背景与目的此项探索性分析回顾评价了晚期非小细胞肺癌(non-small cell lung cancer,NSCLC)患者的结局,旨在确定基线时合并脑转移是否会影响一线应用帕博利珠单抗联合化疗(pembrolizumab plus chemotherapy,PC)比对单用化疗的疗效。患者和方法对KEYNOTE-021队列G(非鳞癌)、KEYNOTE-189(非鳞癌)和KEYNOTE-407(鳞癌)三项研究晚期NSCLC患者的数据进行汇总分析。患者接受含铂两药化疗和/或35个周期帕博利珠单抗治疗(每3周200 mg)。所有研究均纳入了经治或初治的稳定性脑转移患者(KEYNOTE-189与KEYNOTE-407研究纳入初治脑转移患者)。经治的脑转移患者已处于病情稳定的状态≥2周(KEYNOTE-021队列G患者≥4周),无新发或脑转移病灶扩大的证据且入组前至少3天以上未使用激素。初治的无症状脑转移患者需定期接受脑部影像学检查。结果共纳入1298例患者,其中171例有基线脑转移,1127例无脑转移。两组的中位随访时间(范围)在数据截止时分别为10.9(0.1-35.1)个月和11.0(0.1-34.9)个月。合并脑转移和无脑转移患者的总生存期[0.48(95%CI:0.32-0.70)和0.63(95%CI:0.53-0.75)]和无进展生存期[0.44(95%CI:0.31-0.62)和0.55(95%CI:0.48-0.63)]的风险比(PC/化疗)相似。合并脑转移的患者中,PC组和单用化疗组患者的中位总生存期分别为18.8个月和7.6个月,中位无进展生存期分别为6.9个月和4.1个月。无论是否合并脑转移,PC组患者的客观缓解率都高于单用化疗组,且缓解持续时间有显著延长。合并脑转移的患者中,PC组与单用化疗组的治疗相关不良事件发生率分别为88.2%和82.8%;而无脑转移的患者中,两组治疗相关不良事件的发生率分别为94.5%和90.6%。结论无论是否合并脑转移,与单用化疗相比,帕博利珠单抗联合以铂类为基础的组织学特异性化疗可改善所有PD-L1亚组的临床结局,其中包括PD-L1肿瘤比例评分<1%的患者,并且在晚期NSCLC患者中安全性良好。该方案是晚期NSCLC初治患者的标准方案,并可用于合并稳定性脑转移的患者。

Highly stable Ga-doped ZnO/polystyrene nanocomposite film with narrow-band cyan emission

In the present study,a simple method for the preparation of a luminescent flexible gallium doped zinc oxide(GZO)/polystyrene nanocomposite film was *** prepared GZO powder was characterized through different optical and structural *** XRD study revealed the existence of a wurtzite structure with no extra oxide ***-mapping,EDX,FTIR and XPS analyses were used to confirm the presence of elements and the several groups present in the *** excitations of UV,the prepared hybrid nanocomposite showed a strong cyan emission with narrow full width at half the maximum value(20 nm)that has not been reported before.X-ray and laser-induced luminescence results of the hy-brid film revealed novel blue-green emission at room *** prepared composite film showed a strong scintillation re-sponse to ionizing *** strong emissions,very weak deep-level emissions,and low FWHM of composite indicate the de-sirable optical properties with low-density structural defects in the GZO composite ***,the prepared hybrid film can be considered to be a suitable candidate for the fabrication of optoelectronic devices.

Mathematical modeling of electrochemical storage for incorporation in methods to optimize the operational planning of an interconnected micro grid

We extract a mathematical model to simulate the steady-state charging and discharging behaviors of an electrochemical storage over a 24-hour time ***,we develop a model for optimizing the daily operational planning of an interconnected micro grid considering electrochemical *** optimization model is formulated to maximize the total benefit of the micro grid via selling power to its end consumers and also exchanging power with the wholesale energy market so that the constraints of distributed energy resources(DERs) and low-voltage grid are *** optimization problem is solved by a genetic algorithm,and applied on two micro grids operating under different scenarios containing the absence or presence of electrochemical *** of the results of the optimization model for this micro grid,with and without electrochemical storage,shows that the electrochemical storage can improve the economical efficiency of the interconnected micro grids by up to 10.16%.

