Overweight and obesity in young adults with patellofemoral pain:Impact on functional capacity and strength

Purpose:This study aimed to(a)investigate the proportion of overweight/obesity in a cohort of young adults with patellofemoral pain(PFP)and(b)explore the association of body mass index(BMI),body fat,and lean mass with functional capacity and hip and knee strength in people with PFP.Methods:We included a mixed-sex sample of young adults(18-35 years old)with PFP(n=100).Measurements for BMI,percentage of body fat,and lean mass(assessed by bioelectrical impedance)were obtained.Functional capacity was assessed by the Anterior Knee Pain Scale,plank test,and single-leg hop test.Strength of the knee extensors,knee flexors,and hip abductors was evaluated isometrically using an isokinetic dynamometer.The proportion of overweight/obesity was calculated based on BMI.The association between BMI,body fat,and lean mass and functional capacity and strength was investigated using partial correlations,followed by hierarchical regression analysis,adjusted for covariates(sex,bilateral pain,and current pain level).Results:A total of 38%of our cohort had their BMI categorized as overweight/obese.Higher BMI was associated with poor functional capacity(ΔR^(2)=0.06-0.12,p<0.001)and with knee flexion strength only(ΔR^(2)=0.04,p=0.030).Higher body fat was associated with poor functional capacity(ΔR^(2)=0.05-0.15,p≤0.015)and reduced strength(ΔR^(2)=0.15-0.23,p<0.001).Lower lean mass was associated with poor functional capacity(ΔR^(2)=0.04-0.13,p≤0.032)and reduced strength(ΔR^(2)=0.29-0.31,p<0.001).Conclusion:BMI,body fat,and lean mass should be considered in the assessment and management of young people with PFP because it may be detrimental to function and strength.

Background:We compared body mass index(BMI),body fat,and skeletal muscle mass between(1) a mixed-sex nonathletic cohort of people with patellofemoral pain(PFP) and pain-free people,and(2) a nonathletic cohort of people with PFP and pain-free people subgrouped by sex(i.e.,men and women with PFP vs.pain-free men and women).Methods:This cross-sectional study included 114 people with PFP(71 women,43 men) and 54 pain-free controls(32 women,22 men).All participants attended a single testing session to assess body composition measures,which included BMI,percentage of body fat(%BFBioimpedance),and skeletal muscle mass(both assessed by bioelectrical impedance analysis),and percentage of body fat(%BFskinfoid)(assessed by skinfold caliper analysis).A one-way univariate analysis of covariance(age and physical activity levels as covariates) was used to compare body composition measures between groups(i.e.,PFP vs.pain-free group;women with PFP vs.pain-free women;men with PFP vs.pain-free men).Results:Women with PFP presented significantly higher BMI,%BFBiompedance,and %BFSkinfold,and lower skeletal muscle mass compared to pain-free women(p≤0.04;effect size:-0.47 to 0.85).Men with PFP and men and women combined had no differences in BMI,%BFBioimpedance,%BFSkinfold,and skeletal muscle mass compared to their respective pain-free groups(p> 0.05).Conclusion:Our findings indicate that BMI and body composition measures should be considered as part of the evaluation and management of people with PFP,especially in women,who have demonstrated higher BMI and body fat and lower skeletal muscle mass compared to pain-free controls.Future studies should not assess body composition measures in a mixed-sex population without distinguishing men participants from women participants.

A Rapid Parameter of Enzyme-Treated Cellulosic Material Revealed by Reducing Sugar Release

This study was conducted to evaluate the effectiveness of enzymes in purifying and reducing the degree of polymerization of cellulose for the production of dissolving pulp.Our goal was to determine the contributions of xylanase(X)and endoglucanase(EG)in the treatment of pulp,specifically by quantifying the formation of soluble and insoluble reducing sugars using the dinitrosalycilic acid(DNS)test.Predominantly,the release of soluble reducing sugars(RSSol)was enhanced after xylanase treatment,while endoglucanase(EG)treatment led to changes in insoluble reducing sugars(RSIns).The maximum synergism was observed for RSIns when a high ratio of endoglucanase to xylanase(320EG:5X/g pulp)was used.The relative contribution of endoglucanase to RSins was determined to be 15.6%of the total reducing sugar.The viscosity of pulps treated with xylanase decreased only by 7%,whereas endoglucanase treatment significantly reduced viscosity by 45%.Modifications in the particle size were observed after pulp treatment with the combination of endoglucanase and xylanase.In summary,the DNS test is a rapid and effective method for evaluating the efficiency of enzyme treatments on pulps.The measurement of RSIns correlates with changes in pulp viscosity to different extents,providing valuable insights into the effectiveness of enzyme treatments.

