Risk factors, surgical complications and graft survival in liver transplant recipients with early allograft dysfunction

Background: Early allograft dysfunction (EAD) is a severe complication after liver transplantation. The associated risk factors and complications have re-gained recent interest. This study investigated risk factors, survival and complications associated with EAD in a large liver transplant center in Latin America. Methods: Retrospective, unicenter, cohort, based on data from adult patients undergoing first deceaseddonor liver transplant from January 2009 to December 2013. EAD was defined by one or more of the following:(i) bilirubin ≥10 mg/dL on postoperative day 7;(ii) international normalized ratio ≥1.6 on postoperative day 7, and (iii) alanine aminotransferase or aspartate aminotransferase > 2000 IU/L within the first seven days after transplant. Results: A total of 602 patients were included;of these 34.2% developed EAD. Donor risk factors were male ( P = 0.007), age between 50 and 59 years ( P = 0.034), overweight ( P = 0.028) or grade I obesity ( P = 0.012), sodium > 157 mmol/L ( P = 0.002) and grade IV ischemia/reperfusion injury ( P = 0.002). Cold ischemia time ≥10 h ( P = 0.008) and warm ischemia time ≥40 min ( P = 0.013) were the surgical factors. Male ( P rotective factor. Compared with the non-EAD group, patients with EAD were submitted to more reoperations (24.3% vs. 13.4%, P = 0.001) and had higher graft loss rates (37.9% vs. 21.2%, P rocedure and recipient. Donor risk factors for EAD were male, age between 50 and 59 years, donor overweight or grade Ⅰ obesity, sodium > 157 mmol/L and grade Ⅳ ischemia/reperfusion injury. Cold ischemia time ≥10 h and warm ischemia time ≥40 min were the surgical risk factors. Male was the only recipient protective factor. Patients with EAD had higher reoperations and graft loss rates.

目的了解医学院校附属医院教学质量保障体系建设成效,发挥医院教学主体作用,保证医学人才培养质量。方法围绕人才培养方案与教学计划、教学运行、教学质量保障体系等影响因素展开问卷调查,采用卡方检验和方差分析分别对计数和计量资料进行分析。结果受教育程度不同的教师,对人才培养方案了解情况及教学大纲制定参与度有统计学差异(P=0.014;P=0.025),对教师准入制度、教学资源支持程度、师资质量满意度有统计学差异性(P=0.019;P=0.033;P=0.027);职称不同的教师,对教学大纲了解程度存在统计学差异(P=0.039);工作年限不同的教师,对个人所承担的教学工作量态度有统计学差异(P=0.047)。结论教学质量保障体系待进一步完善,建议保障教学经费投入和分配,提高教师教学工作参与度,加强师资队伍建设,优化教学资源整合,完善教学管理制度。

Metabolic syndrome(MetS)might be defined as the simultaneous accumulation of s everal functional changes that frequently occur in adults over 60 years of age(Gomez et al.,2018).The diagnosis of MetS requires the presence of three or more factors such as high body mass,type-2 diabetes mellitus(T2DM),dyslipidemia,and arterial hypertension,which increase the risk of cardiovascular diseases as well as neurological complications,as stroke and dementia(Dyken and Lacoste,2018).Usually,these functional changes coincide and result in insensitivity for example hormones as leptin,adiponectin,and insulin(Dyken and Lacoste,2018).

Patterns of species richness and beta diversity of vascular plants along elevation gradient in Brazilian páramo

Understanding the relationships between plant diversity and the environment is an important step towards conservation of species and *** this study,we hypothesize that community species richness decreases and species composition change along the elevation gradient,with a greater contribution of species turnover than nestedness to beta *** surveyed plant species in 300 plots(1 m×1 m)in four elevational bands(2100 m,2300m,2500 m,and 2700 m)in the CaparaóNational Park,Southeastern *** samples presented a great number of species(97 spp.,59 genera,27families),however,74%of the species were considered rare with their relative abundance beingrom plot to elevational-band).The dissimilarity(βSOR=0.907)in the area was due almost exclusively to spatial species turnover(βSIM=0.879),with very low contribution of species nestedness(βSNE=0.028).Our research showed that species richness does not decrease with elevation in the study area,but the floristic composition *** clear distinction of species along the gradient show that spatial species turnover is the major diversitygenerating process in the studied area.

