The treatment of peritoneal carcinomatosis (PC) of colorectal origin with cytoreductive surgery(CRS) plus hyperthermic intraperitoneal chemotherapy (HIPEC) has a 5-year recurrence-free or cure rate of at least 16%, so it is no longer labeled as a fatal disease, and offers prolonged survival for patients with a low peritoneal carcinomatosis index. Metachronous PC of colorectal origin is so predictable that there is a model which has been used to successfully determine the individual risk of each patient. Patients at risk are clearly identified; those with the highest risk have small peritoneal nodules present in the first surgery (70% probability of developing PC), ovarian metastases(60%), perforated tumor onset or intraoperative tumor rupture(50%). Current clinical, biological and imaging techniques still lack sufficient sensitivity to diagnose PC in its initial stages, when CRS plus HIPEC has a greater impact and a higher cure rate. Second-look surgery with HIPEC or prophylactic HIPEC at the time of the first intervention have been proposed as means of preventing and/or anticipating clinical or radiological relapse in at-risk patients. Both techniques have shown a significant decrease in peritoneal relapses and should be considered essential weapons in the management of colorectal cancer.

Background: Leptospirosis is an important zooanthroponotic disease spread worldwide which infection is recognized as a re-emergent disease. Leptospirosis is a systemic disease of humans and domestic animals, mainly dogs, cattle and swine, characterized by fever, renal and hepatic insufficiency, pulmonary manifestations and reproductive failure. Objective: To study the seroprevalence of anti-Leptospira antibodies in an urban canine population. Materials and Methods: The study was performed in March 2014 in stray dogs that had been captured in the districts of Coyoacan, Iztapalapa, Iztacalco and Benito Juarez in Mexico City;117 dogs were analyzed, from each of which 5 ml of blood were obtained. Serology was performed using a MAT test (Microscopic Agglutination Test), finding that the antigens corresponded to 13 serotypes of Leptospirainterrogans. Three age groups were formed: Group 1) younger than a year (n = 28), Group 2) 1 to 6 years (n = 75), and Group 3) older than 6 years (n = 14). Results: Of the analyzed sera, 28.2% were seropositive to one or more Leptospira serotypes, 74% of the positive seracoagglutinated with two or more serotypes. The most frequent serotypes were: Canicola, Icterohaemorrhagiae and Portland-vere. Serotypes Canicola, Pyrogenes, and Bratislava resulted statistically significant (p 0.001). The presence of anti-Leptospira antibodies in the blood of animals of 1 to 6 years resulted with a p 0.001. The sera from Iztapalapa reacted to nine serotypes. Conclusion: The 28.2% of seropositivity indicates Leptospira transmission in the canine population that was studied could exhibit a potential public health risk.

Epithelioid sarcoma(ES), a mesenchymatous malign neoformation, is often diagnosed in later stages and associated with high recurrence index, metastasis and mortality. We report a case of a 65 years old male, with history of abdominal pain and upper gastrointestinal bleeding. Endoscopy demonstrated a posterior duodenal wall perforation communicating with a solid retroperitoneal neoformation. Endoscopic biopsy was performed, with a final report of ES. The patient was submitted for surgical palliation due to the tumor's unresectability. Retroperitoneal ES is an extremely rare condition with limited reports in the literature where guidelines for its optimal treatment are not well established.

In the last decades we have witnessed a significant increase in the incidence of type 2 diabetes mellitus, and obesity in parallel. So 90% of patients with Diabetes Mellitus type 2 (DM2) are obese. Changes in lifestyle (diet and exercise), behavioral therapies or the use of certain anti-obesity drugs have shown a limited effect and are not maintained over time. Currently, bariatric surgery is the most effective anti-obesity treatment, which is durable and resolves associated comorbidities. The surgical treatment of obesity for the modification of the anatomy of the digestive system is to produce gastric capacity constraints alone or associate with varying degrees of intestinal malabsorption and reduce body weight, improve and maintain the comorbidities associated weight loss by reduced food intake alone or in combination with malabsorption of this. The development of these surgical techniques has occurred in stages. With these techniques, setting to bariatric surgery is an effective, safe and proven procedure for the treatment of obesity and its complications especially DM2, as their post surgery referral mechanisms provide us with information for potential therapeutic treatment aimed at optimizing the control DM2 patients metabolic obese. In this review, we expose the evidence in treatment of DM2 with bariatric surgery and the actual hypothesis trying to explain how it is possible.

