Effect of intra-abdominal pressure on respiratory function in patients undergoing ventral hernia repair

AIM: To determine the influence of intra-abdominal pressure(IAP) on respiratory function after surgical repair of ventral hernia and to compare two different methods of IAP measurement during the perioperative period. METHODS: Thirty adult patients after elective repair of ventral hernia were enrolled into this prospective *** monitoring was performed via both a balloontipped nasogastric probe [intragastric pressure(IGP), Ci MON, Pulsion Medical Systems, Munich, Germany] and a urinary catheter [intrabladder pressure(IBP), Uno Meter Abdo-Pressure Kit, Uno Medical, Denmark] on five consecutive stages:(1) after tracheal intubation(AI);(2) after ventral hernia repair;(3) at the end of surgery;(4) during spontaneous breathing trial through the endotracheal tube; and(5) at 1 h after tracheal extubation. The patients were in the complete supine position during all study ***: The IAP(measured via both techniques) increased on average by 12% during surgery compared to AI(P y 43% during spontaneous breathing through the endotracheal tube(P y, reaching a maximum during the spontaneous breathing trial. The PаO2/Fi O2 decreased by 30% one hour after tracheal extubation(P = 0.02). The dynamic compliance of respiratory system reduced intraoperatively by 15%-20%(P ying from-0.19 mm Hg(2SD 7.25 mm Hg) to-1.06 mm Hg(2SD 8.04 mm Hg) depending on the study stage. Taking all paired measurements together(n = 133), the median IGP was 8.0(5.5-11.0) mm Hg and the median IBP was 8.8(5.8-13.1) mm Hg. The overall r2 value( n = 30) was 0.76(P ysis showed an overall bias for the mean values per patient of 0.6 mm Hg(2SD 4.2 mm Hg) with percentage error of 45.6%. Poly(1-butyl-3-vinylimidazolium bromide) is a polymerized ionic liquid (PILs), a relatively new class of materials that combines the attractive properties of ionic liquids (ILs) and polyelectrolytes and finds wide applications. The backbone of this PIL is composed of quaternary imidazolium salts, which are among the most promising and popular ILs. However, little is known about the physicochemical characteristics of the aqueous solutions of this PIL. In this study, we synthesized and characterized samples of this PIL and obtained experimental data on the viscosity, static and dynamic light scattering, and nuclear magnetic resonance diffusometry for aqueous and aqueous KBr solutions with varying polymer contents at T=298.15 K. We discuss the effects of the polymer concentration and salinity on the behavior of the solution.

Strong multi-kilotesla magnetic fields have various applications in high-energy density science and laboratory astrophysics,but they are not readily *** our previous work[*** et al.,***.130,155101(2023)],we developed a novel approach for generating such fields using multiple conventional laser beams with a twist in the pointing *** method is particularly well-suited for multi-kilojoule petawatt-class laser systems like SG-II UP,which are designed with multiple linearly polarized *** three-dimensional kinetic particle-in-cell simulations,we examine critical factors for a proof-of-principle experiment,such as laser polarization,relative pulse delay,phase offset,pointing stability,and target configuration,and their impact on magnetic field *** general conclusion is that the approach is very robust and can be realized under a wide range of laser parameters and plasma *** also provide an in-depth analysis of the axial magnetic field configuration,azimuthal electron current,and electron and ion orbital angular momentum *** by a simple model,our analysis shows that the axial magnetic field decays owing to the expansion of hot electrons.

The concept of computability is defined more exactly and illustrated as an example of Boolean functions and cryptanalysis. To define a Boolean function is not necessary to record its formula. To do that the reduced (compact) description of values is determined in the truth table or in the statement of the problem. We obtain estimates of computation time, the volume of a compact descriptions and the range of variables under which it takes the value 0 or 1, depending polynomially on the number of arguments.

Pricing strategies in inelastic energy markets： can we use less if we can＇t extract more？

Limited supply of nonrenewable energy resources under growing energy demand creates a situation when a marginal change in the quantity supplied or demanded causes non-marginal swings in price levels. The situation is worsened by the fact that we are currently running out of cheap energy resources at the global scale while adaptation to climate change requires extra energy costs. It is often argued that technology and alternative energy will be a solution. However, alternative energy infrastructure also requires additional energy investments, which can further increase the gap between energy demand and supply. This paper presents an explorative model that demonstrates that a smooth transition from an oil-based economy to alternative energy sources is possible only if it is started well in advance while fossil resources are still abundant. Later the transition looks much more dramatic and it becomes risky to rely entirely on technological solutions. It becomes increasingly likely that in addition to technological solutions that can increase supply we will need to find ways to decrease demand and consumption. We further argue that market mechanisms can be just as powerful tools to curb demand as they have traditionally been for stimulating consumption. We observe that individuals who consume more energy resources benefit at the expense of those who consume less, effectively imposing price externalities on the latters. We suggest two transparent and flexible methods of pricing that attempt to eliminate price externalities on energy resources. Such pricing schemes stimulate less consumption and can smooth the transition to renewable energy.

Traditionally,urbanization has been seen as a negative phenomenon for ***,changes in the environmental parameters induced by urbanization might be favorable for some *** the past half-century,the common hamster has actively populated cities,establishing populations in some European,Russian and Kazakhstan *** on integrative methods,we investigated free-range common hamsters inhabiting Simferopol from 2015 to 2018 to reveal possible adaptations to the urbanized environment across several parameters,including lifespan,hibernation period,reproductive activity and body *** show that in urban areas,the common hamster demonstrates an extremely short hibernation period compared to other localities,possibly due to enhanced food resources from urban forestry(walnuts,locus and hazelnut),allowing the species to start breeding very early(February)and finish as late as *** present the first evidence of polyandry for this species:mating of receptive females with several males and subsequent confirmation of multiple *** high reproductive potential,the lifespan of the common hamster in urban conditions is generally very short(less than 1 year).We speculate that in the process of synurbization,the common hamster’s innate plasticity across many life history traits permits it to successfully colonize throughout a wide range of habitats,with the ability to form novel adaptations to urban environments.
