Pthe hospitalization rate also decreased,from 3.60/100000 in 2000 to 2.17 in *** rates varied during the 20-year period,between 2.06 in 2017 and 3.64 in 2007,and did not follow a linear trend(y=-0.0005049x+2.617;R^(2)=0,00006;P=0.9741).IHM rates also varied between regions,increasing in all but the southeast,which showed a decreasing trend(y=-0.1122x+4.427;R^(2)=0,728;Pthe southeast region accounted for 44.29%of all *** Northeast region had the highest IHM rate(2.86 deaths/100 admissions),with an increasing trend(y=0.1105x+1.110;R^(2)=0.6265;Pt the lowest hospitalization rate(1.15).the Midwest and south regions had the highest hospitalization rates(3.27 and 3.17,respectively).A higher IHM rate was observed for nonelective admissions(2.88),which accounted for 81%of IB

the role of cancer stem cells in gastrointestinal cancer-associated death has been widely *** cancer stem cells(GCsCs)are considered to be responsible for tumor initiation,growth,resistance to cytotoxic therapies,recurrence and metastasis due to their unique *** properties make the current therapeutic trials against GCsCs ***,recent studies have shown that targeting stem cell surface markers or stemness associated pathways might have an additional off-target effect on the immune *** advances in oncology and precision medicine have opened alternative therapeutic strategies in the form of cancer *** approach differs from classical anti-cancer therapy through its mechanism of action involving the activation and use of a functional immune system against tumor cells,instead of aiming physically destruction of cancer cells through radio-or *** immunological approaches for GCsCs targeting involve the use of different immune cells and various immune mechanisms like targeting specific surface antigens,using innate immune cells like the natural killer and t cells,t-cell chimeric antigen receptor technology,dendritic cell vaccine,or immune checkpoint *** this respect,better understandings of immune regulatory mechanisms that govern anti-tumor response bring new hope in obtaining long-term remission for cancer therapy.

Adenocarcinoma of the third and fourth portions of the duodenum: the capsule endoscopy value

Primary adenocarcinoma of the small intestine occurs in over 50% of cases in the duodenum. However,its location in the third and fourth duodenal portions occurs rarely and is a diagnostic challenge. the aim of this work is to report an adenocarcinoma of the third and fourth duodenal portions,emphasizing its diagnostic difficulty and the value of video capsule endoscopy. A man,40 years old,with no medical history,with abdominal discomfort and progressive fatigue,presented four months ago with one episode o f m o d e ra t e m e l e n a. t h e p hy s i c a l e x a m i n a t i o n was normal,except for mucosal pallor. Blood tests were consistent with microcytic,hypochromic iron deficiency anemia with 7.8 g/d l hemoglobin. the upper and lower endoscopy were normal. Additional work-up with video capsule endoscopy showed a polypoid lesion involving the third and fourth portions o f t h e d u o d e n u m. B i o p s y s h o w e d a m o d e ra t e l y differentiated adenocarcinoma. Abdominal computed tomography showed a wall thickening from the third duodenal portion to the proximal jejunum,without distant metastasis. the patient underwent segmental resection(distal duodenum and proximal jejunum) with duodenojejunostomy. the surgical specimen histology confirmed the biopsy diagnosis,with transmural infiltration,without nodal involvement. Conclusion: Adenocarcinoma of the third and fourth portions of the duodenum is difficult to diagnose and capsule endoscopy is of great value.

Gastrointestinal infections are a major cause for serious clinical complications in *** induction of antibody responses by B cells is critical for protective immunity against infections and requires CXCR5+PD-1++CD4+t cells(tFH cells).We investigated the ontogeny of CXCR5+PD-1++CD4+t cells in human *** CXCR5+PD-1++CD4+t cells were absent in fetal intestines,CXCR5+PD-1++CD4+t cells increased after birth and were abundant in infant intestines,resulting in significant higher numbers compared to *** findings were supported by scRNAseq analyses,showing increased frequencies of CD4+t cells with a tFH gene signature in infant intestines compared to ***-cultures of autologous infant intestinal CXCR5+PD-1+/−CD4+t cells with B cells further demonstrated that infant intestinal tFH cells were able to effectively promote class switching and antibody production by B *** together,we demonstrate that functional tFH cells are numerous in infant intestines,making them a promising target for oral pediatric vaccine strategies.