Feeding Behavior, Body Weight and Growth Rate during Post-Deprivation Period in Rats

Deprivation is an environmental experience that organisms are continually exposed to. However, few studies analyze deprivation effects on the consumption pattern during the post-deprivation period systematically. The aim of this study was to evaluate the deprivation effect on feeding pattern, growth rate and body weight during post-deprivation. Sixteen albino rats (3 months old at the beginning of the experiment) were exposed to 72-hour total food deprivation every 15 days. Food and water were freely available during non-deprivation periods. Rats completed three cycles comprised by a free access period followed by food deprivation. Once deprivation was removed, food and water consumption increased and then decreased, while body weight was recovered. Growth rate increased after every deprivation period and reached similar levels to those presented by the control group. Depriving rats from one commodity (i.e., food) cause them to restrain their consumption of the other freely available commodity (i.e., water). Results confirm that food deprivation modifies growth rate, water and food consumption. Additionally, during post-deprivation period, differences between males and females were not registered.

Background:More than a decade of fire suppression has changed the structure of fire-adapted shrubland ecosystems in Spain’s National Parks,which are now at extreme risk of uncontrolled *** burning can mitigate the risk of wildfires by reducing the fuel load but prescribed burning may also alter the soil properties and reduce microbial and fungal activity,causing changes in the availability of nutrients deep in the soil *** fungal communities are a vital part of post-fire restoration,some fire effects remain *** examine the short-term effects of prescribed burning on soil fungal communities in Doñana Biological Reserve(SW Spain),we collected soil samples pre-burn and 1 day,6 and 12 months post-burn from burned plots to perform physicochemical and metabarcode DNA ***:Prescribed burning had no significant effect on the total fungal operational taxonomic unit richness and ***,changes in soil pH,nitrogen and potassium content post-burn affected fungal community *** non-significant changes in pH and phosphorous affected the composition of ectomycorrhizal ***:The ectomycorrhizal fungal community appears to be resilient to the effects of low-to moderate-intensity fires and saprotrophic taxa may benefit from this kind of *** finding revealed that prescribed burning is a potentially valuable management tool for reducing fire hazards in shrublands that has little effect on the total richness and abundance of fungal communities.

Patients with diabetes are characterized by the development of cardiovascular complications: nephropathy, retinopathy, neuropathy, ischemia or hypertensive etc. Therefore, the cardiovascular involvement is the leading cause of death in patients with Diabetes Mellitus type 2 (DM2). Despite intensive treatment on classical factors of cardiovascular disease (blood pressure levels, LDL cholesterol, etc.), patients with diabetes have a high number of cardiovascular events and the onset and prognosis of these are related to glycemic control parameters, glycosylated hemoglobin (HbA1c). On the other hand, the question of the cardiovascular protective effect of some hypoglycemic treatments has been raised, asking what he has done to know more accurately about the safety and cardiovascular effects of the treatments we have today. The two most important incretin hormones are GIP (gastric inhibitory polypeptide) and GLP-1 (glucagon-like peptide-1). Treatment based on GLP-1 is a novel weapon in T2DM that achieves a reduction in HbA1c with other metabolic effects: weight loss and extra effect in dyslipidemia and blood pressure. In the last years other beneficial actions such a protector effect against myocardium ischemia and other actions in basals were reported. In this article we will try to explain the evidence of GLP-1 treatments and its cardiovascular effects.

The objective of this study was to examine longitudinally the effects of exenatide on different physical and biochemical markers, evaluated in adult type 2 diabetic patients with cardiovascular risk. Data were recorded from 10 patients who attended the outpatient primary care health center Mariano Iago Yecla, Murcia province, Spain in the period of December 2009 to October 2011 and who were treated with Byetta®. Differences were statistically significant (p 0.05) in HbA1c from the third month of treatment, and trends of decrease in body weight from the third week of treatment. There was a significant and better glycemic control. Overall effect was interpreted as a sensitizer drug of the parameters evaluated. Randomized studies are recommended with a minimum follow-up of 2 years, to see if the results are maintained over time.
