Background:Warfarin is widely regarded as the main anticoagulant in lowering the risk of *** study used indicators to compare pharmacist-managed anticoagulation services,using a well-prepared protocol,with physician-managed anticoagulation services・methods:A retrospective prospective pilot study was conducted to compare patient outcomes before and after transitioning patients to phannacist-managed anticoagulation services,comparing the proportion of those with therapeutic international normalized ratio(INK),subtherapeutic INK,and supratherapeutic INR,as well as their bleeding occurrences as indicators of assuring quality ***:A significant improvement in anticoagulation management was noted in the transition to pharmacist-managed anticoagulation *** proportion of those with subtherapeutic INR decreased from 61.8%to 11.8%(pm 20.6%to 2.9%(pm 17.6%to 85.3%(pm 11.8%to 0.0%,without significant difference in warfarin doses(median from 4 before the transition to 5 after);in addition,the time to reach therapeutic INR decreased from 12-24 weeks to 2-8 weeks after transitioning to pharmacist-managed anticoagulation ***:Pharmacist-managed anticoagulation services are considered safer and more effective than physician-managed anticoagulation services alone in terms of patientsJ adherence and satisfaction,which provide an excellent opportunity for quality assurance care.

In this paper, three complexes with 8-hydroxyquinoline (8-HQ) were synthesized, their spectral analysis was performed and the antimicrobial effect was examined m>in vitrom>. The stoichiometric ratio of the complex was determined conductometrically and spectrophotometrically. FTIR and UV/VIS spectroscopy were used for structural characterization. Antimicrobial activity was examined by diffusion technique on selected gram-positive and gram-negative bacteria, andm> C. albicansm>. Square planar and octahedral geometry complexes were synthesized by mixing in a molar ratio of 1:2 (m:L). Based on the spectral data, it is concluded that both oxygen and nitrogen atoms from 8-HQ are involved in the formation of the complex. The antimicrobial activity of the complexes is high, with zones of inhibition in the range of 15 - 28 mm. 8-HQ was shown to have a significantly higher ability to inhibit the growth of the tested microorganisms.

methicillin-resistant Staphylococcus aureus (MRSA) infection is a global health concern that has caused severe health threats over the past decade. Leaves extract of C. odontophyllum has been proven previously as an anti MRSA agent. Proteomics provide a technique that used to analyze the differential of protein expression profile between untreated and treated MRSA with subinhibitory concentrations of acetone extract from C. odontophyllum leaves. This study aims to determine the optimum parameter for analysis of protein expression profile using two-dimension gels electrophoresis (2-DE) for MRSA protein after treatment with acetone extract from C. odontophyllum leaves. AIm:To develop and test an Arabic version of the National Eye Institute Visual Function Questionnaire-25(NEI-VFQ-25).mETHODS:NEI-VFQ-25 was translated into Arabic according to WHO translation guidelines. We enrolled adult consenting patients with bilateral chronic eye diseases who presented to 14 hospitals across Egypt from October to December 2012, and documented their clinical findings. Psychometric properties were then tested using ***:We recruited 379 patients, whose mean age was(54.5±15)y. Of 46.2% were males, 227 had cataract,31 had glaucoma, 23 had retinal detachment, 37 had diabetic retinopathy, and 61 had miscellaneous visual defects. Non-response rate and the floor and ceiling numbers of the Arabic version(ARB-VFQ-25) were calculated. Internal consistency was high in all subscales(except general health), with Cronbach-α ranging from0.702-0.911. Test-retest reliability was high(intraclass correlation coefficient 0.79).CONCLUSION:RB-VFQ-25 isareliableandvalidtool for assessing visual functions of Arabic speaking patients. However, some questions had high non-response rates and should be substituted by available alternatives. Our results support the importance of including self-reported visual functions as part of routine ophthalmologic examination.

