Cecal Volvulus: Rare Presentation of Intestinal Obstruction-Case Report

Introduction: Cecal volvulus is a rare cause of intestinal obstruction that occurs 1% - 1.5% of all intestinal obstructions. Causes of volvulus are usually unknown but it can be due to a defective peritoneal fixation of the ascending colon and cecum in 10% and secondary causes (surgical adhesions, colonic carcinoma or diverticulitis). Case Presentation: A 56-year-old woman presented with colicky abdominal pain, bilious vomiting and abdominal distention for two days, who has no history of previous surgery and no other gastrointestinal symptoms or chronic illness. On examinations, she looks ill, dehydrated, abdomen massively distended with exaggerated bowel sound but no signs of peritonitis. CT abdomen with contrast finding: type 11 cecal volvulus seen in midline to the left above the umbilicus reaching 8 cm with ileocecal junction as well as the elongated appendix is reaching the right iliac fossa. Emergency exploratory laparotomy was done and the finding was, obstructing rectosigmoid tumor with cecal volvulus. Discussion: Cecal volvulus is one of the rare causes of mechanical intestinal obstruction which required urgent surgical intervention, and it occurs due to an axial twist of the caecum, ascending colon and terminal ileum around the mesenteric pedicle. Conclusion: Radiological imaging helps in the diagnosis of cecal volvulus especially CT scan with contrast as gold standard for both diagnosis and assessment for complications. However, this should not delay the time of intervention especially if patient presented at late stage with evidence of peritonitis or bowel ischemia and surgical right hemicolectomy is the most effective treatment option. The colonic pathology always should be assessed, as it could be the primary cause of cecal volvulus.

Highly efficient fluoride removal from water using 2D metal-organic frameworks MIL-53(Al)with rich Al and O adsorptive centers

In this study,metal-organic framework MIL-53(Al)was synthesized and studied to understand the different mechanisms between normal MIL-53(Al)and 2D metal-organic framework MIL-53(Al)for removing ***,the 2D MIL-53(Al)had two-dimensional linear morphology rather than block shape,indicating more expose adsorptive sites than normal MIL-53(Al).The batch adsorption experiments were applied to investigate the performance of 2D MIL-53(Al),including pH,adsorption kinetics,and *** 2D MIL-53(Al)(75.50 mg/g)showed better adsorption capacity than normal MIL-53(Al)(35.63 mg/g).The adsorption process of 2D MIL-53(Al)followed the pseudo-firstorder model and Langmuir *** adsorption mechanism of this material was further studied by using experimental characterization and density functional theory calculations in *** main adsorptive sites were Al and O in the 2D MIL-53(Al),and the relationship between fluoride binding with Al and O was HF^(-)_(2)>HF>F^(-).The species of fluoride were HF2,HF,F at different pH and ***,this study provides a significant way on the application of two-dimensional materials for removing fluoride.

Volvulus of Small Bowel Caused by GISTs: Case Report and Literature Review

Introduction: Gastrointestinal stromal tumors (GISTs) originating from the interstitial cells of Cajal and GISTs are the most frequent mesenchymal tumors in the gastrointestinal tract (20% to 30% occur in the small intestine). most small-intestinal neoplasms are asymptomatic. The most common mode of presentation is partial small bowel obstruction, with associated symptoms of colicky abdominal pain, abdominal distention, nausea, and vomiting. Obstruction can be caused by luminal narrowing by the tumor itself or intussusception. Case Presentation: We presented an 85-year-old lady presented with acute intestinal obstruction and CT abdomen with contrast revealed, a large well defined solid mass lesion, enhanced in the arterial phase and inseparable from the jejunal loops and it measures 09 × 08 × 07 cm, likely GISTs with secondary volvulus of the small bowel. Emergency laparotomy with complete resection of the tumor and end-to-end anastomosis of jejunum was done. Discussion: Small-intestinal GISTs should be treated with segmental intestinal resection. If the diagnosis is known prior to resection, wide lymphadenectomy can be avoided as GISTs are rarely associated with lymph node metastases. GISTs are resistant to conventional chemotherapy agents. Imatinib (Gleevec) is a tyrosine kinase inhibitor with potent activity against the tyrosine kinase KIT and is used in those with metastatic disease and as an adjuvant chemotherapeutic agent.

Assessment of tear film parameters post-treatment with commercial eyelid cleaning wipes:a pilot study

AIM:To investigate the short-term effects of commercially available eyelid-cleaning wipes on film ***:This study enrolled 48 healthy participants aged 20-35y(both males and females).Clinical assessment included the Ocular Surface Disease Index(OSDI)questionnaire,non-invasive tear break-up time(NITBUT),tear meniscus height(TMH),and lipid layer pattern(LLP).Based on these initial results,participants were categorized as either non-dry eye or dry *** in each group were randomly allocated to either Blephaclean■or Systane■*** in NITBUT,TMH,and LLP levels before and after lid wipe treatment were ***:The dry eye group exhibited significantly higher OSDI scores and lower NITBUT and TMH levels than in the non-dry eye group(P0.05).Similarly,the LLP levels were significantly improved with Systane■(P<0.001)post-treatment compared to individuals in the non-dry eye ***,the dry eye subjects showed higher posttreatment LLP levels than the untreated group(P<0.02).CONCLUSION:The short-term effects of Systane®eyelid wipes on tear film parameters suggest their effectiveness in dry eye ***,further exploration of their long-term impact is essential to justify their cost effectiveness and efficacy in treating both aqueous deficiency and evaporative dryness.

