Beta-blocker therapy in elderly patients with renal dysfunction and heart failure

OBJECTIVE To assess the role of beta-blockers(BB)in patients with chronic kidney disease(CKD)aged≥75 *** AND rESUlTS From January 2008 to July 2014,we included 390 consecutive patients≥75 years of age with ejection fraction≤35%and glomerular filtration rate(GFr)≤60 m l/min per 1.73 m^*** analyzed the relationship between treatment with BB and mortality or cardiovascular *** mean age of our population was 82.6±4.1 *** ejection fraction was 27.9%±6.5%.GFr was 60-45 m l/min per 1.73 m^2 in 50.3%of patients,45-30 m l/min per 1.73 m^2 in 37.4%,andl/min per 1.73 m^2 in 12.3%.At the conclusion of follow-up,67.4%of patients were receiving *** median follow-up was28.04(Ir:19.41-36.67)*** the study period,211 patients(54.1%)died and 257(65.9%)had a major cardiovascular event(death or hospitalization for heart failure).BB use was significantly associated with a reduced risk of death(Hr=0.51,95%CI:0.35-0.74;Preceiving BB consistently showed a reduced risk of death across the different stages of CKD:stage IIIa(GFr=30-45 m l/min per 1.73 m^2;Hr=0.47,95%CI:0.26-0.86,Pr 30-45 m l/min per 1.73 m^2;Hr=0.55,95%CI:0.26-1.06,P=0.007),and stages IV and V(GFrl/min per 1.73 m~2;Hr=0.29,95%CI:0.11-0.76;P=0.047).CONClUSIONS The use of BB in elderly patients with HFr EF and renal impairment was associated with a better *** of BB should be encouraged when possible.

We describe the development of quinolylnitrones(QNs)as multifunctional ligands inhibiting cholinesterases(ChEs:acetylcholinesterase and butyrylcholinesterase—h BChE)and monoamine oxidases(hMAO-A/B)for the therapy of neurodegenerative *** identified QN 19,a simple,low molecular weight nitrone,that is readily synthesized from commercially available *** 19 has no typical pharmacophoric element to suggest ChE or MAO inhibition,yet unexpectedly showed potent inhibition of h BChE(IC50=1.06±0.31 nmol/l)and h MAO-B(IC_(50)=4.46±0.18μmol/l).The crystal structures of 19 with hBChE and hMAO-B provided the structural basis for potent binding,which was further studied by enzyme *** 19 acted as a free radical scavenger and biometal chelator,crossed the blood—brain barrier,was not cytotoxic,and showed neuroprotective properties in a 6-hydroxydopamine cell model of Parkinson's *** addition,in vivo studies showed the anti-amnesic effect of 19 in the scopolamine-induced mouse model of AD without adverse effects on motoric function and ***,chronic treatment of double transgenic APPswe-PS1δE9 mice with 19 reduced amyloid plaque load in the hippocampus and cortex of female mice,underscoring the disease-modifying effect of QN 19.

Insular marine biotas are often richer in faunal diversity than those from the open sea in the same geo- graphical region. The existence of particular island effects were tested under polar conditions by compar- ing infaunal benthic assemblages on Peter I Island with those of similar latitudes in the open ocean at the Bellingshausen Sea and also from the coast of the Antarctic Peninsula. Sampling was carried out aboard the r/V Hesperides during the expedition named BENTArT-2003 from 24 January to 3 March 2003. Benthic samples were collected at 18 stations ranging from 90 to 2 044 m depth, using an USNEl-type box corer （BC） dredge. representatives of 32 higher taxa of invertebrates were found. Univariate and multivariate analyses revealed various patterns in the data. First, significant differences in polychaete abundance were detected between the stations located in the open sea and the remaining sites （island plus mainland sites）. Bivalve abundances were also distinct between island and mainland sampling sites. Sediment columns taken from the island seafloor exhibited the highest rate of bioturbation by the infauna. These findings confirm the model that islands develop distinct assemblages characterized by the enhancement of the benthic commu- nities even in cold waters. Several abiotic factors were measured simultaneously at the seafloor and along the water column to investigate faunal distribution patterns. Significant correlations were found between the benthic assemblages and a combination of two environmental variables：＂island effect＂（measured as a categorical variable） and the redox state of sediments. richer and more complex benthic assemblages were found in Peter I Island＇s sea bottom, whereas the more depauperate bottoms remained in the open sea.

全球尺度上的凋落物受气候,土壤特征和植被结构的影响,随着植被现存生物量的增加,从草地到森林的凋落物产量增加。然而,在景观尺度上,凋落物的产量与植被结构之间的相同关系却很少被研究,相邻的结构不同的群落之间凋落物格局的比较研究也比较缺乏。本研究使用一种标准化的方法来描述四种稀树草原地貌之间的结构差异,并分析了在巴西塞拉多保护区(Cerrado)中地貌与凋落物变化的关系。我们评估了平均分布在4个地貌的48个地点的木本植被结构和组成,并从2014年4月到2015年3月每月监测凋落物产量。研究结果表明,不同地貌的木本植被密度、断面积、柱状体积和地上生物量各不相同。地上生物量与年凋落物产量之间存在着很强的正相关关系,凋落物产量随植被结构的增加从0.9增加到8.4 Mg ha^(−1)。不同植被类型间的月产量具有季节性和相似性,旱季产量增加。叶组成显性部分(约85%)和凋落物的季节性主要是由于干旱月份落叶集中所致。然而,可能是随着植物群落中落叶物种丰度的减少,全年凋落物的时间格局显示出从开放植被类型到封闭植被类型的季节性逐渐减少。我们的研究结果表明,植被结构的变化可能会影响不同地貌凋落物的空间和时间分布格局,这些格局在整个塞拉多保护区景观中同时发生,可能对该生态系统的整体功能产生影响。此外,这些发现强调了使用标准化方法是正确比较不同环境中凋落物格局的必要条件。

