BACKGROUND The sphericity of the femoral head is a metric used to evaluate hip pathologies and is associated with the development of osteoarthritis and femoral-acetabular *** To analyze the three-dimensional asphericity of the femoral head of asymptomatic pediatric *** hypothesized that femoral head asphericity will vary significantly between male and female pediatric hips and increase with age in both *** Computed tomography scans were obtained on 158 children and adolescents from a single institution in the United States(8-18 years;50%male)without hip *** femoral measurements including the femoral head diameter,femoral head volume,residual volume,asphericity index,and local diameter difference were used to evaluate femoral head *** In both sexes,the residual volume increased by age(Ply smaller femoral head size in older ages(>13 years)in females,there were no sex-differences in residual volume and aspherity *** were no age-related changes in mean diameter difference in both sexes(P=0.07)with no significant sex-differences across different age groups(P=0.06).In contrast,there were significant increases in local aspherity(maximum diameter difference)across whole surface of the femoral head and all quadrants except the inferior regions in males(P=0.03).There were no sex-differences in maximum diameter difference at any regions and age group(P>0.05).Increased alpha angle was only correlated to increased mean diameter difference across overall surface of the femoral head(P=0.024).CONClUSION There is a substantial localized asphericity in asymptomatic hips which increases with age *** 2D measured alpha angle can capture overall asphericity of the femoral head,it may not be sensitive enough to represent regional asphericity patterns.

目的旨在描述制定及评估一项短期留学生研究和教育项目。方法研究分为项目制定与实施及评价2部分。根据1名国际学生的短期访学需求,护理学院的全球护理项目协调员和首席导师共同确定项目基本内容和团队成员。该项目学习活动考虑了该国际学生对老年人和居家临终关怀的兴趣,包括4项内容:.开展研究工作,参与教学活动,参与临床实践活动,以及特殊活动和专业发展培训课程。在为期6个月的访学结束时,对参与此次教学活动的教师、研究生及留学生进行访谈和简短问卷调查。结果通过访谈了解到3个方面的内容:通过参与该项目中获得的经验(增强了教育和研究的多样性,获得了新的全球医疗保健知识,对文化意识的自我反思和促进);项目实施的障碍和挑战(签证申请过程,英语语言障碍,缺乏公共交通设施,时间安排冲突);克服这些障碍和挑战的策略(在课程中增加文化多样性信息;与其他同学一起学习英语;使用不同的沟通技巧;在签证申请过程中与大学全球事务办公室持续合作;提供互动体验)。所有项目参与者都对该项目表示非常满意(4.67±0.50),认为对他们在科研、教学和临床实践方面的护理技能有很大影响(4.67±0.50)。结论此次项目的参与者均获得了相关经验,可用于今后的教学指导活动。国际护理专业学生需要得到帮助才能更好地适应访学环境,教师必须具有文化敏感性,了解学生的学习需求,并倡导营造适合学生学习的环境。

Efficacy of chemotherapy containing bevacizumab in patients with metastatic colorectal cancer according to programmed cell death ligand 1

BACKGROUND Bevacizumab,an anti-vascular endothelial growth factor(VEGF)monoclonal antibody,inhibits angiogenesis and reduces tumor *** VEGF-C,lactate dehydrogenase,and inflammatory markers have been reported as predictive markers related to bevacizumab *** cell death ligand 1(PD-l1)could act upon VEGF receptor 2 to induce cancer cell angiogenesis and *** To investigate the efficacy of bevacizumab-containing chemotherapy in patients with metastatic colorectal cancer(CRC)according to the expression of *** This analysis included CRC patients who received bevacizumab plus FOlFOX or FOlFIRI as first-line therapy between June 24,2014 and February 28,2022,at Samsung Medical Center(Seoul,South Korea).Analysis of patient data included evaluation of PD-l1 expression by the combined positive score(CPS).We analyzed the efficacy of bevacizumab according to PD-l1 expression status in patients with *** A total of 124 patients was included in this *** all patients were treated with bevacizumab plus FOlFIRI or FOlFOX as the first-line *** 77%of patients received FOlFOX,23%received FOlFIRI as backbone first-line *** numbers of patients with a PD-l1 CPS of 1 or more,5 or more,or 10 or more were 105(85%),64(52%),and 32(26%),*** results showed no significant difference in progression-free survival(PFS)and overall survival(OS)with bevacizumab treatment between patients with PDl1 CPS less than 1 and those with PD-l1 CPS of 1 or more(PD-l1l1≥1%;PFS:P=0.93,OS:P=0.33),between patients with PD-l1 CPS less than 5 and of 5 or more(PD-l1l1≥5%;PFS:P=0.409,OS:P=0.746),and between patients with PD-l1 CPS less than 10 and of 10 or more(PD-l1l1≥10%;PFS:P=0.529,OS:P=0.568).CONClUSION Chemotherapy containing bevacizumab can be considered as first-line therapy in metastatic CRC irrespective of PD-l1 expression.

