BACKGROUND Toll-like receptor-2(TlR2) is responsible for recognizing Helicobacter pylori(***) and activating the immune response. Polymorphisms in TlR2 may modulate gastric *** To evaluate whether the TlR2 19216 T/C(rs3804099) and TlR2-196 to-174 ins/del(rs111200466) polymorphisms contribute to gastric carcinogenesis in the Brazilian population, and to determine the influence of both polymorphisms and H. pylori infection on TlR2 mRNA *** DNA was extracted from 854 peripheral blood leukocyte or gastric tissue samples[202 gastric cancer(GC), 269 chronic gastritis(CG), and 383 control/healthy(C)]and genotyped by allele-specific PCR or restriction fragment length polymorphism(RFlP)-PCR. Quantitative polymerase chain reaction by Taq Man■ assay was used to quantify TlR2 mRNA levels in fresh gastric tissues(48 GC, 36 CG, and 14 C).RESUlTS Regarding the TlR2-196 to -174 polymorphism, the ins/del and del/del genotypes were associated with a higher risk of GC by comparison with the C in all of the analyzed inheritance models(codominant, dominant, recessive, overdominant and log-additive;P larly, an increased risk was observed when comparing the GC and CG groups [codominant(P log-additive(P lR2 19216 T/C was associated with a protective effect in the GC group compared to the C group [dominant(P = 0.0420) and log-additive(P =0.0300)]. Regarding the association of polymorphisms with H. pylori infection,individuals infected with H. pylori and harboring the TlR2-196 to-174 ins/del polymorphism had an increased risk of gastric carcinogenesis [codominant(P =0.0120), dominant(P = 0.0051), overdominant(P = 0.0240) and log-additive(P =0.0030)], while TlR2 19216 T/C was associated with a protective effect[codominant(P = 0.0039), dominant(P log-additive(P = 0.0021)]. Modern design of superior multi-functional alloys composed of several principal components requires in-depth studies of their local structure for developing desired macroscopic ***,peculiarities of atomic arrangements on the local scale and electronic states of constituent elements in the single-phase face-centered cubic(fcc)-and body-centered cubic(bcc)-structured high-entropy Alx-CrFeCoNi alloys(x=0.3 and 3,respectively)are explored by element-specific X-ray absorption spectroscopy in hard and soft X-ray energy *** based on the reverse Monte Carlo approach allow to perform a simultaneous fit of extended X-ray absorption fine structure spectra recorded at K absorption edges of each 3d constituent and to reconstruct the local environment within the first coordination shells of absorbers with high *** revealed unimodal and bimodal distributions of all five elements are in agreement with structure-dependent magnetic properties of studied alloys probed by magnetometry.A degree of surface atoms oxidation uncovered by soft X-rays suggests different kinetics of oxide formation for each type of constituents and has to be taken into account.X-ray magnetic circular dichroism technique employed at l2,3 absorption edges of transition metals demonstrates reduced magnetic moments of 3d metal constituents in the sub-surface region of in situ cleaned fcc-structured Al0.3-CrFeCoNi compared to their bulk *** to nanostructured versions of multicomponent alloys,such studies would bring new insights related to effects of high entropy mixing on low dimensions.

Natural regeneration is the interaction of natural processes to restore the forest ecosystem. Its dynamics are influenced by the intensity and extent of a series of abiotic and biotic factors, which may be intrinsic or extrinsic. Knowing the importance of establishing natural regeneration within forest ecosystems, this work aimed to evaluate whether litter depth and trail distance influenced seedling abundance and richness in a forest fragment undergoing natural restoration. The hypothesis tested in this research was that abiotic factors influence the natural regeneration of this forest since they are factors that are directly linked to seed germination and seedling establishment. 30 plots of 1 m2 were randomly analyzed within the forest located on the brown trail. A millimeter ruler was used to diagnose the litter depth and a measuring tape to measure the distance from the plot to the edge. In each plot the seedlings were morpho-specified and each morphospecies had the number of individuals counted. linear regression tests were performed to assess the relationships between species richness and trail distance and litter depth. The same was done for species abundance. All results showed that there is no relationship pattern between any of the variables. Other factors also influence the regeneration of a forest, such as luminosity and seed bank. Furthermore, litter depth is related to the successional stage of the forest. It was concluded that the abiotic factors tested do not influence the regeneration of the study area.

