the interplay between free electrons,light,and matter offers unique prospects for space,time,and energy resolved optical material characterization,structured light generation,and quantum information ***,we study the nanoscale features of spontaneous and stimulated electron–photon interactions mediated by localized surface plasmon resonances at the tips of a gold nanostar using electron energy-loss spectroscopy(EELS),cathodoluminescence spectroscopy(CL),and photon-induced near-field electron microscopy(PINEM).Supported by numerical electromagnetic boundary-element method(BEM)calculations,we show that the different coupling mechanisms probed by EELS,CL,and PINEM feature the same spatial dependence on the electric field distribution of the tip ***,the electron–photon interaction strength is found to vary with the incident electron velocity,as determined by the spatial Fourier transform of the electric near-field component parallel to the electron *** the tightly confined plasmonic tip resonances,our calculations suggest an optimum coupling velocity at electron energies as low as a few *** results are discussed in the context of more complex geometries supporting multiple modes with spatial and spectral *** provide fundamental insights into spontaneous and stimulated electron-light-matter interactions with key implications for research on(quantum)coherent optical phenomena at the nanoscale.

Neurological Disorders among the Outpatients Living with Human Immuno Deficiency Virus (HIV) Following at the Parakou Hospital in 2014

Objective: To study the frequency of neurological disorders associated to HIV infection. Methods: this cross-sectional study was carried out from 16th June to 16th September 2014 and included all 289 HIV-outpatients admitted to the Parakou University hospital. All data were recorded, physical examination was performed and the medical sheet of each patient was assessed. the diagnosis of neurological disorder (central and peripheral) was based on clinical features, past medical history, biological test and CT-scan data after multidisciplinary staff. Epi-data and Epi-Info 6.04C were used to perform analysis. Results: they were 73 (25.3%) male with a mean age of 38.0 years +/−?10.1 years. the mean duration of disease was 48.5 months +/−37.1 months. 150 (51.9%) of them were at Stage 3 of the disease. All patients (99.7%) were infected by HIV1, 188 (76.1%) had CD4 cells at the onset of the follow-up less than 350 cells/mm3 and 260 (90.0%) were on ART. Among the 289 patients, 110 had at least one neurological disorder, the frequency of neurological disorders was 38.1%. the main neurological disorders were peripheral neuropathy (34.3%), isolated headache (5.2%), stroke (0.7%), cerebral toxoplasmosis (0.7%) and one case of dementia (0.7%). the main associated factors were the age of patient and exposure on neurotoxic drug, especially ART. Conclusion: the neurological disorders were more frequent among HIV patients with a high frequency of peripheral neuropathy probably due to ART.

目的白介素5(IL-5)作为参与嗜酸粒细胞(EOS)成熟、分化过程的最主要因子,其作用通过特异性受体白介素5受体α链(IL-5Rα)介导。本研究以支气管上皮细胞株为研究对象,发现支气管上皮细胞在炎症因子的刺激下可以表达IL-5Rα。方法通过促炎症因子肿瘤坏死因子α(TNF-α)和th2型细胞因子IL-4,IL-5,IL-13以及白三烯成分之一的白三烯C4(LTC4)对细胞进行单独或协同刺激,以逆转录聚合酶链反应(RT-PCR)方法、流式细胞检测技术评价上皮细胞对IL-5Rα的基因和蛋白质的表达,同时评价在IL-5和TNF-α的协同刺激下IL-5Rα、细胞内黏附分子1(ICAM-1)和血管细胞黏附分子1(VCAM-1)的协同表达。结果正常支气管上皮细胞在th2型细胞因子、LTC4和TNF-α的刺激下可以表达IL-5Rα基因,在TNF-α单独或与其他因子的协同刺激下表达显著增强,更可以表达IL-5Rα蛋白。流式细胞检测分析显示TNF-α单独或协同刺激后16hIL-5Rα蛋白表达最高,并随时间的延长表达下降。当TNF-α与IL-5协同刺激后上皮细胞可以同时表达VCAM-1和IL-5Rα,而ICAM-1呈持续表达。结论本研究通过对支气管上皮细胞的研究证实了支气管上皮细胞可以在炎症因子TNF-α和th2型细胞因子的协同刺激下进一步表达IL-5特异性受体IL-5Rα,并在IL-5和TNF-α的刺激下进一步表达黏附分子VCAM-1,吸引EOS及其前体细胞浸润至气道参与炎症。说明支气管上皮细胞不仅在支气管哮喘炎症反应中是最主要的受累器官,也是导致炎症的参与者之一。