Tomographic reconstruction of the Earth’s magnetosheath from multiple spacecraft:a theoretical study

Following our earlier work on tomographic reconstruction of the magnetosheath soft X-ray emissions with superposed epoch analysis of many images recorded from a single spacecraft we now explore the instantaneous reconstruction of the magnetosheath and magnetopause using a few images recorded simultaneously from a few *** work is motivated by the prospect of possibly having two or three soft X-ray imagers in space in the coming years,and that many phenomena which occur at the magnetopause boundary,such as reconnection events and pressure pulse responses,do not lend themselves as well to superposed epoch *** the reconstruction is successful-which we demonstrate in this paper that it can be-this collection of imagers can be used to reconstruct the magnetosheath and magnetopause from a single image from each spacecraft,allowing for high time resolution *** this paper we explore the reconstruction using,two,three,and four *** show that the location of the subsolar point of the magnetopause can be determined with just two satellites,and that volume emissions of soft X-rays,and the shape of the boundary,can be reconstructed using three or more satellites.

Background:Physiological and biochemical processes across tissues of the body are regulated in response to the high demands of intense physical activity in several occupations,such as firefighting,law enforcement,military,and sports.A better understanding of such processes can ultimately help improve human performance and prevent illnesses in the work ***:To study regulatory processes in intense physical activity simulating real-life conditions,we performed a multi-omics analysis of 3 biofluids(blood plasma,urine,and saliva)collected from 11 wildland firefighters before and after a 45 min,intense exercise *** profiles ***-exercise were compared by Student’s t-test followed by pathway analysis and comparison between the different omics ***:Our multi-omics analysis identified and quantified 3835 proteins,730 lipids and 182 metabolites combining the 3 different types of *** blood plasma analysis revealed signatures of tissue damage and acute repair response accompanied by enhanced carbon metabolism to meet energy *** urine analysis showed a strong,concomitant regulation of 6 out of 8 identified proteins from the renin-angiotensin system supporting increased excretion of catabolites,reabsorption of nutrients and maintenance of fluid *** saliva,we observed a decrease in 3 pro-inflammatory cytokines and an increase in 8 antimicrobial peptides.A systematic literature review identified 6 papers that support an altered susceptibility to respiratory ***:This study shows simultaneous regulatory signatures in biofluids indicative of homeostatic maintenance during intense physical activity with possible effects on increased infection susceptibility,suggesting that caution against respiratory diseases could benefit workers on highly physical demanding jobs.

There are few pharmacologic options for the treatment of cognitive deficits associated with traumatic brain injury in pediatric *** inhibitors such as donepezil have been evaluated in adult patients after traumatic brain injury,but relatively less is known about the effect in pediatric *** goal of this review is to identify knowledge gaps in the efficacy and safety of acetylcholinesterase inhibito rs as a potential a djuvant treatment fo r neurocognitive decline in pediatric patients with traumatic brain *** queried PubMed to identify literature published from database inception thro ugh June 2023 desc ribing the use of donepezil in young adult traumatic brain injury and pediatric patients with predefined *** on preselected search criteria,340 unique papers we re selected for title and abstra ct ***-two reco rds were reviewed in full after eliminating preclinical studies and pape rs outside the scope of the *** adult traumatic brain injury,we review results from 14 papers detailing 227 subjects where evidence suggests donepezil is well tole rated and shows both objective and patient-reported efficacy for reducing cognitive *** children,3 pape rs report on 5 children recovering from traumatic brain injury,showing limited *** additional 15 pediatric studies conducted in populations at risk for cognitive dysfunction provide a broader look at safety and efficacy in 210 patients in the pediatric age *** its promise for efficacy in adults with traumatic brain injury and tole rability in pediatric patients,we believe further study of donepezil for children and adolescents with traumatic brain injury is warranted.

Mature osteoclasts degrade bone matrix by exocytosis of active proteases from secretory lysosomes through a ruffled ***,the molecular mechanisms underlying lysosomal trafficking and secretion in osteoclasts remain largely ***,we show with GeneChip analysis that RUN and FYVE domain-containing protein 4(RUFY4)is strongly upregulated during *** lacking Rufy4 exhibited a high trabecular bone mass phenotype with abnormalities in osteoclast function in ***,deleting Rufy4 did not affect osteoclast differentiation,but inhibited bone-resorbing activity due to disruption in the acidic maturation of secondary lysosomes,their trafficking to the membrane,and their secretion of cathepsin K into the extracellular ***,RUFY4 promotes late endosome-lysosome fusion by acting as an adaptor protein between Rab7 on late endosomes and lAMP2 on primary ***,Rufy4-deficient mice were highly protected from lipopolysaccharide-and ovariectomy-induced bone ***,RUFY4 plays as a new regulator in osteoclast activity by mediating endo-lysosomal trafficking and have a potential to be specific target for therapies against bone-loss diseases such as osteoporosis.
