Surgical Treatment of Relapsed Clubfoot Using the Ilizarov Method: Anatomical and Functional Outcomes—A Report of 16 Cases at the Order of Malta’s Hospital Center

Introduction: Relapsed Congenital Talipes Equino Varus (CTEV) refers to clubfoot diagnosed after walking age. We refer to this entity as residual deformities of a previously treated clubfoot or a complete recurrence following treatment. For its treatment, several techniques have been used to improve the foot’s functional outcome while remaining minimally invasive. Correction by distraction using the Ilizarov external fixator is a promising technique. This single-center study aims to evaluate the anatomical and functional outcomes of treatment by distraction and compression with the Ilizarov fixator in relapsed club feet. Patients and Method: We carried out a retrospective, single-center study over 5 years. In total 16 clubfeet in 14 patients, including 9 males and 5 females were retained for final analysis. The average age of the patients at the time of the intervention was 15.16 years. K-wires were placed using the original technique. The average duration of the fixator in our series was 4.27 months. The functional assessment of patients was based on pre-and post-operative evaluation of the Foot and ankle function according to the AOFAS score. Results: Anatomic assessment was done according to the Dimeglio classification. There was a significant improvement at post-operative period with an average score of 4.47 against 10.07 at pre-operative period. We obtained 43.8% good and very good functional outcomes according to the global AOFAS score increasing from 63.07 at pre-operative period to 77.88 at post-operative period. Two cases of reinterventions were recorded. Conclusion: Distraction with the Ilizarov external fixator occupies a place of choice in the management of relapsed club foot. A good mastery of the technique and patient selection produce good functional outcomes.

Knee Osteoarthritis Progression after Distal Femur Closing Wedge Osteotomy

Background: Despite the conservative treatment of tibio-femoral osteoarthritis through realignment osteotomies, the rate of total knee replacements following an osteotomy is increasing. The aim of this study was to identify the factors associated with the progression of knee osteoarthritis after a medial closing-wedge distal femoral osteotomy. Methods: Hospital-based observational study on 20 patients who underwent a medial closing-wedge distal femoral osteotomy evaluating the progression of osteoarthritis using the Kellgren and Laurence classification. The Wilcoxon test was used to compare the variation in the progressive stage of the Kellgren and Laurence classification of knee osteoarthritis preoperatively and at the final follow up. Univariate analysis made it possible to determine the factors associated with progression. The final significance threshold for statistical tests was set at 5% (p Results: Overall, the mean follow-up of 46 months ± 6.6 months, with a mean age of 43 years (range: 27 - 69 years) and a female predominance (M: F = 3/7). The progression of tibiofemoral osteoarthritis following a medial closing-wedge distal femoral osteotomy is associated with valgus or varum malalignment been a moderate valgus (OR 6.2 [1.5 - 42.7] at 95% CI;p-value = 0.02), a correction of the mechanical deviation angle with a valgus alignment (OR 2.7 [0.9 - 8.3] at 95% CI), and loss of correction (OR 3.8 [1.3 - 11.6] at 95% CI;p -value) for the lateral compartment while varus alignment (OR 1.7 [0.9 - 8.3] 95% CI, p-value = 0.05) and with rupture of the lateral cortex (OR 2.8 [1.7 - 11.5] 95% CI, p-value = 0.02) were those of the medial compartment. Conclusion: Distal femur closing wedge osteotomy does not definitively interrupt the progression of valgus knee osteoarthritis. The factors associated with the progression of this pathology are modifiable. Taking them into account when performing this surgical technique could improve the osteotomy survival curve.

Introduction: Malaria remains a public health priority in Senegal, particularly in Tambacounda, where it is one of the main causes of child mortality. The objective of this work was to evaluate the care of children under 10 years of age hospitalized at the Tambacounda Health Center and the factors associated with recovery. Methods: An analytical, retrospective, and descriptive cross-sectional study with exhaustive recruitment of children 0 to 120 months hospitalized at the Tambacounda reference health center for severe malaria (according to WHO criteria) between 1 January 2018 and 31 December 2021 was performed. Data collection was done through a questionnaire. Records, hospitalization records, and treatment records were the sources of collection. Data entry and analysis were performed on Epi Info 7.2 and R. Results: A total of 481 children hospitalized with severe malaria were recruited. The highest number of cases was recorded in 2018 (33.05%). In the four years of the study, peaks were always observed between October and November and the highest peak in November 2020 with 95 cases. The mean age was 65.64 months with a standard deviation of 29.28 months and a predominance of male (53.43%). The majority of people were admitted from the outpatient clinic (57.79%) and the rest (42.21%) on the recommendation of a peripheral health post. All hospitalized patients had a positive RDT and/or a positive thick drop. However, the sharp decline at admission or during hospitalization was positive in 93.80% of patients in our series, negative in 5.20% and not achieved in 1.00%. Seizures and severe anemia topped the list of signs of severity with 45.94% and 8.11%, respectively. In terms of evolution, for all hospitalized patients, there were 81.29% recovery, 10.19% referral to the Tambacounda regional hospital center for hospitalization, 4.99% death, 0.83% discharge and 2.70% unknown evolution. Hepatic Artery Aneurysm: An Unusual Cause of Upper Gastrointestinal Bleed in 49 Years Old Patient