High Supracostal Percutaneous Nephrolithotomy Access: Assessing Safety in Access above the Eleventh Rib after Performing Preoperative Planning with Computed Tomography

Objective: To determine if supracostal renal access above the 11th rib during percutaneous nephrolithotomy (PCNL) is a safe option in carefully selected patients determined by preoperative computed tomography (CT) imaging. Patients and Methods: We retrospectively isolated 142 patients who underwent access above the eleventh rib during PCNL, which we term “high supracostal renal access.” We then compared these patients to 113 individuals who underwent access below the twelfth rib. Renal access was achieved by the operative surgeon with fluoroscopic guidance in conjunction with pre-operative computed tomography (CT) scan. Outcomes were compared. Results: Overall surgical outcomes were equivalent when comparing high supracostal versus subcostal access sites. As expected due to proximity, pleural complications occurred in 4% of the high supracostal group (n = 6) compared with 0% of the control (subcostal) group (p = 0.035). Of these six complications, three were managed conservatively with observation and two required cardio-thoracic intervention with video-assisted thoracoscopic pleural repair (1%). In the remaining case, the patient was preoperatively consented for placement of a thoracostomy tube, which was placed during the procedure, due to the difficult location of her upper pole stone and closely adjacent low-lying pleura, and the planned transpleural approach. Hospital stay was not significantly pro-longed between the high supracostal access and subcostal access groups, with an average length of stay of 2.2 ± 2.1 days and 2.0 ± 1.9 days (p = 0.59) respectively. Conclusions: Careful, systematic preoperative planning based on CT and fluoroscopic imaging allows for a confident understanding of a “safety zone” in placement and dilatation of renal access points during PCNL. We have shown that planned upper pole renal access above the 11th rib is achievable with acceptable morbidity and excellent success rates.

Bulk Schottky silicide source/drain n- and p-MOS transistors(SSDTs) with EOT=2.0～2.5nm HfO gate dielectric and HfN/TaN metal gate have been successfully demonstrated using a low temperature process.P-SSDTs with PtSi silicide show excellent electrical performance of I/I～10-10 and subthreshold slop of 66 mV/dec.N-SSDTs with YbSi silicide have also demonstrated a very promising characteristic with a recorded high I/I radio of～10 and subthreshold slope of 75mV/*** the best of our knowledge,these are the best SSDTs data reported so *** implant free low temperature process relaxes the thermal budget of high-K dielectric and metal gate materials. Our results are expected to be further improved when using ultra-thin-body(UTB) SOI structures,showing great potential of this low temperature process SSDTs for future sub-tenth micron CMOS technology.

Context: Exercise is associated with a lower risk of cardiovascular events but may transiently increase the risk of ventricular arrhythmias. Its short-term and long-term associations with risk of sudden cardiac death among women are unclear. Objectives: To compare the risk of sudden cardiac death in women during mo derate to vigorous exertion with the risk of sudden cardiac death during lighter or no exertion; and to assess the long-term association between moderate to vi gorous exercise and sudden cardiac death. Design, Setting, and Participants: Pro spective, nested case-crossover study of 288 cases of sudden cardiac death with in the Nurses’Health Study(1980-2004); and a prospective cohort analysis of 69 693 participants without prior cardiovascular disease followed up from 1986-20 04. Main Outcome Measure: Risk of sudden cardiac death associated with moderate to vigorous exertion. Results: the absolute risk of sudden cardiac death associa ted with moderate to vigorous exertion was exceedingly low at 1 per 36.5 million hours of exertion. In case-crossover analyses, the risk of sudden cardiac death was transiently elevated during moderate to vigorous exertion(relative risk[RR ], 2.38; 95%confidence interval[CI], 1.23-4.60; P=.01) compared with the risk during lesser or no exertion. Habitual moderate to vigorous exertion modified th is transient risk(P=.005 for interaction) and the risk was no longer significant ly elevated among those who exercised 2 or more hours per week. In the cohort an alyses, an increasing amount of moderate to vigorous exercise was associated with a lower long-term risk of sudden cardiac death in ageadjusted and multivariable models that excluded biological intermediates(P=.006 for trend). this relatio nship was attenuated when biological intermediates were included(P=.06 for trend ); however, the reduction in risk remained significant among women who exercised 4 or more hours per week(adjusted RR, 0.41; 95%CI, 0.20-0.83; P=.01) compared wi

