Introduction: One of the most common causes of hyperthyroidism is Graves’ disease. The malignant exophthalmos is a rare condition characterized by the loss of visual function that doesn’t adequately respond to various forms of treatment, requiring, in most cases, surgical correction. Objective: We report a case of conjunctival edema and protrusion of the eyeball in patients secondary to Graves’ disease, characterizing malignant ophthalmopathy and discuss their surgical treatment. Case Report: A 73-year-old male patient presenting bilateral conjunctive hyperaemia, decreased visual acuity, exophtalmos with progressive deterioration. Pulse therapy was performed without improvement and then underwent bilateral ethmoidectomy. Discussion: Late diagnosis is harmful due to the fact that malignant disease is extremely aggressive and fast, as reported in this case. Most often, the ophthalmopathy follows a relatively mild and self-limiting course, making a proper diagnosis difficult if the disease progresses. In extreme cases acute orbital decompression is indicated complementing appropriate medical treatment, which will allow herniation of orbital contents. Many orbital decompression techniques have been used in the treatment of Graves’ ophthalmopathy. The most common of these is the endoscopic sinus surgery which potentiated act in decompression of the medial wall safely and minimally invasive. In our case, open bilateral ethmoidectomy was used, due to the fact that surgeons are most experienced in this technique. Although the post-operative has been successfully submitted, no signs of recurrence were observed after fifteen days due to the evolution of Graves’ disease. Even with outpatient treatment, he walked to the total visual loss. Conclusion: We observed that the malignant ophthalmopathy is rare and aggressive;however, in spite of therapeutic resources used, it can progress to total visual loss.

The percentage of diabetic patients with contaminated lesions increases from 3% to 10%. Treatment with herbal products shows benefits in their use, as well as antioxidant and antifungal activities. Objective: to evaluate the antimicrobial action of 10% banana peel gel and the contraction of diabetic and venous wounds. Methods: individual, analytical, interventional, longitudinal, prospective, randomized study from February to December 2015. Five patients were included in the study;3 with venous ulcer and 2 with diabetic wound. Application of 10% green banana peel gel and weekly samples were performed. After six days, the second collection was performed. The samples were seeded in the Mannitol salt agar culture medium, MacConkey agar and Saboraund agar with chloramphenicol for isolation of cocci and Gram-positive and Gram-negative bacilli;and fungi. The total counts of bacteria were determined by PCA (Plate Count Agar) and measurement of the lesion margin. There was a reduction of microorganisms with the use of the gel in 53.57% of the patients, and reduction of wound areas in 48.1%.

Care Technologies in Nursing for People with Colorectal Neoplasia: Integrative Review

Objective: Identifying in the literature the care technologies used by nurses for people with colorectal neoplasia on Nietzsche’s conceptual perspective. Method: It consists in an integrative review of the literature held in the database: Latin-American Literature and Caribbean in medical health sciences Literature analysis and Retrieval System Online, Database in Nursing, US National Library of Medicine, Cumulative Index to Nursing & Allied Health Literature, Scopus Info Site and Web of Science in December 2015. Results: 28 articles were selected to compose the final sample of this review, which were analyzed and categorized in management technology (86.20%), assistance technology (20.68%) and educational technology (10.34%). Studies published in the last five years have predominated (62.06%), in international territory (82.75%) and with evidence level IV (44.82%). Conclusion: Today the growth on the production about this theme has been highlighted, but there is an important discrepancy between the researches with high and low level of scientific evidence, showing the need of more studies about technologies that strengthen the experience of nursing.

This manuscript collects in a joint and orderly manner the existing evidence at the present time about postoperative treatment with radiotherapy in non-small cell lung *** also systematically reviews the current evidence,the international recommendations in the most relevant guidelines,the most controversial aspects in clinical and pathological staging,the specific technical aspects of radiotherapy treatment,and also collects all the potential risk factors that have been postulated as significant in the prognosis of these patients,evaluating the possibility of segmenting a particularly sensitive subpopulation with a high risk of relapse on which an adjuvant treatment with radiotherapy could have an impact on their clinical ***,currently active trials that aspire to provide more evidence on this topic are reviewed.

Prospects for sociogenomics in avian cooperative breeding and parental care

For the last 40 years,the study of cooperative breeding(CB)in birds has proceeded primarily in the context of discovering the ecological,geographical,and behavioral drivers of *** advent of molecular tools in the early 1990s assisted in clarifying the relatedness of helpers to those helped,in some cases,confirming predictions of kin selection *** for genome-wide analysis of sequence variation,gene expression,and epigenetics promise to add new dimensions to our understanding of avian CB,primarily in the area of molecular and developmental correlates of delayed breeding and dispersal,as well as the ontogeny of achieving parental status in ***,we outline key ways in which modern-omics approaches,in particular genome sequencing,transcriptomics,and epigenetic profiling such as ATAC-seq,can be used to add a new level of ana-lysis of avian *** on recent and ongoing studies of avian social behavior and sociogenom-ics,we review how high-throughput sequencing of a focal species or clade can provide a robust foundation for downstream,context-dependent destructive and non-destructive sampling of spe-cific tissues or physiological states in the field for analysis of gene expression and epigenetics.-Omics approaches have the potential to inform not only studies of the diversification of CB over evolutionary time,but real-time analyses of behavioral interactions in the field or *** of birds represents a new branch in the network of methods used to study CB,and can help clarify ways in which the different levels of analysis of CB ultimately interact in novel and unexpected ways.