Hepatic artery aneurysm (HAA) is a rare disease. HAA is generally asymptomatic disease when symptomatic, they usually present with abdominal pain, upper gastrointestinal (GI) bleeding and/or jaundice, hypovolaemia secondary to rupture or GI bleeding with normal GI endoscopy. Surgical repair and endovascular treatment are the two therapeutic options available at present. Case report: A 49-year-old male presented at the emergency department with high gastrointestinal bleeding, abdominal pain and jaundice. Gastroscopy showed an ulcer with flat pigmented haematin on ulcer base (Forrest IIc) that was controlled by medical treatment. CT angiography was done and showed aneurysm of the proper hepatic artery almost totally thrombosed measuring 100 × 59 mm associated with signs of contained rupture. Emergency surgery was indicated. The laparotomy objectified a rupture of the aneurysm in the biliary tree in per operative excision of aneurysm and ligation of the hepatic pedicle was carried out. After surgery, the evolution was favorable with a follow-up of 8 months. Conclusion: HAA rupture is a rare cause of upper GI bleeding. The mortality rate after rupture is relatively high. CT angiography or MRI can diagnose a ruptured of HAA. Urgent surgery should be the first choice in patients with a ruptured HAA with active hemorrhage causing hemorrhagic shock.

Effect of the Covid-19 Pandemic on Prenatal Consultations for Pathological Pregnancies at the Maternity Unit of Abass Ndao Hospital, Dakar, Senegal

Introduction: The COVID-19 pandemic has led to changes in health systems. The objective was to assess the effect of Covid-19 on the frequency of prenatal consultations (PNCs) for pathological pregnancies at the Abass NDAO Hospital Centre. Materials and Methods: This was primarily a retrospective study conducted at the Abass NDAO Hospital maternity ward. The target was the medical records of women followed for a pathological pregnancy at maternity from July 2019 to November 2020. Second, the department’s qualified staffs were asked about their experiences with the Covid-19 pandemic. Data collection was carried out between October 2021 and February 2022. Results: A total of 690 prenatal charts for pathological pregnancy were collected: 375 (54%) before the pandemic compared to 315 during Covid-19 (46%). The average age of women was 28.15 years. The age group 25 and 35 was the most represented (36.81%). Reasons for consultation were dominated by threats of premature delivery (51%), followed by severe pre-eclampsia (14%) and premature membrane rupture (11%), respectively. The frequency of prenatal consultations (PNCs) was inversely related to pandemic dynamics and significant monthly variations were observed. Qualified health personnel were midwives (71.40%) and gynecologists (28.60%). During the pandemic, more than the majority of staff (67%) worked 3 days a week, and 24% of health workers 5 days. The majority of staff (60%) reported experiencing overwork during the pandemic. Non-compliance with consultation days was noted in 38% of health providers. Protective equipment was not provided to 14% of the staff. Staff difficulties during prenatal consultations were fear of occupational exposure (12%) and patient reluctance (88%). Conclusion: The Covid-19 pandemic is having a major impact on prenatal consultations for pathological pregnancies at Abass NDAO Hospital. Spatial Distribution of Cordex Regional Climate Models Biases over West Africa

The objective of this work is to analyze the spatial distribution of biases of nine (9) regional climate models (RCMs) and their ensemble average used under the framework of COordinated Regional climate Downscaling EXperiment (CORDEX) project over West Africa during the summer period. We assessed the ability of RCMs to represent adequately West African summer rainfall by analyzing some statistical parameters such as the relative bias, the standard deviation, the root mean square error (RMSE) and the correlation coefficient between observation data (GPCP used as reference) and regional climate models outputs. We first analyzed the relative bias between GPCP climatology and the other available observed data (CRU, CMAP, UDEL, GPCC, TRMM and their ensemble mean). This analysis highlights the big uncertainty on the quality of these observed rainfall data over West Africa which may be largely due to the rarity of?in situ?measurement data over this region. The statistical analysis with respect to GPCP rainfall shows the presence of large relative bias values over most part of West Africa for engaged RCMs. However their ensemble mean outperforms individual RCMs by exhibiting the weakest relative change. The RMSE values are weak over West Africa except over and off the Guinea highlands for RCMs and the Era-interim reanalysis. The spatial distribution of the coefficient of correlation between the observation data and RCMs shows that all models (except HIRHAM) present positive values over the Northern Sahel and the Gulf of Guinea. The model of the DMI exhibits the weakest values of correlation coefficient. This study shows that RCMs simulate West African climate in a satisfactory way despite the fact that they exhibit systematic biases.

