Early detection of hepatocellular carcinoma co-occurring with hepatitis C virus infection: A mathematical model

AIm: To develop a mathematical model for the early detection of hepatocellular carcinoma (HCC) with a panel of serum proteins in combination with α-fetoprotein (AFP).mETHODS: Serum levels of interleukin (IL)-8, soluble intercellular adhesion molecule-1 (sICAm-1), soluble tumor necrosis factor receptor II (sTNF-RII), proteasome, and β-catenin were measured in 479 subjects categorized into four groups: (1) HCC concurrent with hepatitis C virus (HCV) infection (n = 192); (2) HCV related liver cirrhosis (LC) (n = 96); (3) Chronic hepatitis C (CHC) (n = 96); and (4) Healthy controls (n = 95). The R package and different modules for binary and multi-class classifiers based on generalized linear models were used to model the data. Predictive power was used to evaluate the performance of the model. Receiver operating characteristic curve analysis over pairs of groups was used to identify the best cutoffs differentiating the different ***: We revealed mathematical models, based on a binary classifier, made up of a unique panel of serum proteins that improved the individual performance of AFP in discriminating HCC patients from patients with chronic liver disease either with or without cirrhosis. We discriminated the HCC group from the cirrhotic liver group using a mathematical model (-11.3 + 7.38 × Prot + 0.00108 × sICAm + 0.2574 × β-catenin + 0.01597 × AFP) with a cutoff of 0.6552, which achieved 98.8% specificity and 89.1% sensitivity. For the discrimination of the HCC group from the CHC group, we used a mathematical model [-10.40 + 1.416 × proteasome + 0.002024 × IL + 0.004096 × sICAm-1 + (4.251 × 10-4) × sTNF + 0.02567 × β-catenin + 0.02442 × AFP] with a cutoff 0.744 and achieved 96.8% specificity and 89.7% sensitivity. Objective: To evaluate the effect of moringa oleifera leaf ethanol extract as an adjunct treatment on lead acetate induced hepatonephrotoxicity in rabbits. methods: Thirty-six male New Zealand White rabbits were assigned into two main groups. The first group(14 rabbits) served as normal control. The second group(22 rabbits) was administered orally with lead acetate at a dose of 40 mg/kg/day, 5 days/week for 8 weeks. At the 4 th and the 8 th week of treatment, 6 animals(3 animals at each period) of the second group were sacrificed while the remaining animals(16 rabbits) were assigned randomly into 2 subgroups(8 rabbits each): treated and non-treated. The first subgroup was orally given 1 m L phosphate-buffered saline for further 4 weeks while the second subgroup was administered orally with moringa oleifera leaf ethanol extract at a dose of 400 mg/kg/day for the same period. Blood samples were collected to determine hematological and serum biochemical indices. Tissue specimens were collected from the liver and kidney for evaluation of the oxidant/antioxidant markers and for histopathological ***: Lead acetate exposure decreased the mean body weight gain, hematocrit, hemoglobin, mean corpuscular volume, and lymphocytes count. moreover, it markedly increased counts of monocytes and platelets, serum enzyme activity, levels of creatinine, total cholesterol, triglycerides, and low-density lipoprotein cholesterol. malondialdehyde level was markedly increased while the reduced glutathione content was significantly decreased in liver tissue of lead intoxicated-rabbits. Histopathological alterations were also noticed in the liver and kidney of lead intoxicated rabbits. moringa oleifera leaf ethanol extract significantly improved hematological and serum biochemical parameters and histopathological structure of the liver and kidney. Hepatitis B genotypes: Relation to clinical outcome in patients with chronic hepatitis B in Saudi Arabia

AIm： To identify the most common hepatitis B virus （HBV） genob/pe in Saudi Arabia, and correlate the prevailing genob/pes with the clinical outcome of patients. mETHODS： Patients were consecutively recruited from the hepatology clinics of two tertiary care referral centers. Patients were categorized into 4 different groups： group 1, patients with hepatitis B and normal liver enzymes; group 2, patients with hepatitis B and abnormal liver enzymes but without cirrhosis; group 3, patients with hepatitis B and liver cirrhosis; group 4, patients with hepatitis B and hepatocellular carcinoma. All patients had a positive hepatitis B surface antigen （HBsAg）. Genob/ping of HBV was performed by nested PCR-mediated amplification of the target sequence and hybridization with sequence-specific oligonucleotides. RESULTS： Seventy patients were enrolled in this study. They were predominantly male （72.9%） in their midforty＇s （mean age 47 years）. Forty-nine （70%） patients were hepatitis B envelope antigen （HBeAg） negative. The majority of patients （64%） acquired HBV through unknown risk factors. Hepatitis B genob/ping revealed that 57 patients （81.4%） were genob/pe D, 1 patient （1.4%） had genob/pe A, 1 patient （1.4%） had genotype C, and 4 patients （5.7%） had genob/pe E, while 7 patients （10%） had mixed genob/pe （4 patients ADG, 1 patient DE, 1 patient DF, and 1 patient ADFG）. Based on univariate analysis of genob/pe D patients, significant predictors of advanced liver disease were age, gender, aspartate transaminase, alanine transaminase, albumin,bilirubin, and alkaline phosphatase （all P 〈 0.001）. In multivariate analysis decreased hemoglobin （r = -0.05; 95% CI： -0.08 to -0.03; P = 0.001） and albumin levels （r = -0.004; 95% CI： -0.007 to -0.001; P = 0.002） were highly significant predictors of advanced liver disease. Effects of Carpobrotus edulis Extract on Oxidative Stress and 158N Oligodendrocyte Death

