This work;It was made to determine the positive and negative usage information, attitudes and behaviors of social networking networks for users. Today, it is well known how the wide social effects of social networking sites are widespread and effective. As far as it can be followed from written and visual media, it causes significant damages for attitudes and behaviors for individuals. Looking at the results of the study, it seems impossible to limit the time and necessity of these networks for people, but it is understood that people force their limits and capacity. It was determined that the general acceptance and rejection areas are mixed and the dominant distinctive effects were weakened. Unable to prevent social dissolution and deterioration was realized by these social networks of networks. However, we cannot only talk about negative effects because their positive effects should be mentioned. Data were obtained in this direction. For example, some segments encourage the users of these sites to have more freedom, to develop democracy and human rights awareness. However, the real problem is when and where to stand. This study;Social networking networks use information, attitudes and behaviors;It was carried out for the purpose of evaluating the use of nicknames and networks for the use of networks. The data of the study were obtained by the application of a questionnaire and the data obtained were analyzed with appropriate statistical analysis methods. In the first part of the study, field study/operational concept scanning is performed and theoretical information is included. In the second part, the purpose of the study, the problem status, the importance of the study, limitations, assumptions, problem question, research method, research universe and sample are specified and informed. Factors Controlling Low Uranium and Thorium Concentrations in the Cayirhan Bituminous Shales(CBS)in the Beypazari(Ankara)Area,Turkey

In this study uranium and thorium contents and depositional characteristics of ay rhan bituminous shales（ BS）, west of Ankara（central Anatolia）, are investigated. Samples used were collected from boreholes opened by Park Holding Ltd. A total of 25 samples were taken from bituminous shale levels in boreholes drilled at 6 different locations in the study area. The H rka formation which hosts bituminous shale deposits is a volcanosedimentary sequence and all lithofacies indicate that a lacustrine environment where the water level was continuously changed. In addition to hydrocarbon generation potential, bituminous shales also accumulate significant amount of radioactive elements such as uranium and thorium. The average uranium and thorium concentrations of BS（1.83/2.62 ppm） are much lower than averages of UC, NASK and PAAS（uranium： 2.70/2.66/3.10 ppm; thorium： 8.50/12.30/14.60 ppm）. Low uranium contents in comparison to those of similar lacustrine environments might be attributed to that waters of depositional environment of BS contain low concentration of dissolved uranium and redox conditions were of oxic and dioxic character. Correlation data indicate that U and Th have a similar source and are associated predominantly with clays and phosphates and dominantly with organic material. Radioactive elements in the basin might be derived from Paleozoic granites and metamorphites（e.g. gneiss, schist） which comprise the basement and volcanism which was active in the region throughout the Miocene period. These elements are probably associated with uraniferous phosphate minerals（e.g. autunite, torbernite） which occur in granite, gneiss and schist. BS with average TOC content of 10.96 % shows very good/perfect source rock potential. Positive correlations between Gamma-Ray values and U, Th and K concentrations imply that radioactivity might be originated from these three elements.

Chronic constipation:Facilitator factor for development of varicocele

Simultaneous Intravitreal Ranibizumab and Dexamethasone Implant Administration at the Same Setting in Eyes with Severe Diabetic Macular Edema

Aim: To share our experience in eyes with severe DME (exhibiting serous retinal detachment or large cysts) treated with simultaneous intravitreal ranibizumab and dexamethasone implant administration at the same setting as the first treatment step. Subjects and Results: Five eyes of three patients with DME who were either treatment naive or relatively undertreated were presented in this report. As optical coherence tomography exhibited serous retinal detachment or severe cystoid edema with large cysts, intravitreal ranibizumab and dexamethasone implant were simultaneously employed at the same setting as the first treatment step in those eyes. Panretinal photocoagulation was also commenced bilaterally a week after the start of injections when at least one eye had retinal neovascularization. Subsequent treatments of intravitreal ranibizumab and/or dexamethasone implant were administered. Patients were followed up for seven, eight and 13 months respectively. All five eyes achieved a relative anatomic stability and experienced visual improvement at the end of follow-up. Conclusion: In some cases with severe DME with or without proliferative diabetic retinopathy, simultaneous intravitreal ranibizumab and dexamethasone implant administration at the same setting may be a better option to initiate the treatment over mono ranibizumab treatment. A randomized study comparing the mono anti-VEGF therapy and mono dexamethasone implant administration with simultaneous treatment may outline the place of this type of therapy in the treatment armamentarium of severe DME.

Helicobacter pylori and serum kynurenine-tryptophan ratio in patients with colorectal cancer