the huge atomic heterogeneity of high-entropy materials along with a possibility to unravel the behavior of individual components at the atomic scale suggests a great promise in designing new compositionally complex systems with the desired ***,we apply multi-edge X-ray absorption spectroscopy(extended X-ray absorption fine structure(EXAFs),Xray absorption near edge structure(XANEs),and X-ray magnetic circular dichroism(XMCD))to probe the structural,electronic,and magnetic properties of all individual constituents in the single-phase face-centered cubic(fcc)-structured nanocrystalline thin film of Cr_(20)Mn_(26)Fe_(18)Co_(19)Ni_(17)(at.%)high-entropy alloy on the local *** local crystallographic ordering and componentdependent lattice displacements were explored within the reverse Monte Carlo approach applied to EXAFs spectra collected at the K absorption edges of several constituents at room temperature.A homogeneous short-range fcc atomic environment around the absorbers of each type with very similar statistically averaged interatomic distances(2.54-2.55Å)to their nearest-neighbors and enlarged structural relaxations of Cr atoms were *** and XMCD spectra collected at the l2,3 absorption edges of all principal components at low temperature from the oxidized and in situ cleaned surfaces were used to probe the oxidation states,the changes in the electronic structure,and magnetic behavior of all constituents at the surface and in the sub-surface volume of the *** spin and orbital magnetic moments of Fe,Co,and Ni components were quantitatively *** presence of magnetic phase transitions and the co-existence of different magnetic phases were uncovered by conventional magnetometry in a broad temperature range.

First characterization of endophytic Corynespora cassiicola isolates with variant cassiicolin genes recovered from rubber trees in Brazil

Corynespora cassiicola is the causal agent of Corynespora leaf Fall(ClF)*** is one of the most important fungal diseases of rubber trees in Asia and Africa but disease outbreaks have not been reported in south ***,a small cysteine-rich glycoprotein secreted by the pathogenic *** isolate CCP,was previously identified as a potential disease effector in rubber ***,the cassiicolin-encoding gene(Cas1)was characterized and shown to be expressed in the early phase of *** this study,we investigated whether previously undetected strains of *** are present in south American rubber plantations by examining the fungal endophyte population found in asymptomatic rubber tree *** isolates were identified as *** encoding new forms of the cassiicolin precursor protein(Cas3 and Cas4)were identified from these *** of four isolates were able to induce symptoms on the cultivar they were isolated from in a detached leaf assay,with different kinetics and *** isolate had the same pathogenicity profile as the pathogenic isolate CCP;the other two isolates developed symptoms late during the course of infection,suggesting saprotrophic ***,no Cas3 or Cas4 transcripts could be detected upon inoculation with the endophytic isolates,whereas the reference gene Cas1 was expressed upon inoculation with the CCP *** work demonstrated that *** is present in south America in an endophytic form and that it may evolve from an endophytic to a saprophytic or even potentially pathogenic life style.

treatment Adherence, Quality of life and Clinical Variables in HIV/AIDs Infection

the purpose of the study was to analyze the relationship between treatment adherence, quality of life and clinical variables in HIV/AIDs Infection. the empirical study was conducted at two Portuguese hospitals (Porto and lisbon) with a sample of 295 outpatients diagnosed with HIV/AIDs attending the Infectology service and on antiretroviral medication, during a 12-month period (February 2009 to February 2010). data were collected by voluntary fulfillment of three questionnaires: one for socio-demographic variables, one to Assess Adherence to Antiretroviral treatment-HIV and the Portuguese version of the WHOQOl-Bref to measure the quality of life (Qol). Clinical records were inspected in order to collect clinical information from the patients. the relationship between these variables was accessed by student’s t-test and ANOVA using tukey and lsD as the post Hoc test. Regarding disease stages, the post Hoc analysis has showed that asymptomatic patients have a better level of adherence and quality of life when compared to those in more advanced stages of the disease. Undetectable viral load 500 cells/mm3 were also associated with higher Qol in all overall domains. Patients on NRtI + NNtRI regimens have higher adherence when compared to those on NRtI + PI regimens and higher Qol indexes when compared to twice daily regimens, patients on single dose per day regimens have higher adherence and higher Qol in overall domain. Patients experiencing adverse effects have lower Qol and lower adherence levels when compared to those not experiencing such events. We consider the multiplicity and the interaction of several determinants of influence in the adaptation process during the HIV/AIDs treatment. the results may have implications for the psychological intervention to improve the adherence’s level to the antiretroviral therapy.