Prognostic Factors for the Occurrence of Blood Exposure Accidents among Health Personnel at the Kaolack Regional Hospital (Senegal)

Caregivers are highly exposed to blood exposure accidents (BEA). The objective of our study is to determine the frequency of BEA and to identify the predictive factors for the occurrence of BEAs among caregivers at the Kaolack Regional Hospital (KRH). It is a descriptive and analytical cross-sectional study of KRH’s caregivers. The data were collected during the period from 16 to 26 April 2018. The multivariate analysis was carried out using the binary logistic regression model with a dependent variable (occurrence of blood exposure accidents) and 5 explanatory variables (sex, age, service of belonging, professional category and seniority in the profession). A total of 115 caregivers were surveyed out of 144. It is noted that 68 caregivers (59.1%) have had at least one BEA in the previous 12 months. The age of caregivers significantly reduces the risk of developing a BEA (p = 0.004, CI [0.04 - 0.21]);belonging to the age group [20 - 40 years] increases the risk of a BEA by 6.66. Sex significantly influences the occurrence of BEA with a (p = 0.013, CI [1.47 - 19.4]);men are 5 times more likely to develop BEA. The risk of a BEA occurring varies according to the professional category. Senior health technicians have a significantly 50 times lower risk of occurrence of a BEA (p = 0.007, CI [0.00 - 0.24)]. Nurses and midwives have a 5.8 lower risk of developing BEAs (p = 0.031, CI [0.03 - 0.75]). The risk of occurrence of BEAs varies according to the service of belonging;Medicine service caregivers have a significantly 50-fold lower risk of developing BEAs (p = 0.004, CI [0.00 - 0.17]). The identified risk factors will be used to better guide our BEAs prevention interventions.

A Cross-Sectional Survey on Non-Communicable Diseases and Risk Factors in the Senegalese Army

Background: The non-communicable diseases (NCDs) have become a public health priority. The objectives of this study were to measure the prevalence and to assess the risk factors of NCDs among the Senegalese military population to initiate an intervention program. Methods: A cross-sectional survey was conducted in 2014. Two-level stratification was used to sample participants. Data were collected following the protocol recommended by the World Health Organization. Data were managed using Epi-Info 6 software and analyzed using R software. Results: A total of 1224 participants were recruited, of whom 96.9% were men. Their ages ranged from 25 to 60 years with a mean of 39.7 ± 9.2 years. Of participants, 17.2% were active smokers. Average duration of active smoking was 19.9 ± 9 years. The prevalence of current alcohol consumption was 11.5%, with an average of 4 ± 2.7 glasses a day. 97.17% of participants consumed fewer than 5 servings of fruits and vegetables a day. 18.63% had insufficient physical activity. The overall prevalence of high blood pressure was 26.9%. The proportion of those who were overweight was 27.2%;3.3% were obese. 3.0% of participants had diabetes and 44.1% had hypercholesterolemia. After adjusting for waist circumference, the risk of high cholesterol was 2.42 in the 35 - 44 age group and 2.86 in the 45 - 60 age group in comparison with the 25 - 34 age group. 32% were classified as having stage 2 chronic kidney. Conclusions: The findings of this study indicate the importance of risk factors for NCDs in the Senegalese military population. Intervention based on prevention and health promotion is needed.

Family Planning Knowledge and Practices among Reproductive Age Women in Widou Thiengoly, Louga Region, Senegal

Family planning is an effective intervention to improve the health of the mother, newborn and child. It was introduced in Senegal in the 1960s. Significant disparities were noted between urban and rural areas. This research aimed at measuring family planning methods knowledge and practices among reproductive age women (RAW) in the village of Widou Thiengoly, Louga region, Senegal. A descriptive and analytical cross-sectional study was carried out from 12 to 20 August 2016 and involved 150 RAW with an average age of 29.34 years. They were mostly married (98%), with a fertility rate of 3.22 children/woman. 67.3% of RAW knew at least one FP method and the contraceptive prevalence was 40.7%. Birth spacing is the main reason or utilization for more than half of users, while for non-users, more than half were ignorant of FP. Knowledge and practices of contraception were not related to the age of WCA, or the number of children. Fears or experiences of side effects were barriers to initiation and continuous use of FP methods, hence the importance of focusing awareness campaigns on alleviating fears about side effects.

What are the Risks of Death in Elderly Patients after a Proximal Femur Fracture at Aristide le Dantec Hospital?

Introduction: Proximal femur fractures are traumatic injuries involving the neck and trochanteric region of the femur. The aim was to identify risk factors for death following proximal femur fractures in elderly patients. Patients and methods: This was a prospective, observational, longitudinal study conducted over an 18-month period in the Orthopedics-Traumatology Department of Aristide Le Dantec Hospital in Dakar. We evaluated a total of 93 patients aged at least 60 years with a proximal femur fracture whose date of onset was less than 3 weeks. The identification of risk factors for death took into account socio-demographic, clinical and therapeutic parameters. Results: The number of patients who died was 19 (20.43%). There was no significant difference in mortality according to age, gender or residence. There was no association between admission time and mortality. The association between the number of comorbidities and mortality was significant. The association between mortality and ASA score and type of treatment was highly significant. Operating time had no influence on mortality. Type of anesthesia, on the other hand, did influence mortality. Conclusion: Mortality due to proximal femur fractures in the elderly remains high. In our practice, this mortality was related to the presence of comorbidity, impaired cognitive function, ASA score, type of treatment and type of anesthesia.
