Soleus arthrogenic muscle inhibition following acute lateral ankle sprain correlates with symptoms and ankle disability but not with postural control

Background:Acute lateral ankle sprains(ALAS)are associated with long-term impairments and instability tied to altered neural *** muscle inhibition(AMI)has been observed in this population;however,relationships with injury-related impairments are unclear,potentially due to the resting,prone position in which AMI is typically *** AMI during bipedal stance may provide a better understanding of this ***:AMI was assessed in 38 young adults(19 ALAS within 72 h of injury:10 males,21.4±2.7 years;19 healthy controls:10 males,21.9±2.2 years;mean±SD)using the Hoffmann reflex(H-reflex)during bipedal *** stimulation was administered to identify the _(max)imal H-reflex(H_(max))and _(max)imal motor response(M_(max))from the soleus,fibularis longus,and tibialis anterior *** primary outcome measure was the H_(max)/M_(max) *** outcomes included acute symptoms(pain and swelling),postural control during bipedal stance,and self-reported ***:No significant group-by-limb interactions were observed for any ***,a significant group main effect was observed in the soleus muscle(F(1,35)=6.82,p=0.013),indicating significantly lower H_(max)/M_(max) ratios following ALAS(0.38±0.20)compared to healthy controls(0.53±0.16).Furthermore,lower H_(max)/M_(max) ratios in the soleus significantly correlated with acute symptoms and self-reported function but not with postural ***:This study supports previous evidence of AMI in patients with ALAS,providing insight into neurophysiologic impacts of musculoskeletal *** results suggest that assessing AMI in a standing position following acute injury may provide valuable insight into how AMI develops and guide potential therapeutic options to curb and offset the formation of joint instability.

Effect of Bacrobial@ Skin Cleanser on the Prevention of Muscle Cramps

Anecdotal evidence supports the use of Bacrobial skin cleanser for preventing muscle cramps; however, empirical evidence is lacking. OBJECTIVE： To examine whether Bacrobial prevents muscle cramping in the triceps surae. DESIGN： randomized, double-blind, placebo-controlled, crossover study. SETTINGS/LOCATION： Laboratory. SUBJECTS： Seventeen cramp-prone participants （11 male, 6 female; age 21 -4- 2 yrs; 79.72 - 19.75 kg; 175.86 - 11.76 cm） volunteered. INTErVENTION： Participants completed two trials, at least one week apart. On the ftrst trial, Bacrobial was liberally applied to the skin over the triceps surae musculature of one leg, while placebo was applied to the other leg. At 1, 5, 10, and 20 min after application, participants were instructed to induce a cramp in the triceps surae via maximal voluntary contraction. For the second trial, treatments were applied to the opposite leg as in trial one and the cramping procedure was repeated. OUTCOME MEASUrES： Incidence of cramping was the main outcome measure and event-odds tables, risk reduction and NNT （numbers needed to treat） or NNH （numbers needed to harm） were calculated at the time intervals of 1, 5, 10 and 20 minutes. rESULTS： No significant differences between treatments were found in cramping incidences at all time intervals. Bacrobial decreased the risk of cramping by 12.5% at one minute （NNT = 8）, while causing a 5.3% increased risk at five minutes （NNH = 5.3）. CONCLUSIONS： Our findings indicate Bacrobial skin cleanser is not an overall effective treatment for muscle cramp prevention. However, the observed results at one minute may be useful to clinicians who treat cramp-prone patients.

Failed biliary cannulation: Clinical and technical outcomes after tertiary referral endoscopic retrograde cholangiopancreatography

AIM: Prospective evaluation of repeat endoscopic retrograde cholangiopancreatography (ErCP) for failed Schutz grade 1 biliary cannulation in a high-volume center. METHODS: Prospective intention-to-treat analysis of patients referred for biliary cannulation following recent unsuccessful ErCP. rESULTS: Fifty-one patients (35 female; mean age: 62.5 years; age range: 40-87 years) with previous failed biliary cannulation were referred for repeat ErCP. The indication for ErCP was primarily choledocholithiasis (45%) or pancreatic malignancy (18%). Successful biliary can- nulation was 100%. The precut needle knife sphincterotomy (NKS) rate was 27.4%. Complications occurred in 3.9% (post-ErCP pancreatitis). An identif iable reason for initial unsuccessful biliary cannulation was present in 55% of cases. Compared to a cohort of 940 nave pa-pilla patients (female 61%; mean age: 59.9 years; age range: 18-94 years) who required sphincterotomy over the same time period, there was no statistical difference in the cannulation success rate (100% vs 98%) or postErCP pancreatitis (3.1% vs 3.9%). Precut NKS use was more frequent (27.4% vs 12.7%) (P = 0.017). CONCLUSION: referral to a high-volume center following unsuccessful ErCP is associated with high technical success, with a favorable complication rate, compared to routine ErCP procedures.

