Role of toll receptors in diabetic nephropathy

Objectives: Diabetic nephropathy is the leading cause of chronic kidney disease. The pathogenesis of DN remains incompletely understood. It has been recently demonstrated that inflammatory processes play a significant role in the development and progression of DN. Toll-like receptors play a fundamental role in the innate immune system by triggering proinflammatory signaling pathways. Our aim is to evaluate the expression of TLRs on monocytes and relate their expression with inflammation in HD patients with & without diabetic nephropathy. Method: In a case control study (60) patients from Alkasr El Aini Hospital on hemodialysis were divided into two groups: Group 1, 30 patients on heamodialysis not due to diabetic nephropathy, Group 2, 30 patients on heamodialysis due to diabetic nephropathy, compared to Group 3, including 30 healthy controls. All participants were subjected to: Full medical history, complete physical examination, Serum creatinine, uric acid, A1C, fundus examination, detection of TLR2, TLR expression by real time PCR in peripheral blood mononuclear cells. Data were statically calculated using SPSS, comparision between groups was done using student T test comparing 2 groups, correlation using spearman’s correlation. Results: Diabetic had significantly increased TLR2, TLR4 mRNA in peripheral blood mononuclear cells compared to controls and non diabetics patient on heamodialysis (p < 0.001), TLR2, TLR4 significantly correlated with dialysis duration in diabetic (p < 0.001), no correlation with A1C in relation to TLR2 (p = 0.078), TLR4 (p = 0.163). Conclusion: TLR2, TLR4 were significantly elevated in diabetic on dialysis initiating event in the pathogenesis of DN, providing a link between hyperglycemia and hypoxia with inflammation and fibrosis within the kidney. Hence, therapeutic interventions aimed at targeting the inflammatory component through interruption of TLR signaling may be a novel strategy to target prevention and treatment of DN.

Coordination Behavior and Biological Activity of Some Transition Metal Complexes with 2-Acetyl and 2-Formyl-3-Amino-1,4-Naphthoquinone Ligands

The aim of this work is to synthesize, characterize and evaluate the biological activity of 2-acetyl and 2-formyl-3-amino-1,4-naphthoquinone (L1 - L2) and their metal-Co(II), Ni(II) and Cu(II) chelates. The newly chelates were characterized by elemental analysis, IR, mass and 1HNMR spectra, thermogravimetric analysis (TGA) and biological activity. The antibacterial and antifungal activities of the ligands and its metal complexes were screened against bacterial species (Staphylococcus aureus, Bacillus subtilis and Escherichia coli) and fungi (Candida albicans). Ampicillin and amphotericin were used as references for antibacterial and antifungal studies. The activity data show that the metal complexes have a promising biological activity comparable with parent free ligand against bacterial and fungal species.

Amman＇s land typology is characterized by hilly slopes, and this presents challenges and opportunities for architects and designers aiming at delivering sustainable buildings. The research focuses on the importance of any site＇s given criteria, mainly its slope and topography on the delivery of sustainable buildings. Amman city consists broadly of two main types of buildings, apartment buildings and villas, by studying each type of building with regard to its environmental context on a given site in the city, the research seeks to identify the sustainable variables that site topography delimit or facilitate, using a set of attributes for each building type. The main objective of this research is to highlight the sustainable approach for building on sloped sites throughout the building project life-cycle in general, and to set a sustainability framework for designers during the initial design phase in particular. A number of case studies for both types of buildings are studied and analysed, and conclusions are given based on syntheses of available data from literature review or case analysis. At the end, the research provides a mechanism for the development of guidelines for sustainable and passive viability on preferred buildings orientation in hilly areas with regard to local climatic data.

The aim of this work is to synthesize, characterize and evaluate the biological activity of 1,4-naphthoquinone derivatives ligands and their metal—Co(II), Ni(II) and Cu(II) chelates. Continuing our work with another derivatives of 1,4-naphthoquinone ligands, this work had been constructed for synthesis of new ligands derived from 1,4-naphthoquinone such as 3-thioacetyl-2-amino-1,4-naphthoquinone and 2-benzoyl-3-amino-1,4-naphthoquinone (L1-L2) which characterized on the basis of elemental analysis, electronic, IR, mass, 1H-NMR spectral data. The synthesized ligands have been carried out to achieve the coordination behavior towards bi-valent metal ions like cobalt, nickel and copper. The solid chelates of the different ligands were prepared and subjected to analytical techniques such as elemental analyses, spectroscopic techniques including mass, 1H-NMR, and IR spectroscopy, and thermal analyses techniques. The chelates were found to have octahedral geometry. The biological activity of the prepared ligands and their binary metals complexes were also screened against different antifungal and antibacterial organisms.

