Efficacy and safety of bamlanivimab in patients with COVID-19:A systematic review and meta-analysis

BACKGROUND Monoclonal antibodies(mAbs)have shown clinical benefits against coronavirus disease 2019(COVID-19)caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2).Several studies have reported the use of bamlanivimab as a promising treatment option for *** To synthesize the latest evidence for the efficacy and safety of bamlanivimab alone in the treatment of adult patients with *** A literature search was conducted in PubMed,Cochrane Library,Web of Science,medRxiv,and Google Scholar using“SARS-CoV-2”,“COVID-19”,“LY-CoV555”,and“Bamlanivimab”keywords up to January 25,*** quality of included studies was assessed using the Cochrane bias *** Comprehensive Meta-Analysis software version 3.0 was used to analyze the *** A total of 30 studies involving 47368 patients were included.A significant difference was observed between the bamlanivimab and standard of care/placebo groups in terms of mortality rate[risk ratio(RR)=50,95%confidence interval(CI):0.36-0.70],hospitalization rate(RR=0.51;95%CI:0.39-0.68),and emergency department(ED)visits(RR=0.69;95%CI:0.47-0.99);while the two groups exhibited no significant difference in terms of intensive care unit(ICU)admission(P>0.05).Compared to other mAbs,bamlanivimab was associated with a higher rate of hospitalization(RR=1.44;95%CI:1.07-1.94).However,no significant difference was detected between the bamlanivimab and other mAbs groups in terms of mortality rate,ICU admission,and ED(P>0.05).The incidence of any adverse events was similar between the bamlanivimab and control groups(P>0.05).CONCLUSION Although the results suggest the efficacy and safety of bamlanivimab in COVID-19 patients,further research is required to confirm the efficacy of this drug for the current circulating SARS-CoV-2 variants.

Management of Post-Mastectomy Radio-Necrotic Ulcers &Osteoradionecrosis

Background: Breast cancer is still the most common indication for chest wall irradiation, despite the advances in RTH techniques complications still occur, and the time of its occurrence vary and could take few months to many years. ORN usually manifests with pain, exposed bone, fistulae and pathological fracture and the proper management needs proper debridement and coverage by thick musculocutaneous flaps. Objective: To evaluate different techniques of surgical management of post-irradiation persistent radio-necrotic ulcer & ORN with emphasis on the outcome of each technique. Patients and methods: A retrospective study from 2010 to 2015 reviewed 20 patients subjected to post-mastectomy radiotherapy as a treatment for breast cancer inNCICairo, and Minia oncology center Hospital, Egypt. Cases were biopsied to exclude local recurrence. The age, sex, type of ulcer, dose of radiotherapy received and the management done for these cases were recorded. Results: Only 4 patients responded to conservative treatment in the form of repeated dressings, and 16 patients needed surgical treatment;5 of them were treated with debridement and coverage with split thickness graft with high rate of complications reaching 80%, the other 11 patients were reconstructed with musculocutanoeus flaps, of these 7 patients were reconstructed with LD flaps with 57.14% success rate and minor complications only, the other 4 cases were reconstructed with TRAM flap with 50% success and major loss of the flap in one case that needed salvage by LD flap. Pre-operative comorbidity was found in 6 patients and all of them experienced complications while only 3 of the other 14 who had no comorbidity had postoperative complications. Conclusion: Pedicled flaps provide a good choice for reconstruction and provide chest wall support without the use of synthetic mesh.

The Poitou-Charentes, located in the Center-West of France, is a region where economy is based mainly on agriculture. This region, made up of 4 Departments (Vienne, Charente, Charente Maritime, Deux-Sèvres), is supplied largely by groundwater exploitation, both for consumption and for irrigation. This resource is thus vital to the region and its preservation is a major issue. The objective of this study is the determination of the groundwater quality in the Dogger aquifer of Poitiers (Vienne Department), which is the main water resource for this area and to achieve a better understanding of the factors influencing groundwater mineralization. Sixty-six wells, distributed over the study area, were sampled and analyzed for major ions (Ca2+, Mg2+, Na+, K+, HCO3-, Cl-, SO42-, NO3-). Data were interpreted using graphical (Piper diagram, binary diagrams) and statistical methods (correlation matrix, principal components analysis). Saturation and chloro-alkaline indices were also computed. It is shown that the groundwater mineralization is mainly associated with Ca2+, K+, HCO3-, Cl-, SO42-, and NO3-. Hydrochemistry of groundwater is determined by both natural processes, and anthropogenic factors. Natural factors are dissolution of carbonate and dolomitic minerals, and cation exchanges with clays, while anthropogenic factors are contaminant infiltration with waste waters and agricultural fertilizers. Nitrate is the main contaminant found in the groundwater and makes this resource unsuitable for consumption at some places.

Assessment of Left Ventricular Torsion in Hypertensive Patients

Background: Hypertension has important effects on left ventricle and its early detection helps to avoid the cardiac complications. LV Twist in systole helps in storing potential energy during diastole;LV untwist releases this energy. Speckle tracking echocardiography (STE) is angle-independent so it allows a more detailed evaluation of myocardial deformation. Objective: This work aimed to assess the left ventricular torsion by speckle tracking in hypertensive patients. Patients and Methods: The study was applied on 40 hypertensive patients and 20 age and sex matched control groups;all had speckle tracking echocardiography of the LV, and the basal and apical rotation were measured to assess the LV torsion. The 18 segments of LV strain were assessed and the net LV global longitudinal strain was calculated and demonstrated as bull's eye figure. Results: Hypertensive patients had higher mean values of both apical rotation and left ventricular twist than control group by a highly significant value (9.76 ± 4.98 vs 4.03 ± 2.15, P value 0.001) and (15.25 ± 4.10 vs 9.90 ± 1.47, P value 0.001) respectively. Hypertensive patients had a higher basal rotation than controls but did not reach a significant value (-6.01 ± 3.25 vs -5.52 ± 2.63, P value 0.558). On the contrary, the global LV longitudinal strain was lower in hypertensive patients than control but did not reach a significant value (-18.48 ± 3.94 VS -19.48 ± 3.57, P value 0.341). Conclusion:Hypertension affects myocardial structure and its systolic and diastolic functions;the left ventricular twist is an essential component of the systolic function that increases in hypertension as an early compensation for the systolic impairment. The detection of these changes achieved accurately by 2-D speckle tracking, can help in monitoring the treatment modalities of the patients for a better direction of treatment and thus, prevent further deterioration.
