Comparative Study of Transvesical and Retropubic Prostatic Adenomectomies in the Urology-Andrology Department of Ignace Deen University Hospital Center

Objective: To compare the results of transvesical and retropubic prostatic adenomectomies at the Urology-Andrology Department of Conakry University Hospital. Methodology: This was a prospective multi-operator analytical study lasting 6 months. It included 70 patients operated on for benign prostatic hypertrophy (BPH), divided into 2 groups of 35 according to the retropubic or transvesical technique. The data was collected using a questionnaire and entered into the Epi info application. The analysis was initially descriptive and then univariate. To identify confounding factors, a multivariate analysis was performed. The application conditions of each test were checked before their application. A p-value Results: Upper adenomectomies represented 53.6% of surgeries performed for BPH. The average age was 73.2 years. The mean prostate volume was 117 ± 51.6 cc and the mean total PSA was 9.2 ± 11.4 ng/ml. The majority of patients had an ASA1 (60%) and ASA2 (37.1%) score. Spinal anesthesia, a double-current silicone probe with hinged hinge 22, and drainage of the prevesical space were carried out in all patients, i.e. 100%. The average overall blood loss was 229 ± 98.2 ml. It was lower in the group of patients operated via the retropubic route (187 ± 60.4 ml). The same was true for the duration of intervention (49.1 ± 7.17 vs 55.4 ± 7.9 min), the catheter wearing time (5.14 ± 0.97 vs 9.29 ± 2 .9 days) and the length of hospitalization (=6.26 ± 1.15 vs 12.1 ± 5.74 days). Conclusion: The retropubic technique has advantages over the transvesical one. These advantages are translated by a gain in the duration of the intervention, blood loss, catheter wearing time and length of hospitalization.

AIM: To retrospective review the laparoscopic management of Meckel Diverticulum(MD) in two Italian Pediatric Surgery ***: Between January 2002 and December 2012, 19 trans-umbilical laparoscopic-assisted(TULA) procedures were performed for suspected MD. The children were hospitalized for gastrointestinal bleeding and/or recurrent abdominal pain. Median age at diagnosis was 5.4 years(range 6 mo-15 years). The study included 15 boys and 4 girls. All patients underwent clinical examination, routine laboratory tests, abdominal ultrasound and technetium-99m pertechnetate scan, and patients with bleeding underwent gastrointestinal endoscopy. The abdominal exploration was performed with a 10 mm operative laparoscope. Pneumoperitoneum was establishedbased on the body weight. Systematic overview of the peritoneal cavity allowed the ileum to be grasped with an atraumatic instrument. The complete exploration and surgical treatment of MD were performed extracorporeally, after intestinal exteriorization through the umbilicus. All patients' demographics, main clinical features, diagnostic investigations, operative time, histopathology reports, conversion rate, hospital stay and complications were registered and ***: MD was identified in 17 patients, while 1 had an ileal duplication and 1 a jejunal hemangioma. Fifteen patients had painless intestinal bleeding, while 4 had recurrent abdominal pain and exhibited cyst like structures in an ultrasound study. Eleven patients had a positive technetium-99m pertechnetate scan. In the patients with bleeding, gastrointestinal endoscopy did not name the source of hemorrhage. All patients were subjected to a TULA surgical procedure. An intestinal resection/anastomosis was performed in 14 patients, while 4 had a wedge resection of the diverticulum and 1 underwent stapling diverticulectomy. Nozzle wall ablation caused by high temperature electric arcs is studied in the context of high voltage SF6 circuit breakers. The simplified ablation model used in litterature has been updated to take into account the unsteady state of ablation. Ablation rate and velocity are now calculated by a kinetic model using two layers of transition, between the bulk plasma and the ablating wall. The first layer （Knudsen layer）, right by the wall, is a kinetic layer of a few mean-free path of thickness. The second layer is collision dominated and makes the transition between the kinetic layer and the plasma bulk. With this new coupled algorithm, it is now possible to calculate the temperature distribution inside the wall, as well as more accurate ablation rates.