Investigating Spatio-Temporal Pattern of Relative Risk of Tuberculosis in Kenya Using Bayesian Hierarchical Approaches

Proper understanding of global distribution of infectious diseases is an important part of disease management and policy making. However, data are subject to complexities caused by heterogeneities across host classes and space-time epidemic processes. This paper seeks to suggest or propose Bayesian spatio-temporal model for modeling and mapping tuberculosis relative risks in space and time as well identify risks factors associated with the tuberculosis and counties in Kenya with high tuberculosis relative risks. In this paper, we used spatio-temporal Bayesian hierarchical models to study the pattern of tuberculosis relative risks in Kenya. The Markov Chain monte Carlo method via WinBUGS and R packages were used for simulations and estimation of the parameter estimates. The best fitting model is selected using the Deviance Information Criterion proposed by Spiegelhalter and colleagues. Among the spatio-temporal models used, the Knorr-Held model with space-time interaction type III and IV fit the data well but type IV appears better than type III. Variation in tuberculosis risk is observed among Kenya counties and clustering among counties with high tuberculosis relative risks. The prevalence of HIV is identified as the determinant of TB. We found clustering and heterogeneity of TB risk among high rate counties and the overall tuberculosis risk is slightly decreasing from 2002-2009. We proposed that the Knorr-Held model with interaction type IV should be used to model and map Kenyan tuberculosis relative risks. Interaction of TB relative risk in space and time increases among rural counties that share boundaries with urban counties with high tuberculosis risk. This is due to the ability of models to borrow strength from neighboring counties, such that nearby counties have similar risk. The Hedonic modelling of Rental Values of Properties in Nasarawa, Nigeria

The purpose of this study is to explore the need to adopt the CAMA （computer assisted mass appraisal） based system into the Nigerian property market with an example data from Nasarawa in Nasarawa State. Using the hedonic regression models on 312 samples, the results reveal the linear form to outperform the semi-log specification. This demonstration shows that CAMA system can be introduced into the local market of Nasarawa and indeed the whole of Nigeria for property taxes.

Efficacy of Acid-treated Sawdust in Decolourization of Tanning Wastewater

Colour removal from dye-bearing effluent is a serious challenge due to the difficulty in treating such wastewater by conventional treatment *** present investigation explores the decolourization of contaminated wastewater using acid-activated sawdust as an *** physicochemical properties of wastewater samples labelled A,B,and C viz average temperature;pH;electrical conductivity;and total dissolved solids were determined using standard methods to be 302.63;6.1;284.47μS/cm;35116.66 mg/L *** removal efficiency of the adsorbent was studied under variable conditions(contact time,rate of agitation,loading).Experimental results demonstrated that the sawdust adsorbent has a significant capacity for colour removal from tannery *** was significant variation in the absorbance of the treated *** dose,stirring rate,and contact time were found to be directly proportional to colour removal while pH variation of the samples show that the effluents became less alkaline(slightly acidic)after decolourization.

The Effect of Evotherm on Hot Mix Asphalt

There has been a paradigm shift globally from high combustion rates of all human activities that contribute to global warming. Environmental awareness has been increasing rapidly over the past years. In these regards, Evotherm, which is a Fatty-Amine derivative of the hydrocarbon chain, was investigated and used in the production of asphalt to examine the effect. The overall influence of the admixture and temperature was examined. Seven (7) Laboratory specimens with different additive contents of 0%, 0.25%, 0.5%, 0.75%, 1.0%, 1.25% and 1.5% were produced and compacted at different compaction temperatures of 100°C, 115°C, 130°C, 145°C and 160°C, to evaluate Marshall Stability parameters. The binder content for the control test and admix samples were all kept constant for the range of 5.0%, 5.5%, 6.0%, 6.5% and 7% by weight of total mix for all the trial mixes for proper and uniform observation and also for the determination of the optimal mix for the control and admix samples. For each specimen mix, two (2) Marshall cylindrical samples were compacted at heavy compaction level of 75 blows, which represent heavy traffic category, and the mean result obtained for accuracy. This was done for all the temperature categories. Therefore, for this research work, 350 cylindrical cubes of Marshall Samples were evaluated in the laboratory. The result shows that despite the modification on the asphalt by the Evotherm product, the admixed asphalt exhibited similar engineering properties as Hot Mix Asphalt even at lower temperature of 115°C, and could be used as a first step in the promotion of improved carbon print from Nigerian Asphalt pavements.

Simple Linear Regression model for Hidden/Hard-to-Reach/Elusive Populations

Estimation methods have over the years been a problem for Statistician especially in sectors that have to do with Hidden/Hard-to-Reach population. In this paper, a regression model was derived for Elusive/Hard-to-Reach/Hidden populations. This was achieved by modelling the Multiplicity Estimator given by Birnbaum and Sirken (1965) into a regression model. The paper also gave the least-squares estimation of the unknown parameters β0 and β1, and σ2.