Impact of repeated loading on mechanical response of a reinforced sand

Pavements constructed over loosely compacted subgrades may not possess adequate California bearing ratio (CBR) to meet the requirements of pavement design codes,which may lead to a thicker pavement design for addressing the required *** have been proven to be effective for mitigating the adverse mechanical behaviors of weak soils as integrated constituents of base and sub-base layers in road *** study investigated the behaviors of unreinforced and reinforced sand with nonwoven geotextile using repeated CBR loading test (followed by unloading and reloading).The depth and number of geotextile reinforcement layers,as well as the compaction ratio of the soil above and below the reinforcement layer(s) and the compaction ratio of the sand bed,were set as variables in this *** layers were placed at upper thickness ratios of 0.3,0.6 and 0.9 and the lower thickness ratio of *** compaction ratios of the upper layer and the sand bed varied between 85% and 97% to simulate a dense layer on a medium dense sand bed for all unreinforced and reinforced testing *** CBR loading tests were conducted to the target loads of 100 kgf,150 kgf,200 kgf and 400 kgf,respectively (1 kgf=9.8 N).The results indicated that placing one layer of reinforcement with an upper thickness ratio of 0.3 and compacting the soil above the reinforcement to compaction ratio of 97% significantly reduced the penetration of the CBR piston for all target repeated load ***,using two layers of reinforcement sandwiched between two dense soil layers with a compaction ratio of 97% with upper and lower thickness ratios of 0.3 resulted in the lowest penetration.

A Study on Nanoparticle Aerosol Filtration via Different Fibrous Filters by Using CFD Approach

Fibrous filters such as nonwoven media are generally characterized by their collection efficiency and pressure drop. The traditional computational studies in this area typically based on unrealistic over-simplified geometries with the fibers placed in a lattice perpendicular to the flow. This paper describes the filter properties made of different Nonwoven materials by using Computational Fluid Dynamics (CFD) approach. In this study, for the first time, a virtual 3-D web is generated based on the fiber orientation information obtaining from analyzing microscopic images of both long-fiber and short-fiber nonwoven structures. Pressure drop and collection efficiency of our virtual filter are simulated and compared with the previous analytical and numerical models as well as experiment.

Triple-negative breast cancer(TNBC)is defined as a type of breast cancer with lack of expression of estrogen receptor,progesterone receptor and human epidermal growth factor 2 *** comparison to other types of breast cancer,TNBC characterizes for its aggressive behavior,more prone to early recurrence and a disease with poor response to molecular target *** TNBC is identified in only 25%-30%of American breast cancer cases annually,these tumors continue to be a therapeutic challenge for clinicians for several reasons:Tumor heterogeneity,limited and toxic systemic therapy options,and often resistance to current standard therapy,characterized by progressive disease on treatment,residual tumor after cytotoxic chemotherapy,and early recurrence after complete surgical ***-surface targeted therapies have been successful for breast cancer in general,however there are currently no approved cell-surface targeted therapies specifically indicated for ***,several cell-surface targets have been identified as candidates for treatment of TNBC and associated targeted therapies are in *** purpose of this work is to review the current clinical challenges posed by TNBC,the therapeutic approaches currently in use,and provide an overview of developing cell surface targeting approaches to improve outcomes for treatment resistant TNBC.

Integrating IPsec within OpenFlow Architecture for Secure Group Communication

Network security protocols such as IPsec have been used for many years to ensure robust end-to-end communication and are important in the context of SDN. Despite the widespread installation of IPsec to date, per-packet protection offered by the protocol is not very compatible with OpenFlow and tlow-like behavior. OpenFlow architecture cannot aggregate IPsee-ESP flows in transport mode or tunnel mode because layer-3 information is encrypted and therefore unreadable. In this paper, we propose using the Security Parameter Index （SPI） of IPsec within the OpenFlow architecture to identify and direct IPsec flows. This enables IPsec to conform to the packet-based behavior of OpenFlow architecture. In addition, by distinguishing between IPsec flows, the architecture is particularly suited to secure group communication.