Dental-derived stem cells(DSCs)are attractive cell sources due to their easy access,superior growth capacity and low immunogenicity.They can respond to multiple extracellular matrix signals,which provide biophysical and biochemical cues to regulate the fate of residing cells.However,the direct transplantation of DSCs suffers from poor proliferation and differentiation toward functional cells and low survival rates due to local inflammation.Recently,elegant advances in the design of novel biomaterials have been made to give promise to the use of biomimetic biomaterials to regulate various cell behaviors,including proliferation,differentiation and migration.Biomaterials could be tailored with multiple functionalities,e.g.,stimuli-responsiveness.There is an emerging need to summarize recent advances in engineered biomaterials-mediated delivery and therapy of DSCs and their potential applications.Herein,we outlined the design of biomaterials for supporting DSCs and the host response to the transplantation.

Contribution of glial cells to the neuroprotective effects triggered by repetitive magnetic stimulation:a systematic review

Repetitive transcranial magnetic stimulation has been increasingly studied in different neurological diseases,and although most studies focus on its effects on neuronal cells,the contribution of nonneuronal cells to the improvement trigge red by repetitive transcranial magnetic stimulation in these diseases has been increasingly suggested.To systematically review the effects of repetitive magnetic stimulation on non-neuronal cells two online databases.Web of Science and PubMed were searched fo r the effects of high-frequency-repetitive transcranial magnetic stimulation,low-frequencyrepetitive transcranial magnetic stimulation,intermittent theta-bu rst stimulation,continuous thetaburst stimulation,or repetitive magnetic stimulation on non-neuronal cells in models of disease and in unlesioned animals or cells.A total of 52 studies were included.The protocol more frequently used was high-frequency-repetitive magnetic stimulation,and in models of disease,most studies report that high-frequency-repetitive magnetic stimulation led to a decrease in astrocyte and mic roglial reactivity,a decrease in the release of pro-inflammatory cyto kines,and an increase of oligodendrocyte proliferation.The trend towards decreased microglial and astrocyte reactivity as well as increased oligodendrocyte proliferation occurred with intermittent theta-burst stimulation and continuous theta-burst stimulation.Few papers analyzed the low-frequency-repetitive transcranial magnetic stimulation protocol,and the parameters evaluated were restricted to the study of astrocyte reactivity and release of pro-inflammatory cytokines,repo rting the absence of effects on these paramete rs.In what concerns the use of magnetic stimulation in unlesioned animals or cells,most articles on all four types of stimulation reported a lack of effects.It is also important to point out that the studies were developed mostly in male rodents,not evaluating possible diffe rential effects of repetitive transcranial magnetic stimulation between sexes.This systematic revi

High-resolution multi-scale air pollution system:Evaluation of modelling performance and emission control strategies

A high percentage of the world's population lives in areas where air pollutant concentrations exceed the World Health Organization guidelines.This work aims to develop and test,a high-resolution multi-scale air pollution modelling system by integrating a set of adequate tools.This system is able to provide detailed air pollutant concentrations in urban areas and support air quality management strategies through a better identification of different atmospheric processes.It also allows furthering the design and assessment of air pollution control measures for a specific area.To evaluate its performance and suitability,the system was applied to the Macao Special Administrative Region(SAR),China,one of the most densely populated areas on earth,during a winter period when this area is affected by high levels of Particulate Matter(PM).Although the developed system tends to underestimate the PM concentrations,it revealed a good performance in reproducing the temporal and spatial air pollution patterns.Several exceedances of the Chinese air quality standards were calculated and high population exposure to PM pollution was estimated.The tested urban atmospheric emission reduction scenarios have shown air quality improvements,indicating that emission reduction measures at urban level should focus on the domestic sector.However,it is crucial to implement joint pollution prevention strategies with neighbouring regions to improve the air quality in Macao SAR.The approach developed in this work can support policymakers in defining new strategies to reduce atmospheric pollution in urban areas.

A discussion about the limitations of the Eurocode’s high-speed load model for railway bridges

High-speed railway bridges are subjected to normative limitations concerning maximum permissible deck accelerations.For the design of these structures,the European norm EN 1991-2 introduces the high-speed load model(HSLM)—a set of point loads intended to include the effects of existing high-speed trains.Yet,the evolution of current trains and the recent development of new load models motivate a discussion regarding the limits of validity of the HSLM.For this study,a large number of randomly generated load models of articulated,conventional,and regular trains are tested and compared with the envelope of HSLM effects.For each type of train,two sets of 100,000 load models are considered:one abiding by the limits of the EN 1991-2 and another considering wider limits.This comparison is achieved using both a bridge-independent metric(train signatures)and dynamic analyses on a case study bridge(the Canelas bridge of the Portuguese Railway Network).For the latter,a methodology to decrease the computational cost of moving loads analysis is introduced.Results show that some theoretical load models constructed within the stipulated limits of the norm can lead to effects not covered by the HSLM.This is especially noted in conventional trains,where there is a relation with larger distances between centres of adjacent vehicle bogies.

