Basosquamous Cell Carcinoma of the Lower Lip Arising from Actinic Cheilitis: Case Report and up Date

Background: Basosquamous carcinoma (BSC) is a rare non-melanoma skin cancer, considered to be a subtype of basal cell carcinoma (BCC). BSC often produces distant metastases with a higher risk of recurrence than that of BCC which is not commonly found in the lip. Case Report: A 57-year-old white female patient presented an ulcer on her lower lip that had an ongoing development for over six months. Physical examination, photo documentation, videoroscopy, scraped cytology, toluidine blue test, and biopsy of the ulcer were carried out. Results: Upon physical examination we observed an actinic cheilitis associated with the ulcer. Videoroscopy revealed the presence of fissures and erosion that had not been seen by oroscopy. Toluidine blue test was only positive for the region of the ulcer. Cytological analysis revealed rare nests compatible with carcinoma. Histopathology of the biopsy revealed a carcinoma with nests lined by basal cells associated with areas of squamous differentiation. The patient was then referred to surgery for the removal of the BCC. analysis of the specimen showed free surgical margins and the immunohistochemical panel did not confirm the initial diagnosis of BCC, indicating a subtype of BSC. After surgery, the patient has been followed by periodic consultations. She is well and without further complications. Coments: BSC is considered to be an aggressive and rare tumor affecting mainly upper face and primarily affects men over 60 years of age. Since our patient is a woman presenting the lesion in the lower lip, this highlights the unusual and interesting presentation of this case report.

A high percentage of the world's population lives in areas where air pollutant concentrations exceed the World Health Organization *** work aims to develop and test,a high-resolution multi-scale air pollution modelling system by integrating a set of adequate *** system is able to provide detailed air pollutant concentrations in urban areas and support air quality management strategies through a better identification of different atmospheric *** also allows furthering the design and assessment of air pollution control measures for a specific *** evaluate its performance and suitability,the system was applied to the Macao Special Administrative Region(SAR),China,one of the most densely populated areas on earth,during a winter period when this area is affected by high levels of Particulate Matter(PM).Although the developed system tends to underestimate the PM concentrations,it revealed a good performance in reproducing the temporal and spatial air pollution *** exceedances of the Chinese air quality standards were calculated and high population exposure to PM pollution was *** tested urban atmospheric emission reduction scenarios have shown air quality improvements,indicating that emission reduction measures at urban level should focus on the domestic ***,it is crucial to implement joint pollution prevention strategies with neighbouring regions to improve the air quality in Macao *** approach developed in this work can support policymakers in defining new strategies to reduce atmospheric pollution in urban areas.

Obesity has a complex multifactorial etiology and is characterized by excessive accumulation of adipose *** adipose tissue has deleterious effects on health because it secretes large amounts of inflammatory *** calorie restriction associated with strength training may be useful in managing chronic systemic *** study aimed to evaluate the acute effect of a single strength-training session on plasma adipokine levels in sedentary,overweight,and obese young *** study included twelve men(Age:[34.95±9.77]years;Height:[174.16±3.66]centimeter[cm];Weight:[97.83±12.87]kilogram(kg);body mass index[BMI]:[32.30±4.51]kg/m2),who performed a single strength training *** strength training protocol consisted of 4 sets of 12 repetitions in the following six exercises,45°leg press,bench press,leg extension,machine row,leg curl,and shoulder *** samples were collected before,immediately after,and 1-h subsequent after strength *** plasma levels of resistin and leptin were measured.A significant decrease in resistin levels were found 1 h after the strength training session if compared to levels before the training session(pre-[before][2390±1199]picograms per milliliter[pg/mL]vs post-1 h[1-h subsequent][1523±798],6 pg/mL,p=0.0028).The plasma leptin levels did not differ at any time *** conclusion,a very well controlled single session of strength training significantly decreased the plasma levels of resistin without altering the concentration of leptin in overweight and obese *** effect,at least in part,supports the benefits of exercise by reducing the low grade inflammation and insulin resistance in obesity.

