Beta-blocker therapy in elderly patients with renal dysfunction and heart failure

OBJECTIVE To assess the role of beta-blockers(BB)in patients with chronic kidney disease(CKD)aged≥75 *** AND RESULTS From January 2008 to July 2014,we included 390 consecutive patients≥75 years of age with ejection fraction≤35%and glomerular filtration rate(GFR)≤60 m L/min per 1.73 m^*** analyzed the relationship between treatment with BB and mortality or cardiovascular *** mean age of our population was 82.6±4.1 *** ejection fraction was 27.9%±6.5%.GFR was 60-45 m L/min per 1.73 m^2 in 50.3%of patients,45-30 m L/min per 1.73 m^2 in 37.4%,and<30 m L/min per 1.73 m^2 in 12.3%.At the conclusion of follow-up,67.4%of patients were receiving *** median follow-up was28.04(IR:19.41-36.67)*** the study period,211 patients(54.1%)died and 257(65.9%)had a major cardiovascular event(death or hospitalization for heart failure).BB use was significantly associated with a reduced risk of death(HR=0.51,95%CI:0.35-0.74;P<0.001).Patients receiving BB consistently showed a reduced risk of death across the different stages of CKD:stage IIIa(GFR=30-45 m L/min per 1.73 m^2;HR=0.47,95%CI:0.26-0.86,P<0.0001),stage IIIb(GFR 30-45 m L/min per 1.73 m^2;HR=0.55,95%CI:0.26-1.06,P=0.007),and stages IV and V(GFR<30 m L/min per 1.73 m~2;HR=0.29,95%CI:0.11-0.76;P=0.047).CONCLUSIONS The use of BB in elderly patients with HFr EF and renal impairment was associated with a better *** of BB should be encouraged when possible.

Mesenchymal stem cell enhances chondral defects healing in horses

The purpose of this study was to evaluate the effect of intralesional Mesenchymal Stem Cells (MSC) on the treatment of experimentally induced articular chondral defects in horses, emphasizing the benefits of this application in veterinary medicine. Chondral defects were induced in the medial femoral trochlea of both hind limbs of four horses. Thirty days post induction;the horses were divided into two groups. The G1 was submitted to treatment with MSC and the G2 was the control group. Clinical evaluations, synovial fluid analysis and synovial Prostaglandin E2 (PGE2) assessment were performed prior to defects and fortnightly up to 120 and 150 days. Macroscopic, histopathological and histochemical evaluations were performed at the end of the experiment. The treatment with MSC reduced the intraarticular inflammatory process. The G1 showed lower PGE2 concentrations in the synovial fluid and greater percentage of mononuclear cells and lower percentages of lymphocytes and neutrophils. The treatment improved the macro and microscopic aspects of repair tissue. No difference was observed in the scores of lameness between the G1 and G2. The use of MSC in the treatment of chondral defects minimized joint inflammation, as confirmed by synovial fluid analysis. The treatment resulted in an improved repair tissue, verified by macroscopic examination, histochemical and histopathological analysis.

This study investigated the efficacy of calcium hydroxide and chlorhexidine gel for the elimination of intratubular Candida albicans (***).Human single-rooted teeth contaminated with *** were treated with calcium hydroxide,2%chlorhexidine gel, calcium hydroxide plus 2%chlorhexidine gel,or saline(0.9%sodium chloride) as a positive *** samples obtained at depths of 0-100 and 100-200μm from the root canal system were analyzed for *** load by counting the number of colony forming units and for the percentage of viable *** using fluorescence ***,the antimicrobial activity of calcium hydroxide and the 2%chlorhexidine gel was evaluated by counting the number of colony forming *** 14 days of intracanal medication,there was a significant decrease in the number of *** colony forming units at a depth of 0-100μm with chlorhexidine treatment either with or without calcium hydroxide compared with the calcium hydroxide only ***,there were no differences in the number of colony forming units at the 100-200μm depth for any of the medications *** viability was also evaluated by vital staining techniques and fluorescence microscopy *** activity against *** significantly increased at both depths in the chlorhexidine groups with and without calcium hydroxide compared with the groups treated with calcium hydroxide *** with only chlorhexidine or chlorhexidine in combination with calcium hydroxide were effective for elimination of ***.