Liver metastasis is a normal result in Ehrlich Ascites Carcinoma Cell (EAC) infected mice. A partially purified protein with Superoxide Dismutase (SOD)-like activity extracted from leaves of Raphnus sativus was tested for its ability to down regulate liver metastasis of Ehrlich Ascites Carcinoma Cell (EAC) intraperitoneally implanted in albino mice. Sixty albino mice were divided equally into 4 groups. Group I mice (G1 control) were treated with physiologic saline solution for 7 days. Group 2 mice (G2) were injected with the partially purified protein. Group 3 mice (G3) were intraperitoneally inoculated with EAC cells and treated with the partially purified protein. Group 4 mice (G4) were inoculated EAC cells and treated with physiologic saline solution. The results showed a disturbance in liver functions. This disturbance was observed as a reduction in the values of serum albumin, hemoglobin, total counts of erythrocytes and platelets, total lipids in liver tissues, erythrocytes SOD activity, serum glucose, reduced glutathione (GSH) and the trace elements (Fe, mn, Zn, and Cu) contents in liver and spleen tissues as well as the elevation of serum transaminases activities, the levels of malondialdehyde in blood, DNA and RNA of liver tissues in the infected mice (G4). On the other hand, in G3, after treatment with the partial purified protein all these parameters restored their normal values and became near from the normal group (GI). moreover, the protein showed no toxicity towards the mice as shown in G2. It could be concluded that a partially purified protein with Superoxide Dismutase (SOD)-like activity extracted from leaves of Raphnus sativus as a nature source has a promised future in controlling liver metastasis.

Serum Y-Box Binding Protein 1 (YBX-1) and Interleukin 6 (IL-6) Are Associated with metastasis in Breast Cancer Patients

Objectives: The aim of this study was to assess the levels of Y-box binding protein 1 (YBX-1) and interleukin 6 (IL-6) in the sera of metastatic and non-metastatic breast cancer patients (BC), investigate their clinicopathological significance and to analyze their potential use as biomarkers of breast cancer metastasis. methods: The study included ninety subjects sub-grouped equally into metastatic BC, non-metastatic BC and healthy volunteers. Serum YBX-1 and IL-6 were quantified using ELISA technique while CA 15-3 was quantified using IRmA kit. Clinical data were collected from patients’ records. Results: YBX-1 (p metastatic and non-metastatic BC patients compared to healthy controls, however, only YBX-1 (p 0.001) showed a significant difference with cancer metastasis. Generally, YBX-1 and IL-6 were correlated with worse histological grade and late clinical stage in breast cancer patients and they were also associated with axillary lymph nodes involvement and positive vascular invasion in metastatic BC patients. Serum YBX-1 and IL-6 levels were positively correlated to each other (rs = 0.615, p mpared to CA 15-3 (p metastasis. Conclusions: Serum YBX-1 and IL-6 are potential biomarkers of breast cancer patients with significant correlation with bad clinicopathological characteristics. Serum YBX-1 and IL-6 have superior sensitivity and specificity compared to CA15-3 and can serve as potential follow up and prognostic markers.

Comparative Study between Intraoperative Frozen Section and Scrape Smear Cytology in the Diagnosis of Ovarian Neoplasm

Objective: The aim of this work is to compare between intraoperative frozen section and scrape smear cytology in the diagnosis of ovarian neoplasm. method: This study was performed between march 2011 and march 2012, on 50 patients presented with ovarian mass. Gross examination of the tumor removed was done by inspection and palpation. The specimen was then cut with a sharp knife into two halves. The area was scraped with a sharp scalpel or the end of a glass slide, depending upon the type of tissue. A semifluid drop thus obtained was spread over a glass slide. One to four slides per case were taken from different representative areas. The slides were labelled and immediately put into 95% ethyl alcohol and stained with hematoxylin-eosin. The specimens were then fixed in formalin. Paraffin blocks of the sections were processed in the routine way and sections were stained with hematoxylin and eosin (H and E). Assessment of the overall accuracy of the intraoperative diagnosis was classified as concordant or discordant. Results: The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of scraping technique in the diagnosis of benign ovarian masses were 100%, 95.2%, 96.7%, 100% and 98% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of scraping technique in the diagnosis of border line ovarian masses were 100%, 93.4%, 25%, 100% and 94% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of scraping technique in the diagnosis of malignant ovarian masses were 80%, 100%, 100%, 88.2% and 92% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of frozen section in the diagnosis of benign ovarian masses were 100%, 100%, 100%, 100% and 100% respectively. Accuracy of Intraoperative Frozen Section in the Diagnosis of Ovarian Neoplasms