Different no-till grain production systems with Urochloa spp.affect soil microbial community structure,biomass and activity in a tropical Ultisol

Integrated grain cropping systems promote soil health(SH)and sustainability.Microbial biomass and activity(MBA)and community structure(MCS)are key to SH.Integration of maize with Urochloa pastures strongly impacts MBA and MCS.MBA is more sensitive than MCS to shifts in grain cropping systems.Systems under continuous Urochloa increased microbial activity and AMF abundance.Tropical soils are prone to degradation.Adoption of conservation agricultural practices is essential to improve soil health,which is influenced by soil microbes.In this study we analyzed shifts in microbial biomass and activity(MBA)and microbial community structure(MCS)based on fatty acid methyl esthers(FAMEs)between five no-till agricultural practices:maize monoculture(MM);maize annualy intercropped with Urochloa decumbens(M/Ud);M/Ud with soybean rotation every other year(M/Ud-S);M/Ud keeping the pasture for the next two years(M/Ud-Ud-Ud);and maize intercropped with U.ruziziensis keeping the pasture for the next two years(M/Ur-Ur-Ur).Results indicated that MBA was affected by the inclusion of Urochloa intercropping and by rotation with soybean.Systems under a longer residence time with Urochloa in the field had higherβ-glucosidase activity and soil basal respiration,indicating a greater microbial activity.MCS was less affected than MBA by the investigated cropping systems.MCS changed only in the continuous pasture systems,which were enriched in arbuscular mycorrhyzal fungi(AMF).Additionally,the continuous pasture systems had lower microbial stress ratios than the other agricultural practices.In sum,our study showed that utilization of Urochloa spp.under longer periods in no-till agricultural practices contributes to increase microbial activity,AMF abundance and decrease microbial stress ratio.These changes are primarily beneficial for soil health.

Assessment of programmed death-ligand 1 expression in primary tumors and paired lymph node metastases of gastric adenocarcinoma

BACKGROUND Anti-programmed death-1/programmed death-ligand 1(PD-1/PD-L1)immuno-therapy has demonstrated promising results on gastric cancer(GC).However,PD-L1 can express differently between metastatic sites and primary tumors(PT).AIM To compare PD-L1 status in PT and matched lymph node metastases(LNM)of GC patients and to determine the correlation between the PD-L1 status and clinicopathological characteristics.METHODS We retrospectively reviewed 284 GC patients who underwent D2-gastrectomy.PD-L1 was evaluated by immunohistochemistry(clone SP142)using the com-bined positive score.All PD-L1+PT staged as pN+were also tested for PD-L1 expression in their LNM.PD-L1(-)GC with pN+served as the comparison group.RESULTS Among 284 GC patients included,45 had PD-L1+PT and 24 of them had pN+.For comparison,44 PD-L1(-)cases with pN+were included(sample loss of 4 cases).Of the PD-L1+PT,54.2%(13/24 cases)were also PD-L1+in the LNM.Regarding PD-L1(-)PT,9.1%(4/44)had PD-L1+in the LNM.The agreement between PT and LNM had a kappa value of 0.483.Larger tumor size and moderate/severe peritumoral inflammatory response were associated with PD-L1 positivity in both sites.There was no statistical difference in overall survival for PT and LNM according to the PD-L1 status(P=0.166 and P=0.837,respectively).CONCLUSION Intra-patient heterogeneity in PD-L1 expression was observed between the PT and matched LNM.This disagreement in PD-L1 status may emphasize the importance of considering different tumor sites for analyses to select patients for immunotherapy.

Background Tuberculosis is one of the most significant infectious diseases for global public health.The reallocation of healthcare resources and the restrictions imposed by the COVID-19 pandemic have hindered access to TB diag-nosis and treatment.Increases in unfavorable outcomes of the disease have been observed in Brazil.The objective of this study was to analyze the spatial distribution of unfavorable TB treatment outcomes in Brazil before and during the pandemic.Methods An ecological study with spatial analysis was conducted with all 5569 municipalities in Brazil.All reported cases of tuberculosis between January 2010 and December 2021,as well as reported cases of COVID-19 from Febru-ary 2020 to December 2021,were included.The outcomes studied encompass loss to follow-up,drug-resistant tuber-culosis,and death.The Getis Ord GI*technique was employed to assess spatial association,and the Kernel density estimator was used to identify areas with concentrated increases or decreases in outcomes.Bivariate Local Moran's/was used to examine the spatial association between outcomes and cOVID-19 incidence.The study was approved by the Research Ethics Committee of Ribeirao Preto Nursing School,University of Sao Paulo.Results There were 134,394 cases of loss to follow-up,10,270 cases of drug resistance,and 37,863 deaths.Clusters of high and low values were identified for all three outcomes,indicating significant changes in the spatial distribu-tion patterns.Increases in concentrations were observed for lost to follow-up cases in the Southeast,while reductions occurred in the Northeast,South,and Midwest.Drug-resistant tuberculosis experienced an increase in the Southern and Southeastern regions and a decrease in the Northeast and South.TB-related deaths showed notable concentra-tions in the Midwest,Northeast,South,and Southeast.There was an increase in high occurrence clusters for deaths after 2020 and 2021 in the Northeast.Conclusions The pandemic has brought additional challenges,emphasizing the importance of enhancing e