In 2019,an oil spill in Brazil,of unknown origin,severely impacted coastal environs with the worst environmental disaster ever recorded in any tropical coastal region globally severely damaging South Pirangi Reef area in the state of Rio Grande do Norte(RN).Here we discuss acute and chronic impacts including chemical contamination and economic consequences all over the world and show some evidence of the oil spill in this biodiverse ***,the lapse between the moment of the disastei;and the action to manage it,was hampered by a political agenda coinciding with local and global tragedies that redirected public *** almost 2 years have passed still without the offending party identification or culpability;and poor communities may continue to absorb its deleterious impacts for decades without consideration or *** disaster occurred during the Brazilian government's current issues involving extensive environmental mismanagement,resulting in a slow response from an inept *** is with urgent necessity to spotlight this tragedy in this unique and sensitive reef habitat experiencing the ongoing damaging effects that include socio-economic losses not yet addressed.

RNAi technology targeting PbGP43 and PbP27 in Paracoccidioides brasiliensis

Efficient technologies for gene silencing would be important to carry out functional analysis with P. brasiliensis genes, as well as for a better understanding of the biology and pathogenesis of this pathogenic fungus. Due to the fact that homologous recombination is unusual in P. brasiliensis, the development of knockout isolates is currently non-feasible. The goal of this work was to assess RNA interference (RNAi) technology as an alternative tool for gene silencing previously employed successfully in H. capsulatum. For this purpose, we built different inverted repeat transgenic hairpin constructs to down-regulate the PbGP43 and PbP27 genes known to codify for two fungal immunogenic proteins that elicit a strong immune response during experimental paracoccidioidomycosis. Using the RNAi strategy, a reduction in the mRNA levels of the PbGP43 and PbP27 genes was observed during the first 20 days after selection;however, in the transformed yeast cells, the gene silencing status proved non-stable through the assay. We demonstrated that electrotransformation was suitable to transform P. brasiliensis yeast cells and integrate the hairpin constructions;nonetheless, gene silencing was not stable along the experimental time. A detailed analysis of the underlying molecular RNAi machinery may provide further insights into the intracellular mechanism that governs this reverse genetic tool.

背景与目的林奇综合征(lynch syndrome,LS)是一种遗传性疾病,LS患者罹患结直肠癌、子宫内膜癌和其他与DNA错配修复基因发生种系突变相关肿瘤的风险较高。LS的传统遗传诊断方法为对与疑似综合征相关的基因进行桑格测序。二代测序技术(next-generation sequencing,NGS)可同时对大量遗传性癌基因进行测序。本研究旨在探究林奇综合征患者中是否存在其他遗传性种系病理癌基因变异。方法我们选取先前已利用桑格测序遗传诊断法确诊的84例LS先证者作为研究对象,通过杂交捕获一组94个商业化的遗传癌基因,随后对这些捕获的样本进行NGS测序。按照美国医学遗传学和基因组学标准,对这些变异的临床意义进行分类。利用桑格测序验证NGS的结果。在可能的情况下,利用桑格测序进一步在先证者的亲属中进行新发现种系变异基因的遗传分析。结果我们从84个家族中鉴定出5(6%)个至少含有2个种系致病变异的家族,这些变异具有高或中等的诱发不同显性癌症的风险,这些家族的种系致病变异如下:[MLH1-BRCA2-NBN]、[MLH1-BRCA1]、[MSH2-ATM]、[MSH6-NF1]和[MLH1-FANCA]。有趣的是,这5个家族中只有1个家族表现出与新的致病变异相关的临床表型。含有3个致病变异基因[MLH1-BRCA2-NBN]的家族显示出LS、乳腺和卵巢癌综合征相关肿瘤的高度聚集特性。结论我们的研究表明,癌症易感基因中,同时发生多个致病变异是LS病例的显著特点。在大部分的病例中,继发性致病变异与临床表型不相关。将来需要进一步验证这些结论并阐明其临床影响。只有表现出复杂的临床表型家族才有必要重新对LS进行分析。

Cell Death Effects of the Phthalate 2-Ethyl-1-Hexanol on Human Linfoblast Cells

Phthalates have been used in a wide variety of consumer goods. Their versatility as plasticizers has translated into worldwide use in a vast array of consumer products. These compounds can leach into matrices, such as food and liquids that can be routed for human exposure. One of the most used phthalates is Diethylhexyl phthalate (DEHP). Diethylhexyl phthalate and its metabolite 2-ethyl-1-hexanol (2-EH) have demonstrated biological effects which merit further evaluation. In this work, we expand on our previous work with DEHP and screen the 2-EH metabolite for different cell death endpoints such as growth inhibition, apoptosis, autophagy, caspase activation, DNA fragmentation, and cell cycle arrest using fluorophores and the NC3000 instrument. Significant results (p 0.05) revealed higher toxicity for the 2-EH metabolite when compared to DEHP. Also, 2-EH presented apoptosis induction with characteristic hallmarks, such as loss of mitochondrial membrane potential, caspase activation, DNA fragmentation and cell cycle arrest at the S phase. In addition, the presence of autophagosome was detected through L3CB protein staining. We conclude that 2-EH presents differences in cell death endpoints that interestingly differ from the DEHP parent compound. Further studies are needed to establish the molecular pathways responsible for the observed effects.

