Incident frailty and cognitive impairment by heart failure status in older patients with atrial fibrillation: the sAGE-AF study

Background Atrial fibrillation(AF) and heart failure(HF) frequently co-occur in older individuals. Among patients with AF, HF increases risks for stroke and death, but the associations between HF and incident cognition and physical impairment remain unknown. We aimed to examine the cross-sectional and prospective associations between HF, cognition, and frailty among older patients with AF. Methods The sAGE-AF(systematic Assessment of Geriatric Elements in AF) study enrolled 1244 patients with AF(mean age 76 years, 48% women) from five practices in Massachusetts and Georgia. HF at baseline was identified from electronic health records using ICD-9/10 codes. At baseline and 1-year, frailty was assessed by Cardiovascular Health survey score and cognition was assessed by the Montreal Cognitive Assessment. Results Patients with prevalent HF(n = 463, 37.2%) were older, less likely to be non-Hispanic white, had less education, and had greater cardiovascular comorbidity burden and higher CHA2 Ds2 VAsC and HAs-BLED scores than patients without HF(all P's sted regression models, HF(present vs. absent) was associated with both prevalent frailty(adjusted odds ratio [a OR]: 2.38, 95% confidence interval [CI]: 1.64-3.46) and incident frailty at 1 year(a OR: 2.48, 95% CI: 1.37-4.51). HF was also independently associated with baseline cognitive impairment(a OR: 1.60, 95% CI: 1.22-2.11), but not with developing cognitive impairment at 1 year(a OR 1.04, 95%CI: 0.64-1.70). Conclusions Among ambulatory older patients with AF, the co-existence of HF identifies individuals with physical and cognitive impairments who are at higher short-term risk for becoming frail. Preventive strategies to this vulnerable subgroup merit consideration.

The goal of this paper was to present knowledge on changes in the morphodynamic structure of the Ochotnica River(Polish Carpathians)and transformations within its active river zone since the end of the 19th *** study used a set of archival and contemporary cartographic materials and the results of three-fold field mapping of the morphodynamic structure of the *** and indirect human interference with the natural environment of the catchment and the fluvial system was taken into account in the *** of changes in the morphodynamic structure of the Ochotnica and its active river zone since the end of the 19th century indicated significant changes in the development trend of the *** contemporary structure of the Ochotnica is more complex than in the *** has been a fragmentation of the riverbed into sections with different morphodynamic *** proportion of erosive sections has increased,which explains the reduced in the width of the active river zone of the *** average width of the active river zone of the river between 1861 and 2022 has changed from 80 m to 18 *** impact has been identified as the main reason for the changes in the morphodynamic structure of the riverbed and its active river ***,only the unregulated sect ion is characterised by free lateral migration of the riverbed and the widest active river zone(~28 m).

The beginnings of laparoscopic liver resection(LLR)were at the start of the 1990s,with the initial reports being published in 1991 and *** were followed by reports of left lateral sectionectomy in *** the years following,the procedures of LLR were expanded to hemi-hepatectomy,sectionectomy,segmentectomy and partial resection of posterosuperior segments,as well as the parenchymal preserving limited anatomical resection and modified anatomical(extended and/or combining limited)resection *** expanded range of LLR procedures,mimicking the expansion of open liver resection in the past,was related to advances in both technology(instrumentation)and technical skill with conceptual *** this period of remarkable development,two international consensus conferences were held(2008 in Louisville,KY,United states,and 2014 in Morioka,Japan),providing up-to-date summarizations of the status and perspective of *** advantages of LLR have become clear,and include reduced intraoperative bleeding,shorter hospital stay,and-especially for cirrhotic patients-lower incidence of complications(e.g.,postoperative ascites and liver failure).In this paper,we review and discuss the developments of LLR in operative procedures(extent and style of liver resections)during the first quarter century since its inception,from the aspect of relationships with technological/technical developments with conceptual changes.

Development and Applications of Ruggedized VIs/NIR spectrometer system for Oilfield Wellbores

The development and applications of a ruggedized visible to near-infrared （VIs/NIR） spectrometer system capable of measuring fluid spectra in oilfield wellbores are presented. Real-time assessment of formation fluid properties penetrated by an oilfield wellbore is critically important for oilfield operating companies to make informed decisions to optimize the development plan of the well and hydrocarbon reservoir. A ruggedized VIs/NIR spectrometer was designed and built to measure and analyze hydrocarbon spectra reliably under the harsh conditions of the oilfield wellbore environment, including temperature up to 175 ~C, pressure up to 170MPa, and severe mechanical shocks and vibrations. The accuracy of hydrocarbon group composition analysis was compared well with gas chromatography results in the laboratory.