目的:通过分析来自中国和约旦两个国家泌尿系结石患者的病例资料探讨中国与约旦在尿路结石的发生率、危险因素及治疗方法上的差异。方法:通过设计好的问卷调查表,我们收集了354例来自一家中国医院(南昌第一附属医院东湖院区和象湖院区)泌尿外科和317例来自两家约旦医院(Al Essra医院和专科医院)泌尿外科的尿路结石患者的资料。利用统计学方法对收集的资料进行分析,比较两国尿路结石患者在危险因素及治疗方法上的差异。结果:在我们的研究中,中国尿路结石的发病率为6.0%,而在约旦发病率大约为9.0%。总体上看男女之间没有明显的差异。年龄的分层分析表明,青壮年比中老年患病人数更多,而在各年龄阶段,两国间没有明显的差异(P>0.05)。所有患者中肾结石、输尿管结石和膀胱结石分别为521例、117例和13例,分别占77.60%、17.43%和1.93%。在约旦患者中更多的伴随有肾盂积水、高血压和癫痫(P0.05)。在中国更多的患者通过CT和超声被诊断,此外在进行手术之前中国的患者会被要求完善KUB x-ray的检查,而在约旦,静脉肾盂造影仍是主要的诊断方法。引起尿路结石的因素很多,在中国的患者中排尿问题、高压力的生活、吃夜宵的习惯和咖啡、茶以及饮料的消费要显著高于约旦;而约旦患者则更多的与家族史、出汗、NSAID的使用、素食、高糖以及大豆的消费有关。结论:约旦尿路结石的发病率较中国更高。青壮年较中老年更为多见。在中国,尿路结石的诊断中CT使用的更为频繁,而约旦更喜欢使用静脉肾盂造影。引起尿路结石的因素很多,在中国可能更多的与高压力的生活以及茶和饮料的消费有关,而在约旦出汗或素食饮食与尿路结石的相关性更高。

Efficacy and safety of treatment of hepatitis C virus infection in renal transplant recipients

AIM: To assess the efficacy and safety of combinedpegylated interferon and ribavirin therapy in hepatitis C virus (HCV) infection in renal transplant recipients. METHODS: This is a retrospective chart review of post renal transplant patients who were positive for anti-HCV and HCV-RNA, and who have received treatment with combination of pegylated interferon and ribavirin between October 2003 and December 2008. Only patients with stable graft function and absence of evidence of cirrhosis and who received the therapy for continuous 48 wk were included. Nineteen patients (13 male and 6 female) were identified and included. The patient's complete blood count, liver and kidney prof ile, and cal- culated glomerular f iltration rate (GFR) were monitored every 6-8 wk while on treatment. HCV-RNA was tested at 12 wk for early virological response, at 48 wk for end of treatment response (ETR), and then retested at 24, and 48 wk after completion of therapy for sustained virological response (SVR). Liver biopsies were obtained before treatment from all patients and graft kidney biopsies were performed as required. RESULTS: Of the entire cohort, 9 patients (47.4%) showed an ETR and 8 had SVR (42.1%). Of the 8 patients with abnormal alanine aminotransferase (ALT) levels at baseline, 78.9% had their ALT normalized (including the virological non responders). ALT was normal in all responders at the end of therapy and at 24 wk post therapy (100%). Only one patient (5.3%) developed an increase in creatinine and decline in GFR from baseline towards the end of treatment. This patient's kidney biopsy revealed borderline rejection. There was no impact on response by HCV-genotype, initial HCV RNA load, age or sex of the patient or duration post transplant before commencement of therapy. All patients tolerated treatment in the same way as non-transplant with no unusual or increased occurrence of side effects. Since Type 1 diabetes(T1DM)occurs whenβ-cells mass is reduced to less than 20%of the normal level due to autoimmune destruction of cells resulting in the inability to secrete insulin,preservation or replenishment of the functionalβ-cells mass has become a major therapeutic focus for this diabetic type ***,this 4-week work plan was designed to determine which mesenchymal stem cells(MSCs)type is more appropriate to alleviate pancreatic hazards resulting from diabetes induction;via tracking a comparative study between MSCs derived from adipose tissue(AD-MSCs)and from bone marrow(BM-MSCs)in management of T1DM considering their immunomodulatory,anti-apoptotic and antioxidative *** were divided randomly into 4 groups;control,STZdiabetic(D),D+AD-MSCs,and D+BM-MSCs *** stem cells types in this study were ***,both oxidative stress and antioxidant markers were evaluated using colorimetric analysis,while inflammatory,immune and apoptotic markers were assessed through flow cytometric *** showed that diabetic rats treated with either AD-MSCs or BM-MSCs exhibited marked pancreatic antioxidant and anti-inflammatory activities that were able to initiate pancreatic immunomodulation and reducingβ-cells apoptotic death,thus,help to restore their normal insulin secretion and hypoglycemic ***,AD-MSCs injection was shown to be superior as a pancreatic regenerative tool in overcoming diabetes;owing to their marked antioxidant,anti-inflammatory,immunomodulatory,and anti-apoptotic characteristics over BM-MSCs treatment.

Aphid-host plant interaction

Black bean aphid, Aphis fabae (Homoptera;Aphididae) is a serious pest causing crop loss. Plant-aphid interaction is a dynamic system subjected to continual variation and changes. Host plants induce various biochemical and physical defense mechanisms due to aphid feeding. Aphids can overcome plant defenses by enzymatic adaptations and sequestering secondary metabolites produced by the plant within their bodies as a defense against their enemies. Many strategies were developed and evolved by aphids in order to overcome plant defense barriers which allowed them to feed, grow and reproduce on their host plants. This study aimed to aid in better understanding of the effect of altering host plant on specialist and generalist aphid *** influence of plant defense on population development of Aphis fabae was also investigated. Analyses for insect enzymes were also demonstrated in addition to further biochemical studies on host plant defences. Generalists showed different ecological and enzymatic adaptations towards host plants than specialist Aphis fabae. The results were fully discussed in details.