Cooperation among service providers, network providers, and access providers in the Internet allows the creation of new services to offer to customers that are in other domains, thus increasing revenue. However, the Internet heterogeneous environment, where each provider has its own policies, infrastructure and business goals, hinders the deployment of more advanced communication services. This paper presents a Quality of Service （QoS） for Inter-Domain Services （QIDS） model that allows inter-domain QoS-aware services to be defined, configured, and adapted in a dynamic and on-demand fashion, among service providers. This is accomplished by： 1） the use of a common communication channel （business layer） where service providers publish and search for services, and interact with each other to contract and manage these services; 2） the templates to specify the business and technical characteristics of the services; 3） the automatic composition of services using service dements （smaller services） a^cording to performance and service-specific QoS parameters; and 4） the creation and enforcement of configuration rules for the underlying infrastructure. A prototype was implemented to validate QIDS and performance tests were conducted on an inter-domain Border Gateway Protocol （BGP）/Multiprotocol Label Switching （MPLS） Virtual Private Network （VPN） scenario.

Electrical characteristics of Co/n-Si Schottky barrier diodes are analysed by current- voltage （I- V） and capacitancevoltage （C- V） techniques at room temperature. The electronic parameters such as ideality factor, barrier height and average series resistance are determined. The barrier height 0. 76 eV obtained from the C - V measurements is higher than that of the value 0. 70 eV obtained from the I - V measurements. The series resistance Rs and the ideality factor n are determined from the d ln（ I） /dV plot and are found to be 193.62 Ω and 1.34, respectively. The barrier height and the Rs value are calculated from the H（I） - I plot and are found to be 0.71 eV and 205.95Ω. Furthermore, the energy distribution of the interface state density is determined from the forward bias I - V characteristics by taking into account the bias dependence of the effective barrier height. The interface state density Nss ranges from 6.484×10^11 cm^-2eV^-1 in （Ec - 0.446） eV to 2.801×10^10 cm^-2eV^-1 in （Ec - 0.631） eV, of the Co/n-Si Schottky barrier diode. The results show the presence of a thin interracial layer between the metal and the semiconductor.

We devise hybrid high-order(HHO)methods for the acoustic wave equation in the time *** frst consider the second-order formulation in *** the Newmark scheme for the temporal discretization,we show that the resulting HHO-Newmark scheme is energy-conservative,and this scheme is also amenable to static condensation at each time *** then consider the formulation of the acoustic wave equation as a frst-order system together with singly-diagonally implicit and explicit Runge-Kutta(SDIRK and ERK)***-SDIRK schemes are amenable to static condensation at each time *** HHO-ERK schemes,the use of the mixed-order formulation,where the polynomial degree of the cell unknowns is one order higher than that of the face unknowns,is key to beneft from the explicit structure of the *** results on test cases with analytical solutions show that the methods can deliver optimal convergence rates for smooth solutions of order O(hk+1)in the H1-norm and of order O(h^(k+2))in the L^(2)-***,test cases on wave propagation in heterogeneous media indicate the benefts of using high-order methods.

The geography of introgression in a patchy environment and the thorn in the side of ecological speciation

When incompletely isolated taxa coexist in a patchy environment （e.g. mosaic hybrid zones, host-race complexes）, patterns of variation may differ between selected traits/genes and neutral markers. While the genetic structure of selected traits/loci tends to coincide with habitat variables （producing Genetic-Environment Association or GEA）, genetic differentiation at neutral loci unlinked to any selected locus rather depends on geographic connectivity at a large scale （e.g. Isolation- By-Distance or IBD）, although these loci often display GEA at a small scale. This discrepancy has been repeatedly taken as evi- dence for parallel primary divergence driven by local adaptation. We argue that this interpretation needs to be addressed more thoroughly by considering the alternative hypothesis that speciation was initiated in allopatry and secondary introgression has subsequently erased the signal of past differentiation at neutral loci. We present a model of neutral introgression after secondary contact in a mosaic hybrid zone, which describes how GEAs dissipate with time and how neutral variation self-organizes accord- ing to the environmental and geographic structures. We show that although neutral loci can be affected by environmental selection they are often more affected by history and connectivity： the neutral structure retains the initial geographic separation more than it correlates with the environment during the colonization and introgression phases, and then converges to a migration-drift balance, the most frequent outcome of which is GEA at a local scale but IBD at a large scale. This is the exact pattern usually attributed to parallel ecological speciation. Introgression is heterogeneous in space and depends on the landscape structure （e.g. it is faster in small patches, which are more impacted by immigration）. Hardy spaces with generalized parameter are introduced following the maximal characterization approach. As particular cases, they include the classical Hp spaces and the Hardy-Lorentz spaces H^p,q. Real interpolation results with function parameter are obtained, Based on them, the behavior of some classical operators is studied in this generalized setting.