Introduction: 68Ga-PSMA-11 is considered the gold standard in detection of micro and oligometastases in advanced prostate cancer, being used for therapeutic planning, as well as, potentially, for evaluating response to treatment. The development of ready-to-use lyophilized kit of PSMA-11 adds quality and safety to the routine use of this radiopharmaceutical and represents a pharmacotechnical challenge as it must preserve the integrity and specificity of the ligand. Methods: PSMA-11 kit formulation was proposed, considering radiolabeling parameters and the preservation of the peptide during the lyophilization process, using mannitol as an excipient. Critical temperature characterization studies were carried out using DSC equipment and the freeze-drying process was developed. The direct radiolabeling conditions were evaluated and standardized using 68Ge/68Ga generator eluate from two different manufacturers (ITG and Eckert & Ziegler). The radiochemical purity was evaluated by TLC and HPLC. Biological evaluation was carried out with lyophilized PSMA-11 to demonstrate the integrity of the peptide and preservation of biological activity after the lyophilization process. Results: Based on critical temperature characterization studies, the freeze-drying cycle was designed to reach a freezing temperature of around −40˚C and primary drying at 2˚C. Using 20 mg of mannitol, an intact and elegant lyophilized cake was obtained. PSMA-11 lyophilized kit was directly labeled with 68Ga eluate from 68Ge/68Ga GMP generators (ITG and Eckert & Ziegler) resulting in % RP > 95% at pH 4.0 to 4.5. The results obtained from in vitro and in vivo biological competition studies confirmed the preservation of PSMA-11 affinity for the receptor after lyophilization. Conclusion: A lyophilized formulation (Kit) of PSMA-11 was successfully obtained, which preserved the integrity and biological activity of the peptide and guaranteed radiolabeling efficiency.

The incidence of Mycobacterium avium complex pulmonary disease (MACPD) has been globally increasing. The present study aims to provide a comprehensive analysis of clinical characteristics and treatment response among patients in the Amazon region of Brazil and conduct a retrospective cohort study at a prominent referral hospital in the State of Par, Brazil, from 2012 to 2020. The M. avium group represented 58.9% of cases, followed by M. intracellulare (35.7%), M. colombiense (3.6%), and M. chimaera (1.8%). The majority of patients (73.2%) were female and with an average age of 63 years. Primary clinical manifestations included persistent cough and sputum production. The Charlson comorbidity index (CCI) averaged 2.47, with 36.3% of patients having a CCI score equal to higher than 3. Notably, over 70% of patients had received treatment for pulmonary tuberculosis before initiating treatment for MAC infection. The fibrocavitary radiologic form was the most prevalent (55.4%), frequently exhibiting a bilateral distribution (53.6%). Antimicrobial susceptibility revealed a significant prevalence of MAC resistance to drugs in the therapeutic regimen. despite this, most of the patients experienced clinical improvement (50%). In conclusion, this study highlights a higher prevalence of MAC infections among middle-aged women, with a history of pulmonary tuberculosis treatment and presenting the fibrocavitary radiological form was predominant.

Dietary supplementation with free methionine or methionine dipeptide mitigates intestinal oxidative stress induced by Eimeria spp. challenge in broiler chickens

