Urinary Retention in Adults Male Patients: Causes and Complications among Patients Managed in a Teaching Hospital in North Western Nigeria

Background: Urinary retention is one of the common urologic emergencies constituting a significant workload of urologists and non-urologists alike. Aims and Objectives: This study was undertaken to identify the causes and complications associated with urinary retention in adult male patients at AKTH, Kano. Patients and Methods: It was a prospective hospital-based study of 110 consecutive adult male patients who presented to Aminu Kano Teaching Hospital, Kano with urinary retention over 12 months. On presentation, a brief history was taken and rapid physical examination done;the urinary retention was relieved by urethral catheterization or suprapubic cystostomy. A detailed history and thorough physical examination findings were later obtained. Results: One hundred and ten patients were seen during the study period. There ages ranged from 17 to100 years with a mean of 56 ± 19.3 SD years. Patients within the age range 55 - 74 years accounted for the largest group, (25.5% + 23.6% = 49.1%). The most common cause of urinary retention in this study was benign prostatic hyperplasia (51.8% of the patients). This was followed by urethral stricture (20%), cancer of the prostate (7.3%), urethral injury (7.3%) and bladder tumour (6.4%). More than half of the patients (53.4%) presented with acute urinary retention, 30.5% presented with chronic urinary retention, and 16.1% were diagnosed to have acute-on-chronic urinary retention. The complications of urinary retention found were: urinary tract infection (in 24.5% of patients), renal impairment (14.5%), and anaemia (11.8%). Conclusion: Urinary retention is commoner among the middle aged and the elderly. Benign prostatic hyperplasia remains the leading cause of urinary retention. Acute urinary retention was the commonest type of urinary retention;however, complications due to the retention were associated with chronic and acute-on-chronic urinary retention.

This study developed a systematic decision-making process for water supply capacity expansion using the analytic hierarchy process. The decision-making criteria were categorized into environmental, economic, technical and socio-cultural aspects. Capacity expansion of three water resources (Kpong, Weija and Teshie plants) of Accra-Tema Metropolitan Area (Ghana) was studied as a test case. The research resulted in the environmental criterion with the highest priority weight (52.4%), followed by the economic (30.6%), technical (11.3%) and socio-cultural criteria (5.8%). The overall analysis ranked the Kpong plant with a score of 36.1% followed by the Weija and Teshie plants with scores 33.8% and 30.2%, respectively.

Tuberculosis is one of the most important infectious diseases, particularly in developing countries. Involvement of the prostate gland is very rare. We present a 70-year-old man who presented with low back pain and progressive weakness of the lower limbs associated with irritative lower urinary tract symptoms. Prostate gland was enlarged, hard and nodular. Prostate specific antigen was 6 ng/ml, Erythrocytes sedimentation rate was 130 mm/hr and Monteux was positive. Prostate biopsy revealed features of tuberculosis and lumbosacral radiograph revealed features of Pott’s disease. Diagnosis of disseminated tuberculosis with prostate involvement was made and patient was placed on anti-tuberculosis drugs with good response.

Integration for coexistence？ Implementation of intercultural health care policy in Ghana from the perspective of service users and providers

OBJECTIVE： In spite of the World Health Organization＇s recommendations over the past decades, Ghana features pluralistic rather than truly integrated medical system. Policies about the integration of complementary medicine into the national health care delivery system need to account for individuallevel involvement and cultural acceptability of care rendered by health care providers. Studies in Ghana, however, have glossed over the standpoint of the persons of the illness episode about the intercultural health care policy framework. This paper explores the health care users, and providers＇ experiences and attitudes towards the implementation of intercultural health care policy in Ghana. METHODS： In-depth interviews, augmented with informal conversations, were conducted with 16 health service users, 7 traditional healers and 6 health professionals in the Sekyere South District and Kumasi Metropolis in the Ashanti Region of Ghana. Data were thematically analysed and presented based on the a posteriori inductive reduction approach. RESULTS： Findings reveal a widespread positive attitude to, and support for integrative medical care in Ghana. However, inter-provider communication in a form of cross-referrals and collaborative mechanisms between healers and health professionals seldom occurs and remains unofficially sanctioned. Traditional healers and health care professionals are skeptical about intercultural health care policy mainly due to inadequate political commitment for provider education. The medical practitioners have limited opportunity to undergo training for integrative medical practice. We also find a serious mistrust between the practitioners due to the ＂diversity of healing approaches and techniques.＂ Weak institutional support, lack of training to meet standards of practice, poor registration and regulatory measures as well as negative perception of the integrative medical policy inhibit its implementation in Ghana. First principle calculation within the Density Functional Theory (DFT) and Density Functional Perturbation Theory (DFPT) using Local Density Approximation as implemented in Quantum ESPRESSO has been significantly used to investigate the structural and Piezoelectric, properties of Perovskite ZrTi(PbO3)2. From structural properties calculation, the ground state total energy of -2417.12 eV has been obtained which led to an equilibrium lattice constant of a= 5.620Åfor ZrTi(PbO3)2. Our obtained optimized atomic positions and atomic effective charge shows that the optimized ZrTi(PbO3)2 is stable and the Piezoelectric stress tensor is calculated using Berry-phase approach within density functional perturbation theory (DFPT). From our calculation, we have obtained the stress tensor elements with values of d1,5 = 6.81, d3,1 = 1.69, and d3,3 = 6.18, which is in agreement with the values obtained for tetragonal PbTiO3.