Concise review on the comparative efficacy of endoscopic ultrasound-guided fine-needle aspiration vs core biopsy in pancreatic masses, upper and lower gastrointestinal submucosal tumors

Endoscopic ultrasound(EUS)-guided fine needle aspiration with or without biopsy(FNA/FNB) are the primary diagnostic tools for gastrointestinal submucosal tumors. EUS-guided fine needle aspiration(EUS-FNA) is considered a first line diagnostic method for the characterization of pancreatic and upper gastrointestinal lesions, since it allows for the direct visualization of the collection of specimens for cytopathologic analysis. EUSFNA is most effective and accurate when immediate cytologic assessment is permitted by the presence of a cytopathologist on site. Unfortunately, the accuracy and thus the diagnostic yield of collected specimens suffer without this immediate analysis. recently, a EUS-FNB needle capable of obtaining core samples(fine needle biopsy, FNB) has been developed and has shown promising results. This new tool adds a new dimension to the diagnostic and therapeutic utility of this technique. The aim of the present review is to compare the efficacy of EUS-FNA to that afforded by EUS-FNB in the characterization of pancreatic masses and of upper and lower gastrointestinal submucosal tumors.

Diagnosis of a submucosal mass at the staple line after sigmoid colon cancer resection by endoscopic cuttingmucosa biopsy

A 48-year-old man underwent laparoscopic sigmoid colon resection for cancer and surveillance colonoscopy was performed annually thereafter. Five years after the resection, a submucosal mass was found at the anastomotic staple line, 15 cm from the anal verge. Computed tomography scan and endoscopic ultrasound were not consistent with tumor recurrence. Endoscopic mucosa biopsy was performed to obtain a definitive diagnosis. Mucosal incision over the lesion with the cutting needle knife technique revealed a creamy white material, which was completely removed. Histologic examination showed fibrotic tissue without caseous necrosis or tumor cells. No bacteria, including mycobacterium, were found on culture. The patient remains free of recurrence at five years since the resection. Endoscopic biopsy with a cutting mucosal incision is an important technique for evaluation of submucosal lesions after rectal resection.

“阴三针”对卵巢早衰小鼠卵巢功能及卵巢TNF-α、Caspase-3蛋白表达的影响

目的:通过观察卵巢早衰(POF)模型小鼠血清中雌二醇(E_(2))、卵泡刺激素(FSH)、促黄体生成素(LH)和下丘脑中β-内啡肽(β-EP)水平的变化、卵巢组织形态变化及TNF-α、Caspase-3蛋白在卵巢中的表达,探讨卵巢早衰的生理病理变化及“阴三针”对卵巢早衰的改善、治疗作用。从而以“经脉所过,主治所及;腧穴所在,主治所及”的针灸理论为基础,以“阴三针”为治疗组方,为临床上电针延缓生殖轴衰老的作用机制提供依据。方法:用雷公藤多苷片45 mg/kg灌胃模拟出卵巢早衰小鼠模型,6周后将小鼠随机分为空白对照组、模型组、电针组、坤泰胶囊药物组与假电针组。干预3周后,利用酶联免疫吸附法(ELISA)检测血清中E_(2)、FSH、LH水平及下丘脑中β-EP水平;HE染色观察卵巢组织形态的变化;免疫印迹法检测TNF-α、Caspase-3蛋白在卵巢中的表达。结果:与空白对照组相比,POF模型小鼠血清中FSH、LH升高,E_(2)和下丘脑中β-EP显著下降,卵巢颗粒细胞层减少、生长卵泡和黄体减少及闭锁细胞增多,卵巢中TNF-α蛋白的表达显著升高,Caspase-3蛋白的表达无显著变化;与POF模型组小鼠相比,电针组小鼠血清中LH降低,E_(2)和下丘脑中β-EP提高,卵巢颗粒细胞层增多、闭锁细胞减少、生长卵泡和黄体明显增加,卵巢中TNF-α蛋白的表达显著降低,Caspase-3蛋白的表达无显著变化。结论:本研究结果表明“阴三针”可以促进下丘脑、垂体和卵巢分泌相关激素,抑制颗粒细胞凋亡,使卵巢功能得到改善或恢复,最终达到下丘脑、垂体对卵巢及卵巢对下丘脑、垂体的正负反馈调节。