Background: This study evaluated the effects of Eimeria spp. challenge and dietary supplementation with free methionine or methionine dipeptide on animal performance;expression of genes associated with the immune system, antioxidant system, and amino acid transport in the jejunum;and redox status of the jejunum of broiler ***: A randomized, 2 × 3 factorial design was used, in which Eimeria spp. challenge was the first factor(Eimeria-challenged, EC, or unchallenged, UC, broilers) and methionine supplementation was the second factor(non-supplemented, NS;free DL-methionine, DL-Met;and methionine dipeptide, DL-methionyl-DL-methionine, DLMMet). At 14 days of age, chickens were inoculated orally with sporulated oocysts of Eimeria acervulina, Eimeria praecox, Eimeria maxima, and Eimeria mitis. Birds were killed by cervical dislocation 144 h post-inoculation(PI), and the jejunum was collected for biochemical and molecular ***: EC broilers had a 13% lower feed intake(FI), 37% lower body weight gain(BWG), and 39% higher feed conversion ratio(FCR) than UC broilers. Chickens fed the DL-Met diet had higher BWG(about 12% higher) and better FCR(about 12% lower) than chickens fed the NS diet. EC chickens had lower relative weight of the bursa of Fabricius(51.8%) and higher relative weights of the spleen and whole intestine(53.6% and 26.3%, respectively) than UC chickens. Eimeria spp. challenge led to an increase in the levels of oxidative substances, such as nitrite and thiobarbituric acid reactive substances(TBARS), in the jejunum of chickens 144 h PI. Among UC chickens, those fed the DL-Met diet had higher total antioxidant capacity(TAC) and lower catalase(CAT) and superoxide dismutase(SOD) activities. EC chickens that received the NS diet had higher carbonylated protein content(CP). This result was associated with their lower TAC and catalase activity. Astyanax mexicanus has repeatedly colonized cave environments,displaying evolutionary parallelisms in many troglobitic *** being a model system for the study of adaptation to life in perpetual darkness,the parasites that infect cavefish are practically *** this study,we investigated the macroparasite communities in 18 cavefish populations from independent lineages and compared them with the parasite diversity found in their sister surface fish populations,with the aim of better understanding the role that parasites play in the colonization of new *** the cavefish populations,we identified 13 parasite taxa,including a subset of 10 of the 27 parasite taxa known for the surface *** infecting the cavefish belong to five taxonomic groups,including trematodes,monogeneans,nematodes,copepods,and *** are the most dominant group,found in 14 *** macroparasites include species with direct life cycles and trophic transmission,including invasive ***,paired comparisons indicate higher parasite richness in the cavefish than in the surface *** variation in parasite composition across the caves suggests historical and geographical contingencies in the host-parasite colonization process and potential evolution of local *** base-line data on parasite diversity in cavefish populations of *** provides a foundation to explore the role of divergent parasite infections under contrasting ecological pressures(cave *** environments)in the evolution of cave adaptive traits.

Serum levels of angiotensin converting enzyme as a biomarker of liver fibrosis

The renin angiotensin system(RAS) is classically conceived as a circulating hormonal system involved in blood pressure control and hydroelectrolyte balance. The discovery that RAS components are locally expressed in a wide range of organs and tissues,including the liver,pointed to a role for this system in the pathogenesis of several conditions including hepatic fibrosis and cirrhosis. It has been widely reported that the classical RAS axis composed by the angiotensin converting enzyme(ACE)-angiotensin(Ang) Ⅱ-Ang type 1(AT1) receptor mediates pro-inflammatory,pro-thrombotic,and pro-fibrotic processes. On the other hand,the alternative axis comprising ACE2-Ang-(1-7)-Mas receptor seems to play a protective role by frequently opposing Ang Ⅱ action. Chronic hepatitis B(CHB) is one of the leading causes of liver fibrosis,accounting for the death of nearly one million people worldwide. Liver fibrosis is a key factor to determine therapeutic interventions for patients with CHB. However,the establishment of non-invasive and accurate methods to detect reversible stages of liver fibrosis is still a challenge. In an elegant study published in the 36 th issue of the World Journal of Gastroenterology,Noguchi et al showed the predictive value of serum ACE levels in detecting not only advanced stages of liver fibrosis but also initial and intermediate fibrotic stages. The serum levels of ACE might represent an accurate,non-invasive,widely available,and easy method to evaluate fibrosis related to CHB. Moreover,therapies involving the inhibition of the classical RAS axis components might be promising in the control of CHB-related liver fibrosis.

Measuring External debt in a Context of Macroeconomic Imbalances

The exploitation of new methodologies and indicators is considered valuable in measuring a key macroeconomic indicator like the external debt. The recent Portuguese economic developments illustrate the need and usefulness of having a multidimensional approach of this indicator, with several balance of payment items, such as the current and the capital account balances, the foreign direct investment or the reserves assets being carefully analysed when reading the external conditions faced by the economy. Only such an approach can provide with a comprehensive measure of external debt consistent across the range of debt instruments, institutional sectors and valuation methods used. This paper develops an assessment of external debt measures and concludes about their potential advantages and disadvantages. Comparisons are made by focusing on alternatives like gross external debt against net external debt, external debt vis-h-vis international investment position, and external debt at nominal value against external debt at market value.