Optical and scintillation properties of Nd-doped srAl2O4 crystals

Nd-doped srA1204 （Nd：srA1204） crystals were prepared by a floating zone （FZ） method with different dopant concentra- tions. The photoluminescence （PL） measurements under visible light excitations confirmed strong near infrared （NIR） emissions around 890 nm, which is due to the 4f-4f transitions of Nd3＋, and the decay time constants were 300-400 μs. Furthermore, VUV ex- citations revealed two broad emissions around 250450 and 500-700 run, which were attributed to self-trapped excitons （sTEs） perturbed by Nd3＋. Moreover, the X-ray induced scintillation spectra showed some small peaks at around 380 nm in addition to NIR emissions at 1064 nm similarly seen in PL. The former scintillation decay time constants were 30-40 Its. In the undoped sample, these emissions mentioned above were not present but a weak broad emission around 450 nm appeared. In thermally stimulated lu- minescence （TsL） after X-ray irradiation, strong TsL glow peaks were observed in the Nd-doped samples at around 100 and 250 ℃.

Late Neoproterozoic adakitic magmatism within the Eastern Arabian Nubian shield has been dated at633.2±9.0 Ma(2σ).These magnas intrude the forearc Ad Dawadimi Basin,which is composed of metapelitic schists and greywacke along with ophiolitic melanges of boninitic affinity which underwent inversion and deformation by^620 *** adakitic magmatism,while intruding parts of the Ad Dawadimi Basin,predates this deformation,but is possibly coincident with basin *** adakitic magmatism requires melting of an amphibolite or eclogitic source,empirical and experimental constraints require anomalously hot supra-subduction zone *** that this magmatism immediately predates basin inversion,these magmas possibly pinpoint the timing of the slab breaking,marking the terminal stages of arc magmatism,terrane accretion and the influx of hot oceanic asthenospheric *** influx of hot asthenospheric mantle may also be responsible for postcolltsional A-type magmatism.

Development and course of heart failure after a myocardial infarction in younger and older people

Background Acute myocardial infarction （AMI） is a common cause of heart failure （HF）, which can develop soon after AMI and may persist or resolve or develop late. HF after an MI is a major source of mortality. The cumulative incidence, prevalence and resolution of HF after MI in different age groups are poorly described. This study describes the natural history of HF after AMI according to age. Methods Patients with AMI during 1998 were identified from hospital records. HF was defined as treatment of symptoms and signs of HF with loop diuretics and was considered to have resolved if loop diuretic therapy could be stopped without recurrence of symptoms. Patients were cate- gorised into those aged 〈 65 years, 65-75 years, and 〉 75 years. Results Of 896 patients, 311,297 and 288 were aged 〈 65, 65-75 and 〉75 years and of whom 24%, 57% and 82% had died respectively by December 2005. Of these deaths, 24 （8%）, 68 （23%） and 107 （37%） oc- curred during the index admission, many associated with acute HF. A further 37 （12%）, 63 （21%） and 82 （29%） developed HF that persisted until discharge, of whom 15, 44 and 62 subsequently died. After discharge, 53 （24%）, 55 （40%） and 37 （47%） patients developed I-IF for the first time, of whom 26%, 62% and 76% subsequently died. Death was preceded by the development of HF in 35 （70%）, 93 （91%） and 107 （85%） in aged 〈 65 years, 65-75 years and 〉75 years, respectively. Conclusions The risk of developing HF and of dying after an MI in- creases progressively with age. Regardless of age, most deaths after a MI are preceded by the development of HF.

Classification of solar sail attitude dynamics with and without angular momentum

This paper describes attitude dynamics properties of spinning,momentum-biased and zero-momentum solar sail *** model called“Generalized sail Dynamics Model”(GsDM)is introduced,which can deal with general and practical sail configurations,such as arbitrary optical property distribution,shape and surface *** stability criteria and other key dynamical characteristics are derived and compared by compact analytical equations induced from the *** newly derived zero-momentum sail dynamics is compared with that of spinning and momentum-biased *** is shown that the spinning and momentum sails have an advantage in terms of dynamical stability whereas zero-momentum sails are only statically *** this special property,angular momentum-stabilized sails can realize a sun-pointing stable attitude with almost zero-fuel,which are discussed with actual space flight experience of the JAXA’s two interplanetary missions,IKAROs and Hayabusa2.
