Antithrombotic treatment tailoring and risk score evaluation in elderly patients diagnosed with an acute coronary syndrome

Age is an important prognostic factor in the outcome of acute coronary syndromes (ACS). A substantial percentage of patients who experience ACS is more than 75 years old, and they represent the fastest-growing segment of the population treated in this setting. These patients present different patterns of responses to pharmacotherapy, namely, a higher ischemic and bleeding risk than do patients under 75 years of age. Our aim was to identify whether the currently available ACS ischemic and bleeding risk scores, which has been validated for the general population, may also apply to the elderly population. The second aim was to determine whether the elderly benefit more from a specific pharmacological regimen, keeping in mind the numerous molecules of antiplatelet and antithrombotic drugs, all validated in the general population. We concluded that the GRACE (Global Registry of Acute Coronary Events) risk score has been extensively validated in the elderly. However, the CRUSADE (Can Rapid risk stratification of Unstable angina patients Suppress ADverse outcomes with Early implementation of the ACC/AHA Guidelines) bleeding score has a moderate correlation with outcomes in the elderly. Until now, there have not been head-to-head scores that quantify the ischemic versus hemorrhagic risk or scores that use the same end point and timeline (e.g., ischemic death rate versus bleeding death rate at one month). We also recommend that the frailty score be considered or integrated into the current existing scores to better quantify the overall patient risk. With regard to medical treatment, based on the subgroup analysis, we identified the drugs that have the least adverse effects in the elderly while maintaining optimal efficacy.

Corrosion Investigation by Scanning Electrochemical Microscopy of AISI 446 and Ti-Coated AISI 446 Ferritic Stainless Steel as Potential Material for Bipolar Plate in PEMWE

The components of proton exchange membrane water electrolysers frequently experience corrosion issues, especially at high anodic polarization, that restrict the use of more affordable alternatives to titanium. Here, we investigate localized corrosion processes of bare and Ti-coated AISI 446 ferritic stainless steel under anodic polarization by scanning electrochemical microscopy (SECM) in sodium sulphate and potassium chloride solutions. SECM approach curves and area scans measured at open-circuit potential (OCP) of the samples in the feedback mode using a redox mediator evidence a negative feedback effect caused by the surface passive film. For the anodic polarization of the sample, the substrate generation-tip collection mode enables to observe local generation of iron (II) ions, as well as formation of molecular oxygen. For the uncoated AISI 446 sample, localized corrosion is detected in sodium sulphate solution simultaneously with oxygen formation at anodic potentials of 1.0 V vs. Ag/AgCl, whereas significant pitting corrosion is observed even at 0.2 V vs. Ag/AgCl in potassium chloride solution. The Ti-coated AISI 446 sample reveals enhanced corrosion resistance in both test solutions, without any evidence of iron (II) ions generation at anodic potentials of 1.2 V vs. Ag/AgCl, where only oxygen formation is observed.

This work addresses the alloying of titanium aluminides used in aircraft engine applications and automobiles. The oxidation resistance behavior of two titanium aluminides of α2 ＋ γ （Ti3Al ＋ TiAl） and orthorhombic Ti2NbAl, recognized as candidates for high-temperature applications, was investigated by exposure of the alloys for 100 h in air. Thus, oxidation resistance was expressed as the mass gain rate, whereas surface aspects were analyzed using scanning electron microscopy in conjunction with energy-dispersive X-ray spectroscopy, and the type of oxidation products was analyzed by X-ray diffraction and Raman spectroscopy. The orthorhombic Ti2NbAl alloy was embrittled, and pores and microcracks were formed as a result of oxygen diffusion through the external oxide layer formed during thermal oxidation for 100 h.

We report on the generation and delivery of 10.2 PW peak power laser pulses,using the High Power Laser System at the Extreme Laser Infrastructure–Nuclear Physics *** this work we demonstrate for the first time,to the best of our knowledge,the compression and propagation of full energy,full aperture,laser pulses that reach a power level of more than 10 PW.

Power Quality Comparative Analysis between UPF and SAPF

This paper makes a technical analysis of the converter’s topologies that are conventionally employed in three-phase three-wire in increasing the power factor and harmonics eliminating,namely unit power factor(UPF)and shunt active power filter(SAPF)and highlights the improvement of energy quality in the supply *** paper highlights that the mathematical model of the three-phase PWM rectifier in the dq coordinate system is similar for both *** structure of the control schemes of the two topologies for increasing the energy quality is similar by transformation of three-phase stationary coordinate system(a,b,c),to synchronous rotating(d,q)coordinate system that synchronously rotates with the fundamental voltage of the *** analysis and simulation results show that the control in synchronous rotating coordinate system has high power factor and low THDi for the both topologies.

Comparative Analysis of the Methods Used for Active Power Filtering in a Grid-Connected Photovoltaic System

This paper presents a comparison of the results obtained from the integration of the photovoltaic system to the electrical grid through the Active Power *** innovative aspect of this article consists of the elaboration of 4 models in Matlab/Simulink for 4 methods of control of the Active Power Filter introduced between the photovoltaic system and the alternating current grid and the highlighting of the performances obtained regarding the quality and quantity of electricity transferred for each control method in *** the analysis of the results,it was found that by using the Indirect Control(CI)method of the Active Power Filter,the best results are obtained THDu=3.24%,THDi=2.46%if the photovoltaic system produces a larger amount of energy than the nonlinear load,and by using the methods of control of the Active Power Filter Synchronous Reference Frame(DQ)and Instantaneous Power(PQ)the best results are obtained:THDu=3.22%,THDi=5.26%for the method(DQ)and THDu=3.41%,THDi=5.17%for the method(PQ)if the photovoltaic system produces a smaller amount of energy than the nonlinear load.

Circulating microparticles and micro RNAs as players in atherosclerosis

Microparticles(MPs) are extracellular membrane vesicles released from normal, apoptotic and pathological cells following a process of detachment from cells of origin. MPs are typically defined by their size, exposure of phosphatidylserine, the expression of surface antigens, proteins and genetic material, originating from their donor cells, and as important vehicles of intercellular communication across numerous biological processes. MPs contain the major source of systemic RNA including micro RNA(mi RNA) of which aberrant expression appears to be associated with stage and progression of atherosclerosis. The involvement and influence of mi RNA during the onset and progression of atherosclerotic disease have generated a lot of inter-est in assessing the feasibility of therapeutic regulation of mi RNAs to manipulate them with a special focus on cardiovascular disease. We speculate on the future de velopments of MPs which contain mi RNA as new thera peutic targets for proliferative vascular diseases such as atherosclerosis.
