Assessment of Dose and Lifetime Risk of Exposure Induced Cancer in Adult Common Computed Tomography Scans in Douala-Cameroon

Background: Among medical technologies that use ionizing radiation, CT is currently the radio diagnostic technic that can deliver the highest radiation to the Patient compared with other conventional procedures. In developing countries, the uses and risks of CT have not been well characterized. Objective: To estimate the lifetime attributable risk (LAR) incidence and mortality for cancer for each procedure for adult’s patients who had Computed Tomography examinations in 10 imaging centers in the city of Douala-Cameroon so as to provide a reference data. Materials and Methods: We conducted a cross-sectional study describing radiation dose associated with the 8 most common types of diagnostic CT studies performed on 1287 consecutive adult patients at 10 Douala radiology department. We estimated lifetime attributable risks of cancer by study type from these measured doses. Estimation of LAR for cancer incidence and mortality was based on the effective dose, patient’s sex and age at exposure using the BIER VII preferred models. Results: Mean effective dose from CT scans examinations varied from: 0.30 and 8.81 mSv. The highest doses were observed for lumbar spine CT (8.81 mSv), followed by abdomen-pelvis procedure (6.46 mSv), chest-abdomen-pelvic CT (6.61 mSv), chest CT (3.90 mSv), cervical Spine CT (3.05 mSv), head CT (1.7 mSv) and lower for sinus CT (0.30 mSv). The LAR values of all cancer from patients’ CT scans obtained vary from 67.13 excess per 100,000 (about 1 in 1489) and 0.45 excess per 100,000 (about 1 in 222,222). All cancer risk was high for lumbar spine CT in women 20 years old (67.13 excess deaths in 100,000 scans) followed by chest-abdomen-pelvic CT (50.36 excess deaths in 100,000 scans) and abdomen-pelvic CT (49.22 excess deaths in 100,000 scans) for the same age group. Background: CT in pregnant patients requires careful consideration of the radiation dose and corresponding radiation risks from ionizing radiation to the unborn child. The determination of foetal dose in diagnostic radiology is of interest as a basis for risk estimates from medical exposure of the pregnant patient. Objective: To evaluate the foetal-maternal radiation doses delivered during the CT-Pelvimetry procedure and to estimate the risk to the unborn child to develop a cancer in childhood and hereditary disease. Materials and Methods: We investigate the foetal-maternal radiation doses during CT-scan Pelvimetry in Douala (Cameroon). Data of 194 helical acquisition CT-Pelvimetry were collected between May 2017 and May 2019. An average DLP for the examination was established and the average effective dose was evaluated. The fetal dose was calculated and the FetDose V5 program was used for risk estimations. Results: The average dose length product (DLP) was 56.17 mGy·cm (range: 51.69 - 59.21 mGy·cm). The average effective dose received by women pregnant was 0.78 mSv. The mean individual fetal dose was 1.5 mGy (range: 0.76 - 1.87 mGy). The risk of Childhood Cancer calculated was: range 1 in 16,000 to 1 in 10,000 and 1 in 260,000 to 1 in 106,000 to the risk of Hereditary Disease, respectively. Conclusion: This study shows that the foetal-maternal doses delivered during CT-Pelvimetry examinations are very low and the risks of childhood cancers and hereditary diseases are derisory, the technology should be further investigated to ensure its full potential for optimal diagnostic accuracy.

Multiple Scanning Effects in Radiochromic Film Dosimetry: A Method to Reduce the Increase of Optical Density

In this work, a method to reduce increase in optical density (OD) caused by multiple scanning in radiochromic film dosimetry in combination with a flatbed scanner is presented. Gafchromic EBT3 films are scanned with Epson Pro 1680 Expression scanner and time intervals of 15 minutes and 30 minutes are observed between consecutive scans to reduce the increase in temperature of the scanner. The maximum variations in OD after consecutive scans are calculated and compared to the values obtained for scans without interruption. For film irradiated to 3 Gy, a time interval of 15 minutes between two successive scans leads to a reduction of the OD increase of 56.2% compared to when films are scanned without interruption. Reductions of OD increase of 86.72% and 78.72% respectively for film irradiated to 1 Gy and 2 Gy are obtained when a time interval of 30 minutes is left between two successive scans. These results show that when time intervals are observed between consecutive scans, the increase in OD has significantly reduced. However, the method has the drawback of increasing the time needed to perform radiochromic film dosimetry.