Effective prophylaxis with rFVIIa in young haemophiliacs with inhibitors using a schedule similar to FVIII prophylaxis in non-inhibitor patients

Objective: To assess the role of early prophylaxis with recombinant activated factor VII (rFVIIa) in young haemophiliacs with inhibitors and to determine whether it can reduce bleeding episodes and prevent joint damage. Patients and Methods: Ten severe haemophiliacs, less of three years old, with up to four joint bleeds and inhibitors to FVIII who started early prophylaxis with rFVIIa, were included. A number of haemorrhages/year/patient and haemarthroses/ year/patient were compared before the start of prophylaxis, which include both the time before (initial period) and after inhibitor diagnosis (inhibitor period), with those during prophylaxis (prophylaxis period). Results: The mean time of inhibitor diagnosis was 15.6 months (range: 2.3-34.1). The mean time between inhibitor diagnosis and the start of prophylaxis was 7.1 months (range: 0-23.2), shorter than the time of rFVIIa prophylaxis (mean: 10.3 months;range: 4.1-32.0). Bleeding episodes for the three time periods were 45, 36 and 17, respectively, or 0.29 and 0.51 haemorrhages/year/patient in the two periods prior to prophylaxis compared to 0.17 during prophylaxis. Total haemarthroses for the three-time periods were 7, 5 and 5, respectively. The haemarthroses/year/patient in the pre-prophylaxis period were 0.032 and 0.070, compared to0.049 inthe prophylaxis period. rFVIIa schedules were either 90 μg/kg three times weekly or 90 μg/kg daily. Conclusions: Early prophylaxis with rFVIIa may be efficacious in young haemophiliacs with inhibitors, reducing joint bleeds. After the risky period of inhibitor presence, they are able to continue rFVIII prophylaxis with success assured to prevent arthropathy.

Risk factors and classifications of hilar cholangiocarcinoma

Cholangiocarcinoma is the second most common primary malignant tumor of the *** cholangiocarcinoma or Klatskin tumor represents more than 50% of all biliary tract cholangiocarcinomas.A wide range of risk factors have been identified among patients with Perihilar cholangiocarcinoma including advanced age,male gender,primary sclerosing cholangitis,choledochal cysts,cholelithiasis,cholecystitis,parasitic infection(Opisthorchis viverrini and Clonorchis sinensis),inflammatory bowel disease,alcoholic cirrhosis,nonalcoholic cirrhosis,chronic pancreatitis and metabolic *** classifications have been used to describe the pathologic and radiologic appearance of *** three systems most commonly used to evaluate Perihilar cholangiocarcinoma are the Bismuth-Corlette(BC) system,the Memorial Sloan-Kettering Cancer Center and the TNM *** BC classification provides preoperative assessment of local *** Memorial Sloan-Kettering cancer center proposes a staging system according to three factors related to local tumor extent:the location and extent of bile duct involvement,the presence or absence of portal venous invasion,and the presence or absence of hepatic lobar *** TNM classification,besides the usual descriptors,tumor,node and metastases,provides additional information concerning the possibility for the residual tumor(R) and the histological grade(G).Recently,in 2011,a new consensus classification for the Perihilar cholangiocarcinoma had been *** consensus was organised by the European Hepato-PancreatoBiliary Association which identified the need for a new staging system for this type of *** classification includes information concerning biliary or vascular(portal or arterial) involvement,lymph node status or metastases,but also other essential aspects related to the surgical risk,such as remnant hepatic volume or the possibility of underlying disease.

TLR2 and TLR4 polymorphisms influence m RNA and protein expression in colorectal cancer

AIM: To evaluate the effect of promoter region polymorphisms of toll-like receptor(TLR)2-196 to-174 del and TLR4-1607T/C(rs10759932) on m RNA and protein expression in tumor tissue and of TLR4+896A/G(rs4986790) on colorectal cancer(CRC) ***: The TLR2-196 to-174 del polymorphism was investigated using allele-specific polymerase chain reaction(PCR) and the TLR4-1607T/C and TLR4+896A/G by PCR-restriction fragment length p o l y m o r p h i s m( R F L P). W e g e n o t y p e d 4 3 4 D N A samples from 194 CRC patients and 240 healthy individuals. The m RNA relative quantification(RQ) was performed in 40 tumor tissue samples by quantitative PCR Taq Man assay, using specific probes for TLR2 and TLR4 genes, and ACTB and GAPDH reference geneswere used as endogenous controls. Protein expression was analyzed by immunohistochemistry with specific primary ***: No association was found for TLR4-1607T/C and TLR4+896A/G by three statistical models(logadditive, dominant and recessive). However, based on dominant and log-additive models, the polymorphic variant TLR2-196 to-174 del was associated with increased CRC risk [dominant: odds ratio(OR) = 1.72, 95%CI: 1.03-2.89; P = 0.038 and log-additive: OR =1.59, 95%CI: 1.02-2.48; P = 0.039]. TLR2 m RNA expression was increased in tumor tissue(RQ = 2.36) when compared to adjacent normal tissue(RQ = 1; P analysis of TLR2 and TLR4 protein expression was concordant with the findings of m RNA expression. In addition, the TLR2-196 to-174 del variant carriers showed m RNA relative expression 2.19 times higher than wild-genotype carriers. The TLR2 protein expression was also higher for the TLR2-196 to-174 del variant carriers [117 ± 10 arbitrary unit(a.u.) vs 95 ± 4 a.u., P = 0.03].
