BACKGROUND：Liver ischemia reperfusion（IR）injury triggers a systemic inflammatory response and is the main cause of organ dysfunction and adverse postoperative outcomes after liver ***（PTX）and hypertonic saline solution（HTS）have been identified to have beneficial effects against IR *** study aimed to investigate if the addition of PTX to HTS is superior to HTS alone for the prevention of liver IR ***： Male Wistar rats were allocated into three groups. Control rats underwent 60 minutes of partial liver ischemia, HTS rats were treated with 0.4 mL/kg of intravenous 7.5% NaCl 15 minutes before reperfusion, and HPTX group were treated with 7.5% NaC1 plus 25 mg/kg of PTX 15 minutes before reperfusion. Samples were collected after reperfusion for determination of ALT, AST, TNF-α, IL-6, IL-10, mitochondrial respiration, lipid peroxidation, pulmonary permeability and myeloperoxidase. RESULTS： HPTX significantly decreased TNF-α 30 minutes after reperfusion. HPTX and HTS significantly decreased ALT,AST, IL-6, mitochondrial dysfunction and pulmonary myelo- peroxidase 4 hours after reperfusion. Compared with HTS only, HPTX significantly decreased hepatic oxidative stress 4 hours after reperfusion and pulmonary permeability 4 and 12 hours after reperfusion. CONCLUSION： This study showed that PTX added the beneficial effects of HTS on liver IR injury through decreases of hepatic oxidative stress and pulmonary permeability.

From Simulation to Reality: A Comprehensive Study on the Efficacy of a Rotating Monopolar and Bipolar Radiofrequency System through In-Silico Modeling and Pre-Clinical and Clinical Validation

Background: Skin aging is an unavoidable process aggravated by environmental agents. Among other energy devices, non-invasive radiofrequency (RF) technology is widely used for skin tightening and body contouring as it is simpler and more affordable than other technologies that also minimize pain and side-effects. However, most of the current RF devices do not provide automatic skin temperature control and it is difficult to achieve controlled, deep, and harmless thermal increase, so treatment performance and safety is dependent on the operator’s movements and expertise. Objective: To show the potential of numerical simulations for optimizing the design of monopolar and bipolar RF electrodes that are capable of providing homogeneous, deep and controlled heating. Materials and methods: In-silico models were developed and analyzed using Comsol Multiphysics software to simulate the RF effect produced in tissue by rotating monopolar and bipolar electrodes with different geometries from the Sculpt & Shape RF device (Sinclair, Spain), operating at frequencies of 0.5 and 1 MHz. Ex-vivo and in-vivo proof-of-concept tests were carried out to validate the simulations. Finally, treatments were performed on 16 subjects and a total of 78 body areas to assess the clinical results generated by the RF electrodes for skin tightening and body contouring. Results: In-silico studies emulated the superficial and deep dispersion of heat due to the release of RF energy into human skin tissue. The rotating electrodes (monopolar and bipolar) and the selected RF frequency (0.5 and 1 MHz) determined the homogeneity of the thermal distribution, the penetration depth (between 4.37 mm and 25.0 mm) and the heating dynamics (between 30 and 100 seconds to reach the target skin temperature), which were confirmed by ex-vivo and in-vivo tests. Marigold(Tagetes erecta): The Potential Value in the Phytoremediation of Chromium

Environmental pollution with chromium （Cr） is harmful to humans, animals and plants, while in plants it causes diminished growth, anatomical alterations and death. In the present study, the potential value of marigold （Tagetes erecta） in the phytoremediation of Cr has been investigated. The randomized experimental design involved the exposure of plants to nutrient solutions containing 0.00, 0.04, 0.08, 0.12, 0.16 or 0.24 mmol L-1 Cr（III）. Chromium toxicity was observed at Cr（III） concentrations ≥ 0.12 mmol L-1 as demonstrated by diminished growth of the aerial parts and reduced density of the root system. Increasing Cr（III） concentrations in the nutrient solution resulted in a higher bioaccumulation of total Cr in the tissues, although translocation from roots to aerial parts was not efficient （maximum value of 25% at 0.12 mmol L-1 Cr（III））. The Cr bioaccumulation was up to 11-fold greater in roots than in the aerial parts. Tagetes erecta exhibited leaf plasticity when exposed to Cr, indicating the existence of a tolerance mechanism to Cr in this species. Chromium caused a reduction in xilem vases, resulting in a plastic effect in T. erecta leaves that increased the metal tolerance in culture solution. Tagetes spp. are potential Cr hyperaccumulators; at Cr（III） concentrations up to 0.12 mmol L-1, the plants accumulated levels above that proposed for hyperaccumulators and still maintained a considerable growth and even flourished. However, this study was conducted in nutrient solution, and studies on species confirmation as Cr hyperaccumulator should be conducted in soils for further clarification.