Chronic pain, a multidimensional experience affecting individuals’ sensory, cognitive, and emotional aspects, significantly impacts their quality of life. Post-laminectomy syndrome, a condition characterized by persistent back pain following spinal surgery, often leads to disability and increased healthcare utilization. Methods: This randomized, controlled, blind clinical trial aimed to investigate the efficacy of Transcranial Direct Current Stimulation (tDCS) in managing pain from post-laminectomy syndrome in patients. Twenty-four participants were assigned to three groups: sham stimulation, active stimulation over primary motor cortex (M1), or stimulation over dorsolateral prefrontal cortex (DlPFC). Stimulation was administered for five consecutive days, 20 minutes per session, using a current of 1.5 mA through 25 cm2 electrodes. Pain intensity was assessed using Visual analog Scale (VAS) before, during, and after intervention. Results: An ANOVA model demonstrates significant reduction in pain intensity compared to baseline in VAS, (F(7, 285) = 12.292;p 0.001;Power = 1.000;η2p = 0.534), in tDCS applied to M1, after five days of intervention. After stimulation, a significant improvement was observed in WHOQol-Bref Quality of life item 1 (p = 0.04), considering statistical significant difference p 0.05. Correlation between the variables: quality of life, depression, anxiety and pain also demonstrates reduction in depression and anxiety according to Beck’s Depression and Anxiety Inventories (BDI and BAI), p 0.05. This effect was not observed in DlPFC stimulation group. Patients who believed they received active stimulation, in sham group, demonstrated potential for effective blinding. Conclusion: The tDCS applied to primary motor cortex effectively improved pain management and psychiatry symptoms in post-laminectomy syndrome patients. An Instrument for Perceiving Ethical Problems in Primary Healthcare: Psychometric Parameters and Ethical Components

Ethical issues in primary (health) care (PC) are common and are confronted daily. In the scarce literature that was available, Zoboli and Silva developed an instrument (IPE-APS) for perceiving ethical conflicts. Two empirical ex post facto studies to gather psychometric evidence for the instrument were outlined. In the first study, 88 nurses and physicians from Brazil’s Family Health (Saúde da Família—SF) responded to the IPE-APS, and the results confirmed its validity and accuracy. In total, 14 items were grouped in Factor I (relationship/responsibility), which was described as having human rights and deontological nature (Cronbach’s alpha = 0.92), and 11 items were grouped in Factor II, which was described as having a labor structure nature (Cronbach’s alpha = 0.90). In the second study, 207 SF professionals in Goiania, Goiás responded to the IPE-APS. The results showed that Factor I had a high factorial congruence index (0.98), which revealed high similarity and showed that the instrument displayed practical validity to identify and assess ethical problem perception in PC.

Cutaneous leishmaniasis in an indigenous infant with Down’s syndrome: A case report

Rationale: leishmaniasis, caused by the protozoan leishmania and transmitted by sandflies, is endemic to the tropical and subtropical areas of Brazil. It is one of the neglected tropical diseases in the world. Patient concerns: A 20-month-old indigenous infant with severe malnutrition and Down’s syndrome presented with a facial ulcer for 5 months. Diagnosis: Giemsa staining of scraped ulcer tissues indicated the presence of the amastigote form of leishmania sp., and positive Montenegro’s intradermal test helped in diagnosing the condition as cutaneous leishmaniasis. Interventions: The child was hospitalized and received intravenous treatment for cutaneous leishmaniasis(1 mg/kg/day of amphotericin B). Outcomes: The condition was cured with amphotericin B(1 mg/kg/day for 14 d). lessons: Because of infanticide practices in indigenous cultures, indigenous infants with Down’s syndrome rarely survive. Thus, no similar case has been reported in the literature.

Background： Brazil is among the nations with the greatest rates of annual cocaine usage. Pharmacological treatment of cocaine addiction is still limited, opening space for nonconventional *** Q-potencies of opium and Erythroxylum coca have been tested in the integrative treatment of cocaine craving among homeless addicts, but this setting had not proven feasible, due to insufficient ***： This study investigates the effectiveness and tolerability of homeopathic Q-potencies of opium and E. coca in the integrative treatment of cocaine craving in a community-based psychosocial rehabilitation ***, setting, participants, and interventions： A randomized, double-blind, placebo-controlled, parallelgroup, eight-week pilot trial was performed at the Psychosocial Attention Center for Alcohol and Other Drugs（CAPS-AD）, Sao Carlos/SP, Brazil. Eligible subjects included CAPS-AD patients between 18 and 65 years of age, with an International Classification of Diseases-10 diagnosis of cocaine dependence（F14.2）. The patients were randomly assigned to two treatment groups： psychosocial rehabilitation plus homeopathic Q-potencies of opium and E. coca（homeopathy group）, and psychosocial rehabilitation plus indistinguishable placebo（placebo group）.Main outcome measures： The main outcome measure was the percentage of cocaine-using days.