Objective: To examine whether the presence of motor signs has predictive value for important outcomes in Alzheimer disease (AD). Methods: A total of 533 patients with AD at early stages (mean Folstein Mini-Mental State Examination [MMSE] 21/30 at entry) were recruited and followed semiannually for up to 13.1 years ( mean 3) in five University-based AD centers in the United States and European U nion. Four outcomes, assessed every 6 months, were used in Cox models: cognitive endpoint (Columbia Mini-Mental State Examination ≤20/57 [～MMSE ≤10/30]), functional endpoint (Blessed Dementia Rating Scale ≥10), institutionalization equ ivalent index, and death. Using a standardized portion of the Unified PD Rating Scale (administered every 6 months for a total of 3,149 visit-assessments, aver age 5.9 per patient), the presence of motor signs, as well as of individual moto r sign domains, was examined as time-dependent predictor. the models controlled for cohort, recruitment center, sex, age, education, a comorbidity index, and b aseline cognitive and functional performance. Results: A total of 39%of the pat ients reached the cognitive, 41%the functional, 54%the institutionalization, a nd 47%the mortality endpoint. Motor signs were noted for 14%of patients at bas eline and for 45%at any evaluation. their presence was associated with increase d risk for cognitive decline (RR, 1.72; 95%CI, 1.24 to 2.38), functional declin e (1.80 [1.33 to 2.45]), institutionalization (1.68 [1.26 to 2.25]), and dea th (1.38 [1.05 to 1.82]). Tremor was associated with increased risk for reaching the cognitive and bradykinesia for reaching the functional endpoints. Posturalgait abnormalities carried increased risk for institutionalization and mortality. Fas ter rates of motor sign accumulation were associated with increased risk for all outcomes. Background: Delusions and hallucinations are common in Alzheimer disease (AD) and there are conflicting reports regarding their ability to predict cognitive decline, functional decline, and institutionalization. According to all previous literature, they are not associated with mortality. Objective: To examine whethe r the presence of delusions or hallucinations has predictive value for important outcomes in AD. Design, Setting, and Participants: A total of 456 patients with AD at early stages (mean Folstein Mini-Mental State Examination [MMSE] score of 21 of 30 at entry) were recruited and followed up semiannually for up to 14 years (mean, 4.5 years) in 5 university-based AD centers in the United States and Europe. Using the Columbia University Scale for Psychopathology in AD (administered every 6 months, for a total of 3266 visit-assessments, average of 7.2 per patient), the presence of delusions and hallucinations was extracted and examined as time-dependent predictors in Cox models. the models controlled for cohort effect, recruitment center, informant status, sex, age, education, a comorbidity index, baseline cognitive and baseline functional performance, behavioral sympt oms, and use of neuroleptics and cholinesterase inhibitors. Main Outcome Measure s: Cognitive (Columbia MMSE score of ≤20/57 [approximate Folstein MMSE score o f ≤10/30]), functional (Blessed Dementia Rating Scale [parts I and II] score of ≥10), institutionalization equivalent index, and death. Results: During the full course of follow-up, 38%of patients reached the cognitive, 41%the functiona l, 54%the institutionalization, and 49%the mortality end point. Delusions were noted for 34%of patients at baseline and 70%at any evaluation. their presence was associated with increased risk for cognitive (risk ratio [RR], 1.50; 95%confidence interval [CI], 1.07-2.08) and functional decline (RR, 1.41; 95%CI, 1.02-1.94).