Habitat, occurrence and conservation status of the Saharo-Macaronesian and Southern-Mediterranean element Fagonia cretica l. (Zygophyllaceae) in Italy

Fagonia cretica l. is an important component of Mediterranean dry grasslands and a rare and isolated species of Italian flora. In this study, an assessment is presented on the distribution, habitat, and conservation status of F. cretica in Italy. The results of field investigation and herbarium analysis show that this species grows in a small area within the southern Calabria region characterized by a warmest and driest Mediterranean climate on the Italian peninsula. F. cretica is a semi-desert plant species growing in Italy in only one peripheral and isolated population at the northern limit of its distribution. Plant community analysis, using the phytosociological method, shows that F. cretica grows in wintergreen perennial dry grasslands dominated by lygeum {partum and Hyparrhenia hirta. F. crelica plant communities are located in thermo-xeric habits such as south-, southeast- and east-facing slopes on clays and sandy clays in southern Calabria. The population of F. cretica is fragmented in six neighbouring localities, with two of which belonging to a Site of Community Importance （SC1）. The conservation status of F. cretica population is not very good, and is defined as ＂Critically Endangered＂ in accordance with IUCN criteria. There are many threats affecting the F. crelica population in ltaly, primarily the changes in land uses due to urban expansion and reforestation with exotic plants. The southern end of the Italian peninsula hosts other plants from thermo-xeric habits that do not adapt to the current local climate. This territory can be considered as a microrefugia for plants currently distributed in the arid territory of the southern Mediterranean. These results contribute to the discussion of some conservation measures, and the possibility of establishing a micro-reserve. For all these reasons, we propose to include F. cretica in the lists of protected plant species at regional （Calabria） and country （Italy） scales in Italy.

The essential oil from Melaleuca leucadendra *** has been widely used as a perfume and traditional remedy,cosmetics and pharmaceutical products ingredient since many years *** common technology to recover the oil is hydro-distillation and ***,all oil can not be fully extracted from the leaves by this method due to the recalcitrant structure of leaves that hindrance the access of the *** a submerged fermentation as a pretreatment step prior to the extraction process helped to loosen the lignocellulose structure and enhance oil release in the extraction *** this study,the raw materials were collected from the natural forest in Buru Island,Maluku,*** biological agents applied in these processes were Phanerochaete chrysosporium ITBCC136 and Trichoderma viride *** oil extraction process was conducted by method of steam-distillation,the oil was analysed using gas chromatography-mass spectroscopy(GC-MS),and the lignocellulose content in the biomass was measured by the fractionation *** treatment using *** provided the highest increase in yield percentage up to 3.47%as compared with control of 1.45%,with the lowest percentages of the remained cellulose,while the fermentation with the presence of *** did not affect the oil yield even the lignin content was decrease as much as 21%.The percentages of 1,8-cineole in the oil were almost unchanged,which was about 20%of the oil.

From conventional two-stage hepatectomy to AlPPS:Fifteen years of experience in a hepatobiliary surgery unit

Background:Hepatectomy in patients with large tumor load may result in postoperative liver failure and associated complications due to excessive liver parenchyma *** two-stage hepatectomy(TSH)and associating liver partition and portal vein ligation for staged hepatectomy(AlPPS)technique are possible solutions to this *** liver metastases(CrlM)is the most frequent indication,and there is a need to assess outcomes for both techniques to improve surgical and long-term oncological outcomes in these ***:A single-center retrospective study was designed to compare TSH with AlPPS in patients with initially unresectable bilateral liver tumors between January 2005 and January *** was performed from January 2012 onwards as the technique of ***-term overall survival(OS)and disease-free survival(DFS)were evaluated as primary outcome in CrlM *** morbidity,mortality and liver growth in all patients were also ***:A total of 38 staged hepatectomies were performed:17 TSH and 21 *** resection rate was 76.5%(n=13)in the TSH group and 85.7%(n=18)in the AlPPS group(P=0.426).Overall major morbidity(Clavien-Dindo≥3 a)(stage 1+stage 2)was 41.2%(n=7)in TSH and 33.3%(n=7)in AlPPS patients(P=0.389),and perioperative 90-day mortalities were 11.8%(n=2)vs.19.0%(n=4)in each group,respectively(P=0.654).Intention-to-treat OS rates at 1 and 5 years in CrlM patients for TSH(n=15)were 80%and 33%,and for AlPPS(n=17)76%and 35%,*** rates at 1 and 5 years were 36%and 27%in the TSH group vs.33%and 27%in the AlPPS group,***:AlPPS is an effective alternative to TSH in bilateral affecting liver tumors,allowing higher resection rate,but patients must be carefully *** CrlM patients similar long-term OS and DFS can be achieved with both techniques.