Purpose: The aim of this study was to isolate and identify potentially pathogenic airborne fungi from Hospital das Clínicas Samuel Libanio in the city of Pouso Alegre-MG and evaluate their susceptibility to natural and industrial products. Methods: The air collection was performed by passive sedimentation in the morning during the autumn and winter seasons. Petri dishes were open as the location of air conditioning. The isolates were subjected to pathogenicity test. The identification of the fungi was performed according to the macroscopic evaluation and micromorphology. Potentially pathogenic isolates were susceptibility tested by disc diffusion method. The agents used were insecticides and industrial cleaning products and essential oils of citronella plants, lemon grass, eucalyptus and Melaleuca extracted by steam distillation method. Results: We obtained a total of 356 fungal isolates. The inside door environments were 126 (35.39%) and the outside environments were 230 (64.6%) isolates. The 22 (6.18%) isolates from the inside and 25 (6.18%) outside the hospital showed pathogenic potential. This isolates were identified as Acremonium spp., A. niger, A. terreus, A. versicolor, Curvularia sp., Penicillium sp. and Scopulariopsis sp. Susceptibility testing it was observed that most of the isolates were susceptible to the principle product containing sodium hypochlorite. Citronella oil showed enormous potential inhibition against all isolates. Already lemon grass oil was effective only against isolates of Penicillium spp. Conclusions: All genres identified are significant allergens, which can cause respiratory disease in both immunocompromised individuals such as asthmatics and people with any immune deficit. The monitoring of environmental sources should be performed, especially in special areas with immunocompromised patients. Despite efforts to try to reduce fungal infections hospital there are still flaws in the strategies employed.

Estimates and Spatial Distribution of Emissions from Sugar Cane Bagasse Fired Thermal Power Plants in Brazil

Sugar cane bagasse is one of the largest fuels used for electricity generation in Brazil and its usage has continuously increased to supply the energy demand. This paper presents emission inventory based on power plants burning sugar cane bagasse. The inventory involves the spatial distribution and the estimated flows for the following major pollutants: nitrogen oxides (NOx), particulate material (PM), carbon dioxide (CO2) and total organic carbon (TOC). A total of 384 power plants were inventoried, representing a generated power of 9.9 GW, about 26% of the energy produced by thermal power plants sector. The plants are concentrated in two main poles: one of them in S?o Paulo State and nearby areas and the other one in coast of Brazilian Northeast. The limits proposed by the AP-42 Regulations of the US Environmental Protection Agency (USEPA) for the emission factors were applied. Additional emission factors identified in the scientific literature were also included in the analysis in order to assess the uncertainties associated to the estimative. The estimated emissions showed values in the range 16.0 - 20.5 Gg?year?1 for NOx, 18.0 - 267.0 Gg?year?1 for MP and 20.5 - 26.7 Tg?year?1 for CO2. The contribution of TOC showed a minor contribution around 10 - 20 Mg?year?1. PM showed to be the most representative pollutant emitted by the thermal plants burning sugar cane bagasse, but with a large range of uncertainty. There is a high level of uncertainty associated to the preparation of cane as well as the use of collectors to control particulate emissions. The adequate control over all stages could reduce the bagasse ash content in 90% or more.

Nursing Diagnoses of the Domain Safety/Protection and Socioeconomic and Clinical Aspects of Critical Patients

Objective: The objective is to correlate the nursing diagnoses of the domain Safety/Protection of NANDA-I in critically ill patients with sociodemographic and clinical data. Method: A cross-sectional study with 86 individuals was conducted, from October 2013 to May 2014 in the Intensive Care Unit of a university hospital in northeastern Brazil, through a formal interview and physical examination. Results: It was possible to identify a total of 20 significant statistical associations, and 15 were clinically justified by the literature, namely: risk for aspiration and reason for admission;impaired dentition and age;risk for peripheral neurovascular dysfunction and sex and comorbidity;skin integrity and comorbidity;risk for impaired skin integrity and gender and reason for admission;impaired tissue integrity and gender and reason for admission;risk for perioperative positioning injury and reason for admission;risk for thermal injury and age and comorbidity;delayed surgical recovery and reason for admission;risk for poisoning and years of schooling;and risk for imbalanced body temperature and age. Conclusions: By understanding the relationship between customers’ answers and the sociodemographic and clinical profile, positive health outcomes can be achieved in particular in the prevention of risks facing vulnerability characteristics, providing greater safety and protection for the critical customer.