Estimating Urban Households’ Willingness-to-Pay for Upland Forest Restoration in Vietnam

Increased urbanization coupled with increased reliance of urban communities on rural areas for ecosystem service provision is a challenge faced by many nations. The ability of urban households to directly support restoration efforts in surrounding rural regions represents an underappreciated funding stream for ecological restoration. This study explored the willingness of urban households to support forest restoration in Vietnam. We surveyed 211 households (HHs) in the capital city Hanoi, Vietnam. A Maximum Likelihood Estimator (MLE) model allowed us to obtain the parameters of our model and quantify mean Willingness-to-Pay (WTP) for a program of forest restoration in addition to identifying factors influencing the decision of WTP. Generally, over forty percent of the households surveyed are willing to pay for forest restoration and the mean value of WTP is 37,830 VND ($1.73) per household per month. WTP depends on endogenous and exogenous factors including level of education, income, female-to-male ratio in the household, attitude toward payment for monthly electricity consumption, and awareness of payment for environmental service. Our results suggest that urban household’s demand for forest restoration is real, and represents an untapped source of restoration funding. Policy-makers should take actions to apply charges on water bills to turn this potential into reality for restoration projects in Vietnam if the benefits from restoration outweigh the costs based on our findings.

We present qualitative data from a study in Ghana （2011）, where the National Health Insurance Scheme （NHIS） was introduced to improve access to health care. In 2011 membership enrolment and retention in the scheme was stalling. To obtain better insights into socio-cultural factors that influence utilization of healthcare services and the NHIS this study compared Explanatory Models of healthcare clients with those of primary healthcare providers and the NHIS regarding illness, the need for, the quality of, and the control over heaithcare and health insurance services. We found critical disparities in socio-cultural beliefs and perceptions of healthcare and health insurance between these three stakeholder groups, such as the clients＇ holistic view on illness versus healthcare providers＇ bio-medical view; the clients＇ inter-relational focus in perceiving quality of services versus the providers＇ medical technical focus. These differences are leading to misconceptions, blame practice, poor services, non-adherence and low trust. The findings increase our understanding of clients＇ behavior and that of their service providers. We conclude with key messages for policy leaders and operational managers that can guide them in improving services and facilitating client trust and interest to participate in health insurance and utilize healthcare services.

BACKGROUND Oral candidiasis(OC)is an oral health disease that could influence patients’oral health quality of *** To estimate prevalence of OC among disabled and non-disabled individuals and its potential risk factors in the Al-Baha region,Saudi *** An observational cross-sectional study was carried out among 148 disabled and non-disabled *** technique of concentrated oral rinse employing the Sabouraud Dextrose Agar medium accompanied with 0.05%chloramphenicol was conducted to assess and isolate *** examination using the World Health Organization guidelines was conducted to examine participants’oral hea-lth status.A pre-designed questionnaire was also used to evaluate sociodemo-graphic,medical history,and oral hygiene habits of the studied *** Out of 148 participants(n=57,38%)had colonized *** of the studied population had visible Candida ***,Candida was found in the oral rinses without the subject presenting any lesions or issues caused by Candida(asymptomatic colonization).The most common prevalent OC among participants were Candida albicans,Candida glabrata,Candida dubliniensis,Candida krusei,Candida tropicalis,and Candida parapsilosis(n=35,61%;n=8,14%;n=6,10%;n=5,9%;n=2,4%;and n=1,2%)***,smoking,poor plaque,and gingival status were key potential risk factors that significantly associated with candida’s density and presence(P=0.001,P=0.001,P=0.01,and P=0.01)*** status had no statistically significant effect on presence and density of *** The prevalence of OC is almost third of the studied population;thus,may provoke a need to develop preventive strategies to reduce the OC rate and establish solid treatment plans.