Aims Increasing evidence worldwide indicates that high mountain regions are not immune to invasion by alien ***,we aimed to address whether three major woody invaders of low-mountain areas are constrained to lower altitudes due to climatic restrictions or just by low propagule *** hypothesize that the increasing climatic harshness towards higher altitudes restricts seedling establishment and survival of these woody invaders and thus their potential for altitudinal *** the study was carried out in the sierras Grandes Mountains,Córdoba,central Argentina(32°50′s,64°90′W).We addressed the hypothesis with an experimental approach,dissociating the effect of the environmental gradient from that of propagule pressure,by discarding the influence of seed *** translocated seeds of Gleditsia triacanthos l.,ligustrum lucidum *** Pyracantha angustifolia(Franch.)*** the altitudinal gradient(from 900 to 2700 m asl).seven sites were selected along the altitudinal gradient,spaced every 200-400 *** plots(4×4 m)were selected within each altitudinal site and excluded from livestock;100 seeds of each species were sown within each plot(6300 seeds in total).seedling emergence and survival was monitored during two growing seasons and soil temperature and moisture was recorded as *** field experiment was complemented with lab *** Findings Climate along this altitudinal gradient might be least restrictive at mid elevations,as a result of the intersection between opposite soil temperature and moisture *** seeds germinated along the whole altitudinal gradient and seedlings successfully established and over-wintered well above their current altitudinal distribution(up to 2200 m for ligustrum and 2400 m for Gleditsia and Pyracantha).Additional lab assays confirmed field patterns and gave some insights into contrasting regeneration strategies between these invaders that might help to overcome stochastic e

Use of Color Doppler Ultrasound for the Diagnosis of subarachnoid Hemorrhage in Asymptomatic Full-term Neonate:A Case Report

Cerebral hemorrhages are fairly common in full-term neonates with no history of traumatic birth, mostly limited, and with benign evolution. We report a case of a full-term neonate from vaginal birth with caput succedaneum in the right parietal area. the neonate underwent cranial ultrasonography and color Doppler which showed extra-axial blood effusion. Color Doppler showed vessels crossing the collection area, which allowed the diagnosis of subarachnoid hematoma.

this paper brings the comparison of performances of CO_(2)conversion by plasma and plasma-assisted catalysis based on the data collected from literature in this field,organised in an open access online *** tool is open to all users to carry out their own analyses,but also to contributors who wish to add their data to the database in order to improve the relevance of the comparisons made,and ultimately to improve the efficiency of CO_(2)conversion by *** creation of this database and database user interface is motivated by the fact that plasma-catalysis is a fast-growing field for all CO_(2)conversion processes,be it methanation,dry reforming of methane,methanolisation,or *** a result of this rapid increase,there is a need for a set of standard procedures to rigorously compare performances of different ***,this is currently not possible because the fundamental mechanisms of plasma-catalysis are still too poorly understood to define these standard *** however,the accumulated data within the CO_(2)plasma-catalysis community has become large enough to warrant so-called“big data”studies more familiar in the fields of medicine and the social *** enable comparisons between multiple data sets and make future research more effective,this work proposes the first database on CO_(2)conversion performances by plasma-catalysis open to the whole *** database has been initiated in the framework of a H_(2)0_(2)0 European project and is called the“PIONEER data Base”.the database gathers a large amount of CO_(2)conversion performance data such as conversion rate,energy efficiency,and selectivity for numerous plasma sources coupled with or without a *** data set is associated with metadata describing the gas mixture,the plasma source,the nature of the catalyst,and the form of coupling with the *** the database itself,a data extraction tool with direct visualisation features or advance