AIM: To evaluate how Helicobacter pylori(H. pylori) is able to evade the immune response and whether it enhances systemic immune tolerance against colorectal ***: This prospective randomized study involved 97 consecutive colorectal cancer patients and 108 cancer-free patients with extra-digestive diseases. Colorectal cancer and cancer-free patients were assigned into subgroups according to H. pylori Ig G seropositivity. Exposure to H. pylori was determined by Ig G seropositivity which was detected by enzyme linked immunoassay(ELISA). Serum neopterin levels were measured by ELISA. Serum tryptophan, kynurenine, and urinary biopterin concentrations were measured by high performance liquid chromatography. Serum nitrite levels were detected spectrophotometrically. Serum indoleamine 2,3-dioxygenase activity was estimated by the kynurenine to tryptophan ratio and by assessing the correlation between serum neopterin concentrations and the kynurenine to tryptophan ratio. The frequencies of increased serum kynurenine to tryptophan ratio of H. pylori seronegative and seropositive colorectal cancer subgroups were estimated by comparing them with the average kynurenine to tryptophan ratio of H. pylori seronegative tumor-free ***: Compared with respective controls, in both H. pylori seronegative and seropositive colorectal cancer patients, while serum tryptophan levels were decreased(controls vs patients; seronegative: 20.37 ± 0.89 μmol/L vs 15.71 ± 1.16 μmol/L, P < 0.05; seropositive: 20.71 ± 0.81 μmol/L vs 14.97 ± 0.79 μmol/L, P < 0.01) the kynurenine to tryptophan ratio was significantly increased(controls vs patients; seronegative: 52.85± 11.85 μmol/mmol vs 78.91 ± 8.68 μmol/mmol, P < 0.01, seropositive: 47.31 ± 5.93 μmol/mmol vs 109.65 ± 11.50 μmol/mmol, P < 0.01). AIM： To evaluate the frequency of neural cell adhesion molecule （NCAM）-180 expression in fresh tumor tissue samples and to discuss the prognostic value of NCAM-180 in routine clinical practice.METHODS： Twenty-six patients （16 men, 10 women） with colorectal cancer were included in the study. Fresh tumor tissue samples and macroscopically healthy proximal margins of each specimen were subjected to flow-cytometric analysis for NCAM-180 expression.RESULTS： Flow-cytometric analysis determined NCAM-180 expression in whole tissue samples of macroscopically healthy colorectal tissues. However, NCAM-180 expression was positive in only one case （3.84%） with well-differentiated Stage Ⅱ disease who experienced no active disease at 30 months follow-up. CONCLUSION： As a consequence of the limited number of cases in our series, it might not be possible to make a generalisation, nevertheless the routine use of NCAM-180 expression as a prognostic marker for colorectal carcinoma seems to be unfeasible and not cost-effective in clinical practice due to its very low incidence.

Determination of Genotoxic Pollution of Some Hospital Wastewater with Salmonella Ames Test

Wastewater of hospitals contains materials that would be a threat to alive. These water needs to be checked by a biological purification before leaving to nature. Hospital wastewater has differences than domestic waste because of especially blood, body waste, drugs, chemicals, medical device waste and radioactive materials. We aimed to determine genotoxic effects of total pollution in hospital wastewater on alive by Salmonella microsome test method. In this study, we decided on three hospitals which weren’t checked as biological purification of waste. The samples were taken for six 1-week periods between March 2009 and June 2009. Mutagenite studies of samples taken from hospitals were made with ,Salmonella typhimurium TA 98 and TA 100. Wastewater samples were evaporated. 27 different test materials were prepared using DMSO, ethanol and acetone solvents, two different MGA plaques were used for each test material. Each experiment was made for 3 times with known results of mutagens and we made it ready for “Ames” test method. We had genotoxicity 50% in Istanbul University Medical Faculty Hospital, 56% in Haseki Hospital and 61% in Vak?f Gureba Hospital. According to three hospitals result there are 9 positives, 9 negatives in DMSO;9 positives, 9 negatives in ethanol;12 positives, 6 negatives in acetone. These values are totally 56%. Our results give important information about mutagenic effect of total pollution in hospital wastewater. It is first time researched in Turkey that effect on DNA of pollution is from hospital wastewaters. In prospective studies, it is necessary to use this system as a method to monitor mutagenic genotoxic pollution in hospital wastewaters. These kinds of studies present applicability and importance of our method because of placing in the literature. Method constitutes a new approach to check mutagenite of pollution in hospital wastewater.

Imatinib response of gastrointestinal stromal tumor patients with germline mutation on KIT exon 13:A family report

Familial gastrointestinal stromal tumor(GIST) is a rare autosomal dominant disorder associated with mutations in the KIT gene in the majority of cases. Although, exon 11 appears to be the hot spot region for approximately 95% of germline mutations, pathogenic variations have also been identified in exon 8, 13 and 17. Exon 13 germline mutations are extremely rare amongst familial GISTs and seven families with a germline mutation have been reported to date. Moreover, the role of imatinib mesylate in this rare familiar settings is not completely known so far. We describe here clinical, imaging, pathological and genetic findings of a family with four affected members; grandmother, his son and two grand-sons having a germline gain-of-function mutation of KIT in exon 13 and discuss the imatinib mesylate treatment surveillance outcomes towards disease management.

Adult Poisoning Cases in Ankara: Capital City of Turkey

Poisoning is the disruption of functions of a living organism by any agent. The aim of this study is to identify the clinical and demographic characteristics of intoxication cases admitted to the Numune Training and Research Hospital emergency department. Characteristics such as age and gender of the patients, intoxication agents, and the results (discharge, forwarding, hospitalization, death etc.) were recorded to the prepared form. Data were assessed with the SPSS 15.0 software package and expressed. Quantitative variables were summarised as mean ± standard deviation. Frequency and percent distribution were used for the evaluation of data. The chi-squared test was used for comparison of the quailtative variables. p 0.05 was accepted significant. The mean age of the receiving 651 poisoned patients was 32.04 ± 12.24 years. Of them, 67.7% were female. The most causes of poisoning agents were a pharmaceuticals (79.4%), carbonmonoxide (8.9%) and alcohols (7.1%). Gastric lavage was performed in 76% patients. Activated charcoal was administered in 81% patients. Antidotes treatment was applied in 7.4% patients. Most patients were evaluated and then treated and discharged from the ED (91.9%).