Objective: The aim of the work is to evaluate the accuracy of intraoperative frozen section in the diagnosis of ovarian neoplasms in Zagazig University. Design: A prospective cross sectional cohort study. method: This study was performed between march 2011 and march 2012, on 50 patients presented with ovarian mass. Gross examination of the tumor removed was done by inspection and palpation. The specimen was then cut with a sharp knife into two halves. The most appropriate area thought to be representative of lesion was chosen. The number of sections frozen was depended on the type and size of the tumor. Seven to eight μm sections were obtained and stained with hematoxylin-eosin. The specimens were then fixed in formalin. Paraffin blocks of the sections were processed in the routine way and sections were stained with hematoxylin and eosin (H and E). The diagnosis obtained by intraoperative frozen section based on cellularity and cell morphology was compared with final histopathological diagnosis in terms of diagnostic sensitivity, to differentiate between benign and malignant lesions. Assessment of the overall accuracy of the intraoperative diagnosis was classified as concordant or discordant. Results: There was no statisticaly significant differencre in the studied patients as regard the clinical data, macroscopic and intraoperative picture, while there was statisticaly significat association as regard the laterality of the ovarian masses. The validity of frozen section in the diagnosis of benign tumour was 100% with 100% accuracy, specificity, positive predictive value, negative predictive value, while sensitivity & negative prediction for borderline tumour and specificity & positive prediction of malignant tumour were 100%, specifecity for borderline tumours was 95% while the positive predictive value was 33.3% with 96% accuracy for both malignant and borderline tumours. Conclusion: Intraoperative frozen section is accurate for rapid diagnosis of ovarian tumors. I

Feasibility of Concurrent Radiotherapy and Paclitaxel-Based Chemotherapy after Conservative Surgery for Breast Cancer

Purpose: Our prospective phase II trial aims to show the feasibility of adjuvant paclitaxel-based concurrent chemoradiotherapy (CCRT) following doxorubicin and cyclophosphamide (AC) to get the survival benefit of taxanes addition and avoid delay of radiotherapy. Patients and methods: A total of 63 patients with pT1-2, and pN1-3, m0 breast cancer underwent conservative surgery followed by adjuvant 4 cycles AC followed by 4 cycles Paclitaxel 175 mg/m2 every 3 weeks. Adjuvant radiotherapy started during the first and second cycle of paclitaxel (CCRT). Toxicities evaluated at the base time, weekly during radiation therapy and every 3 months for 24 months for skin, pulmonary, cardiac, lymphedema, subcutaneous fibrosis and cosmoses. Survival reported at 2-year median follow-up. Results: At median follow up time of 24 months (6 - 30), we did not report any toxicity postpone or stop treatment and only two patients had grade III acute dermatitis. Fifty-two patients (82.5%) had satisfactory cosmoses and none of the patients developed local recurrence. Conclusion: Three-weekly paclitaxel during radiotherapy is considered safe without significant complications and acceptable cosmoses with excellent local control and could be considered to avoid radiotherapy delay.

Antimicrobial effect of malaysian vegetables against enteric bacteria

Objective: To investigate the antibacterial activities of green vegetables(pennywort,mint, garlic, parsley and celery) against four common enteric bacteria [Salmonella enterica(ATCC 25957)(S. enterica), Shigella flexneri(ATCC 12022)(S. flexneri),Escherichia coli(ATCC 43889)(E. coli) and Enterobacter cloacae(ATCC 13047)(E. cloacae)] as an alternative medicine for controlling food borne diarrhea disease and the synergistic effect of green vegetables against those ***: Five common vegetables(pennywort, mint, garlic, parsley and celery) were purchased and extracted. The antimicrobial activities of these extracts were tested against four common enteric bacteria(S. enterica, S. flexneri, E. coli and E. cloacae). Ten different concentrations of the extracts(from 640 to 1.25 mg/m L) were prepared and used for the study. The minimal inhibitory concentration(mIC) was determined by the broth dilution method. The antimicrobial activities were assessed by using both well diffusion and disc diffusion ***: Garlic extract showed excellent inhibitory effects on all enteric bacteria. Other plants(parsley, celery, mint and pennywort) were not effective against enteric *** mIC of garlic against S. flexneri and E. cloacae was 40 mg/m L. The mIC of S. enterica and E. coli were 20 and 10 mg/m L, respectively. The performance of the well diffusion method was better than that of the disc diffusion method with clear and sharp inhibition zones of tested bacteria against plant ***: Garlic had excellent antimicrobial effects against enteric bacteria and was recommended to be given to patients with gastroenteritis. The other vegetables(pennywort, mint, parsley and celery) showed no inhibitory effects on enteric bacteria but still can be used for its richness in vitamins and fibers. The performance of the well diffusion method was better than that of the disc diffusion method in detecting the antibacterial effects of green vegetables.
