Distribution of atrial cardiomyopathy markers and association with atrial fibrillation detected after ischaemic stroke in the SAFAS study

Background Atrial cardiomyopathy(AC)is an emerging concept explaining the pathophysiology of cardioembolic strokes in absence of atrial fibrillation(AF).A definition based on the presence of electrical abnormality(P-wave terminal force in lead V1(PTFV1)>5000µV×ms),N-Terminal pro-B-type natriuretic peptide(NT pro BNP)>250pg/mL and/or indexed left atrial diameter(LADI)>3cm/m^(2)is currently tested in the ARCADIA(AtRial Cardiopathy and Antithrombotic Drugs In prevention After cryptogenic stroke)*** set out to estimate the prevalence of AC as defined in the ARCADIA trial,its determinants and its association with AF detected after stroke(AFDAS).Methods Stepwise screening for silent Atrial Fibrillation After Stroke(SAFAS)study prospectively included 240 ischaemic stroke *** markers were complete for 192 of them and 9 were not included in this analysis because AF had been diagnosed on *** A total of 183 patients were analysed,of whom 57%(104 patients)met the AC criteria(79 NT-proBNP,47 PTFV1,4 LADI).In the multivariate logistic regression,C reactive protein>3mg/L(OR(95%CI)2.60(1.30 to 5.21),p=0.007)and age(OR(95%CI)1.07(1.04 to 1.10),p34mL/m^(2),OR 2.35(CI 1.09 to 5.06)p=0029).Conclusion AC as defined in ARCADIA is mostly based on NT pro BNP elevation(76%of patients)and is associated with age and ***,AC was not independently associated with AFDAS at *** ARCADIA trial,which compares aspirin to apixaban in patients with embolic strokes of undetermined source with AC markers and must,therefore be analysed in the light of these limitations.

Determination of Total Heavy Metals Content in Local Antidote Manufacturing Resources

The present work was initiated for searching and evaluating the total heavy metal content in some selected local resources used by traditional healers in locally made antidotes to search for their toxicity. Elekat public slaughterhouse in the city of Bukavu, the central market of Kadutu and in the Ruzizi plain, precisely in Sange city, have been the purchased centers. The samples of bull and cow horns, oil palm nut shells, cow bones, ginger, black pepper and Mwenga salt were used. The spectrometric method has been used to carry out heavy metal content in each local resource using the atomic absorption spectrometer (VARIAN 1275). The heavy metals found and their average content are as follows: copper (Cu: 124.79 mg/kg), lead (Pb: 76.41 mg/kg), chromium (Cr: 33.9 mg/kg), zinc (Zn: 190.86 mg/kg), Arsenic (As: 107.41 mg/kg) and cadmium (Cd: 0.05 mg/kg). The averages of total heavy metal contents for all samples are below the standard of each one. The determination of the heavy metal contents in these samples allows evaluating human intoxication risk. These results show that antidote technology can be run without any toxicity risk and showed statistically significant differences compared to the controls (p 0.05) for chromium, zinc and cadmium have been recorded. However, most of the heavy metals molecules being thioloprives and then bioccumulatives, antidotes made from them shall not be consumed during a long period.

Chemical Analysis of Activated Carbon from Bull and Cow Horns Pyrolysis to Be Used as Antidotes

The purpose of this study is to compare the results of chemical analysis of two types of activated from the pyrolysis of bull horn and that of cow. Six samples were used to measure pH, carbon, calcium and to determine adsorbent power. The pH was measured at a temperature of 20˚C using an “ANION 7010 ionomer” pH meter, the carbon (C) content was analyzed using a “EURO EA 3000” analyzer. and the electronic balance: “Sartorius CP-2P”, calcium (Ca) was analyzed using a DFS-8 spectrograph. For the adsorbency test, the 0.15% methylene blue R solution was used. At the end of this study, we found that the activated carbon from the bull horn demonstrated a carbon content that is higher than that of the cow horn (20.79% against 15.63%), activated carbon of cow horn is richer in calcium than that of bull horn (16.27% against 3.69%) and then the pH. The cow horn is higher than that of the bull horn (7.43 versus 6.5). For the adsorbent power, the sample (75% bull horn and 25% cow horn) was recorded with the greatest adsorbent power. Thus, from this study, it can be recommended as an activated carbon antidote to be used for poisonings treatment.

Assessment of Dose and Lifetime Risk of Exposure Induced Cancer in Adult Common Computed Tomography Scans in Douala-Cameroon

Background: Among medical technologies that use ionizing radiation, CT is currently the radio diagnostic technic that can deliver the highest radiation to the Patient compared with other conventional procedures. In developing countries, the uses and risks of CT have not been well characterized. Objective: To estimate the lifetime attributable risk (LAR) incidence and mortality for cancer for each procedure for adult’s patients who had Computed Tomography examinations in 10 imaging centers in the city of Douala-Cameroon so as to provide a reference data. Materials and Methods: We conducted a cross-sectional study describing radiation dose associated with the 8 most common types of diagnostic CT studies performed on 1287 consecutive adult patients at 10 Douala radiology department. We estimated lifetime attributable risks of cancer by study type from these measured doses. Estimation of LAR for cancer incidence and mortality was based on the effective dose, patient’s sex and age at exposure using the BIER VII preferred models. Results: Mean effective dose from CT scans examinations varied from: 0.30 and 8.81 mSv. The highest doses were observed for lumbar spine CT (8.81 mSv), followed by abdomen-pelvis procedure (6.46 mSv), chest-abdomen-pelvic CT (6.61 mSv), chest CT (3.90 mSv), cervical Spine CT (3.05 mSv), head CT (1.7 mSv) and lower for sinus CT (0.30 mSv). The LAR values of all cancer from patients’ CT scans obtained vary from 67.13 excess per 100,000 (about 1 in 1489) and 0.45 excess per 100,000 (about 1 in 222,222). All cancer risk was high for lumbar spine CT in women 20 years old (67.13 excess deaths in 100,000 scans) followed by chest-abdomen-pelvic CT (50.36 excess deaths in 100,000 scans) and abdomen-pelvic CT (49.22 excess deaths in 100,000 scans) for the same age group. Oral cancer (OC) is the most common form of head and neck cancer. Despite the high incidence and unfavourable patient outcomes, currently, there are no biomarkers for the early detection of OC. This study aims to discover, develop, and validate a novel saliva-based microRNA signature for early diagnosis and prediction of OC risk in oral potentially malignant disorders (OPMD).The Cancer Genome Atlas (TCGA) miRNA sequencing data and small RNA sequencing data of saliva samples were used to discover differentially expressed miRNAs. Identified miRNAs were validated in saliva samples of OC (n=50), OPMD (n=52), and controls(n=60) using quantitative real-time PCR. Eight differentially expressed miRNAs (miR-7-5p, miR-10b-5p, miR-182-5p, miR-215-5p,miR-431-5p, miR-486-3p, miR-3614-5p, and miR-4707-3p) were identified in the discovery phase and were validated. The efficiency of our eight-miRNA signature to discriminate OC and controls was:area under curve (AUC):0.954, sensitivity:86%, specificity:90%,positive predictive value (PPV):87.8%and negative predictive value (NPV):88.5%whereas between OC and OPMD was:AUC:0.911,sensitivity:90%, specificity:82.7%, PPV:74.2%and NPV:89.6%. We have developed a risk probability score to predict the presence or risk of OC in OPMD patients. We established a salivary miRNA signature that can aid in diagnosing and predicting OC,revolutionising the management of patients with OPMD. Together, our results shed new light on the management of OC by salivary miRNAs to the clinical utility of using miRNAs derived from saliva samples.