Many plants pollinated by nectar-foraging animals have to maintain a balance between legitimate visitor attraction strategies and mechanisms that minimize illegitimate *** study investigated how floral display and neighboring species composition influences nectar robbing by hummingbirds in the tropical ornithophilous herb Heliconia *** tested the role of inflorescence display,flower abundance,and neighboring species in the reduction of nectar robbing in *** results indicate that nectar robbing hummingbird activity was higher in moderately large inflorescence displays and that the frequency of nectar robbing in *** decreases with increased flower abundance and the presence of neighboring plant *** non-ornithophilous plants decreased the frequency of nectar robbing in *** flowers to a greater extent than ornithophilous *** results suggest that nectar robbing hummingbirds are attracted to similar conditions that attract legitimate visitors and that spatial aggregation and mixed-species displays may represent a mechanism to dilute nectar robbing effects at an individual level.

This paper proposes the multiple-input multiple-output(MIMO)detection scheme by using the deep neural network(DNN)based ensemble machine learning for higher error performance in wireless communication *** the MIMO detection based on the ensemble machine learning,all learning models for the DNN are generated in offline and the detection is performed in online by using already learned *** the offline learning,the received signals and channel coefficients are set to input data,and the labels which correspond to transmit symbols are set to output *** the online learning,the perfectly learned models are used for signal detection where the models have fixed bias and *** performance improvement,the proposed scheme uses the majority vote and the maximum probability as the methods of the model combinations for obtaining diversity gains at the MIMO *** simulation results show that the proposed scheme has improved symbol error rate(SER)performance without additional receive antennas.

Weighted Gauss-Seidel Precoder for Downlink Massive MIMO Systems

In this paper,a novel precoding scheme based on the Gauss-Seidel(GS)method is proposed for downlink massive multiple-input multiple-output(MIMO)*** GS method iteratively approximates the matrix inversion and reduces the overall complexity of the precoding *** addition,the GS method shows a fast convergence rate to the Zero-forcing(ZF)method that requires an exact invertible ***,to satisfy demanded error performance and converge to the error performance of the ZF method in the practical condition such as spatially correlated channels,more iterations are necessary for the GS method and increase the overall *** efficient approximation with fewer iterations,this paper proposes a weighted GS(WGS)method to improve the approximation accuracy of the GS *** optimal weights that accelerate the convergence rate by improved accuracy are computed by the least square(LS)*** the computation of weights,the different weights are applied for each iteration of the GS *** addition,an efficient method of weight computation is proposed to reduce the complexity of the LS *** simulation results show that bit error rate(BER)performance for the proposed scheme with fewer iterations is better than the GS method in spatially correlated channels.

Computed tomography and magnetic resonance imaging findings of metastatic rectal linitis plastica from prostate cancer:A case report and review of literature

Linitis plastica is a rare condition showing circumferentially infiltrating intramural anaplastic carcinoma in a hollow viscus, resulting in a tissue thickening of the involved organ as constricted, inelastic, and rigid. While most secondary rectal linitis plastica(RLP) is caused by metastasis from stomach, breast, gallbladder, or bladder cancer, we report an extremely rare and unique case of secondary RLP due to prostate cancer with computed tomography(CT) and magnetic resonance imaging(MRI) findings, including diffusion weighted imaging(DWI). A 78-year-old man presented with approximately a 2-mo history of constipation and without cancer history. On sigmoidoscopy, there was a luminal narrowing and thickening of rectum with mucosa being grossly normal in its appearance. On contrast-enhanced CT,marked contrast enhancement with wall thickening of rectum was noted. On pelvic MRI, rectal wall thickening showed a target sign on both T2-weighted imaging and DWI. A diffuse infiltrative lesion was suspected in the prostate gland based on low signal intensity on T2-weighted imaging and restricted diffusion. A transanal full-thickness excisional biopsy revealed metastasis from a prostate adenocarcinoma invading the submucosa to the muscularis propria consistent with metastatic RLP. We would like to emphasize the CT and MRI findings of metastatic RLP due to prostate cancer.