Objective Age-related diseases, including neurodegenerative diseases, are associated with oxidative stress and lipid peroxidation, and increase the levels of cholesterol auto-oxidation products such as 7β-hydroxycholesterol(7β-OHC). Thus, it is imperative to identify agents that can prevent 7β-OHC-induced side-effects. methods We evaluated the potential protective effects of Carpobrotus edulis ethanol-water extract(EWe) on murine oligodendrocytes(158 N) cultured in the absence or presence of 7β-OHC(20 μg/mL, 24 h). The cells were incubated with EWe(20-200 μg/mL) 2 h before 7β-OHC treatment. mitochondrial activity and cell growth were evaluated with the mTT assay. Photometric methods were used to analyze antioxidant enzyme [catalase(CAT) and glutathione peroxidase(GPx)] activities and the generation of lipid and protein oxidation products [malondialdehyde(mDA), conjugated diene(CD), and carbonylated proteins(CPs)]. Results Treatment with 7β-OHC induced cell death and oxidative stress(reflected by alteration in CAT and SOD activities). Overproduction of lipid peroxidation products(mDA and CDs) and CPs was also reported. The cytotoxic effects associated with 7β-OHC were attenuated by 160 μg/mL of EWe of C. edulis. Cell death induced by 7β-OHC treatment was ameliorated, GPx and CAT activities were restored to normal, and mDA, CD, and CP levels were reduced following C. edulis extract treatment. Conclusion These data demonstrate the protective activities of C. edulis EWe against 7β-OHC-induced disequilibrium in the redox status of 158 N cells, indicative of the potential role of this plant extract in the prevention of neurodegenerative diseases.

Orthogonal Codes-Based Dynamic Spectrum Access in Cognitive Radio Networks

Dynamic spectrum access(DSA) in cognitive radio(CR) networks became a challenging research area recently. In CR technology, the DSA between primary users(PUs) and secondary users(SUs) simultaneously can be achieved without degrading the performance of the PUs by SUs interference. This can be achieved by donating incentive power to the PUs in order to compensate the interference caused by the SUs. Consequently, PUs allow SUs to share the spectrum. In this paper, orthogonal codes-based dynamic spectrum access(OC-DSA) technique is proposed. OC-DSA technique employs orthogonality between PUs and SUs transmitted data symbols in addition to the incentive power donation. Compared to other techniques, the proposed technique uses a simple encoder at the SU network for the same PU network infrastructure. By applying orthogonal codes, the interference caused by SUs is canceled and hence the donated power to incentivize the PUs is reduced. Also, the SU packet rate is increased significantly. The simulation results show that the proposed technique provides effective improvements over other existing techniques in the signal strength and the bit error rate performance of both the PU network and the SU network at the receiver side. moreover, the proposed technique requires less donated power to incentivize the PU and has higher packet rate.

Impact of maternal HBsAg carrier status on pregnancy outcomes in Duhok city, Iraq

Objective: To investigate the relationship between hepatitis B virus(HBV) positivity and pregnancy outcomes. Also, the association between HBV-related risk factors and HBV status was ***: A total of 100 HBV positive pregnant women were recruited and the pregnancy outcomes were compared with 301 HBV negative women. Blood samples were collected and tested for HBV by HBs Ag elISA. Data were collected for recruited subjects using interview ***: Data analysis showed that 51/100(51%) of the HBV-positive subjects gave a history of HBV in the family which was significantly higher than that of HBV-negative patients [41/301(13.6%) P = 0.001]. A significant association was found between positive history of surgery and HBs Ag positivity(P = 0.009). Then, pregnancy outcomes were stratified according to the HBV positivity. No significant association was found between HBV status and pregnancy outcomes(P > 0.05 for all).Conclusions: Positive family history of HBV and previous surgical procedures are associated with higher rate of HBV positivity. No association is found between HBV positivity and pregnancy outcomes.

The cornea is maintained in an avascular state by maintaining an environment whereby anti-angiogenic factors take the upper hand over factors promoting angiogenesis. many of the common pathologies affecting the cornea involve the disruption of such equilibrium and the shift towards new vessel formation, leading to corneal opacity and eventually-vision loss. Therefore it is of paramount importance that the molecular underpinnings of corneal neovascularization（CNV） be clearly understood, in order to develop better targeted treatments. This article is a review of the literature on the recent discoveries regarding pro-angiogenic factors of the cornea（such as vascular endothelial growth factors,fibroblast growth factor and matrix metalloproteinases）and anti-angiogenic factors of the cornea（such as endostatins and neostatins）. Further, we review the molecular underpinnings of lymphangiogenesis, a process now known to be almost separate from（yet related to） hemangiogenesis.