XEN^(■) implant and trabeculectomy medium-term quality of life assessment and comparison of results

AIM: To evaluate and compare the quality of life of patients submitted to XEN~? implant or trabeculectomy and the relationship with potentially involved ***: A cross-sectional study of patients with advanced open-angle glaucoma who underwent implantation of XEN~?(group 1) and trabeculectomy(group 2) between October 2015 and February 2017. The studied variables were: age, gender, follow-up time, need of topical anti-hypertensive therapy, visual acuity and intraocular pressure(IOP). The quantification of the quality of life was attained through the Glaucoma Symptom Scale(GSS) questionnaire. RESULTS: Totally 34 eyes(34 patients) were included, 17 in each group. The mean GSS scores for group 1 were 42.6±6.8(median, 47; p25, 36.5; p75, 48.5) and for group 2 it was 41.6±7.0(median, 43; p25, 36.5; p75, 47.0; P=0.34). There was a strong negative correlation between the need for topical anti-hypertensive drugs and the GSS result in both groups(r=-0.88, Panalysis demonstrates the noninferiority of medium-term quality of life of one group in relation to the other(XEN~? implant and trabeculectomy). The number of topical anti-hypertensive drugs and IOP negatively influenced the quality of life.

The Amazon basin has experienced an extreme drought that started in the austral summer of 2022-23 and extends into 2024. This drought started earlier than other previous droughts. Although some rain fell during the austral summer, totals remained below average. Higher temperatures during austral winter and spring 2023, which affected most of Central South America, then aggravated drought conditions. This coincided with an intense El Niño and abnormally warm tropical North Atlantic Ocean temperatures since mid-2023. Decreased rainfall across the Amazon basin, negative anomalies in evapotranspiration (derived from latent heat) and soil moisture indicators, as well as increased temperatures during the dry-to-wet transition season, September-October-November (SON) 2023, combined to delay the onset of the wet season in the hydrological year 2023-24 by nearly two months and caused it to be uncharacteristically weak. SON 2023 registered a precipitation deficit of the order of 50 to 100 mm/month, and temperatures +3˚C higher than usual in Amazonia, leading to reduced evapotranspiration and soil moisture indicators. These processes, in turn, determined an exceptionally late onset and a lengthening of the dry season, affecting the 2023-2024 hydrological year. These changes were aggravated by a heat wave from June to December 2023. Drought-heat compound events and their consequences are the most critical natural threats to society. River levels reached record lows, or dried up completely, affecting Amazonian ecosystems. Increased risk of wildfires is another concern exacerbated by these conditions.

Standardizing MI-TLIF, a Proposal for a Reproducible Technique

Background: Minimally invasive transforaminal lumbar interbody fusion (MI TLIF) is a widely known and performed technique, however its versatility among different physicians continues to hinder its replication and results. Therefore, this study aimed to provide a step-by-step surgical guide to perform a safe MI-TLIF, based on the results obtained in patients operated on by a single surgeon over a period of 12 years. Patients and methods: A retrospective, single center, longitudinal, and observational cohort study was conducted with 931 patients who underwent MI TLIF by a single surgeon between 2010 and 2022 using the technique described on this paper, each with a minimum follow-up of 12 months. Criteria included Schizas classification, listhesis according to Meyerding classification, number of levels treated, cage size, and complications (screw repositioning or cerebrospinal fluid leak). Patient clinical outcomes were assessed using the Oswestry Disability Index (ODI), Visual analog Scale (VAS) for pre- and postoperative radicular pain. Thin slice CT scans were used to assess the progression of the fusion using the Bridwell classification. In the statistical analysis, percentages, median, and interquartile range (IQR) were calculated. Results: Nine hundred and thirty one patients underwent MI TLIF using the technique described, eight hundred and eighty (94.5%) had a single level treated and fifty one (5.5%) had a 2 level procedure (982 levels), an 8mm cage was placed on five hundred and seventeenlevels (52.7%), six hundred and sixty three levels(67.6%) achieved grade I fusion, two hundred and sixty six levels (27.1%) achieved grade II fusion, 52 levels (5.3) achieved grade III fusion and one level (0.1) achieved a grade IV fusion or non-union. Revision surgery was performed on 3 patients (0.3%) for screw repositioning, cerebrospinal fluid leak was present on 2 patients during surgery and treated before closure. Ethanol exposed maturing rat cerebellar granule cells show impaired energy metabolism and increased cell death after oxygen-glucose deprivation

Alcohol, a widely abused drug, has deleterious effects on the immature nervous system. This study investigates the effect of chronic in vitro ethanol exposure on the metabolism of immature rat cerebellar granular cells(CGCs) and on their response to oxygen-glucose deprivation(OGD). Primary CGC cultures were exposed to ethanol(100 mm in culture medium) or to control ethanol-free medium starting day one in vitro(DIV1). At DIV8, the expression of ATP synthase gene ATP5 g3 was quantified using real-time PCR, then cultures were exposed to 3 hours of OGD or normoxic conditions. Subsequently, cellular metabolism was assessed by a resazurin assay and by ATP level measurement. ATP5 g3 expression was reduced by 12-fold(P = 0.03) and resazurin metabolism and ATP level were decreased to 74.4 ± 4.6% and 55.5 ± 6.9%, respectively after chronic ethanol treatment compared to control values(P < 0.01). Additionally, after OGD exposure of ethanol-treated cultures, resazurin metabolism and ATP level were decreased to 12.7 ± 1.0% and 9.0 ± 2.0% from control values(P < 0.01). These results suggest that chronic ethanol exposure reduces the cellular ATP level, possibly through a gene expression down-regulation mechanism, and increases the vulnerability to oxygen-glucose deprivation. Thus, interventions which improve metabolic function and sustain ATP-levels could attenuate ethanol-induced neuronal dysfunction and should be addressed in future studies.
